Does Anti-Vascular Endothelial Growth Factor Treatment Delay Photoreceptor Maturation in Preterm Infants?

Purpose: To investigate the effects of intravitreal anti-vascular endothelial growth factor (VEGF) for treatment of type-1 or aggressive posterior retinopathy of prematurity (ROP) on photoreceptor maturation in preterm infants using spectral domain optical coherence tomography (SD-OCT). Methods: A study group of 36 patients with ROP treated with anti-VEGF therapy was compared to a control group of 80 patients with ROP. All patients had undergone SD-OCT imaging at least once between 40 and 44 weeks post-menstrual age (PMA). Two imaging specialists evaluated the images for ellipsoid zone (EZ) development, cystoid macular edema (CME), sub-retinal fluid (SRF) at the fovea, and cone outer segment tips (COST). The outer nuclear layer thickness and the distance from the EZ to the central fovea was measured. Results: EZ development was observed in 18 (50%) patients in the study group and 68 (85%) in the control group (p = 0.002). CME was observed in 17 (47.2%) patients in the study group and 17 (21.2%) in the control group. The distance between the EZ and central fovea was 868 ± 440 μm in the study group and 725 ± 380 μm in the control group (p = 0.02). COST was not observed in any patient. Regression analysis indicated that anti-VEGF treatment and CME affect EZ development. Conclusions: Early photoreceptor maturation is correlated with preterm birth week but not ROP stage. Anti-VEGF treatment delays photoreceptor maturation in preterm infants.

目的:评估中心性浆液性脉络膜视网膜(CSCR)病变中盐皮质激素受体基因-2G/C单核苷酸多态性,以及基因多态性和血浆皮质醇水平的关系。方法:选取60例中心性浆液性脉络膜视网膜病变患者和50例正常人作为研究对象。患者皆患有急性中心性浆液性脉络膜视网膜病变,即浆液性视网膜脱离和视网膜色素上皮脱离或功能障碍(排除其它可能导致渗出的疾病,比如脉络膜新生血管、炎症或浸润病变)。为避免皮质醇水平的昼夜变化,上午8时到10时之间采集外周血样,检测盐皮质激素受体基因多态性(rs2070951)和血浆皮质醇水平。结果:CSCR组的基因型频率分布为G/C(46.6%),G/G(26.7%)和C/C(26.7%)。两组间基因型分布无统计学差异(P=0.96)。研究结果显示,CSCR组的血浆皮质醇水平为401.2±162.1 nmol/L,对照组为296.8±130.1nmol/L,两组间差异有统计学意义(P<0.01)。血浆皮质醇水平在G/C(345.0±137.0 nmol/L),G/G(369.2±165.3 nmol/L)和C/C(395.3±188.8 nmol/L)基因型之间不存在差异(P=0.50)。结论:盐皮质激素受体基因多态性与中心性浆液性脉络膜视网膜病变和血浆皮质醇水平无关。

Background：Obstructive sleep apnea syndrome （OSAS） is a disease with increasing prevalence,which is mainly characterized by increased cardiopulmonary mortality and *** is well-known that OSAS patients have increased prevalence of cardiovascular diseases including coronary heart disease,heart failure,and *** aim of this study was to evaluate the presence of prolonged and fragmented QRS complexes,which have previously been associated with cardiovascular mortality,in OSAS ***：Our study included 51 patients （mean age 41.6 ± 10.1 years） who were recently diagnosed with OSAS （apnea-hypopnea index [AHI] ≥5 events/h） and never received *** control group consisted of 34 volunteers （mean age 43.1 ± 11.6 years） in whom OSAS was excluded （AHI 〈5 events/h）.The longest QRS complexes was measured in the 12-lead electrocardiogram （ECG） and the presence of fragmentation in QRS complexes was ***：Fragmented QRS frequency was significantly higher in patients with OSAS （n =31 [61%] vs.n =12 [35%],P =0.021）.QRS and QTc durations were also significantly longer in OSAS patients than controls （99.8 ± 13.9 ms vs.84.7 ± 14.3 ms,P 〈 0.001;411.4 ± 26.9 ms vs.390.1 ± 32.2 ms,P =0.001,respectively）.Analysis of the patient and controls groups combined revealed a weak-moderate correlation between AHI and QRS duration （r =0.292,P =0.070）.OSAS group had no correlation between AHI and QRS duration （r =-0.231,P =0.203）.Conclusions：In our study fragmented QRS frequency and QRS duration were found to increase in OSAS *** parameters are related with increased cardiovascular *** the prognostic importance of ECG parameters,it may be reasonable to recommend more detailed evaluation of OSAS patients with fragmented or prolonged QRS complexes with respect to presence of cardiovascular diseases.

A novel mutation of SGK-1 gene in central serous chorioretinopathy

AIM: To investigate the association of serum glucocorticoid kinase gene-1(SGK-1) DNA variants with chronic central serous chorioretinopathy(CSC).METHODS: We enrolled 32 eyes of 32 patients who were diagnosed with chronic CSC and composed 32 normal eyes as a control group. Peripheral blood was used for DNA extraction and polymerase chain reaction amplification. SGK1 gene was sequenced by using Big Dye Terminator v3.1 cycle sequencing Kit(Applied Biosystems, Foster City, CA, USA). The SGK-1 gene and its variants were investigated in CSC patient group and control ***: We identified a new polymorphism M32 V in two person in the patient group [Minor allele frequency(MAF) =0.009] on the region of 1-60 amino acids. The rs1057293 was located in the encoder region of the SGK- 1 gene but not associated with CSC(P =0.68). An intrinsic rs1743966 is also not associated(P =0.28).CONCLUSION: The new polymorphism M32 V is located on the region of 1-60 amino acids which is necessary for localization to the mitochondria in CSC patient. This mutation is probably important for the energy metabolism and plays an important role in the cellular response to hyperosmotic stress and other stress stimuli. Both rs1057293 and rs1743966 are not associated with CSC.

Within the scope of this study,it is aimed to contribute to the efforts for forming a database including the natural radiation levels throughout the country and developing a foundation for radiation *** portion of the Euphrates valley within the provincial border of Erzincan was chosen for the investigation and natural radiation levels were detected by analyzing the samples taken in terms of radioactivity,natural environment’s radiation *** the region of Karasu,29 different samples from the water and 17 from the soil were taken and their total alpha,beta activities were *** take samples from the water and then they were measured by low background counter(Berthold brand LB770 10-channel counter).Total alpha and beta analyses were made according to the standard *** values of alpha and beta concentrations of samples were measured as 0.0421 Bq/L and 1.806 Bq/L,*** the soil samples HPGe gamma spectrometer system was used and radioisotope activity concentrations were found within the intervals of 2.7-27.8 Bq/kg for 226Ra;3.2-39.4 Bq/kg for 232Th;98-714 Bq/kg for 40K;<0.4-57.8 Bq/kg for 137Cs,*** radioisotopes were detected in some of the samples taken from streaming beds joining river water.
