Efficacy of Intravenous versus Intrathecal Meperidine on Post-Spinal Shivering after Knee Arthroscopy: A Randomized Controlled Study

Background and Aims: Hypothermia is considered a common problem with anesthesia. Spinal anesthesia, affects the process of temperature regulation. The aim of this study was to compare the prophylactic effect of intravenous (IV) meperidine with intrathecal (IT) meperidine on the prevention of shivering during spinal anesthesia in patients underwent knee arthroscopy. Aim of the Study: The aim of this work is to study the effect of meperidine either intravenous (IV) vs intrathecal (IT) on controlling postoperative shriving after knee arthroscopy. Materials and Methods: This study will be conducted in accordance with the principles of the ethics committee Benha University hospitals. Full written informed consent will be obtained from all patients before inclusion in the study, and a prospective randomized double-blinded study was conducted in adult patients undergoing knee arthroscopy under spinal anesthesia. Cases are divided into three equal groups (20 patients in each). Group IT: will receive spinal anesthesia (12.5 mg heavy bupivacaine) with intrathecal meperidine (25 mg). Group IV: will receive spinal anesthesia (12.5 mg heavy bupivacaine) with intravenous meperidine (25 mg). Group C: will receive spinal anesthesia (12.5 mg heavy bupivacaine) only postoperatively. Incidence and degree of shivering (SS), sedation score (RSS), pain score (VAS), hemodynamics, and PONV will be measured at the following intervals: 0 min (once arrived at PACU), 20 min later, 40 min later, 80 min later. Results: Shivering was observed in 0%, 0%, and 10.5% of patients in Groups IT, IV, and C, respectively. There was a significant difference between Group IT and IV compared to Group C (P Conclusion: We concluded that IT meperidine and IV meperidine comparably can decrease intensity and incidence of shivering compared to control group as well as decrease the requirement for additional doses of meperidine for shivering control without any hemodynamic side effect. Effect of Height Adjusted Dose of Intrathecal Hyperbaric Bupivacine for Elective Cesarean Section: A Study of 2 Different Concentrations

Introduction: Study compare spinal anesthesia using hyperbaric bupivacine (0.5% - 0.75%) between fixed dose and height adjusted dose during elective cesarean section. Methods: Hundred parturients who had given their consent and were scheduled for elective cesarean section under spinal anesthesia, were divided into four groups—first group received 0.5% hyperbaric bupivacine fixed dose, the second group received 0.5% hyperbaric bupivacine in a dose adjusted to the height, the third received 0.75% hyperbaric bupivacine fixed dose, the fourth group received 0.75% hyperbaric bupivacine in a dose adjusted to height. The anesthesia onset time, haemodynamic changes, side effects and fetal outcome observed. Results: Spinal block provide excellent surgical anesthesia in all patients. Anesthesia onset time is longer in adjusted than fixed groups, in 0.5% (5 ± 0.816 vs. 3.84 ± 0.746) (P Conclusion: Adjusting dose of hyperbaric bupivacine (0.5% - 0.75%) to patient’s height, decreases the dose of bupivacine in use, it also provide adequate anesthesia for elective cesarean section with decrease the use of vasopressors, the incidence and severity of maternal hypotention also markedly reduced.

Effect of Magnesium Sulfate Nebulization on Stress Response Induced Tracheal Intubation;Prospective, Randomized Study

