Fertility after Neosalpingostomy through Laparoscopic Way in Point “G” Teaching Hospital

The endoscopic surgery is actually the golden standard for the management of infertility. Its results look better than classical surgery. The aim of the study was to assess the fertility rate among infertile women who have undergone a laparoscopic distal tubal plasty and to estimate factors influencing this fertility. From January 2007 to December 2016 a descriptive and analytical cross sectional retrospective study was conducted in the departments of general and laparoscopic surgery of the Point “G” Teaching Hospital in Bamako, Mali. Clinical records of 120 laparoscopic patients with distal tubal occlusion who have undergone a neosalpingostomy were collected. Average period of infertility was 6.7 years (from 1 to 25 years), and the average duration before laparoscopic surgery was 5 years. Main abnormalities were: 50 cases of bilateral tubal distal occlusion (41.7%), 14 patients with unilateral tubal distal occlusion (11.7%), 44 cases of bilateral hydrosalpinx (36.6%) and 12 cases with unilateral occlusion (10.0%). Seventeen patients had adhesions (14.1%) including 4.0% of tubo-peritoneal adhesions, 35.5% of tubo-ovary adhesions and Fitz Hugh and Curtis syndrom adhesions in 17.7% of cases. More than half of the patients (58.0%) had a grade II tubal score. Neosalpingostomy had led to bilateral tubal repermeabilization among 60.0% of the patients. Thirty two point five percent (39/120) pregnancies have been observed after the management. Conception probability was null after the 4th post operative year. The majority of pregnancies have been observed in the 11th month after laparoscopic management. During the follow-up, thirty one patients had intrauterine pregnancy (79.5%), four ectopic pregnancies and three miscarriages. The occurrence of pregnancy was influenced by the desire of the woman, the absence of co-morbidity, the tubal score, the result and the quality of the laparoscopic gesture and the duration of the fertility. Splenic infarction is a rare condition (1/500,000 and 1/100,000) but potentially fatal. The prognosis depends on the diagnostic delay and above all on the speed of treatment. It usually occurs in a particular area such as myeloproliferative syndrome, sickle cell anemia, thalassemia etc. The objective of this work was to report a clinical case of necrosis of the spleen.

Water is essential to life and to the sustainable socio-economic development of a nation, It is therefore interesting to have a better knowledge of the quality of this water. The aim was to determine the degree of microbiological and physico-chemical pollution of the water of the “Mamouwol” river in the town of Mamou. To do this, we chose four (4) sampling sites spread throughout the town of Mamou. During the month of March (2024), 4 water samples were analyzed to monitor the water quality of this river. The average values of the flora analyzed show that these water are heavily contaminated with bacteria indicative of fecal and metal pollution. This study showed that sites: Mam4;Mam2 and Mam1 contain the highest loads, with Total Coliform counts ranging from 1534 CFU/100 ml to 2100 CFU/100 ml, the number of faecal coliforms varies between 526 and 1240 CFU/100 ml, and that of faecal streptococci between 526 and 841 CFU/100 ml. Metal content, BOD5 and COD all comply with the laboratory’s analysis criteria, although they vary from point to point.

The monitoring of our watercourses for their preservation must be an imperative concern of the whole *** aim of this study is to describe the main causes of degradation and destruction of the natural resources of the Mamouwol River in the Mamou *** were collected using a survey method(questionnaire,interviews and observations).The study revealed that extensive agro-pastoral activities are the main socio-economic activity of the population,accounting for 78.5%-90%and 20.30%*** follows(1.2%).Related activities include handicrafts and petty trade;Other human activities:hunting(11%),charcoal burning(58%),brick making and firing(45.6%).Riverbanks and water resources in the vicinity of dwellings are damaged and polluted by socio-economic activities and the use of agricultural *** absence of industrial units on the path for the data collection.

