Objective:To isolate the phytoconstituents from the methanolic extracts of the stem bark of Pongamia glabra and Ficus glomerata,characterize spectroscopically and screen for in vitro immunomodulatory activity on human ***:A flavonoid(PGF)and an alkaloidal compound(PGA)from the extract of *** and a steroidal compound(FGS)and tannin fraction(FGT)from the extract of *** were isolated using column chromatography technique and were subjected for the spectroscopic(FT-IR,1HNMR and LC-MS)and TLC studies to identify the *** isolated compounds were screened for in vitro immunomodulatory activity on human neutrophils using nitroblue tetrazolium(NBT)dye test,phagocytosis of Candida albicans and neutrophil locomotion and chemotaxis assay at the concentration range of 100,50,25,12.50 and 5.00μg/***:From the spectroscopic and TLC studies data,the isolated compounds were identified as glabrin(PGA),karanjin(PGF),β-sitosterol(FGS),and tannin fraction(FGT).The isolated compounds PGA,PGF,FGS,and FGT exhibited significant(P<0.05)in vitro immunomodulatory activity in all the parameters ***:The steroidal compound,*** was found to be more immunopotent than all constituents alkaloid,flavonoid and ***,these constituents could be attributed to the immunomodulatory property of the plants.

A Double Tension Band Technique for Patellar Fractures—A New Concept

Amongst the many known techniques used for the management of fractures of the patella, the tension band wire fixation is a reliable treatment modality and has given consistently good results. In transverse patellar fractures, a combination of cerclage wiring and tension band fixation gives a good mechanical stability and an environment conducive to healing. The fracture described herein is classified as 45-C1 (based on Orthopaedic Trauma Association classification) of the right knee of a forty three years old male patient. It was fixed with three vertical 1.5 mm K wires and double cerclage wire looped proximally through the quadriceps tendon and distally through the patellar ligament in a double figure-of-eight configuration. The average follow-up period was 7 months. The Activity of Daily Living Scale (ADLS) of the Knee Outcome Survey was used to assess symptoms and functional capability of the knee. The fracture union was achieved at an average of 10 weeks. Full range of knee motion was achieved by end of the third postoperative month. No complications, such as infection and implant failure were noted. The technique aims to stress the advantage of giving a balanced fixation addressing the bony elements (the fractured ends) as well as the soft tissue (the extensor apparatus).

The overall purpose of performing measurements in science is to increase knowledge and document about some physical quantity. The measurand has a true value but, except for straightforward counting situations, the true value can never be fully determined. When a quantity is measured, the outcome depends on the measuring system, the measurement procedure, the skill of the operator, the environment and other effects. In practice the uncertainty on the result may arise from many possible sources, including examples such as incomplete definition, sampling, matrix effects and interferences, environmental couditions, uncertainties of weights and volumetric equipment, reference wtlues, approximations and assump- tions incorporated in the measurement method and procedure, and random wlriation. The objective of the paper is how to identify the sources of uncertainty in measurement and their importance on the result.

Umbilical granuloma is a very common cause of umbilical discharge. It is managed by chemical cauterization or simple thread ligation. However, it can be a differential diagnosis of patent vitello intestinal duct and this should be ruled out before managing such patients. We report a case of a 10-week-old male infant referred by his General Practitioner for silver nitrate cauterisation, with a diagnosis of suspected umbilical granuloma (UG). The child underwent subsequent exploratory laparotomy and bowel anastomosis.

Cytogenetic abnormalities are the hallmark of leukemias. We report here two cases of unusual cytogenetic abnormalities associated with Philadelphia chromosome, one with mixed phenotypic acute leukemia showing monosomy 7 and t(9;22)(q34;q11.2) and the other with chronic myeloid leukemia and additional translocation involving chromosomes 10 and 13. Both patients achieved complete remission following imatinib based treatment.

We present a case of extracranial internal carotid artery (ICA) aneurysm, which presented as an inflammatory submandibular swelling in the upper part of the right side of the neck. The lack of frank pulsatility and signs of inflammation though was a bit confusing, the Doppler and CT angiogram clinched the diagnosis. We were able to surgically resect and reform the ICA using the native vessel itself, which is an unusual technique, which we thought was worth presenting.
