Obstetric Fistula: Epidemiological, Social and Therapeutic Aspects: Prospective Study of 196 Patients Admitted to the ZINDER Mother and Child Health Centre (CSME)

Introduction: Obstetric fistula (OF) is an abnormal communication between the genital and urinary tracts and/or associated with a recto-vaginal communication resulting from prolonged dystocic delivery. It is a frequent pathology in underdeveloped countries. In Zinder no study has been carried out on obstetric fistula. The aim of this study was to evaluate the epidemiological, social impact and therapeutic aspects of obstetric fistula at the CSME of Zinder. Patient and Method: This was a descriptive cross-sectional study of obstetric fistula care at CSME during the period of (January 2018 to June 2020). Results: A total of 196 cases were collected over 30 months. This represents an annual incidence of 78.4 fistulas. The median age of the patients was 18.63 years, and more than 65% were over 20 years old. One hundred and ninety-one patients (97.45%) did not attend school, and 56.12% (n = 110) lived in polygamous households. One hundred and fifty-one women (n = 151), 77.04% had full assistance from their husband during the first four (4) months of the pathology. Twenty-four (12.24%) had been notified of repudiation. Obstetrical risk of dystocia was found in 39.79% (n = 78) of the patients, of whom 24.49% (n = 48) had a focused antenatal consultation (CPNR). The labour lasted more than 24 hours in 100%. The majority of women are multiparous and 60% had at least 2 pregnancies. vesicovaginal fistula is the main pathological type with 86.23%. Fistulas were closed in 83.16% and 68.87% exited without urine loss. Conclusion: Obstetric fistula is a devastating disease affecting girls and young women in Niger, as in other countries in Africa and Asia. It is a major concern worldwide and is a social tragedy because it is disabling and depressing.

Assessment of Domestic Wastewater Management Practices in the Communal District I of Maradi City, Niger Republic

Wastewater treatment is a problem with much acuity in the city of Maradi in general, and particularly in the Communal District I of Maradi. It is for this reason that the present study was conducted which deals with wastewater sanitation. The main objective of this study conducted through a survey is to investigate in the domestic wastewater sanitation in the Communal District I of Maradi city. Specifically, the study aims to estimate the amount of wastewater produced by households and to analyze the sanitation system at household and at district levels. The wastewater management practices in this district were assessed through a descriptive cross sectional study in which a total of 129 households were selected by sample distribution technique in the district neighborhoods and studied using interviewer and administered questionnaires at communal and household levels. The results of the study showed that the amount of domestic wastewater generated is quite large, about 32.27 m3 per person per day, and this comes from several sources including showers, laundry, dishwashing and income generating activities. The results also revealed that the unhealthy state of the Communal District I of Maradi is related to the absence of a wastewater treatment system, the methods and practices used in its management, and the population is also largely responsible for this unhealthy state and not willing to participate to a sanitation project.

Environmental Factors, Constraints and Risks of Rainwater Runoff in Commune II of Maradi (Republic of Niger)

The Urban Commune of Maradi in Niger experienced increasingly frequent flooding following rainy episodes generating volumes and water flows that are difficult to control in recent years. This study aims to determine the impacts of runoff water in Commune II of Maradi City and to assess the role of urban planning in runoff management, as well as the strengths and weaknesses of urban sanitation in Maradi. The methodological approach used consisted of: 1) an interpretation of documentary data, 2) gathering information on runoff at several sites in the study area, and 3) interviews and questionnaires with local communities. This approach made it possible to understand the degree of vulnerability of the populations to flooding and then to propose sustainable solutions to reduce the vulnerability of the affected populations, through the implementation of effective urban rainwater management practices (USWMP). The results show that the impacts of runoff are mainly related to the lack of adequate storm water drainage networks in the area, but also to the nature of the habitats. Geomorphological factors such as the nature of the soil, the slope of the terrain and the altitude increase the degree of risk. In addition, the inadequacy and dilapidation of the drainage systems of the canals and above all the anarchic occupation of the land linked to accelerated urbanization are the cause of flooding by stagnation of rainwater. A phenomenon now linked to numerous deteriorations of urban equipment, the runoff of rainwater in the city of Maradi carries so many pollutants that municipalities, businesses and individuals should put in place pollution prevention measures.

