Patient satisfaction and penile morphology changes with postoperative penile rehabilitation 2 years after Coloplast Titan prosthesis

A common complaint after inflatable penile prosthesis surgery is reduced penile length. we previously reported how using the Coloplast Titan inflatable penile prosthesis with aggressive new length measurement technique （NLMT） coupled with postoperative IPP rehabilitation of the implant for 1-year helped to improve patient satisfact on and erectile peni e measurements Th s is a 2 years follow-up of a prospective, three-center, study of 40 patients who underwent Titan prosthesis placement, with new length measurement technique for erectile dysfunction. Patient instructions were to inflate daily for 6 months and then inflate maximally for 1-2 h daily for 6-24 months. Fifteen penile measurements were taken before and immediately after surgery and at follow-up visits. Measurement changes were improved at 24 months as compared to immediately postoperative and at 12 months. 67.8% of subjects were satisfied with their length at 2 years, and 77% had perceived penile length that was longer （30.8%） or the same （46.2%） as prior to the surgery. 64.3% and 17.9% of subjects had increased and unchanged satisfaction, respectively, with penile length as compared to prior to penile implant surgery. All but one subject （96.5%） was satisfied with the overall function of his implant. This study suggests using the Coloplast Titan with aggressive cylinder sizing, and a postoperative penile rehabilitation inflation protocol can optimize patient satisfaction and erectile penile measurements at 2 years postimplant.

The worldwide prevalence of spinal cord injury （SCI） ranges from 233 to 755 per million inhabitants, whereas the report- ed incidence lies between 10.4 and 83 per million inhabitants annually （wyndaele and wyndaele, 2006）. Thus, the socio- economic impact of SCI associated with cervical trauma is high enough that it could become an important concern in the vast majority of developed countries.

Beacon-Less Geographic Routing in Real wireless Sensor Networks

Geographic Routing （GR） algorithms require nodes to periodically transmit HELLo messages to allow neighbors to know their positions （beaconing mechanism）. Beacon-less routing algorithms have recently been proposed to reduce the control overheads due to these messages. However, existing beacon-less algorithms have not considered realistic physical layers. Therefore, those algorithms cannot work properly in realistic scenarios. In this paper we present a new beacon-less routing protocol called BoSS. Its design is based on the conclusions of our open-field experiments using Tmote-sky sensors. BoSS is adapted to error-prone networks and incorporates a new mechanism to reduce collisions and duplicate messages produced during the selection of the next fo.warder node. we compare BoSS with Beacon-Less Routing （BLR） and Contention-Based Fo.warding （CBF） algorithms through extensive simulations. The results show that our scheme is able to *** almost perfect packet delivery ratio （like BLR） while having a low bandwidth consumption （even lower than CBF）. Additionally, we carried out an empirical evaluation in a real testbed that shows the correctness of our simulation results.

A 4-year retrospective study of add-on therapy to the fixed combination of dorzolamide/timolol for the treatment of PoAG

AIM:To evaluate the long-term response to the fixed combination of dorzolamide/timolol in patients with primary open angle glaucoma(PoAG)and the addition of other intraocular pressure(IoP)lowering medications such as prostaglandin analogs and ***:A retrospective,non-randomized,and descriptive clinical study was performed with 182 eyes diagnosed with *** were divided into three groups:a group with fixed combination of dorzolamide/timolol only,a second group with prostaglandin analogs plus fixed combination of dorzolamide/timolol,and a third group with the addition of brimonidine to the same fixed *** data were gathered retrospectively and the differences between groups were ***:IoP was reduced satisfactorily in all three groups;however,a progressive IoP reduction was noted in the group with the fixed combination plus prostaglandin *** this group,a progressive,significant and more homogeneous response of the reduction was noted in comparison with the other ***:IoP reduction was efficacious in all three *** addition of prostaglandin analogs showed progressive IoP reduction,progressive responseand absence of long-term *** did not show a significant additive effect.

Incidental findings at MRI-enterography in patients with suspected or known Crohn's disease

AIM:To determine the frequency and clinical impact of incidental findings detected with magnetic resonance imaging(MRI)-enterography in patients with suspected or known Crohn's disease(CD). METHoDS:Incidental findings were defined as unexpected lesions outside the small intestine,not previously known or suspected at the time of referral, and not related to inflammatory bowel *** a systematic review of medical charts we analyzed the clinical impact of incidental findings,and compared the MRI findings with subsequent diagnostic procedures. RESULTS:A total of 283 patients were included in the analysis,and MRI detected active CD in 31%,fistula in 1.4%and abscess in 0.7%.Extra-intestinal findings notrelated to CD were recorded in 72 patients(25%),of which 58 patients(20%)had 74 previously unknown *** or incompletely characterized findings were detected in 17 patients(6.0%).Incidental findings led to 12 further interventions in 9 patients(3.2%) revealing previously unknown pathological conditions in 5(1.8%).one patient(0.4%)underwent surgery and one patient was diagnosed with a malignant disease. MRI detected incidental colonic lesions in 16 patients of which additional work-up in 4 revealed normal anatomy. Two patients(0.7%)benefitted from the additional examinations,whereas incidental findings led to unnecessary examinations in 9(3.2%). CoNCLUSIoN:In a minority of patients with suspected or known CD,important incidental findings are diagnosed at ***,a substantial number of patients experience unnecessary morbidity because of additional examinations of benign or normal conditions.

Low temperature microwave-assisted vs conventional pyrolysis of various biomass feedstocks

A comparison between conventional pyrolysis and a novel developed low-temperature microwave-assisted pyrolysis methodology has been performed for the valorisation of a range of biomass feedstocks including waste residues. Microwave pyrolysis was found to efficiently deliver comparable evolution of bio-gases in the system as compared with conventional pyrolysis at significantly reduced temperatures （120-180 ℃vs 250-400 ℃）. The “Brain Stress Timing” phenomenon and other misinterpretations of randomized clinical trial on aneurysmal subarachnoid hemorrhage

Clipping and coiling are currently the two alternatives in treatment of ruptured cerebral aneurysms. In spite of some meritorious analysis, further discussion is helpful to understand the actual state of art. Retreatment and rebleeding rates clearly favors clipping, although short-term functional outcome seems to be beneficial for clipping, while this different is not such if we perform the comparison at a longer follow up. Longterm follow ups and cost analysis are mandatory to have a clear view of the current picture in treatment of subarachnoid hemorrhage. Treatment strategy should be made by a multi-disciplinary team in accredited centers with proficient experience in both techniques.
