Hepatocellular carcinoma incidence post direct-acting antivirals in hepatitis C-related advanced fibrosis/cirrhosis patients in Australia

Background:Despite efficacy in HCV eradication,direct-acting antiviral(DAA)therapy has raised controversies around their impact on hepatocellular carcinoma(HCC)*** we reported the first Australian data on HCC incidence in DAA-treated HCV patients with advanced fibrosis/***:We conducted a retrospective single center study of DAA-treated HCV patients with advanced fibrosis/cirrhosis from April 2015 to December *** with prior HCC were included if they had complete response to HCC ***:Among 138 patients who completed DAA therapy,133(96.4%)achieved sustained virologic response(median follow-up 23.8 months).Ten had prior HCC and 5/10(50.0%)developed recurrence,while de novo HCC developed in 7/128(5.5%).Median time from DAA to HCC diagnosis was 34 weeks in recurrent HCC *** novo 52 weeks(p=0.159).In patients with prior HCC,those with recurrence(***)had shorter median time between last HCC treatment and DAA(12 vs.164 weeks,pponse at 12 weeks(SVR12)(p=0.011),platelets(p=0.005),model for end-stage liver disease(MELD)score(p=0.029),alpha fetoprotein(AFp)(p=0.013),and prior HCC(pprior HCC(OR=4.80;95%CI:1.47–48.50;p=0.010),failed SVR12(OR=2.83;95%CI:1.71–16.30;p=0.016)and platelets(OR=0.97;95%CI:0.95–0.99;p=0.009).Conclusions:Our study demonstrates a high incidence of recurrent HCC in HCV patients with advanced fibrosis/cirrhosis treated with *** associated with HCC development post-DAA were more advanced liver disease,failed SVR12 and prior HCC,with higher rates of recurrence in those who started DAA earlier.

Unmet supportive care needs in families of children with chronic health conditions: an Australian cross‑sectional study

Background The aim of this study was to identify similarities and diferences in the unmet supportive care needs(USCN)of families of children with major chronic health conditions(CHCs)using a universal need assessment *** A cross-sectional online survey involving parents of children with congenital heart disease(CHD),type 1 diabetes mellitus(T1D),cancer,and asthma diagnosed within the last 5 years recruited via social media and support ***-four items assessing the USCN across six domains(care needs,physical and social needs,informational needs,support needs,fnancial needs,child-related emotional needs)were responded to on a 4-point Likert scale[no need(1)to high need(4)].Descriptive statistics identifed the level of need,and linear regressions identifed factors associated with higher need domain *** to small numbers,the asthma group was excluded from comparisons across *** One hundred and ninety-four parents completed the survey(CHD:n=97,T1D:n=50,cancer:n=39,and asthma:n=8).parents of children with cancer were most likely to report at least one USCN(92%),followed by parents of children with T1D(62%).The fve most commonly reported USCN across CHCs were drawn from four domains:child-related emotional,support,care,and *** need items were included in the top fve needs for all conditions.A higher USCN was associated with a greater frequency of hospital visits and the absence of parental *** Using a universal need assessment tool,this is one of the frst studies to characterize USCN in families of children diagnosed with common *** proportions endorsing diferent needs varied across conditions,the most endorsed needs were similar across the illness *** suggests that support programs or services could be shared across diferent CHCs.

Neurotoxicity and transcriptome changes in embryonic zebrafish induced by halobenzoquinone exposure

Halobenzoquinones(HBQs) are emerging disinfection byproducts(DBps) with a widespread presence in drinking water that exhibit much higher cytotoxicity than regulated DBps. However, the developmental neurotoxicity of HBQs has not been studied in vivo. In this work, we studied the neurotoxicity of HBQs on zebrafish embryos, after exposure to varying concentrations(0-8 μmol/L) of three HBQs, 2,5-dichloro-1,4-benzoquinone(2,5-DCBQ), 2,6-dichloro-1,4-benzoquinone(2,6-DCBQ), and 2,5-dibromo-1,4-benzoquinone(2,5-DBBQ) for 4 to 120 hr post fertilization(hpf). HBQ exposure significantly decreased the locomotor activity of larvae, accompanied by significant reduction of neurotransmitters(dopamine and γ-aminobutyric acid) and acetylcholinesterase activity. Furthermore, the expression of genes involved in neuronal morphogenesis( gfap, α1-tubulin, mbp, and syn-2 α) were downregulated by 4.4-, 5.2-, 3.0-, and 4.5-fold in the 5 μmol/L 2,5-DCBQ group and 2.0-, 1.6-, 2.1-, and 2.3-fold in the 5 μmol/L 2,5-DBBQ group, respectively. Transcriptomic analysis revealed that HBQ exposure affected the signaling pathways of neural development. This study demonstrates the significant neurotoxicity of HBQs in embryonic zebrafish and provides molecular evidence for understanding the potential mechanisms of HBQ neurotoxicity.