Adult Urological Cancers at Zinder National Hospital

The aim of this study was to present the epidemiological, clinical and therapeutic aspects of adult urological cancers in the department of urology at Zinder National Hospital. Patients and Methods: This was a cross-sectional study of adult primary urologic cancers during the period of January 2019 to December 2023. Data were obtained from the record of patients admitted to urology, and the medical oncology department. The parameters studied were;frequency, age, sex, urogenital distribution, anatomopathological type, and therapeutic aspects. Data were recorded on Excel 2013 and analyzed with Epi-info version 7.2.5. Results: Epidemiology: About 289 cancers were diagnosed through 6017 consultations during the period, corresponding to 4.80% of urological pathologies: prostate cancer (n = 221;76.47%), bladder cancer (n = 46;15.92%), kidney cancer (n = 13;04.5%), testis cancer (n = 09;03.11%). Median age of patients was 50.42 years. Prostate cancer: was Adenocarcinoma in 100% (n = 221) with Gleason scores > 7 (77.83%) and prostatique specific antigen (PSA) > 20 ng/ml in 90.04%. Extension evaluation 134 cases (60.63%). Treatment was chirurgical castration in (62.44%) and resistance to castration appeared between 8 and 11 months. Hospital mortality for prostate cancer was 16.29%. Bladder cancer was found in 46 cases with Sex ratio 7/1. Cystoscopy was performed for all. Histology revealed squamous cell carcinoma (n = 41;89.13%), and (n = 5;10.87%) urothelial. Endoscopic resection performed (n = 14;30.43%). Kidney cancers were 13 cases (10 men, 03 women). Nephrectomy was performed in (n = 8;61.54%) cases and Histology revealed renal cell cancer in 76.92%. Testicular cancers were diag 09 cases. History of cryptorchidism was found in 4 cases, 4-year infertility in 3 cases. Orchidectomy was done in all cases. Chemotherapy in 3 cases. Analysis of the Energy and Environmental Sustainability of a Built Space System: The Case of Patte d’Oie University Campus in Ouagadougou

The aim of this study was to carry out a dynamic simulation of the energy and environmental performance of a built space system, with a view to assessing its energy and environmental class. The use of a simulation and modeling tool, supported by various methodological references, formed the basis of our approach. Adopting a systemic perspective, we described the structural and functional aspects of the systems making up built spaces, as well as the associated energy flows. Our approach was also based on a typology, taking into account typical days, structural and functional configurations at different scales and angles of observation. The analysis tool we developed in Java was applied to the built space system of the Patte d’Oie university campus in Ouagadougou. Annual electricity consumption was measured at 124387.34 kWh, closely aligned with the average annual electricity bill (125224.31 kWh), with a maximum relative deviation of 1%, followed by a carbon emission balance of 58337.66 kg eq CO2 per year. This validation confirmed the effectiveness of our tool. In addition, following the analysis of electricity consumption using our tool, the university campus was classified in energy class B and environmental class C. These results will be based on the emission factors of the energy mix of the West African Economic and Monetary Union (WAEMU) territory, with particular emphasis on Burkina Faso.

Numerical Modelling of Coupled Heat and Mass Transfer in Porous Materials: Application to Cinder Block Bricks

In this work, we present numerical modelling of coupled heat and mass transfer within porous materials. Our study focuses on cinder block bricks generally used in building construction. The material is assumed to be placed in air. Moisture content and temperature have been chosen as the main transfer drivers and the equations governing these transfer drivers are based on the Luikov model. These equations are solved by an implicit finite difference scheme. A Fortran code associated with the Thomas algorithm was used to solve the equations. The results show that heat and mass transfer depend on the temperature of the air in contact with the material. As this air temperature rises, the temperature within the material increases, and more rapidly at the material surface. Also, thermal conductivity plays a very important role in the thermal conduction of building materials and influences heat and mass transfer in these materials. Materials with higher thermal conductivity diffuse more heat.

Contribution of Garcinia kola to the Productivity and Economic Profitability of the Community Agricultural Area of Kedougou: Financial and Comparative Analysis of Production Factors

