The past decade has seen an unprecedented increase in our understanding of the biology and etiology of head and neck squamous cell carcinomas(HNSCC).Genome-wide sequencing projects have identified a number of recurrently mutated genes,including unexpected alterations in the NOTCH pathway and chromatin related ***-expression profiling has identified 4 distinct genetic subtypes which show some parallels to lung squamous cell carcinoma *** identification of the human papilloma virus as one causative agent in a subset of oropharyngeal cancers and their association with a favorable prognosis has opened up avenues for new therapeutic *** expanding knowledge of the underlying molecular abnormalities in this once very poorly understood cancer should allow for increasingly rational clinical trial design and improved patient outcomes.

Improved mechanical, degradation, and biological performances of Zn–Fe alloys as bioresorbable implants

Zinc(Zn)is a promising bioresorbable implant material with more moderate degradation rate compared to magnesium(Mg)and iron(Fe).However,the low mechanical strength and localized degradation behavior of pure Zn limit its clinical *** is one of the most effective ways to overcome these *** screening the alloying element candidates regarding their potentials for improvement on the degradation and biocompatibility,we proposed Fe as the alloying element for Zn,and investigated the in vitro and in vivo performances of these alloys in both subcutaneous and femoral *** showed that the uniformly distributed secondary phase in Zn–Fe alloys significantly improved the mechanical property and facilitated uniform degradation,which thus enhanced their biocompatibility,especially the Zn-0.4Fe ***,these Zn–Fe alloys showed outstanding antibacterial *** together,Zn–Fe alloys could be promising can-didates as bioresorbable medical implants for various cardiovascular,wound closure,and orthopedic applications.

Chronic obstructive pulmonary disease is an inflammatory condition with increased risk of lung cancer. We hypothesized that curcumin/ bioperine (CB), which has anti-inflammatory effects, may reduce cytological abnormalities in the sputum of patients with COPD. We conducted a 3-month, three-to-one randomized, doubleblind, pilot trial of escalating doses of CB in patients with moderate or worse COPD who were capable of producing sputum. The primary efficacy endpoint was changed in sputum cytology. We also explored changes in fluorescence in situ hybridization (FISH). We obtained sputum samples for cytology and chromosome abnormalities at baseline and each monthly follow-up visit. We enrolled 57 participants, with 35 completing the study. The participants’ mean age (standard deviation [SD]) was 66.6 (8.2) years, and they were mainly male (91.2%), with an average of 63.8 pack-years of smoking history. Also, 42.1% of participants were active smokers and the mean (SD) FEV1 was 37% (13%). At baseline, 13 subjects had moderate or worse dysplasia (22.8%). Subjects with moderate to severe sputum dysplasia had more chromosome abnormalities in epithelial cells and neutrophils, as measured by deletion and aneuploidy in 10q22.3. The changes in sputum cytology and chromosome abnormalities did not differ between the active and placebo arms. CB was well tolerated at the bid doses of 1, 1.5, and 2 gm of curcumin and 5 mg of bioperine, with minor side effects related to the gastrointestinal tract. In this short pilot trial, CB compared to placebo did not alter cytological and chromosomal abnormalities seen in sputum of patients with COPD.

Texture analysis on parametric maps derived from dynamic contrast-enhanced magnetic resonance imaging in head and neck cancer

AIM: To investigate the merits of texture analysis on parametric maps derived from pharmacokinetic modeling with dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI) as imaging biomarkers for the prediction of treatment response in patients with head and neck squamous cell carcinoma(HNSCC). METHODS: In this retrospective study,19 HNSCC patients underwent pre- and intra-treatment DCEMRI scans at a 1.5T MRI scanner. All patients had chemo-radiation treatment. Pharmacokinetic modeling was performed on the acquired DCE-MRI images,generating maps of volume transfer rate(Ktrans) and volume fraction of the extravascular extracellular space(ve). Image texture analysis was then employed on maps of Ktrans and ve,generating two texture measures: Energy(E) and ***: No significant changes were found for the mean and standard deviation for Ktrans and ve between pre- and intra-treatment(P > 0.09). Texture analysis revealed that the imaging biomarker E of ve was significantly higher in intra-treatment scans,relative to pretreatment scans(P < 0.04). CONCLUSION: Chemo-radiation treatment in HNSCC significantly reduces the heterogeneity of tumors.

Dear Editor,Non-small cell lung carcinoma(NSCLC)accounts for more than 85%of all lung cancers[1].Lung adenocarcinomas(ADCs)arising from never-smokers have been frequently found to be associated with oncogenic driver mutations,including anaplastic lymphoma kinase(ALK)*** microtubule-associated protein-like 4(EML4)is the most common fusion partner of the ALK gene[2].Tyrosine kinase inhibitors(TKIs)are currently the first-line treatment for advanced EML4-ALKrearranged lung ADC.
