Sensory Disturbance as a Characteristic Symptoms of Methylmercury Poisoning in Niigata, Japan

Fifty years have passed from the first official recognition of the methylmercury （MeHg） poisoning in Niigata （so called the second Minamata disease）, Japan in 2015. The first case of MeHg poisoning was found in January 1965, when two surveys were carried out for 20,000 inhabitants lived within 15 km from the mouth of the Agano river. From these surveys, 26 cases （including 5 cases of death） were officially recognized as MeHg poisoning. Moreover, 9 cases who had no symptoms but whose hair mercury concentration were over 200 ppm were found. Within 20 cases, 15 cases are among 26 cases officially recognized as having MeHg poisoning and 5 cases are among 9 cases who had no symptoms but whose hair mercury concentration were over 200 ppm in 1965. The distal dominant sensory disturbances of the extremities lasted for more than several decades among 20 cases. The sensory disturbance of the bilateral distal extremities could be used as the gold standard for the diagnosis of MeHg poisoning in the exposed areas. The distal dominant sensory disturbance of the extremities is a typical characteristic and important sign of the MeHg poisoning.

Introduction: To investigate a long-term in vivo deterioration of polymethylmethacrylate (PMMA) bone cement over time, we evaluated retrieved PMMA cement in terms of chemical elements presenting in the cement using energy dispersive analysis of X-rays;Knoop hardness;and the Young’s modulus using scanning acoustic microscopy. Materials and Methods: For mechanical evaluation, we could neglect the influences of entrapped air bubbles or blood by the use of small specimens. The study was based on thirteen cement samples (six used in the acetabulum and seven in the femur) derived from eight patients (age at revision surgery: mean 72.5, range 68 to 79). All of these samples were Simplex-P?cement. They were functioning well at least ten years after the previous surgery. Duration until revision surgery was ranged 12 to 25 years (average, 17.4 years). The reason for revision was aseptic mechanical loosening. Twenty samples of Simplex-Preg;cement were served by manually mixing as a control. Results: The average of the hardness of the cement was 17.0 ± 1.2 (range, 13.4-20.6). In the control, the hardness was 17.8 ± 1.5 (range, 14.0-24.6). There was no significant difference between these values. The mean of Young’s modulus of the cement was 5.61 ± 0.19 GPa (range, 5.09-6.10). In the control, the modulus was 6.04 ± 0.13 GPa (range, 5.68-6.45). Although the modulus was significantly less than that of the control, there was only 7% decrease in average between twelve and twenty-five years in vivo. Conclusions: Our results suggest that long-term implantation and functional loading in vivo may not be the limiting factor in the mechanical integrity of the bone cement.

Solar Influences on the North Atlantic Oscillation by Wavelet-Based Multifractal Analysis

There is increasing interest in the relation between the solar activity and climate change. Regarding the solar activity, the fractal property of the sunspot number (SSN) has been studied by many previous works. In general, fractal properties have been observed in the time series of the dynamics of complex systems. The purpose of this research is to investigate the relationship between the solar activity, total ozone, and the North Atlantic Oscillation (NAO) from a viewpoint of multi-fractality. To detect the changes of multifractality, we performed the wavelets analysis, and plotted the τ-function derived from the wavelets of these time series. We showed that the solar activity relate to the NAO, by observing the matching in monofractality or multifractality of these indices. When the SSN increased and the solar activity was stable, the NAO also became stable. When the SSN became maximum, the fractality of the SSN, F10.7 flux, geomagnetic aa, and NAO indices changed from multifractality to monofractality and those states became stable for most of the solar cycles. When the SSN became maximum, the fluctuations became large and multifractality became strong, and a change from multifractal to monofractal behavior was observed in the SSN, F10.7 flux, geomagnetic aa, and NAO indices. The strong interactions of the solar flux, geomagnetic activity, total ozone, and NAO occur in the SSN maximum. The strong interactions were inferred from the similarity of fractality changes and the wavelet coherence. The influence of the solar activity on the NAO was shown from a viewpoint of multi-fractality. These findings will contribute to the research on the effects of the solar activity on climate change.

Flood Discharge Changes by Urbanization from Farmland Based on the Unit Flood Concept at the Kurabe River, Japan

Flood runoff models of urbanization from farmland based on the physical characteristics of a basin have been minimally used in previous research until today. Consequently, the runoff analysis has not been performed that is based on physical basis. Therefore, this research undertook flood discharge analysis from urbanization using the unit flood discharge concept that is enhanced the previous research. The study area was selected at the Kurabe River basin, which is 17.5 km2 in area having a very steep landscape. Twenty-one rainfall events at 10-minute intervals were selected, and five urbanized years were tested. From 1976 to 2009 during 35 years, the flood discharge increased approximately 2.0 times, in which residential areas increased from 23% to 48%;the maximum specific discharge was 21.7 m3·s-1·km-2 in a some block, which is a remarkably large amount. Furthermore, following issues investigated: changes in the hydrograph were associated with urbanization, the effect of a small reservoir aiming to cut down the peak discharge and the relationship between the unit discharge, and the relationship between our method and the discharge estimated by a “Rational Formula”. In particular, the effect of the small reservoir for flood control was found to be remarkably efficient. Finally, the validity of our method was confirmed at the study area in the observed discharge. This result is very useful for estimating runoff discharge changes by urbanization from farmland.

