Treatment patterns and clinical outcomes in patients with metastatic breast cancer treated with palbociclib-based therapies:real-world data in the Han population

Background:This study aimed to reveal the treatment patterns and clinical outcomes of diverse palbociclib-based regimens in Han patients with estrogen receptor-positive(ER+)metastatic breast cancer in routine clinical practice.Methods:The clinical data of patients with ER+metastatic breast cancer treated with palbociclib were collected from the National Cancer Center database.The efficacy profile of palbociclib in this Han population was evaluated,especially for various combination regimens.The efficacy of palbociclib-based therapy in patients with prior everolimus treatment was also assessed.Results:A total of 186 patients from 89 cities in 18 provinces in China were enrolled.The median progression-free survival(PFS)was similar among different palbociclib-combined groups(P=0.566):10.0 months(95%confidence interval[CI]3.8-16.1)in the+exemestane group,9.7 months(95%CI 6.3-13.1)in the+letrozole group,7.8 months(95%CI 5.5-10.2)in the+fulvestrant group,7.2 months(95%CI 3.2-11.3)in the+toremifene group,and 6.1 months(95%CI 1.2-11.0)in the+anastrozole group.Thirty-four patients(18.3%)had received everolimus for their metastatic disease before the prescription of palbociclib.The disease control rate was significantly lower in patients who had received previous everolimus than in the everolimus-naïve group(50.0%vs.82.2%,Plimus had significantly worse PFS than those in the everolimus-naïve group(3.4 months vs.8.8 months,P=0.001).After propensity score matching,patients pre-treated with everolimus had similar PFS(4.4 months,95%CI 0.5-8.2)compared with everolimus-naïve patients(6.1 months,95%CI 4.7-7.5,P=0.439).Conclusions:Various palbociclib-based regimens have promising efficacy in ER+metastatic breast cancer in real-world settings,even in patients who had been pre-treated with everolimus.

Earthworms （Eiseniafetida） were used to study the impact of low-dose cadmium in treated artificial soil （0, 0.6, 3, 6, 15, 30 mg/kg） and contaminated natural soil （1.46 mg/kg）. The changes of earthworms＇ physiological related gene expressions of metallothionein （MT）, armetocin, calreticulin and antimicrobial peptides were detected using real-time PCR after a 70-day incubation period. The results showed that low doses of cadmium could up regulate earthworms＇ MT and down regulate annetocin gene expression and show a significant positive and negative correlation respectively. The expression of two other genes, calreticulin and anti-microbial peptides, was induced at low doses of cadmium （highest gene expression at 0.6 mg/kg for calreticulin and 6 mg/kg for anti-microbial peptides） and inhibited at high doses. No significant correlation was found for these two genes. This study shows that MT and annetocin genes expression found in earthworms in contaminated soil have the potential to be developed as biomarkers of soil cadmium pollution.

Primordial germ cells(PGCs)are precursors of both male and female gametes as fundamental materials for organism development.The transcriptome,methylome,and chromatin accessibility profiles of PGCs in both mice and humans have been recently reported.However,little is known about the characteristics of PGCs at the protein levels,which directly exert cellular functions.Here,we construct landscapes of both proteome and 3D spatial distribution of mouse PGCs at E11.5,E13.5 and E16.5 days,the three critical developmental windows for PGCs'sex differentiation,female meiosis initiation and male mitotic arrest.In each developmental stage of PGCs,nearly 2,000–3,000 proteins are identified,among which specific functional pathways such as oxidative phosphorylation,DNA damage repair,and meiotic cell cycle are involved for different events during PGCs development.Interestingly,by 3D modeling we find that PGCs spatially cluster into around 1,300 nests in genital ridge at E11.5 and the nest number is not increased by the exponential proliferation of PGCs.Comparative analysis of our proteomic data with published transcriptomic data does not show a close correlation,meaning that the practically executive factors are beyond the transcriptome.Thus,our work offers a valuable resource for the systematic investigations of PGC development at protein level and spatial map.

Here,we report the synthesis of hard carbon materials(RH) made from natural rice husk through a single pyrolysis process and their application as an anode in sodium-ion batteries.The studies show that the electrochemical properties of RHs are affected by the treatment temperatures,which determine the materials morphology,in particular,their degree of graphitization and extent of continuous channels(nanovoids).The latter are accessible to sodium ions and significantly contribute to charge storage capacity of the produced anodes.The RHs obtained at 1600 °C deliver the highest reversible capacity of276 mAh g^(-1) mainly due to insertion of sodium ions into the nanovoids.This work deepens the basic understanding of the influence of the carbonization temperature on the sodium storage mechanism.

