AIM:To demonstrate that CD14 + cells are an important source of the growth factor YKL40 in acute and chronic liver ***:Rats were inoculated with one dose of CCl4 to induce acute *** biopsies were obtained at 0,6,12,24,48 and 72 *** chronic damage,CCl4 was administered three days per week for 6 or 8 *** samples were collected,and cellular populations were isolated by liver digestion and purified by cell ***40 mRNA and protein expression were evaluated by realtime polymerase chain reaction and western ***:Acute liver damage induced a rapid increase of YKL40 mRNA beginning at 12 *** peaked at 24 h,with a 26fold increase over basal *** 72 h however,YKL40 expression levels had nearly returned to control *** the other hand,chronic damage induced a sustained increase in YKL40 expression,with 7and 9fold higher levels at 6 and 8 wk,*** pattern of YKL40 expression in different subpopulations showed that CD14+cells,which include Kupffer cells,are a source of YKL40 after acute damage at 72 h[0.09 relative expression units(REU)]as well as after chronic injury at 6 wk(0.11 REU).Hepatocytes,in turn,accounted for 0.06 and 0.01 REU after 72 h(acute)or 6 wk(chronic),*** rest of the CD14cells(including T lymphocytes,B lymphocytes,natural killer and natural killer T cells) yielded 0.07 and 0.15 REU at 72 h and 6 wk,***40 protein expression in liver was detected at 72 h as well as 6 and 8 wk,with the highest expression relative to controls(11fold;P≤0.05)seen at 6 *** were stimulated by *** demonstrate that under these conditions,these cells showed maximum expression of YKL40 at 12 h,with P<0.05 compared with ***:Hepatic CD14 + cells are an YKL40 mRNA and protein source in acute and chronic liver injury,with expression patterns similar to growth factors implicated in inflammationfibrogenesis.

Introduction: Mechanical or physical restraint is an exceptional therapeutic resource to immobilize a subject and thus guarantee the safety of the patient and/or third parties in the face of high-risk behaviors, but it entails multiple crossings (bioethical, philosophical, medical, psychological, legal). Framed in the so-called “safety culture” developed by the WHO, based on the Protocol for its implementation of the CABA and attentive to its frequent use in CABA by different hospital services (medical clinic, geriatrics, intensive care and medical guards) we consider it necessary its study in terms of compliance with the risks it entails and its management. Objectives: Identify regulatory compliance with the GCABA Mechanical restraint (MR) Protocol from a patient safety perspective, as well as describe the clinical and medicolegal aspects, and propose the usefulness of a tool for its management and control. Methodology: Observational, descriptive, transversal and prospective work through the analysis of Clinical Records with indication of MR using a rubric-type form. 177 cases were analyzed between September-November 2023 from three hospitals of the Government of the City of Buenos Aires, statistical parameters were applied and graphs were made. Results: Only 12.99% complied with the Protocol. In the mental health specialized hospital compliance was almost 5 times greater than in the general one, and in the emergency services compliance was 12 times greater than in Inpatient services. We found that the start or end time of MR was not recorded and only 43% described the causes/justifications for the indication (mostly in Emergency and Specialized hospitals), with the MR average time being shorter in Emergency. Conclusions: Only 1.3 out of 10 patients reliably completed the Protocol and it was mostly in the mental health specialized hospital and the emergency services. Sports have scored significant attention among the public in this multifaceted world. Diverse training strategies are followed by many athletics and even flexible to adapt comfortable and optimal techniques. This fact has led physicians and educators to encourage remote health surveillance as one of the core strategies in athletic training. The need for innovative data exploration methodologies capable of facing Big Data’s influence to make remote monitoring services viable has been raised by the growing ties of networks that deliver high quantities of real-time data. This paper presents an interactive healthcare data exploration and visualization(IHDEV) model to enhance multi-scaling data analysis and visualization in the athletic health vision platform. This paper aims to simplify optimization methods to measure sportsperson muscle tension. This model illustrates a three-layer architecture with a raw data acquisition layer, data analysis layer, and visualization layer. The first layer considers the acquisition of health-related data from the athletes for remote monitoring using IoT and stores it into the cloud. The data analysis layer adapts artificial intelligence(AI) in data mining. The final layer introduces an intelligent interactive data visualization model assisted by a reactive workflow mechanism, enabling analysis and visualization solutions to be composed in a personalized data flow appropriate to the athletic training. This experimental study extended with two healthcare datasets to show the feasibility of IHDEV in promoting healthcare based athletic monitoring and improves the accuracy ratio of 96.7%, prediction ratio of 96.2%, an efficiency ratio of 96.8%, Pearson correlation coefficient of 98.2%, and reduces the error rate of 18.7% compared to other conventional models.