Analyses of Incidents of Institutional Food Service Practice Management for One Year in the Kitchen of a General Hospital, Chubu Region, Japan

This study aimed to assess the incidence of institutional food service practice management errors in the kitchen of a general hospital, Chubu region, Japan. Clarifying the cause of incidents of institutional food service operation can reduce institutional food service practice management errors. This is the first study to analyze incidents at a general hospital kitchen in Japan in English. Data were collected from incident reports over the course of one year, between October 1, 2011 and November 30, 2012. Forty two reports of incidents whose locations were the kitchen were analyzed, for example, incident dates, incident times, workloads, incident contents, incident causes, accident levels, and the presence or absence of negligence. The frequency of incidents for months, for each third of month, for day of week, for time of day, for workload levels of staff member, and for types of incidents were analyzed using chi-square test following Ryan multiple comparison. The incidents were more likely to occur at the start of the business year, during the 11：00 to 12：59 time blocks, and when staffs were busy. Breakdown of incident contents of＂foreign contamination （hair）＂ was 10 reports （24%）. To prevent incidents, new staff should also be trained to acquire skills to avoid making errors during food preparation, ensuring that there is a clear route for communicating changes in the number of hospitalized patients, and building a system that allows for changes to be made to the number of meals to be served and the content of those meals in an accurate and prompt fashion. Furthermore, work uniform should be changed promptly as such contamination can reportedly be improved by having workers wear a work cap.

The charge interaction and corresponding doping effect between single-walled carbonnanotubes (SWNTs) and various fullerene derivatives, namely, C60, phenyl-C61-butyricacid methyl ester (PC61BM), methano-indenefullerene (MIF), 10,100,40,400-tetrahydrodi[1,4]methanonaphthaleno[5,6]fullerene (ICBA), 1,4-bis(dimethylphenylsilylmethyl)[60]fullerene (SIMEF-1), and dimethyl(orthoanisyl) silylmethyl(dimethylphenylsilylmethyl)[60]fullerene (SIMEF-2), are investigated. A variety of analytical techniques,including field-effect transistors (FETs) made of horizontally aligned arrays ofSWNTs, is used as a means of investigation. Data from different measurements haveto be used to obtain a concrete evaluation for the fullerene-applied SWNTs. The datacollectively points toward the conclusion that fullerenes with high molecular orbitalenergy levels, namely, MIF, SIMEF-1, SIMEF-2, and PC61BM, induce p-type doping,while fullerenes with low molecular orbital energy levels, namely, ICBA and C60,induce n-type doping on the carbon nanotubes. Nevertheless, the SWNTs retained ptypecharacteristics because n-doping induced by the fullerenes are weak compared tothe p-doping of the water and oxygen on carbon nanotubes. This means that fullerenederivatives have the ability to fine-tune the energy levels of carbon nanotubes, whichcan play a crucial role in carbon nanotube-based electronics, such as solar cells, lightemittingdevices, and FETs.

Combined thoracoscopic and endoscopic surgery for a large esophageal schwannoma

A 47-year-old woman presented to our hospital with complaints of dysphagia. Esophagogastroduodenoscopy identified a submucosal tumor in the left wall of the esophagus that was diagnosed as a benign schwannoma on biopsy. Computed tomography revealed a tumor of length 60 mm in the thoracic esophagus, with its cranial edge at the level of the aortic arch. On endoscopy, a submucosal tunnel was created 40 mm proximal to the cranial edge of the tumor, and its oral end was dissected from the mucosal and muscular layers. This was followed by the resection of the entire tumor by left-sided thoracoscopy. The esophageal defect was closed in layer by continuous suture from the thoracic side. Endoscopic closure was achieved by using clips. No postoperative complications were observed. Oral diet was resumed from postoperative day 7 and the patient was discharged on postoperative day 9. This combined approach has not been described for similar tumors. Our experience demonstrated that large esophageal tumors can be safely excised with minimally invasive surgery by using a combination of thoracoscopy and endoscopy.

Common carotid arterial thrombosis associated with ulcerative colitis

A 26-year-old woman with ulcerative colitis was transferred to our hospital with left hemiparesis due to cerebral infarction. Cervical ultrasonography and magnetic resonance imaging angiography revealed thrombosis at the right common carotid artery and the right internal carotid artery. Antithrombotic and anticoagulant therapies were commenced. After about 2 wk of the treatment, the frequency of her diarrhea increased. She underwent emergency subtotal colectomy, but 10 d later an abundant hemorrhage from the remnant rectum occurred, so the remnant rectum was resected and an ileal pouch anal anastomosis was performed. Antithrombotic and anticoagulant therapies were continued, but neither her neurological status nor magnetic resonance imaging angiography findings showed subsequent changes. She was discharged 3 mon after operation. This is a rare case of common carotid arterial thrombosis occurring as a complication of ulcerative colitis, in which antithrombotic and anticoagulant therapies are considered to provoke a deterioration of the patient’s bowel disease.

CTNNB1 （or beta-catenin） is regarded as a central effecter in molecules of the wingless/Wnt signalling pathway, It is cadherin mediated cell to forms a complex with a key component of the cell adhesion system and the protein product of adenomatous polyposis coil （APC）, glycogen synthase kinase 3β （GSK3β） and conductin. One mutation of APC is also responsible for activation of wingless/Wnt signalling pathway and accumulation of free beta-catenin in the cell. Beta-catenin upregulates oncogenes, such as cyclin D1 and c-myc. Beta-catenin expression in cytoplasm and nuclei was reported to increase in many cases of intestinal tumorigenesis. In addition, the hyperexpression of integrin linked kinase （ILK） in colonic polyposis has been demonstrated.
