Unusual sites of metastases are recognized in patients with renal cell carcinoma (RCC). However, the prognostic implications of these sites are not well understood. We used the Memorial Sloan-Kettering Cancer Center (MSKCC) risk classification for metastatic RCC to evaluate 912 consecutive patients with RCC managed at the Singapore General Hospital between 1990 and 2009. Among these patients, 301 had metastases either at diagnosis or during the course of illness. Nasal metastases, all arising from clear cell RCC, were identified histologically in 4 patients (1.3% of those with metastasis). All 4 patients were classified as MSKCC poor prognosis by current risk criteria. Nasal metastases were significantly associated with lung and bone metastases. The frequency of nasal metastases in patients with metastatic RCC is about 1%, occurring predominantly in patients with clear cell RCC. Nasal metastases are associated with poor prognosis as estimated by the MSKCC risk classification, with attendant implications for selection of targeted therapy, and are usually associated with multi-organ dissemination, including concurrent lung and bone involvement.

探讨马来西亚社区大学高收入毕业生职业生涯发展中的妥协问题。当毕业生面对个人与工作特性不匹配时,必须做出妥协,即只有放弃或者调整职业发展目标才能获得真正的成功。本研究采用多案例研究法,探讨影响妥协的四个因素和调整职业生涯目标的三个妥协过程。影响妥协的因素分别是:有限的就业前景、性别限制、家庭不支持、经济不景气。妥协的表现有:接受声望不太高的工作,选择要求更高、更具有挑战性的工作,选择学习深造。

Spontaneous rectus sheath hematoma(SRSH) is a rare entity that mimics acute *** sheath hematoma is the accumulation of blood in rectus sheath due to muscle or epigastric vessel injury. However, SRSH without a trauma or anticoagulation is rare. It frequently mimics acute abdomen and it may lead to misdiagnosis and unnecessary laparotomy. In this article, we described two cases of SRSH with their diagnostic and therapeutic strategy. Both of our patients in report were neither consumed antiplatelet nor coagulopathy. They both had abdominal muscle straining before their symptoms presentation. We hypothesized that the SRSH may be induced by rectus muscle injury secondary to inappropriate straining or posture. To diagnose SRSH, clinical findings are important but radiologic imaging such as computed tomography, can be diagnostic. In expanding SRSH, percutaneous arterial embolization of epigastric artery is useful to secure the bleeding. If embolization is not feasible, surgical exploration hemostasis is curative. In non-expanding SRSH, it can be managed nonoperatively. SRSH is an important initial differential for acute abdomen. Radiologic imaging helps in *** SRSH can be managed conservatively with good outcome.

Retained placenta is a known cause of post-partum haemorrhage and maternal mortality. A recent systemic review has confirmed that the incidence of retained placenta had increased all over the world, which is more common in developed countries. Failure of retroplacental myometrium contraction is the main cause of retained placenta. Maternal age greater than 35 years, grandmultipara, preterm labor, history of previous retained placenta, and caesarean section were the risk factors for retained placenta. Manual removal of the placenta has been the treatment of choice. Attempts had been made by clinician and researchers to find a safe, effective and reliable method to avoid the need for surgical intervention. The efficacy and safety of prostaglandin, nitroglycerin or acupuncture in the management of retained placenta are yet to be further evaluated. Nonetheless, till date only intraumbilical vein oxytocin has been studied extensively but with varied success. More randomized clinical trials are needed to address this issue. However, if immediate manual placenta removal service is unavailable, a trial of intra-umbilical vein oxytocin 100 IU at a totalvolume of at least 40 m L while preparing for transfer to a tertiary center or theatre may result in spontaneous expulsion of the placenta.