Background and Aims: Multiple, nonpharmacological and pharmacological measures have been used for attenuating stress response of intubation with variable results. N-methyl-D-aspartate (NMDA) receptors have a role in nociception and inflammation. NMDA receptors are located in peripheral and the central nervous system. Magnesium is also a NMDA receptor blocker. Aim of the Study: To evaluate the effect of nebulized magnesium sulfate on reduce the stress response induced tracheal intubation Material and Methods: Following institutional ethical committee approval and written informed consent, a prospective randomized double-blinded study was conducted in 100 cases divided into two equal groups. Patients included in the study were of either gender belonging to American Society of Anesthesiologists (ASA) status 1 or 2 undergoing elective surgery requiring tracheal intubation. Patients will be recruited in the study divided into two equal groups;Patients in Group A: will receive nebulized magnesium sulfate in 3 ml (240 mg) over 15 min. While group B: will receive nebulized normal saline in 3 ml over 15 min, ending 5 min before the induction of anesthesia. SBP, DBP, HR, and blood glucose level will be measured at the following intervals;Baseline (before induction), after premedication (sedation), after induction, after ETT intubation, 3 min later, 6 min later. Results: Preoperative magnesium sulfate nebulization has a significant effect (p value Conclusion: Magnesium sulfate significantly reduces the stress response of intubation.

Multiphysics simulation of VVER-1200 fuel performance during normal operating conditions

Nuclear fuel performance modeling and simulation are critical tasks for nuclear fuel design optimization and safety analysis under normal and transient *** performance is a complicated phenomenon that involves thermal,mechanical,and irradiation mechanisms and requires special multiphysics *** this study,a fuel performance model was developed using the COMSOL Multiphysics *** modeling was performed for a 2D axis-symmetric geometry of a UO2fuel pellet in the E110 clad for VVER-1200 *** modeling considers all relevant phenomena,including heat generation and conduction,gap heat transfer,elastic strain,mechanical contact,thermal expansion,grain growth,densification,fission gas generation and release,fission product swelling,gap/plenum pressure,and cladding thermal and irradiation *** model was validated using a code-to-code evaluation of the fuel pellet centerline and surface temperatures in the case of constant power,in addition to validation of fission gas release(FGR)*** prediction proved that the model could perform according to previously published VVER nuclear fuel performance parameters.A sensitivity study was also conducted to assess the effects of uncertainty on some of the model *** model was then used to predict the VVER-1200 fuel performance parameters as a function of burnup,including the temperature profiles,gap width,fission gas release,and plenum pressure.A compilation of related material and thermomechanical models was conducted and included in the modeling to allow the user to investigate different material/performance *** the model was developed for normal operating conditions,it can be modified to include off-normal operating conditions.

With the rapid development in business transactions,especially in recent years,it has become necessary to develop different mechanisms to trace business user records in web server log in an efficient *** business transactions have increased,especially when the user or customer cannot obtain the required *** example,with the spread of the epidemic Coronavirus(COVID-19)throughout the world,there is a dire need to rely more on online business *** order to improve the efficiency and performance of E-business structure,a web server log must be well utilized to have the ability to trace and record infinite user *** paper proposes an event stream mechanism based on formula patterns to enhance business processes and record all user activities in a structured log *** user activity is recorded with a set of tracing parameters that can predict the behavior of the user in business *** experimental results are conducted by applying clustering-based classification algorithms on two different datasets;namely,Online Shoppers Purchasing Intention and Instacart Market Basket *** clustering process is used to group related objects into the same cluster,then the classification process measures the predicted classes of clustered *** experimental results record provable accuracy in predicting user preferences on both datasets.

On the Computing of the Minimum Distance of Linear Block Codes by Heuristic Methods

The evaluation of the minimum distance of linear block codes remains an open problem in coding theory, and it is not easy to determine its true value by classical methods, for this reason the problem has been solved in the literature with heuristic techniques such as genetic algorithms and local search algorithms. In this paper we propose two approaches to attack the hardness of this problem. The first approach is based on genetic algorithms and it yield to good results comparing to another work based also on genetic algorithms. The second approach is based on a new randomized algorithm which we call 'Multiple Impulse Method (MIM)', where the principle is to search codewords locally around the all-zero codeword perturbed by a minimum level of noise, anticipating that the resultant nearest nonzero codewords will most likely contain the minimum Hamming-weight codeword whose Hamming weight is equal to the minimum distance of the linear code.

