Evaluation of Bilateral Mental Nerve Block as an Alternative to General Anesthesia for Resection of Lower Lip Tumors

Objective: Nerve blockade is commonly used in oncosurgical procedures as an alternative to general anesthesia for older patients and those with significant medical problems. We report a single tertiary oncology center experience in performing various lip resections and reconstructive techniques using this technique. Patients and methods: sixty patients with lower lip tumors were enrolled into this study with exclusion of refusing, non-compliant and critically ill patients. A solution of 4 ml 2% Lidocaine and epinephrine was put in a 5 ml syringe. Half the solution (2 ml) was injected into each side with a 23 gauge needle and after 5 minutes the surgical resection was carried out after testing for anesthesia. Results: The mean age was 68 ± 6.2 years. The mean ASA score was 3 ± 0.75. There were 4 cases who expressed painful sensation and their operations were completed with fentanyl increments. Mean hospital stay was 1 ± 0.75 days. There were three cases of wound gaping who were treated with secondary closure in an outpatient basis. One patient expressed postoperative acute ischemic heart pain that was managed with anti-ischemic measurements. Conclusion: Bilateral mental nerve block is a safe and effective alternative to general anesthesia in lower lip tumors’ resection especially in older patients and those with poor tolerance for general anesthesia provided that there is no need for cervical nodal dissection.

Triple Negative Breast Cancer(TNBC)immunotherapy has recently shown promising ***,some TNBC patients presented with *** of the reasons was attributed to the excessive release of cytokines at the tumor microenvironment(TME)such as Tumor necrosis factor alpha(TNF-α)and Interleukin-10(IL-10).Fine regulation of these cytokines’levels via non-coding RNAs(ncRNAs)might alleviate the immune quiescent nature of TME at TNBC ***,the extrapolation of ncRNAs as therapeutic tools is highly ***,disentanglement the nature for the isolation of natural compounds that could modulate the ncRNAs and their respective targets is an applicable translational therapeutic ***,this study aimed to targeting the chief immune suppressive cytokines at the TME(TNF-αand IL-10)via ncRNAs and to examine the effects of Rosemary aerial parts extract on the expression levels of these ncRNAs in *** revealed miR-17-5p as a dual regulator of TNF-αand ***,an intricate interaction has been shown between miR-17-5p and the oncogenic lncRNAs:MALAT1 and *** down of MALAT1 and/or H19 caused an induction in miR-17-5p and reduction in TNF-αand IL-10 expression ***-17-5p was found to be down-regulated,while TNF-α,IL-10,MALAT1 and H19 were up-regulated in BC *** expression of miR-17-5p in MDA-MB-231 cells reduced TNF-α,IL-10,MALAT1 and H19 expression levels,as well as several BC *** a translational approach,ursolic acid(UA)isolated from rosemary induced the expression of miR-17-5p,MALAT1 and decreased H19 expression *** conclusion,this study suggests miR-17-5p as a tumor suppressor and an immune-activator miRNA in BC through tuning up the immunological targets TNF-α,IL-10 at the TME and the oncological mediators MALAT1 and H19 lncRNAs.

Association between Dietary Habits and Body Mass Index to Female Students College Sciences and Arts Campus 1 at Khamis Mushayt King Khalid University

Background: In the Kingdom of Saudi Arabia (KSA), recent studies revealed increasing consumption of animal products and refined foods in the diet at the expense of vegetables and fruits. These dietary changes were accused of increasing the prevalence of both overweight and obesity observed among Saudi children, adolescences and adults in the last few decades. Objectives: The present study was a cross sectional study aimed at exploring the BMI distribution among students of college of Sciences and Arts for girls Khamis Mushayt Campus 1 at King Khalid University (KKU) and investigated how Dietary habits of students female affected body mass index (BMI). Methods: A total of 240 female students aged 18 - 22 years were about 25% from total students randomly chosen from college of Arts and Science Campus 1 at Khamis Mushayt, King Khalid University, KSA for the present study. A self-reported questionnaire about the student’s dietary habits was conducted and their body mass index (BMI) was measured. Data were analyzed using SPSS statistical software and Chi-square test conducted for variables. Results: About half of the students (47%) were within normal weight, 27.4% were under weight, 16% were overweight and 9.6% were categorized obese. The majority of the students reported eating meals with their family while more than half of sample (58.7%) had eating meals during watching TV. It was worth mentioning that 84.5% of students reported eating snacks such as chocolate and chips 3 or more times per week. There were no significant differences between BMI category and dietary habits. Conclusion and Recommendations: There were no significant difference between body mass index category and dietary habits. Increasing educational programs introduced healthy dietary concepts to improve the dietary habits of female students.

Function coupling of otoferlin with gad65 acts to modulate GABAergic activity

Otoferlin,an integral membrane protein implicated in a late stage of exocytosis,has been reported to play a critical role in hearing although the underlying mechanisms remain ***,its widespread tissue distribution infers a more ubiquitous role in synaptic vesicle ***,an excitatory neurotransmitter,is converted to its inhibitory counterpart,g-aminobutyric acid(GABA),by L-glutamic acid decarboxylase(gad),which exists in soluble(gad67)and membrane-bound(gad65)*** the first time,we have revealed a close association between otoferlin and gad65 in both HEK293 and neuronal cells,including SHSY5Y neuroblastoma and primary rat hippocampus cells,showing a direct interaction between gad65 and otoferlin’s C2 *** primary rat hippocampus cells,otoferlin and gad65 co-localized in a punctate pattern within the cell body,as well as in the axon along the path of vesicular ***,GABA is virtually abolished in otoferlin-knockdown neuronal cells whereas otoferlin overexpression markedly increases endogenous *** attenuation in otoferlin-knockdown primary cells is correlated with diminished L-type calcium *** previously unknown and close correlation demonstrates that otoferlin,through gad65,modulates GABAergic *** discovery of otoferlin–gad65 functional coupling provides a newavenue for understanding the molecular mechanism by which otoferlin functions in neurological pathways.
