Improving Confidence and Practical Skills in Plastic Surgery for Medical Students and Junior Doctors: A One-Day Session

Objectives: The current medical school curriculum offers scant exposure to plastic surgery. Medical students and junior doctors are often inadequately prepared for in-hospital placements which may impact on patient safety and quality of care. We aimed to deliver a one-day “hands-on” practical session on basic surgical skills and principles in plastic surgery, and to assess its effectiveness at improving confidence to the specialty among junior trainees. Methods: The one-day practical session covered knot tying, suturing, LA administration, skin lesion excision and local flaps, with short concurrent lectures on principles of plastic surgery. Self-assessment confidence scores were recorded before and after the course on seven domains: a) knowledge: wound assessment and management, suture selection, and concepts of flaps;b) practical skills: Handling of surgical instrument, suturing, LA administration, and skin lesion excision. Results: Thirty-five participants attended: Six core surgical trainees, fifteen foundation doctors, and fourteen medical students. The overall course satisfaction was 9.13/10. Self-assessment confidence scores in all domains significantly improved after the course (p < 0.0001). Suturing (58.6%) and skin lesion excision (74.5%) demonstrated the largest improvement. Qualitative feedback was extremely positive. Conclusions: The hands-on nature of the course offered participants great opportunities to acquire the necessary confidence and practical skills required for in-hospital placements. It is vital for students and doctors interested in a career in plastic surgery to prepare adequately for this diverse specialty, which is covered in limited depth in medical school. The need for more practical skills courses on plastic surgery for this demographic is intuitive.

A photochemistry coupled computational fluid dynamics （CFD） based numerical model has been developed to model the reactive pollutant dispersion within urban street canyons, particularly integrating the interrelationship among diurnal heating scenario （solar radiation affections in nighttime, daytime, and sun-rise/set）, wind speed, building aspect ratio （building-height-to-street-width）, and dispersion of reactive gases, specifically nitric oxide （NO）, nitrogen dioxide （NO2） and ozone （O3） such that a higher standard of air quality in metropolitan cities can be achieved. Validation has been done with both experimental and numerical results on flow and temperature fields in a street canyon with bottom heating, which justifies the accuracy of the current model. The model was applied to idealized street canyons of different aspect ratios from 0.5 to 8 with two different ambient wind speeds under different diurnal heating scenarios to estimate the influences of different aforementioned parameters on the chemical evolution of NO, NO2 and 03. Detailed analyses of vertical profiles of pollutant concentrations showed that different diurnal heating scenarios could substantially affect the reactive gases exchange between the street canyon and air aloft, followed by respective dispersion and reaction. Higher building aspect ratio and stronger ambient wind speed were revealed to be, in general, responsible for enhanced entrainment of 03 concentrations into the street canyons along windward walls under all diurnal heating scenarios. Comparatively, particular attention can be paid on the windward wall heating and nighttime uniform surface heating scenarios.

Centenarians,as excellent examples of extreme yet successful aging,not only display a much longer lifespan but can also delay or even escape major agerelated diseases,such as cardiovascular disorders,neurodegenerative diseases,and cancer [1].Studies have suggested that genetic factors account for 20%-30%of human longevity [2];however,only a handful of longevity-associated genes/mutations have been observed in long-lived individuals,even with the use of genome-wide scanning/sequencing assays [3,4].In contrast,evidence has shown the crucial roles of epigenetic alternations in aging and age-related diseases [5],suggesting that epigenetic modifications play central roles in human healthy aging.Thus,a comprehensive analysis of the epigenomic landscape of centenarians is urgently needed.

Background The neurogenesis in retina of adult mammals is generally abolished, and this renders the retina lack of regenerative capacity. Despite this, there is a small population of nestin-positive cells in the ciliary epithelium which retains neurogenic potential. The present study aimed at investigating the effect of two drugs, corticosterone and paroxetine, on the cell proliferation of the ciliary body. Methods Adult Sprague-Dawley rats were given vehicle, corticosterone, paroxetine, or both corticosterone and paroxetine treatment for 14 days. Cell proliferation in the ciliary body was quantified using 5-bromo-2-deoxyuridine （BrdU） immunohistochemistry. Co-labelling of BrdU and stem cell marker was used to phenotype the BrdU immunoreactive cells. Results Corticosterone treatment suppressed while paroxetine treatment increased the cell proliferation of the ciliary body. Co-labelling with cell markers revealed that the BrdU positive cells also showed nestin expression but not glial fJbrillary acidic protein （GFAP）. Conclusions The results illustrate that proliferation of retinal progenitor cells situated in ciliary body are subjected to regulation by selective serotonin reuptake Jnhibitors （SSRI） and corticosteroJd, which is similar to our previous findings in neurogenic regions in central nervous system （CNS）. Paroxetine treatment could reverse the suppressive effect of corticosterone on ciliary body cell proliferation. This provides information for future investigation of retinal stem cell biology and potential treatment of retinal degenerative diseases.

