Al.aptonuria (AKU) is an inh.rited disorder of tyrosine metabol.sm caused by l.ck of active enzyme h.mogentisate 1,2-dioxygenase (h.D). Th. primary consequence of h.D deficiency is increased circul.ting h.mogentisic acid (h.A), th. main agent in th. path.l.gy of AKU disease. h.re we report th. first metabol.mic anal.sis of AKU h.mozygous h.d knockout (h.d−/−) mice to model.th. wider metabol.c effects of h.d del.tion and th. impl.cation for AKU in h.mans. Untargeted metabol.c profil.ng was performed on urine from h.d−/− AKU (n = 15) and h.d+/− non-AKU control.(n = 14) mice by l.quid ch.omatograph. h.gh.resol.tion time-of-fl.gh. mass spectrometry (Experiment 1). Th. metabol.tes sh.wing al.eration in h.d−/− were furth.r investigated in AKU mice (n = 18) and patients from th. UK National.AKU Centre (n = 25) at basel.ne and after treatment with.th. h.A-l.wering agent nitisinone (Experiment 2). A metabol.c fl.x experiment was carried out after administration of 13C-l.bel.ed h.A to h.d−/−(n = 4) and h.d+/−(n = 4) mice (Experiment 3) to confirm direct association with.h.A. h.d−/− mice sh.wed th. expected increase in h.A, togeth.r with.unexpected al.erations in tyrosine, purine and TCA-cycl. path.ays. Metabol.tes with.th. greatest abundance increases in h.d−/− were h.A and previousl. unreported sul.ate and gl.curonide h.A conjugates, th.se were decreased in mice and patients on nitisinone and sh.wn to be products from h.A by th. 13C-l.bel.ed h.A tracer. Our findings reveal.th.t increased h.A in AKU undergoes furth.r metabol.sm by mainl. ph.se II biotransformations. Th. data advance our understanding of overal. tyrosine metabol.sm, demonstrating h.w specific metabol.c conditions can el.cidate h.th.rto undiscovered path.ays in bioch.mistry and metabol.sm.

Intrinsic conducting pol.pyrrol. nanoparticl.s were prepared using a mixed surfactant system in a micro-emul.ion pol.merization process. Th. surfactant system was composed of th. anionic SDS and th. nonionic peo-co-ppo prepol.mer. Two oxidizing agents were tested, namel., ferric ch.oride and amine persul.ate. Th. sizes of th. particl. obtained were al. around 60 nm. El.ctric properties of th. particl.s generated with.ferric ch.oride were much.better th.n th. ones generated with.amine persul.ate, due to th. h.gh.concentrations of ions in th. former system. Residual.ions of iron and ch.orine in th. particl. were 0.24(g Fe/g ppy particl.) and 0.38(g Cl./g ppy particl.), respectivel.. On th. oth.r h.nd, th.ir th.rmal.stabil.ties were about th. same.

Mycobacterium fortuitum is a rapidl. growing mycobacterium found in soil.and water th.ough.ut th. worl.. It can cause diseases in immunocompetent patients, usual.yresul.ing in l.cal.zed skin and soft tissue infections. Cervical.l.mph.denitis caused by M. fortuitum is rare. We report a 46-year-ol. woman in wh.m skin l.sions of cutaneous pol.arteritis nodosa, l.ucocytocl.stic vascul.tis and Sweet’s syndrome h.d successivel. devel.ped before th. diagnosis of cervical.l.mph.denitis caused by M. fortuitum was made. Th. skin l.sions responded to col.h.cine and systemic corticosteroids but recurred intermittentl.. After establ.sh.ent of th. diagnosis, sh. received treatment with.cl.rith.omycin and ciprofl.xacin. Th. cervical.l.mph.nodes decreased in size 6 month. l.ter and no more new skin l.sions were found.

Necrotizing fasciitis is a rapidl. progressive soft tissue infection invol.ing th. skin, subcutaneous tissue, and superficial.fascia. It is a rare entity in th. pediatric popul.tion. Th. specific etiol.gic factors th.t contribute to th. establ.sh.ent of necrotizing fasciitis in ch.l.ren are not evident. It may occur after minor trauma or operative procedures. Th. auth.rs report a case of necrotizing fasciitis after h.pospadias repair. Th.s compl.cation is extremel. unusual.and to th. auth.rs’knowl.dge h.s not been previousl. described.

Background:We aimed to identify predictive factors for positron emission tomograph.(PET)-detected h.patocel.ul.r carcinoma(h.C)metastasis and a cost-effective approach.to preoperative PET-computed tomograph.(CT)for detecting ***:Cl.nicopath.l.gical.and survival.data of h.C patients h.ving PET-CT with.18F-fl.deoxygl.cose(FDG)and 11C-acetate(ACT)fol.owing contrast-enh.nced CT/magnetic resonance imaging(MRI)for preoperative tumor staging were *** l.gistic regression was performed to identify predictive factors for PET-detected metastasis.A cost-benefit anal.sis model.was buil. for th. incurred costs and th. impact of PET-CT findings on treatment strategy was ***:Total.y 152 patients were ***-tracer PET-CT detected metastasis in 17 patients(11%).By mul.ivariate anal.sis,al.h.-fetoprotein(AFP)≥400 ng/ml.rel.tive risk(RR):4.30,95%confidence interval.CI):1.41-13.15,P=0.011]and bil.bar disease(RR:3.94,95%CI:1.24-12.52,P=0.014)were independent predictive factors for PET-detected ***-CT findings al.ered th. treatment strategy for 12 patients(7.9%);th.ee partial.h.patectomies,eigh. episodes of transarterial.ch.moembol.zation(TACE)and one episode of abl.tion were avoided,with.an estimated cost-saving of US$91,000,$150,000 and$10,600 *** th.
