The Prognostic Value of BMI, Serum Glucose, Endometrial Echo Pattern and Uterine Artery Doppler Velocimetry as a Predictor for Endometrial Pathology in Women with Postmenopausal Bleeding (Prospective Observational Study)

Background: Post-menopausal bleeding is a warning sign that accounts for about 5% of all outpatient gynaecologic visits and is a common indication for referral to rapid access clinics because of the fear of underlying malignancy. Endometrial malignancies differ from other malignancies in that early symptomization is common, allowing early cure. Patients and Methods: During the study period, 100 women with post-menopausal bleeding having inclusion criteria were evaluated in Al Hussein University Hospital. For each patient full history, general, abdominal and pelvic examination was performed. Routine pre-operative investigations were done. Patients were divided into four groups: Group 1 included 29 patients with endometrial polyp. Group 2 included 34 patients with endometrial hyperplasia. Group 3 included 21 patients with atrophic endometrium. Group 4 included 16 patients with endometrial carcinoma. Results: As regards the predictive value of BMI, in the study there was a high statistical significance in comparison between the endometrial carcinoma group and all other benign groups. When discussing the predictive value of blood glucose level, in the study there was a high statistical significance in comparison between the endometrial carcinoma group and all other benign groups. It is worth to mention that the predictive value of endometrial thickness, in the study, was with high statistical significance in comparison between the endometrial carcinoma group and all other benign groups providing the highest specificity and sensitivity. At the last the predictive value of uterine artery velocimetry, in the study, was with high statistical significance in comparison between the endometrial carcinoma group and all other benign groups. Conclusion: BMI, blood glucose level, endometrial thickness and uterine artery velocimetry indices, improve the prediction of endometrial carcinoma in women with post-menopausal bleeding.

Background: Ketamine or dexmedetomidine as an adjuvant to bupivacaine in local wound infiltration attenuated postoperative stress response, especially with ketamine in patients undergoing total abdominal hysterectomy. Objectives: Compare effect of local wound infiltration with ketamine or dexmedetomidine added to bupivacaine to bupivacaine alone on inflammatory cytokine response after total abdominal hysterectomy. Methods: Sixty female patients with endometrial carcinoma underwent total abdominal hysterectomy and scheduled to receive local wound infiltration before wound closure either with one of three;40 ml of 0.25% bupivacaine alone (C Group) or with the addition of 2 mg/kg ketamine (K Group) or 2 μg/kg dexmedetomidine (D Group). After extubation, they were followed up for postoperative interleukin 6 (IL6), IL1β, IL10, and TNF-α levels were assessed at baseline, pre-infiltration, 6, and 24 h by blood samples obtained from each patient, hemodynamic variables, analgesic profile and side effects. Results: Inflammatory cytokines response was attenuated in K and D groups, evidenced by decreased mean pro-inflammatory cytokines IL6, TNF-α, and increased anti-inflammatory IL10 at 6 and 24 h postoperatively compared to pre-infiltration levels (p ≤ 0.01) with preservation of IL1β at its preoperative level (p > 0.05). Attenuation was more in K and D groups than in the C group and was highest in the K group with decreased 1st request, total morphine consumption without serious side effect. Conclusion: Local wound infiltration with ketamine or dexmedetomidine added to bupivacaine has a good postoperative analgesic profile and attenuated cytokines inflammatory response more than bupivacaine alone after total abdominal hysterectomy, with highest attenuation in ketamine group.

This paper represents a review of the recent researches that investigate the behavior of the gas turbulent flow laden with solid particles. The significant parameters that influence the interactions between the both phases, such as particle size, loading ratio and the gas velocity, have been extensively reviewed. Those parameters are presented in dimensionless numbers in which the applicability of studying its effect in terms of all circumstances of the gas turbulent channel flow at different condition is possible. The represented results show that the turbulence degree is proportional to the particle size. It was found that at the most flow conditions even at low mass ratio, the particle shape, density and size significantly alter the turbulence characteristics. However, the results demonstrate that the particle Reynolds number is a vital sign: the turbulence field becomes weaker if particle Reynolds number is lower than the critical limit and vies verse. The gas velocity has a strong effect on the particles settling along the channel flow and as a result, the pressure drop will be affected.

Objective: Despite the growing evidence that lipopolysaccharide binding protein (LBP) plays a major role in cardiovascular disease (CVD) pathophysiology and obesity, data regarding this association in children are rare. Therefore, our objectives were to assess whether there was a difference between overweight/obese and normal-weight children in plasma LBP levels and to assess the cardiovascular changes in both groups. Methods: In an observational, case-control study, a total of 30 children as obese and overweight children. Obese children with body mass index (BMI) above 95th percentile, and overweight children with BMI between 85th and 95th percentile were recruited if they aged between 8-16 years old. A similar number of matched controls were included. Serum LBP was measured by enzyme-linked immunosorbent assay (ELISA) technique. Cardiovascular disease remains the leading cause of mortality and morbidity despite the identification of major risk factors and risk reduction strategies. Myocardial infarction (MI) is a relevant cardiovascular worldwide event for morbidity and mortality. In most cases, sudden cardiac death is triggered by ischemia-related ventricular tachyarrhythmia and accounts for 50% of deaths from cardiovascular disease in developed countries. This is a descriptive analytical case control study aimed to determine D-dimer, PT and PTT level and among patients with acute myocardial infarction conducted in Sudan cardiac center hospital. Thirty patients after MI and twenty normal controls have been studied. The MI patients also include co-exist disease diabetes and hypertension, they receive different anticoagulants therapy. The result demonstrates a significant increase post MI in the mean level of D-dimer (p = 0.00) whereas none significantly compares to control group. There are no differences between INR (0.393), PTT (0.648) and PT (0.393), parameters between cases and controls. In conclusion, our study reveals higher D-dimer level among patients than the control. In conclusion, serum D-dimer levels appear to be useful for diagnosing MI and may assist in the prediction of mortality among those patients which are presented with acute chest pain or known diagnosed with MI and should be done as indicator for thrombosis risk during therapy in post MI.