Inland fishing predominates in the Kedougou region, which has no coastline. This type of fishing does not yet meet the local population’s demand for fish products. In response to this concern, a Community Agricultural Estate housing an integrated fish farm was set up in the commune of Bandafassi, in the village of Itato. Since its creation, this production unit has been faced with the problem of sourcing high-quality, low-cost, monosex male Oreochromis niloticus fry. In order to overcome this constraint, the present research focuses on the contribution of Garcinia kola to the productivity and economic profitability of the Itato farm. The aim of the research is to assess fish production in the experimental set-up and the production costs of tilapia in a controlled environment. The comparative study of the various production factors shows disparities only in the input factor, where scenarios 2 and 3 use additional products. These are 17-α-methytestosterone for scenario 2 and Garcinia kola for scenario 3. These products significantly interfere with fish production, with a fairly high mortality rate for scenario 2 (25% for two production cycles/ cohort2 (B5, B6, B7 and B8) treated with 17-α-methytestosterone). As for scenarios 1 and 3, mortalities are 5% with or without recourse to additional products (G. kola). In addition, average fish production for the three (03) scenarios is estimated at 28687.5 kg/2 cycles. It varies from one scenario to another, i.e. 30937.5 kg/2 cycles for scenarios 1 and 3 and 24187.5 kg/2 cycles for scenario 2. It is therefore higher in scenarios 1 and 3 than in scenario 2. This difference is due to the fairly large losses of individuals in scenario 2. Furthermore, the analysis of the profit and loss accounts for tilapia production varies from one scenario to another depending on the type of farm: 476 Franc CFA for scenario 1, 610 Franc CFA for scenario 2 and 472 Franc CFA for scenario 3 (F CFA = franc of the French Colonies of Africa). Fish Assemblage and Abundance Distribution in Nanji Islands Marine Nature Reserve in Relation to Season Change

The fishing survey station located in the warm waters of the East China Sea is one of the abundant fishery resources of waters;with a large number of economic fish breeding grounds and economic juvenile feeding grounds. Several fish species according to this bottom trawl survey results which appear in the annual survey from winter to autumn give us a total of 58 species, which have appeared throughout the year. But the vast majority of species appears only in certain seasons, according to the sea fish survey and based on seasonal migratory species or species mainly small endemic species. The survey of fish is tropical and subtropical warm water species and warm temperate species, and the history of the area has several types of 397 different species. There are many reasons, in addition to recent years, human disturbance and adverse consequences of environmental changes brought about, but also with the relevant sampling methods and sampler time, the survey sampled only trawl based, from Ping yang Ocean and Fisheries Bureau, the waters are still other jobs method net, gill nets, fishing industry, etc. Survey results show that the type of fish in the summer (58 species) > spring (55 species) > winter (51 species) > autumn (42 species). The type of season was among spring to summer rose, summer to autumn decline. The survey found that in the reef area of the Nanji Islands, no one species of fish is the dominant species throughout the year (according to the dominant species, each species is dominant only in one season). From the dominant species turnover accounting for the four seasons, in winter it has 53.72% of the total biomass or annual biomass, spring it has 41.53% of the overall biomass, summer it has 31.85% and autumn it has 38.56% of total biomass. Could ERA5 Deliver Better Climate Services Than ERA-Interim over the West African Sahel Region?

Our paper assessed the improvement performance of the reanalysis(ERA5)compared to ERAI(ERA-Interim)both from the ECMWF(European Center for Medium-Range Weather Forecast)in representing the WAM(West African Monsoon)*** aim is to evaluate the reliability of ERA5 to deliver better climate services than ERAI in the West African Sahel *** complementary observational databases namely the CRU(Climate Research Unit)and the GPCC(Global Precipitation Climatology Center)data are used to evaluate precipitation and temperature representation by the two ***,the representation of some major features of the WAM system,such as the SHL(Saharan Heat Low),the AEJ/TEJ(African and Tropical Easterly Jets)was assessed using the two reanalysis *** obtained results show a better representation of the seasonal accumulated precipitation and average temperature by ERA5 compared to ERAI with higher spatial correlation and lower bias relative to the ***,ERAI appears to be rainier than ERA5 but ERA5 produces more heavy rainfall *** the period of intense monsoon,the frequency of the SHL is higher for ERAI which would favor intensification of monsoon inflow and *** lower SHL frequency observed in the ERA5 could explain the observed weakening intensity of AEJ which is favorable for moist conditions over the *** findings confirm the progress made by ERA5 compared to ERAI in representing the WAM dynamic and demonstrate its reliability for delivering better climate services over the West African Sahel.

A Rare Etiology of a Life-Threatening Subdural Hematoma: Case Report and Literature Review

Background: Life-threatening subdural hematoma is commonly related to trauma and rarely revealed by neoplasm. Observation: We report a case of a 53-year-old suffering from mild headache and without a history of trauma, was admitted unconscious due to a subdural hematoma on radiological investigations. Beside the left subdural hematoma, there was also alytic lesion of the sphenoid wing and the temporal bone on the same side. An emergent removal of the subdural hematoma and an excision of the bone lesion were performed. Pathological examination diagnosed a cavernous hemangioma of the skull. The postoperative period was uneventful with a dramatic recovery of the patient. Conclusion: A quick worsening of a chronic headache, acute impairment of an uncommon headache deserve prompt investigation and emergent surgical management in case of intracranial hematoma. Any bone and dural abnormalities at the vicinity of a subdural hematoma require total excision with clean border and pathological examination.
