Traumatic facial nerve paralysis dilemma. Decision making and the novel role of endoscope

Objective: The management of traumatic facial nerve paralysis(FNP) has remained a controversial issue with conflicting findings arguing between surgical decompression and conservative ***, recent advances in endoscopic surgery may consolidate the management plan for this ***: This prospective clinical study included patients with posttraumatic FNP at a tertiary referral center. Patients were categorized in two main groups: surgical and conservative. Indications for surgery included patients with immediate and complete FNP, no improvement in facial function on medical treatment, with electroneurography showing >90% degeneration or electromyography showing fibrillation potential. Patients who did not satisfy this criterion received the conservative approach. The transcanal endoscopic approach(TEA) or endoscopic assisted transmastoid approach was performed for facial nerve decompression in the surgical ***: The main outcome was facial function improvement, assessed using the House Brackmann grading scale(HBGS) 6 months after surgery, and hearing state assessed using the air bone gap(ABG).Results: The study included 38 patients, of whom 15 underwent had surgical decompression and 23underwent conservative therapy. A significant improvement in facial nerve function from a mean of4.66 ± 0.97 to 1.71 ± 0.69(P = 0.001) and ABG from a median of 30(10-40) to 20(10-25)(P = 0.002)was ***: Decision-making in cases of traumatic FNP is critical. The geniculate ganglion and tympanic segment were the most commonly affected areas in FNP cases. The TEA represents the most direct and least invasive approach for this area.

Learning from imbalanced data is one of the greatest challenging problems in binary classification,and this problem has gained more importance in recent *** the class distribution is imbalanced,classical machine learning algorithms tend to move strongly towards the majority class and disregard the ***,the accuracy may be high,but the model cannot recognize data instances in the minority class to classify them,leading to many *** methods have been proposed in the literature to handle the imbalance problem,but most are complicated and tend to simulate unnecessary *** this paper,we propose a simple oversampling method based on Multivariate Gaussian distribution and k-means clustering,called *** new method aims to avoid generating noise and control imbalances between and within *** experiments have been carried out with six classifiers and four oversampling *** results on different imbalanced datasets show that the proposed Gk-Means outperforms other oversampling methods and improves classification performance as measured by F1-score and Accuracy.

Spot urinary sodium for assessing dietary sodium restriction in cirrhotic ascites

AIM： To evaluate the accuracy of spot urinary Na/k and Na/creatinine （Cr） ratios as an alternative to 24-h urinary sodium in monitoring dietary compliance in patients with liver cirrhosis and ascites treated with diuretics. METHODS： The study was carried on 40 patients with liver cirrhosis and ascites treated with diuretic therapy. Patients were divided into two groups according to 24-h urinary sodium. We measured spot urine Na/k ratio, Na/ Cr ratio and 24-h urinary sodium. Student＇s t test was used to compare the interval variables and x^2 test to compare the nominal variables between the two groups. Receiver operator characteristic curve was used to identify the best cutoff point for Na/k and Na/Cr ratio. RESULTS： The best cutoff point for Na/k ratio was 2.5 （P 〈 0.001） and area under the curve （AUC） was 0.9, and for Na/Cr ratio, the best cutoff point was 35 （P 〈 0.001） and AUC was 0.885. Na/k ratio showed higher sensitivity and accuracy compared to Na/Cr ratio （87.5% and 87% for Na/k ratio; 81% and 85% for Na/Cr ratio, respectively）. CONCLUSION： Spot urine Na/k ratio has adequate accuracy for assessment of dietary sodium restriction compared with 24-h urinary sodium in patients with liver cirrhosis and ascites.

Due to rapid urbanization around the world,high concentrations of vehicular pollutants have deteriorated the outdoor air quality,which can affect the physical and psychological well-being of *** strategies have been proposed to overcome these harmful impacts by improving the dispersion of air ***,a question arises regarding the potential effects of building morphology on the dispersion of ***,transient three-dimensional Computational Fluid Dynamics(CFD)simulations are performed to examine the effect of building morphology on PM10 *** cases with various prototypes and morphological methods are compared with a simple building form to identify the patterns of PM10 dispersion within a given time sequence under a prevailing inflow *** results indicate that the different designs of building morphology with varying Relative compactness(RC)indicator highlight the importance of considering morphological factors to improve outdoor air *** addition,the proposed prototypes can reduce PM10 concentrations by approximately 30%e90%at specific points in the studied time *** particular,the vertical,horizontal,and grid folded prototypes can be considered more effective as an approximate decrease between 70%and 90%in PM10 concentrations is observed,which reflects the influence of building morphology on improving outdoor air quality.