Assessment of Iron Profile among Transfused Dependent Chronic Renal Failure Sudanese Patients

Background: Anemia is one of the complications of chronic kidney disease (CKD) which necessitates multiple blood transfusions. These multiple transfusions are usually accompanied by Iron over load. The current study aimed to assess the Iron profile in Sudanese patients with chronic renal failure received regular blood transfused. Materials and Methods: A descriptive analytical case control study was performed in Alturki Teaching Hospital, Khartoum, Sudan. A total of 90 Sudanese were enrolled in this study (30 CKD patients under hemodialysis and undergo multiple blood transfusion, 30 CKD patients under hemodialysis but not blood transfusion and 30 healthy controls, 51 (57%) were male and 39 (43%) were females, their age ranged between 28 to 75 years. Iron profile (serum iron, serum ferritin and total iron binding capacity (TIBC)) were measured using full automated Chemical analyzer Mindray BS 200 (China). Data were analyzed by using statistical package for social sciences (SPSS) verssion16. Results: The present study showed that serum iron and serum ferritin of those under hemodialysis and received regular blood transfusion patient were significantly higher compared to those under hemodialysis and did not received regular blood transfusion patients (P value 0.000 and 0.01) respectively. Conclusion: Patients with chronic renal failure and under hemodialysis and receive regular and multiple transfusions are prone to iron overload.

Background: Hindfoot fusion, is a fusion of the talonavicular, talocalcaneal, and calcaneocuboid joints and is commonly performed for patients complaining of pain, chronic instability and gross deformity as a result of hindfoot pathology. Objective: This study aimed to assess the functional outcomes of the patients underwent hindfoot fusion, the post-operative complications and patient’s satisfaction. Methodology: This study is a prospective cohort study was conducted in Future hospital, in Khartoum, Sudan. It involved 30 patients from July 2015-July 2022. Data was collected by the primary researcher using data collection sheet, then it was cleaned and entered into Microsoft excel data sheet and was analysed using SPSS version 28 software. Results: 33.3% of the patients are aged 51 - 60 years, and >60 years. Male: Female equals 1:1. 56.7% are for the left side, only 23.3% developed complications. 63.3% patients are fully satisfied. There is a significant correlation between the AOFAS score before and after the operation. Also, a significant correlation between the means of AOFAS score after with the complications and the satisfaction of the patients. Conclusion: Minimally invasive tibio-talo-calcaneal is an alternative procedure for the treatment of severe Charcot arthropathy of the ankle. The expected functional outcome is impressive and comparable with the open surgery. However, the post-operative complications are prevalent in 23.3% of patients. Most candidates are fully satisfied with the procedure.

Hypertension (HTN) has been defined as a blood pressure level at which an otherwise healthy person would have an increased risk of cardiovascular disease that could be mitigated through blood pressure-lowering treatment. Worldwide, hypertension is one of the most common causes of death. The prevalence of hypertension and cardiovascular disease increases with age, and has been found to be higher in those of South Asian and African ancestry, and in Aboriginal populations. Hypertension is not only one of the most important risk factors for cardiovascular disease, but also the number one modifiable risk factor for stroke. Alterations in blood coagulation system have been reported in patients of hypertension. Fibrinogen has been identified as a major independent risk factor for cardiovascular diseases. This was a descriptive cross-sectional study, conducted in elobied teaching hospital during period from March to June 2014. The aim of this study is to determine the level of fibrinogen among hypertensive patients. Seventy percent of the patients (about 35 patients) had high fibrinogen level above 400 mg/dl compared with the control and about 30% (15 patients) were within the normal range of 200 mg - 400 mg/dl. This study has concluded that there was a significant increase in fibrinogen level in hypertensive patients compared to control, while the gender has no effect on the level of fibrinogen.

Background and Objectives: The main benefits of laparoscopic surgery in comparison to open surgery involve the rapid discharge from recovery room, decreased postoperative hospital stays, reduced postoperative discomfort, easier getting back to work and faster return to ordinary daily life as well as cosmetic surgical wounds. The anesthesia type has an essential role in attenuation of the surgical stress and achievement of these advantages. We aimed to determine the outcome of giving general anesthesia in conjunction with thoracic epidural analgesia (TEA) compared to general anesthesia alone on stress response to surgery and anesthesia by investigating cytokine reaction (interleukin 6 and 8 levels), hemodynamic changes (BP, HR, RR, SPO2), and Visual Analogue Scale (VAS) scores postoperatively in patients subjected for laparoscopic cholecystectomy. Methods: This study included 40 patients aged 20 - 60 years old with American Society of Anesthesiologists physical status (ASA) I and II. They were planned for laparoscopic cholecystectomy at Aswan University Hospital from April 2017 to March 2018. They were randomly allocated into two groups. Group A (n. 20) received general anesthesia only and Group B (n. 20) received general anesthesia in conjunction with thoracic epidural analgesia using fentanyl and bupivacaine in the epidural catheter.
