Background The lifetime occurrence of urinary stones is approximately 1%–15%,and the peak age of occurrence is 30 *** one fifths of urinary tract stones are found in the ureter,of which two thirds are in the distal *** drugs,including phosphodiesterase-5 inhibitors(PDE5Is)andα-blockers,are used to relax the smooth muscles in medical expulsive therapy(mET).We aimed to compare the combination of tadalafil and tamsulosin versus tadalafil alone as mET for stones in the L1/3 ureter of 10 mm or *** and methods A total of 150 patients with L1/3 ureteric stones measuring 10 mm or less were enrolled in the study and randomly assigned to one of 3 equal groups using a computer-generated random *** in group A prescribed tadalafil 10 mg/***,those in group B were prescribed tamsulosin 0.4 mg and tadalafil 10 mg/d,whereas those in group C received a placebo once *** expulsion rate and pain recurrence were evaluated after 14 *** The stone expulsion rate was significantly higher in the tadalafil and tamsulosin groups and the tamsulosin group than in the placebo group in the current study by 68%in the combination group,64%in the tadalafil alone group,and 42%in the placebo group(p=0.019).In the current study,a combination was associated with lower pain recurrence than tadalafil alone or placebo,with means of 1.06,1.9,2.98(with a p value of 0.001).Stone size was not effective in any *** The combination of PDE5Is andα-blockers effectively increases the expulsion of lower ureteric stones(5–10 mm),but with the same effect as PDE5Is alone,with the advantage of decreasing pain *** size did not affect the expulsion rate in patients who received mET for stones less than 1 cm in size.

Background:Hepatocellular carcinoma (HCC) is the most common cause of cancer-related death in Saudi Arabia. Our study aimed to investigate the patterns of HCC and the effect of TNm staging, Alfa-fetoprotein (AFP), and Child-Turcotte Pugh (CTP) on patients' overall survival (OS).methods:A retrospective analysis was conducted on 43 HCC patients at a single oncology center in Saudi Arabia from 2015 to 2020. All patients had to fulfill one of the following criteria: (a) a liver lesion reported as definitive HCC on dynamic imaging and/or (b) a biopsy-confirmed diagnosis.Results:The mean patient age of all HCC cases was 66.8 with a male-to-female ratio of 3.3:1. All patients were stratified into two groups: viral HCC (n = 22, 51%) and non-viral HCC (n = 21, 49%). Among viral-HCC patients, 55% were due to HBV and 45% due to HCV. Cirrhosis was diagnosed in 79% of cases. Age and sex did not significantly statistically differ in OS among viral and non-viral HCC patients (-value > 0.05). About 65% of patients had tumor size >5 cm during the diagnosis, with a significant statistical difference in OS (-value = 0.027). AFP was >400 ng/ml in 45% of the patients. There was a statistically significant difference in the OS in terms of AFP levels (-value = 0.021). A statistically significant difference was also observed between the CTP score and OS (-value = 0.02). CTP class B had the longest survival. BSC was the most common treatment provided to HCC patients followed by sorafenib therapy. There was a significant statistical difference in OS among viral and non-viral HCC patients (-value = 0.008).Conclusions:The most common predictors for OS were the underlying cause of HCC, AFP, and tumor size. Being having non-viral etiology, a tumor size >5 cm, an AFP > 400 ng/mL, and a CTP score class C were all negatively associated with OS.

Solar-driven hydrogen production from seawater attracts great interest for its emerging role in decarbonizing global energy consumption. Given the complexity of natural seawater content, photocatalytic vapor splitting offers a low-cost and safe solution, but with a very low solar-to-hydrogen conversion efficiency. With a focus on cutting-edge photothermal–photocatalytic device design and system integration, the recent research advances on vapor splitting from seawater, as well as industrial implementations in the past decades were reviewed. In addition, the design strategies of the key processes were reviewed, including vapor temperature and pressure control during solar thermal vapor generation from seawater, capillary-fed vaporization with salt repellent, and direct photocatalytic vapor splitting for hydrogen production. moreover, the existing laboratory-scale and industrial-scale systems, and the integration principles and remaining challenges in the future seawater-to-hydrogen technology were discussed.