Over the years, pineapple production in the Republic of Guinea has become less competitive in the West African sub-region, with a world ranking of 144th. It is therefore only natural to review certain parameters in order to improve this ranking. To do this, certain physico-chemical parameters of soil samples from Friguiagbé and Maferinyah (in the Kindia and Forécariah prefectures) were taken and analysed using the following techniques: Pipette de Robinson, Anne, Bray II, Kapen HICDVITZ, Mc. Lead (1982). The analytical results show that the soils at Friguiagbé in Kindia and Maferinyah in Forécariah are acidic, with pH values of 4.4 and 4.7 (fields I and II) and 4.8 and 4.7 (fields I and II) respectively. The soils have a silty-sandy texture. This study could therefore serve as a guide for the Ministry of Agriculture of the Republic of Guinea.

Problem of Invasive Cervical Cancer’s Management in the Obstetric Gynecologic Department of the National Hospital Donka, Conakry

Introduction: The objectives of this study were to describe the diagnostic and therapeutic aspects, identify problems and evaluate the survival rate of invasive cervical cancer patients. Patients and method: It was a retrospective descriptive study of 7 years from 1 January 2006 to 31 December 2012. Records of invasive cervical cancer were histologically confirmed. The clinical and therapeutic data collected were transferred to Epi info 7 and SSPS version 18 software with a significance level p as used to provide information on the vital condition after confidential agreement. Kaplan Meier was used to assess the overall survival rate. Results: Invasive cancer of the cervix was frequent (58.79%) with an annual median rate of 16.7%, with extremes of 5.8% and 20.6%. 88.70% of the patients was referred from regions of the country (54.02%);the median age was 50 years with extremes of 16 and 84 years and a peak of 29.6% between 35 and 44 years;76.20% were housewives;uneducated women were about 60.12%;Stage III was about 45.3%. Most of the patients were confirmed histologically after 30 days (68%). The means of treatment were surgery about 91 (29.26%), palliation 75 (24.12%), radiotherapy 59 (18.97%) and chemotherapy 41 (13.18%) with surgery (9, 65%) or radiotherapy (4.82%). The main complication was metrorrhagia, 164 cases (56.55%), with overall survival rate of 51.8% at 2 years and 5.1% at 5 years. Conclusion: Invasive cervical cancer in later stages is treated for low survival in the context of our work. Early detection and treatment of precancerous lesions would reduce the incidence and mortality of this cancer.

The issue of wound care has always played an important role in the practice of medicine. This is evidenced by Ambroise Paré’s decision to publish his first work on this subject, “La manière de traiter les Plaies” (The Way of Treating Wounds) in 1545. Objective: To evaluate the therapeutic methods we practice and to describe the impact of the pathology on socio-economic and professional development. Patients and Method: This was a retrospective study carried out at the Fousseyni Daou Hospital in Kayes from January 1, 2018, to December 31, 2020. We included all patients with an ulcero-necrotic wound on immunocompetent terrain hospitalized in the department. Patients with ulcerative necrotic wounds of diabetes, HIV, or cancerous origin were not included. The parameters studied were: etiologies, local care, sequelae, and socio-economic and professional aspects. Results: We collected 57 patients of whom 43 were men and 14 were women, i.e. a sex ratio of 3. The mean age was 40.7 years with a standard deviation of 8.4 with extremes (7 years and 80 years). The average consultation time was 25.1 days. The most represented socio-professional stratum was agropastoralism in 37 cases (65%). The predominant etiological factor was neglected traumatic wounds in 34 cases (59.6%). The site was the lower limb in 39 cases (68.5%). The germ found was Staphylococcus aureus in 21 cases (36.8%). The particularity during local care was the use of table sugar in 9 cases (15.7%) and maggot therapy in 2 cases (3.5%). Hyperthermia was the clinical sign of aggravation in 22 cases (38.6%) and we diagnosed 2 cases (3.5%) of tetanus. The mortality rate was 15.7% (9 cases) and 30 cases (52.7%) of sequelae after recovery. The average length of hospitalization was 38 days. Hospital care was provided by the social welfare service in 35 cases (61.4%). Nineteen (19) patients (39.5%) were unable to resume their socio-professional activity.