Oil palm or Elaesis guineensis is originated from West *** was first introduced into Malaya in 1870 and was known as an ornamental ***,about 4.50 million hectares of land in Malaysia is under oil palm cultivation;producing 17.73 million tons of palm oil and 2.13 tons of palm kernel *** is one the largest producers and exporters of palm oil in the world,accounting for 11% of the world’s oils and fats production and 27% of export trade of oils and fats（Anon,2009）.In Malaysia,the palm oil industry generates huge amounts of residues and wastes such as fruit fibers,shell,empty fruit bunches,fronds,trunks and palm oil mill *** palm trunk（OPT） is among the most abundant biomass with high potential for energy and material source because oil palm trees are felled and replanted approximately every 25 *** gasification of OPT was carried out using downdraft fixed-bed gasifier to determine its suitability as a gasification feedstock in *** experimental work was carried out under atmospheric pressure（1atm）,constant temperature（9000C） and constant reaction time（60min）.The effects of different gasifying agent and steam to carbon ratio（S/C） on syngas production were *** syngas ratio was increased when steam（1.40） was applied as gasifying agent compare to air（1.33） due to water-gas shift reaction was occurred between carbon and steam to produce more *** value of nitrogen（N2） content in producer gas was reduced almost 90% by replace with carbon dioxide（CO2） as ***2 removal gave zero effect to greenhouse gasses due to recycling process of CO2 for natural growth process.

Fanconi anemia (FA) is a fatal heterogeneous autosomal recessive disorder, characterized by progressive bone marrow failure, congenital defect and cancer predisposition. Cell culture from FA fibroblast (FAF) displays certain abnormalities as compared to normal human dermal fibroblast (NHDF). This prompted us to investigate the effect of a specific nutrient mixture (NM) containing ascorbic acid, lysine, proline and green tea extract, which has demonstrated a broad spectrum of pharmacological activities, on FAF compared to NHDF. We investigated the in vitro effect of NM on FAF and NHDF cell proliferation by MTT assay, MMPs secretion by zymography, morphology by H&E staining and apoptosis by green caspase assay. FAF (FA-A: PD20, FA-A: PD220) and NHDF were cultured in modified Dulbecco Eagle media. At near confluence, the cells were treated with different concentrations of NM (0, 50, 100, 250, 500 and 1000 μg/ml) in triplicate. The cells were also treated with PMA to induce MMP-9 activity. NM had no effect on FAF cell viability in both cell lines compared to control. In contrast NM exhibited 20% at 50 and 100, 50% at 250, 60% at 500 and 70% toxicity at 1000 μg/ml on NHDF cells. Zymography demonstrated MMP-2 and MMP-9 on PMA stimulation in FAF and NM inhibited the activity of both MMP-2 and MMP-9 in a dose response fashion with total block at 500 μg/ml. In contrast, NHDF exhibited only MMP-2, both active and inactive forms, and NM inhibited their activities in a dose-dependent manner with total block at 1000 μg/ml. H&E staining did not indicate any morphological changes in FAF nor induced apoptosis at higher concentrations, as seen by caspases assay. However, although no morphological changes in NHDF were noted up to NM 100 μg/ml, progressive changes in cell shrinkage, rounding and nuclear condensation, pertaining to apoptosis, were observed at higher concentrations. Background and Objectives:The aims of this study were(1)to examine the diagnostic accuracy of resting timeaveraged distal coronary pressure(Pd)to mean aortic pressure(Pa)ratio to predict hyperemic fractional flow reserve(FFR)and(2)to identify a resting Pd/Pa value that can preclude the need for hyperemic FFR assessed with use of a monorail pressure ***:A total of 191 stenoses were *** exclusions,157 FFR data sets from 103 patients were ***:Resting Pd/Pa showed poor agreement with hyperemic FFR(r=0.619,Pnormal FFR of 0.80 or *** results were consistent regardless of the vessels studied,the location of lesions,and the severity of ***:Resting Pd/Pa showed poor agreement with hyperemic FFR assessed with use of a monorail pressure ***,resting Pd/Pa of 0.96 or greater had excellent sensitivity and negative predictive value to predict normal hyperemic FFR,and resting Pd/Pa of 0.82 or less had excellent specifi city and positive predictive value to predict abnormal hyperemic FFR.