AIM： To determine the incidence of cystoid macular edema（CME） after Descemet＇s stripping automated endothelial keratoplasty（DSAEK）.METHODS： This study included all consecutive patients operated in a Spanish tertiary reference hospital over a period of four years. A total of 55 eyes from 47 patients matched the selection criteria. CME was diagnosed clinically at the slit-lamp and confirmed by optical coherence tomography. RESULTS： Six cases of CME were diagnosed postoperatively, which represented an incidence of 11%. Three patients had previously undergone DSAEK alone（7%; 3/41） and the other three, DSAEK combined with phacoemulsification（21%; 3/14）. Five out of six patients with CME responded to standard ***： CME is a possible complication after DSAEK and can be treated with standard therapy. CME appears more frequently when DSAEK is combined with phacoemulsification and posterior chamber（PC） intraocular lens（IOL） implantation. Intraoperative damage to the corneal endothelial cells might play a role in the pathogenesis of CME. As long as the causes remain unclear, we recommend administering prophylaxis when risk factors are present or when combined surgery is planned.

This document presents a computer vision system for the automatic recognition of Mexican Sign Language (MSL), based on normalized moments as invariant (to translation and scale transforms) descriptors, using artificial neural networks as pattern recognition model. An experimental feature selection was performed to reduce computational costs due to this work focusing on automatic recognition. The computer vision system includes four LED-reflectors of 700 lumens each in order to improve image acquisition quality;this illumination system allows reducing shadows in each sign of the MSL. MSL contains 27 signs in total but 6 of them are expressed with movement;this paper presents a framework for the automatic recognition of 21 static signs of MSL. The proposed system achieved 93% of recognition rate.

Microstructural evolution during mechanical alloying of Ag and Zn, and subsequent heat treatments were investigated. The mechanical alloying was carried out in a SPEX 8000D miller. The microstructural characterization was obtained by X-ray diffraction （XRD）, transmission electron microscopy （TEM） and scanning electron microscopy （SEM）. The thermal behavior was studied using differential scanning calorimetry （DSC）. Based on the results obtained, it can be concluded that at the early stages of milling was possible to detect the ε, β, β′, α solid solutions and remaining Zn. Later, the ε, β, β′ and Zn phases disappeared while the Zn concentration of the α solid solution was strongly increased. After 7.2 ks of milling, the mechanical alloying process reached a steady state. During this period, both the composition and crystallite size of the α solid solution remained practically unchanged. On the other hand, subsequent heat treatments of milled powders showed that the α solid solution could also be obtained by the combination of mechanical alloying and heat treatment. Finally, the evolution of the microstructure during milling and annealing was combined to propose an optimal processing route in order to obtain a α solid solution.

Depending on its time of evolution, leptospirosis presents two phases: acute and chronic. The chronic leptospirosis is widely accepted in animals and, until recently, this was denied in humans. Since the 1990s, we have studied dozens of cases demonstrating the chronicity of leptospirosis, including an asymptomatic or undetermined form that may remain so throughout life time or may evolve into a definitive chronic disease and/or worsen, becoming severe cases. A fundamental part for its demonstration is the isolation and characterization of the etiologic agent. In this work, the first 14 isolations of Leptospira sp. are reported from human chronic leptospirosis and an African lion diagnosed with chronic leptospirosis, obtained between 2002 and 2006, in urine, blood, and skin microbiopsies. From the 14 isolations from patients with chronic leptospirosis, one of the cases is apparently congenital and two are from patients with chronic asymptomatic leptospirosis. The diagnosis was performed by means of videorecording in darkfield microscopy in blood and urine, specific culture media, and MAT serology. Argentic impregnation and detection of specific antigen by immunostaining in blood were performed in seven patients;in two patients, skin microbiopsies and biopsies were performed. All cases were positive by video-recording in darkfield, one of them MAT was 1:160, and the rest were positive at low titers (≤1:80). The two skin biopsies were positive by argentic impregnation and immunostaining. Eleven isolates were obtained from blood, two from skin, and one from urine. The isolates were characterized by monoclonal antibodies as Leptospira serovar Pomona. L. Pomona is considered of low virulence and high adaptability to the host, and is frequently associated to chronic leptospirosis in animals.
