Suicide is the second leading cause of death in youth globally; however, there is uncertainty about how best to intervene. Suicide rates are typically higher in males than females, while the converse is true for suicide attempts. We review this "gender paradox" in youth, and in particular, the age-dependency of these sex/gender differences and the developmental mechanisms that may explain them. Epidemiologic, genetic,neurodevelopmental and psychopathological research have identified suicidal behaviour risks arising from genetic vulnerabilities and sex/gender differences in early adverse environments, neurodevelopment, mental disorder and their complex interconnections. Further, evolving sex-/gender-defined social expectations and norms have been thought to influence suicide risk. In particular, how youth perceive and cope with threats and losses(including conforming to others' or one's own expectations of sex/gender identity) and adapt to pain(through substance use and helpseeking behaviours). Taken together, considering brain plasticity over the lifespan, these proposed antecedents to youth suicide highlight the importance of interventions that alter early environment(s)(e.g., childhood maltreatment) and/or one's ability to adapt to them. Further, such interventions may have more enduring protective effects, for the individual and for future generations, if implemented in youth.

背景：尽管血管近距离放射治疗是唯一批准用于治疗裸金属支架植入后再狭窄的方法，但是药物洗脱支架目前亦用于再狭窄的治疗。比较上述两种治疗方法各自优势的资料还很有限。目的：与血管近距离放射治疗裸金属支架内再狭窄比较，评价西罗莫司（雷帕霉素）洗脱支架的安全性及有效性。设计、地点及研究对象：于384例支架内再狭窄患者中进行前瞻性、多中心、随机试验。患者于2003年2月2004年7月入选自26家学院及社区医疗中心。本文资料均随访至2005年6月30日。干预：血管近距离放射治疗（n=125）或植入西罗莫司洗脱支架（n=259）。主要观测指标：术后9个月靶血管治疗失败（心源性死亡、心肌梗死或靶血管血运重建）。结果：两组患者的基线特征匹配良好。血管近距离放射治疗组与西罗莫司洗脱支架组患者病变长度相似（均值[SD]分别为16．76[8．55]laln和17．22[7．97]mln；P=0．61）。血管近距离放射治疗组手术成功率为99．2％（124／125），西罗莫司（雷帕霉素）洗脱支架组为97．3％（250／257）（P=0．28）。血管近距离放射治疗组靶血管治疗失败率为21．6％（27／125），西罗莫司洗脱支架组为12．4％（32／259）（相对危险度[relativerisk，RR]，1．7；95％可信区间[confidence interval，CI]，1．1～2．8；P=0．02）。血管近距离放射治疗组靶病变需要血运重建发生率为19．2％（24／125），西罗莫司洗脱支架组为8．5％（22／259）（RR，2．3；95％CI，1．3～3．9；P=0．004）。在血管造影随访中，血管近距离放射治疗组分析节段血管造影再狭窄率为29．5％（31／105），西罗莫司洗脱支架组为19．8％（45／227）（RR，1.5；95％CI，1.0—2．2；P=007）。6个月随访时，西罗莫司洗脱支架组最小管腔直径大于血管近距离放射治疗组（均值[SD]，1．52[0．63]mm比1．80[0．63]mm；P〈0．001），表明前者分析节段血管管腔净获益更大，其主要原因是植入支架以及无边缘再狭窄（0．68[0．60]mm比1．0[0．61]mm；P〈0．001）。结论：与血管近距离放射治疗裸金属支架内再狭窄比较，西罗莫司洗脱支架具有更好的临床及血管造影效果。

The Giant Radio Array for neutrino Detection（GRAnD） is a planned large-scale observatory of ultra-high-energy（UHE） cosmic particles, with energies exceeding 10～8 Ge V. Its goal is to solve the long-standing mystery of the origin of UHE cosmic rays. To do this, GRAnD will detect an unprecedented number of UHE cosmic rays and search for the undiscovered UHE neutrinos and gamma rays associated to them with unmatched sensitivity. GRAnD will use large arrays of antennas to detect the radio emission coming from extensive air showers initiated by UHE particles in the atmosphere. Its design is modular: 20 separate, independent sub-arrays, each of 10000 radio antennas deployed over 10000 km2. A staged construction plan will validate key detection techniques while achieving important science goals early. Here we present the science goals, detection strategy, preliminary design, performance goals, and construction plans for GRAnD.