A new europium(Ⅲ)complex containing(4-Methyl-2-oxo-2 H-chromen-7-yloxy)-acetic acid moiety(CMMC)was synthesized,characterized,and confirmed as antitumor agent and fluorescent *** spectroscopic measurements of Eu(Ⅲ)in the presence of CMMC were obtained in different *** results show that the strongest Eu(Ⅲ)emission bands were monitored in iso-propyl alcohol while the weakest Eu(Ⅲ)emission band was observed in *** interaction of Eu(Ⅲ)-(CMMC)2 complex with DNA was monitored using absorption and emission *** fluorescence titration measurements,the binding constants of DNA with Eu(Ⅲ)-(CMMC)_2 complex were found to be 1.04×10~5 L·mol^(-1) in Tris-HCl and 1.17×10~7 L·mol^(-1) in DMSO-Tris-HCl buffer(9∶1 V/V).Hypochromism was observed from the absorption titration experiment which indicates the intercalation of Eu(Ⅲ)-complex between the base pair of *** result further confirmed by fluorescent Ethidium bromide displacement *** fluorescence calibration curve was used for the determination of DNA with LOD of 1.2 ng in DMSO-Tris-HCl buffer(9∶1 V/V)and 5 ng in Tris-HCl *** preliminary antitumor investigation shows promising cytotoxicity against MDA-MB-231,MCF-7(mammary cancer),and PC-3(prostate carcinoma)cell lines with IC50 values of 40.63,25.42 and 30.25μmol·L^(-1),respectively.

Utility of 2-Methyl-quinazolin-4(3H)-one in the Synthesis of Heterocyclic Compounds with Anticancer Activity

Quinolino[2,1-b]quinazolines 3 and 4, pyrrolo[2,1-b]quinazoline 5 and various substituted 2-(4-chlorostyryl)quinazoline derivatives including: 4-amino derivative 8, 4-hydrazino derivative 9, thiourea derivative 10, thiosemicarbazide derivative 19, 4-benzylidene hydrazinyl derivative 21, 4-amino thiazolidene derivatives 11, 12, 13, 22, imidazoquinazolines 15, 16, quinazolinium chloride 14, triazino[4,3-c]quinazolines 17, 18, tetrazino[1,6-c]quinazoline 20, 4-amino azetidinyl derivative 23, triazolo[4,3-c]quinazoline 24, 4-amino substituted quinazolines 25, 26, 27, 29 and quinazolino quinazoline 28 were synthesized through different chemical reactions. The obtained compounds were evaluated for their in vitro antitumor activity against HEPG2 and MCF-7 cell lines compared to the reference drug (doxorubicin). Compounds 18, 19, 20, 23 and 24 were found to be the most active against both cell lines exhibiting IC50 values ranging from 10.82 - 29.46 μM/L and 7.09 - 31.85 μM/L against Hep-G2 and MCF-7 cell lines, respectively, in addition to docking study of these five compounds against thymidylate synthase and dihydrofolate reductase enzymes active sites.

Immunological,hematological and biochemical changes induced by short term exposure to cadmium in catfish(Clarias gariepinus)

Objective:To investigate the hematological,biochemical and immunological changes in catfish(Clarias gariepinus)(***)experimental exposed to ***:*** were exposed to different concentrations of cadmium(Cd)(0,2,5,and 10 mg/L)for 3 *** samples were collected for assessing some hematological,biochemical and immunological studies at the end of ***:neutrophilia and lymphopenia in 5,10 mg/L in cadmium exposed *** the blood level activities of ALT and AST significantly increased,as well as glucose,creatinine,urea,potassium and uric *** total protein,albumin and sodium were significantly decreased at 5,10 mg/L of cadmium exposed *** immunological parameters in cadmium exposed experimental dose groups decreased serum bactericidal activity,lysozyme,neutrophils adhesion test as well as decreased resistance to Aeromonas hydrophilla with increasing exposure dose seemed to correspond with suppressive of non-specific immune ***:The treatment of *** with cadmium under the same conditions had immunosuppressive and decrease diseases resistance in a dose-dependent effect.
