Impact of Epidural Analgesia in Labour on Neonatal and Maternal Outcomes

Aim: To evaluate the effect of epidural analgesia during labour on neonatal-maternal outcomes. Methods: A retrospective cohort study of nulliparous parturients who gave birth in Vasternorrland County, Sweden, over a 2-year period between 2015 and 2016. Neonatal outcomes (Apgar score at 5 min and umbilical cord arterial blood gases), maternal outcomes (perineal injury, total bleeding volume and maternal satisfaction with birth) and labour parameters (mode of delivery and the durations of labour and postpartum hospital stay) were evaluated. Results: The study cohort consisted of 1449 women with singleton pregnancies. Patients were divided into two groups according to whether during labour they were administered epidural analgesia using bupivacaine and sufentanil (EDA group, n = 615) or not (non-EDA group, n = 834). The rate of assisted vaginal delivery was significantly higher in the EDA group than in the non-EDA group (15.6% and 11.3%, respectively, p < 0.05), whereas the rates of caesarean section were similar. The duration of the active phase of labour was significantly longer in the EDA group than in the non-EDA group (489 ± 217 min versus 371 ± 210 min, respectively, p < 0.001). The Apgar score at 5 min and umbilical cord blood pH were lower and the base deficit greater in the EDA group (p < 0.001, p < 0.001 and p < 0.01, respectively). Bleeding volume was similar between the groups after adjusting for gestational age. Women in the EDA group were more satisfied with their labour experience, as measured by the visual analogue scale (p Conclusion: The results of this study suggest that EDA affects delivery and neonatal-maternal outcomes negatively, but increases the mother’s satisfaction with labour.

Effect of human autologous serum and fetal bovine serum on human corneal epithelial cell viability,migration and proliferation in vitro

AIM： To analyze the concentration-dependent effects of autologous serum （AS） and fetal bovine serum （FBS） on human corneal epithelial cell （HCEC） viability, migration and proliferation. METHODS： AS was prepared from 13 patients with non- healing epithelial defects Dulbecco＇s modified eagle medium/ Ham＇s F12 （DMEM/F12） with 5% FBS, 0.5% dimethyl sulphoxide （DMSO）, 10 ng/mL human epidermal growth factor, 1% insulin-transferrin-selenium, then were incubated in serum media： DMEM/F12 supplemented by 5%, 10%, 15% or 30% AS or FBS. HCEC viability was analyzed using cell proliferation kit XTI＇, migration using a wound healing assay, proliferation by the cell proliferation enzyme-linked immunosorbent assay （ELISA） BrdU kit. Statistical analysis was performed using the generalized linear model, the values at 30% AS or 30% FBS were used as the baselines. RESULTS： HCEC viability was the highest at 30% AS or 15% FBS and the lowest at 10% AS or 30% FBS application. HCEC migration was the quickest through 30% AS or 30% FBS and the slowest through 5% AS or 5% FBS concentrations. Proliferation was the most increased through 15% AS or 5% FBS and the least increased through 30% AS or 30% FBS concentrations. HCEC viability at 10% and 15% AS was significantly worse （P=0.001, P=0.023） compared to baseline and significantly better at 15% FBS （P=0.003） concentrations. HCEC migration was significantly worse （P〈0.007） and HCEC proliferation significantly better （P〈0.001） in all concentration groups compared to baseline. CONCLUSION： For the best viability of HCEC 30% AS or 15% FBS, for HCEC migration 30% AS or 30% FBS, for proliferation 15% AS or 5% FBS should be used. Therefore, we suggest the use of 30% AS in clinical practice.

Influence of Increasing Shares of Miscanthus on Physical and Mechanical Properties of Pellets Produced in an Industrial Softwood Pellets Plant

It has been possible to pelletize wood Miscanthus mixtures （12.5%, 25% and 50%） without modifying production process settings of a softwood pellets plant. Pure Miscanthus material tested in the same conditions has led to unstable production, mainly explained by hammermill overfeeding. The unstable production has been identified as the main responsible factor of the low quality of pellets produced with pure Miscanthus for these trials. The produced pellets were tested in a 25 kW boiler and compared with agro-pellets of various origins： winter barley straw, rapeseed straw, reed, old hay, Miscanthus, and wood pellets. Results gained during these combustion trials allowed to describe combustion of these uncommon fuels and to characterize its emissions. The trials confirm general characteristics of agricultural fuel combustion： influence of the ash content on the gross calorific value, influence of the N content in the fuel on the NOx emissions, influence on the alkali content on the C12 emissions. But the influence of known parameter （S. alkali and earth alkali content） on SO2 emission might not be highlighted, while a relation with the fuel moisture content could be considered.

Using immune cell-based bioactivity assays to compare the inflammatory activities of oil sands process-affected waters from a pilot scale demonstration pit lake

In this study,we provide evidence that oil sands process-affected waters(OSPW)contain factors that activate the antimicrobial and proinflammatory responses of immune ***,using the murine macrophage RAW 264.7 cell line,we establish the bioactivity of two different OSPW samples and their isolated ***,we directly compared the bioactivity of two pilot scale demonstration pit lake(DPL)water samples,which included expressed water from treated tailings(termed the before water capping sample;BWC)as well as an after water capping(AWC)sample consisting of a mixture of expressed water,precipitation,upland runoff,coagulated OSPW and added *** inflammatory(*** activating)bioactivity was associated with the AWC sample and its organic fraction(OF),whereas the BWC sample had reduced bioactivity that was primarily associated with its inorganic fraction(IF).Overall,these results indicate that at non-toxic exposure doses,the RAW 264.7 cell line serves as an acute,sensitive and reliable biosensor for the screening of inflammatory constituents within and among discrete OSPW samples.