AIM: Elevation of alanine aminotransferase (ALT) level is commonly seen among patients suffering from severe acute respiratory syndrome (SARS). We report the progression and clinical significance of liver derangement in a large cohort of SARS patient.METHODS: Serial assay of serum ALT was followed in patients who fulfilled the WHO criteria of SARS. Those with elevated ALT were compared with those with normal liver functions for clinical outcome. Serology for hepatitis B virus (HBV) infection was checked. Adverse outcomes were defined as oxygen desaturation, need of intensive care unit (ICU) and mechanical ventilation and death.RESULTS: Two hundred and ninety-four patients were included in this study. Seventy (24%) patients had elevated serum ALT on admission and 204 (69%) patients had elevated ALT during the subsequent course of illness. Using peak ALT >5×ULN as a cut-off and after adjusting for potential confounding factors, the odds ratio of peak ALT>5x ULN for oxygen desaturation was 3.24 (95%CI 1.23-8.59, P = 0.018), ICU care was 3.70 (95% CI 1.38-9.89, P = 0.009), mechanical ventilation was 6.64 (95%CI 2.22-19.81, P = 0.001) and death was 7.34(95%CI 2.28-24.89, P = 0.001). Ninety-three percent of the survived patients had ALT levels normalized or were on the improving trend during follow-up. Chronic hepatitis B was not associated with worse clinical outcomes.CONCLUSION: Reactive hepatitis is a common complication of SARS-coronavirus infection. Those patients with severe hepatitis had worse clinical outcome.

Background:Despite an increasing number of surveys and a growing interest in birdwatching,the population and distribution of Asian Dowitcher(Limnodromus semipalmatus),a species endemic to the East Asian-Australasian and Central Asian Flyways,remains poorly understood,and published information about the species is largely outdated.In boreal spring 2019,over 22,432 Asian Dowitchers were recorded in a coastal wetland at Lianyungang,Jiangsu Prov-ince,China,constituting 97.5%of its estimated global population.Methods:In 2019 and 2020,we conducted field surveys at Lianyungang to determine the numbers of Asian Dow-itchers using the area during both southward and northward migrations.We also assessed the distribution and abun-dance of Asian Dowitchers elsewhere along the China coast by searching literature and consulting expert opinion.Results:The coastal wetlands of Lianyungang are the most important stopover site for Asian Dowitchers during both northward and southward migrations;they supported over 90%of the estimated global population during north-ward migration in two consecutive years(May 2019 and 2020).This area also supported at least 15.83%and 28.42%(or 30.74%and 53.51%using modelled estimates)of the global population during southward migration in 2019 and 2020 respectively.Coastal wetlands in the west and north of Bohai Bay also have been important stopover sites for the species since the 1990s.Although comprehensive,long-term monitoring data are lacking,available evidence sug-gests that the population of the species may have declined.Conclusions:The high concentration of Asian Dowitchers at Lianyungang during migration means the species is highly susceptible to human disturbances and natural stochastic events.The coastal wetlands of Lianyungang should be protected and potentially qualify for inclusion in China’s forthcoming nomination for World Heritage listing of Migratory Bird Sanctuaries along the Coast of Yellow Sea-Bohai Gulf of China(Phase II)in 2023.Additional research is needed to understand Asian Dowi

In 2018,the STAR collaboration collected data from^(96)_(44)Ru+^(96)_(44)Ru and^(96)_(40)Zr+^(96)_(40)Zr at√^(S)NN=200 Ge V to search for the presence of the chiral magnetic effect in collisions of nuclei.The isobar collision species alternated frequently between 9644 Ru+^(96)_(44)Ru and^(96)_(40)Zr+^(96)_(40)Zr.In order to conduct blind analyses of studies related to the chiral magnetic effect in these isobar data,STAR developed a three-step blind analysis procedure.Analysts are initially provided a"reference sample"of data,comprised of a mix of events from the two species,the order of which respects time-dependent changes in run conditions.After tuning analysis codes and performing time-dependent quality assurance on the reference sample,analysts are provided a species-blind sample suitable for calculating efficiencies and corrections for individual≈30-min data-taking runs.For this sample,species-specific information is disguised,but individual output files contain data from a single isobar species.Only run-by-run corrections and code alteration subsequent to these corrections are allowed at this stage.Following these modifications,the"frozen"code is passed over the fully un-blind data,completing the blind analysis.As a check of the feasibility of the blind analysis procedure,analysts completed a"mock data challenge,"analyzing data from Au+Au collisions at√^(S)NN=27 Ge V,collected in 2018.The Au+Au data were prepared in the same manner intended for the isobar blind data.The details of the blind analysis procedure and results from the mock data challenge are presented.

High transverse momentum(pT)particle production is suppressed owing to the parton(jet)energy loss in the hot dense medium created in relativistic heavy-ion collisions.Redistribution of energy at low-to-modest pT has been difficult to measure,owing to large anisotropic backgrounds.We report a data-driven method for background evaluation and subtraction,exploiting the away-side pseudorapidity gaps,to measure the jetlike correlation shape in Au+Au collisions at √sNN=200 GeV in the STAR experiment.The correlation shapes,for trigger particles pT>3GeV/c and various associated particle pT ranges within 0.5