Mineral and bone disorder(MBD)in chronic kidney disease(CKD)is tightly linked to cardiovascular disease(CVD).In this study,we aimed to compare the prognostic value of nine MBD biomarkers to determine those associated best with adverse cardiovascular(CV)outcomes and *** 5217 participants of the German CKD(GCKD)study enrolled with an estimated glomerular filtration rate(eG FR)between 30–60 mL·min-1 per 1.73 m2 or overt proteinuria,serum osteoprotegerin(OPG),C-terminal fibroblast growth factor-23(FGF23),intact parathyroid hormone(iP TH),bone alkaline phosphatase(BAP),cross-linked C-telopeptide of type 1 collagen(CTX1),procollagen 1intact n-terminal propeptide(P1nP),phosphate,calcium,and 25-OH vitamin D were measured at *** with missing values among these parameters(n=971)were excluded,leaving a total of 4246 participants for *** a median follow-up of6.5 years,387 non-CV deaths,173 CV deaths,645 nonfatal major adverse CV events(MACEs)and 368 hospitalizations for congestive heart failure(CHF)were *** and FGF23 were associated with all outcomes,with the highest hazard ratios(HRs)for *** the final Cox regression model,adjusted for CV risk factors,including kidney function and all other investigated biomarkers,each standard deviation increase in OPG was associated with non-CV death(HR 1.76,95%CI:1.35–2.30),CV death(HR 2.18,95%CI:1.50–3.16),MACE(HR 1.38,95%CI:1.12–1.71)and hospitalization for CHF(HR 2.05,95%CI:1.56–2.69).Out of the nine biomarkers examined,stratification based on serum OPG best identified the CKD patients who were at the highest risk for any adverse CV outcome and mortality.

[背景]人类在生命早期的发育期间可能特别容易受到环境暴露的影响。有关这一主题的人体研究一般集中在单一暴露与健康效应之间的关系。而"暴露组"的概念涵盖了从受孕开始的的全部暴露,完善了基因组。[目的]人类生命早期暴露组(HELIX)项目是一个新的合作研究项目,目的是为了采用新的暴露评估和生物标志分析方法来描述生命早期多种环境因素的暴露,并将这些与生物标志组学和儿童健康结局相关联,从而刻画"生命早期暴露组"。本文描述了该项目的总体设计。[方法]HELIX将利用欧洲现有的6个出生队列研究来估计产前、产后暴露的一系列化学和物理暴露。建立全部队列中总共32 000对母亲和儿童的暴露模型,并在一个包含1 200对母亲和儿童的子队列中测量生物标志。嵌套重复采样的定组研究(n=150)将收集生物标志的变化数据,利用智能手机来评估流动性和体力活动,并监测个体暴露。采用组学技术确定与暴露相关的分子学特征(代谢组、蛋白质组、转录组和表观基因组)。对于多次暴露,采用统计方法估计胎儿和儿童的成长、肥胖、神经发育和呼吸系统结局中的暴露-效应关系。项目还将进行一项健康效应评估测试,以评价组合暴露的风险和收益。[结论]HELIX是描述欧洲人群生命早期暴露组并解开它与组学标志物和儿童健康之间关联的首次尝试之一。作为对暴露组学这一概念的验证,该项目向生命过程中暴露组迈出了重要的第一步。