AIM: To assess whether the polymorphisms of NOD2/ CARD15 , autophagy-related 16-like 1 (ATG16L1 ), and interleukin-23 receptor (IL23R ) genes play a more critical role in the susceptibility of childhood-onset than in adult-onset Crohn’s disease (CD). METHODS: polymorphisms R702W, G908R, and 3020insC of NOD2/CARD15 ; rs2241880 A/G of ATG16L1 , and rs11209026 (R381Q) of IL23R gene were assessed in 110 childhood-onset CD, 364 adult-onset CD, and 539 healthy individuals. Analysis of polymorphisms R702W, G908R, and 3020insC of NOD2/CARD15 genotyping was performed by allele specific polymerase chain reaction (pCR) or by pCR-restriction fragment length polymor-phism analysis. The polymorphisms rs2241880 A/G of the ATG16L1 , and rs11209026 (R381Q) of the IL23R gene in the children’s cohort were genotyped by pCR and melting curve analysis whereas adult group genotyping was performed using the Affymetrix Genome-Wide Human SNp Array 5.0 (500K). RESULTS: The 3020insC allele in NOD2/CARD15 was significantly higher in childhood than in adult-onset CD (p = 0.0067). Association with at least 1 NOD2/CARD15 variant was specific for ileal disease (with or without co- lonic involvement). Even if the frequency of G allele of the rs2241880 ATG16L1 polymorphism was increased in both paediatric and adult CD patients compared to con- trols (p = 0.017 and p = 0.001, respectively), no difference was observed between the childhood and the adult cohort. The rare Q allele of IL23R rs11209026 polymorphism was underrepresented in both paediatric and adult CD cases (p = 0.0018 and p = 0.04, respectively) and no difference was observed between the childhood and the adult cohort. The presence of the rs2241880 ATG16L1 and rs11209026 IL23R polymorphisms did not influence disease phenotype. CONCLUSION: polymorphism 3020insC in NOD2/ CARD15 occurs statistically significantly more often in patients with childhood-onset CD than in patients with adult-onset CD. The liver is an organ in which antigen-specific T-cell responses manifest a bias toward immune tolerance. This is clearly seen in the rejection of allogeneic liver transplants, and multiple other phenomena suggest that this effect is more general. These include tolerance toward antigens introduced via the portal vein, immune failure to several hepatotropic viruses, the lack of natural liverstage immunity to malaria parasites, and the frequent metastasis of cancers to the liver. Here we review the mechanisms by which T cells engage with hepatocellular antigens, the context in which such encounters occur, and the mechanisms that act to suppress a full T-cell response. While many mechanisms play a role, we will argue that two important processes are the constraints on the cross-presentation of hepatocellular antigens, and the induction of negative feedback inhibition driven by interferons. The constant exposure of the liver to microbial products from the intestine may drive innate immunity, rendering the local environment unfavorable for specific T-cell responses through this mechanism. Nevertheless, tolerance toward hepatocellular antigens is not monolithic and under specific circumstances allows both effective immunity and immunopathology.

Effects of Exhaust Gas Recirculation on the Homogeneous Charge Combustion process of n-Heptane at Different Load Conditions

Effects of exhaust gas recirculation （EGR） on homogeneous charge combustion of n-heptane was studied through simulation and experiment. Experiments were carried out in a single cylinder, four-stroke, air cooled engine and a single cylinder, two-stroke, water cooled engine. In the four-stroke engine, experiments of the effects of EGR were examined using heated N2 addition as a surrogate for external EGR and modifying engine to increase internal EGR. The ignition timing was sensitive to EGR due to thermal and chemical effects. EGR or extra air is a key factor in eliminating knock during mid-load conditions. For higher load operation the only way to avoid knock is to control reaction timing through the use of spark ignition. Experimental and modeling results from the two-stroke engine show that auto-ignition can be avoided by increasing the engine speed. The two-stroke engine experiments indicate that high levels of internal EGR can enable spark ignition at lean conditions. At higher load conditions, increasing the engine speed is an effective method to control transition from homogeneous charge compression ignition （HCCI） operation to non-HCCI operation and successful spark ignition of a highly dilute mixture can avoid serious knock.