Objective: To study the effects of folic acid and vitamin therapy on Hcy metabolism, endothelial injury and placental blood perfusion in preeclampsia. Methods: Preeclampsia patients in our hospital during May 2014-March 2017 were randomly divided into observation group receiving folic acid, vitamin and conventional treatment, the control group receiving conventional treatment;before and after 2 weeks treatment, serum Hcy metabolism index and endothelial injury index were determined;after delivery, contents of oxidative stress index and apoptosis index in placenta were determined. Results: Two weeks after treatment, serum Hcy, ET-1, sEng, sFlt-1 contents of observation group were significantly lower than the control group;VEGF, PLGF, folic acid and VitB12 contents were significantly higher than the control group;after delivery, MDA, AOPP, 8-OHdG, Fas, FasL, Bax, Caspase-3 contents in placenta of observation group patients were significantly lower than the control group;SOD, GSH-Px, XIAP and Survivin in placenta were significantly higher than control group. Conclusion:The treatment of preeclampsia by folic acid and vitamins can improve the metabolism of Hcy, alleviate the injury of the endothelial cells and increase the blood flow perfusion of the placenta.

Vinylidene and its heavier analogues are fundamentally important in synthetic chemistry,but their stabilization and structural characterization remains extremely challenging.Herein we describe that the reduction of an NHSi-stabilized stannyl-stannylene(2)with one molar equivalent of[(^(Mes)Nacnac)Mg]_(2)affords an NHSi-stabilized distannavinylidene(3).Single-crystal X-ray crystallography and density functional theory(DFT)calculations show that compound 3 features a pronounced Sn=Sn double bond and one lone pair of electrons at the two-coordinate Sn atom.most strikingly,3 undergoes an interconversion with distannyne(4)upon the addition and removal of one molar equivalent of N-heterocyclic carbene.Compound 3 readily reacts with Ph_(2)C=C=O,Ad C≡P and(Al Cp^(*))_(4)to give the structurally interesting stannacycles(5,6)and an aluminyl distannyne(7),demonstrating its unique reactivity derived from the high substituent lability.

BACKGROUND Anti-melanoma differentiation-associated gene 5 antibody-positive(anti-MDA5 Ab+)dermatomyositis complicated with rapidly progressive interstitial lung disease(anti-MDA5 Ab+DM-RP-ILD)has an unclear underlying mechanism with no recommended unified treatment plan.Herein,one of the cases that we report(Case 2)was successfully treated with tocilizumab despite having lung infection.CASE SUMMARY Case 1 was a 30-year-old woman who was admitted due to recurrent rash for 5 mo,fever and cough for 1 mo,and chest tightness for 3 d.She was diagnosed with non-myopathic dermatomyositis(anti-MDA5 Ab+)and interstitial pneumonia,and was treated with the combination of hormone therapy and cyclophosphamide followed by oral tacrolimus.Case 2 was a 31-year-old man admitted due to systemic rash accompanied by muscle weakness of limbs for more than 1 mo,and chest tightness and dry cough for 4 d.He was diagnosed with dermatomyositis(anti-MDA5 Ab+)and acute interstitial pneumonia with Pneumocystis jirovecii and Aspergillus fumigatus infections and was treated with hormone therapy(without cyclophosphamide)and the combination of tocilizumab and tacrolimus.The condition of both patients eventually improved and they were discharged and showed clinically stable condition at the latest follow-up.CONCLUSION Tocilizumab could be a salvage treatment for patients with anti-MDA5 Ab+DMRP-ILD who are refractory to intensive immunosuppression.

Objective: To study the effects of Kang Ai injection + conventional chemotherapy on tumor markers, apoptotic molecules and immune response in patients with advanced non-small cell lung cancer (NSCLC). Methods: Patients diagnosed as advanced NSCLC in our hospital during March 2015 - October 2017 were randomly divided into the experimental group receiving Kang Ai injection + conventional chemotherapy and the control group receiving conventional chemotherapy. The contents of tumor markers, apoptotic molecules and immunologic cytokines in serum and the contents of immune cells in peripheral blood were determined before and after chemotherapy. Results: after chemotherapy, the content of CEA, CYFRA21-1, SCC-Ag, GDF15 and livin in the serum of the two groups decreased significantly, and the content of PDCD5, P53 and Bax increased significantly, and the content of CEA, CYFRA21-1, SCC-Ag, GDF15, livin in the serum of the experimental group were lower than those of the control group, and the content of PDCD5, P53 and Bax were higher than those of the control group;the content of CD3+ and CD4+ cells in peripheral blood and the content of IFN-γ, IL-2 in serum of control group significantly decreased, whereas the content of CD8+ cells in peripheral blood and the content of IL-4, IL-5 in serum significantly increased after chemotherapy;while the content of CD3+ and CD4+ cells in peripheral blood and the content of IFN-γ, IL-2 in serum of experimental group significantly increased, whereas the content of CD8+ cells in peripheral blood and the content of IL-4, IL-5 in serum significantly decreased after chemotherapy. Conclusion: Kang Ai injection + conventional chemotherapy in the treatment of advanced NSCLC can more significantly decrease the serum tumor markers, regulate the secretion of apoptotic molecules and anti-tumor immune response than conventional chemotherapy.