背景：对于非ST段抬高急性冠脉综合征（acute coronary syndromes，ACS）患者而言，与单独应用普通肝素相比，依诺肝素或者血小板糖蛋白Ⅱb／Ⅲa受体拮抗剂替罗非班与普通肝素联合应用都显示出较好的疗效。目前，尚不清楚依诺肝素和替罗非班联合应用是否像普通肝素和替罗非班标准联合方案一样安全有效。目的：在非ST段抬高的ACS患者中评价依诺肝素和替罗非班联合疗法与普通肝素和替罗非班联合方案的疗效及安全性。设计、地点及参试者：国际前瞻性、开标（open—label）、随机、非劣势（noninferiority）试验。在接受替罗非班和阿斯匹林治疗的非ST段抬高ACS患者中分别给予依诺肝素1mg／kg（n=2026）每12小时一次或经体重校正的静脉普通肝素（n=1961），然后进行比较。这项A to Z试验的A阶段试验是于1999年12月至2002年5月进行的。主要观察指标：意向治疗人群7天时死亡、再发心肌梗死和难治性缺血的发生情况，该意向治疗人群是根据优效性和非劣势原则确定的。应用“心肌梗死溶栓治疗试验”（Thrombolysis in Myocardial Infarction，TIMI）中之出血分级系统，通过监测出血发生率判定用药安全性。结果：在治疗第7天时，随机分配至依诺肝素组的2018例患者中有169例（8．4％）发生死亡、心肌梗死或者难治性缺血，而普通肝素组的1952例患者中有184例（9．4％）发生上述事件（风险比[hazard ratio，HR]为0．88，95％可信区间[confidence interval，CI]为0．71～1．08）。该结果符合预先设定的非劣势标准。除死亡外，所有一级和二级复合终点事件分析结果均提示，依诺肝素更为有益。死亡仅见于1％的患者（依诺肝素组23例，普通肝素组17例）。任何TIMI分级的出血发生率都很低（依诺肝素组为3．0％，普通肝素组为2．2％；P=0．13）。最差情况分析（合并了两种独立的出血判定策略）表明，采用依诺肝素每治疗200例患者就会发生一次TIMI严重出血。结论：对于接受替罗非班和阿斯匹林治疗的非ST段抬高ACS患者而言，依诺肝素是除普通普通肝素之外的另一种适当选择。依诺肝素可使一级终点相对减少12％，绝对减少1％，符合非劣势标准。该结果与以往未联用糖蛋白Ⅱb／Ⅲa受体拮抗剂的试验一致。

背景与目的鼻美学畸形可明显导致对体像的不满意。目前，仅有的对此类患者心理疾病的诊断种类是身体畸形恐惧症。本文将对身体畸形恐惧症在鼻成形术患者中存在的普遍性进行大规模的研究，并对此类患者的症状和临床属性进行阐释和评估。方法本组共226例患者，采用人口学特征量表（DemographicCharacteristics）、评价鼻外形的视觉类比量表（VisualAnalogScale）、评价身体畸形恐惧症的改良Yale-Brown强迫症量表（Yale-BrownObsessiveCompulsiveScale）让患者进行问卷调查，以评估其症状的严重程度，并采用通用生活质量调查表和Derriford外貌59量表（DerrifordAppearanceScale59）对与外貌损毁相关的患者13常生活情况进行评估。由独立观察者分别对患者的鼻外形进行评分。结果本组共有39％的患者表现出至少中等程度的身体畸形恐惧症症状。有美学目标（P〈0．001）、希望行鼻成形术修复手术（P=0．003）和有心理疾病史（P：0．031）者被认为有严重的身体畸形恐惧症。对鼻外形的客观和主观评分结果间无关联。评价身体畸形恐惧症的改良***强迫症量表结果与鼻主观评分结果呈负相关（n=210，P〈0．001），与鼻客观存在的畸形无关。身体畸形恐惧症可显著降低患者的普通生活质量（n,=160，P〈0．001），并严重损害与面容相关的日常生活（n=161，P〈0．001）。结论中度至重度身体畸形恐惧症在美容鼻整复手术患者中具有普遍较高的发病率。患者行鼻成形修复手术和有心理疾病史尤其是诊断有身体畸形恐惧症的高危因素。身体畸形恐惧症可显著降低患者的普通生活质量，并严重损害患者与面容相关的日常生活。

全球变化科学数据是人类宝贵的科技资源。兼顾保护数据知识产权和推动数据广泛共享是科学数据界长期存在的普遍难题。数据出版(元数据、实体数据、数据论文关联出版)是解决这个难题的有效机制。发展中国家科学数据共享原则执行过程中,亟需数据出版基础设施和能力建设。"全球变化科学研究数据出版系统"(中英文)在保护数据知识产权和促进数据共享方面作出实践性案例,在数据产权认证、数据质量标准、同行专家评审、数据长期保藏、数据开放共享、国际资质联网等方面的成果为数据的增值起到关键性作用。《全球变化数据学报》(中英文)是该系统重要组成部分。该系统成为世界各国全球变化科学研究领域学者、特别是发展中国家学者可信赖的、国际组织(WDS、GEOSS、CODATA、Clarivate Analytics等)认可的、数据出版和共享世界性基础设施。