Bariatric surgery induces alterations in effective connectivity between the orbitofrontal cortex and limbic regions in obese patients

Obese subjects show enhanced brain responses in motivation and reward neurocircuitry encompassing sensory and somatic integration-interception, motivation-reward(striatal), emotion, and memory processes, which attenuate frontal region activation during food cues. Bariatric surgery(BS) is the only reliable treatment for morbid obesity. Unfortunately, it is unknown how BS affects neurocircuitry after weight loss. We aimed to examine effects of BS on the basal activity of brain areas involved in reward and motivation processing, emotion, memory, and gut-brain interaction. We combined resting-state fMRI with amplitude of low-frequency fluctuation(ALFF) and Granger causality analysis(GCA) to assess interactions between regions within the frontal-mesolimbic circuitry in 16 obese subjects(OB) and 22 normal-weight(NW) *** OB group was studied at baseline and 1 month post BS. Comparisons between OB and NW, and preand post BS showed significant differences in ALFF in areas involved in drive(caudate, orbitofrontal cortex(OFC)), arousal(thalamus), and conditioning/memory(amygdala, hippocampus)(P < 0.05, FDR correction). GCA revealed that in the OB group, the OFC had greater connectivity to limbic regions(amygdala,hippocampus, and medial thalamus) and the caudate. Post BS, the connectivity of the OFC to limbic regions decreased, whereas the connectivity from the amygdala and hippocampus to the caudate and thalamus was enhanced, particularly in subjects with lower body mass index(BMI). OFC activation in the OB group was associated with BMI prior to surgery, and changes in OFC post surgery were associated with alterations in BMI. Overall, the functional connectivity of the OFC was significantly decreased. Landfill Siting Evaluation Using GIS and Multi-Criteria Decision-Making Method: A Case Study: Dammam Municipal Solid Waste Landfill

Waste landfills are a prominent global issue that does not concern a country without the other because of its long-term effects especially in the environmental aspect. As these pose an environmental threat to water, air and soil;and directly affect human life and health. Accordingly, this research uses the integration of Geographic Information Systems (GIS) and Multi-Criteria Decision-Making (MCDM) approaches to assess the location of the municipal solid waste dump in Dammam, which was established in (2000AD) twenty years ago, due to the lack of recent studies assessing the current status of the landfill and whether it is in a suitable location and keeps pace with the population and urbanization increase in the region. For this purpose, the researchers are using the tools and techniques of GIS to evaluate eight criteria on the study area. The required standards were directed to the landfill site and transformed into maps that represent the most important economic, social, environmental, geological, geomorphological and Public Acceptance standards. These standards have been processed using the tools of spatial and statistical analyses. Analytic Hierarchy Process (AHP) is implemented in geographical information system for building the suitable cartographic model, and thus allowed to determine the suitable degree of Dammam municipal solid waste landfill site in light of the study criteria. The study concluded that the current landfill site achieves a good degree of suitability, as it achieved 63% measured against the quality standards set by the study, and that the current landfill site conforms to most of the criteria in this study to a good degree. But it violates two standards, which are distance from wells standard and distance between landfill and the center of waste generation standard.

To screen clinically relevant microRNAs (miRNAs) silenced by DNA methylation in human hepatocellular carcinoma (HCC).METHODSKnockdown of DNA methyltransferases (DNMTs) using siRNAs and miRNA profiling in HCC cell lines were performed to identify DNA hypermethylation-mediated miRNA downregulation. Confirmation using individual quantitative real-time PCR (qRT-PCR) assays was then performed followed by DNA methylation quantification at the promoter of the miRNA genes. Quantification of DNA methylation and miRNA expression was then performed in primary HCC tumor samples and related with clinicopathological *** profiling after DNMT knockdown in HCC cell lines revealed upregulation of miR-23, miR-25 and miR-183. After qRT-PCR confirmation and CpG island methylation quantification of these miRNAs in cell lines, further analysis in primary HCC specimens showed that hsa-miR-183 is hypermethylated in 30% of HCC (n = 40). Expression of mature miR-183 showed an inverse correlation with DNA methylation levels. In HCC cells, DNMT knockdown and 5-aza-2’-deoxycytidine treatment reduced methylation and stimulated expression of miR-183. In HCC patients, hypermethylation at hsa-miR-183 promoter significantly correlates with poor survival (log-rank test P = 0.03). DNA methylation analysis in healthy liver, benign liver tumors (hepatocellular adenoma and focal nodular hyperplasia) and their corresponding adjacent tissues showed absence of hypermethylation supporting the notion that aberrant methylation at hsa-miR-183 is specific for the malignant transformation of *** data indicate that hypermethylation of hsa-miR-183 is a frequent event in HCC and potentially useful as a novel surrogate diagnostic and prognostic marker.
