Aim of the study: Immunohistochemical evaluation of hormone receptors, Her2/neu, CK5/6, e-cadherin, beta-catenin, p53 and PTeN on Tissue Micro Array (TMA) of 46 Moroccan invasive breast carcinomas. Materials and Methods: The cases comprised 40 invasive ductal carcinomas, 4 invasive lobular carcinomas, 1 mixed carcinoma and 1 invasive colloid carcinoma. TMA paraffin blocs were prepared with the Beecher manual arrayer and immunostaining was performed using standard immunoperoxidase techniques. Results: 58.69% of the cases were eR positive. 43.18% (19/44) were triple negative breast cancers (TNBC) of which 15.78% (3/19) were of the basal phenotype expressing CK5/6. On the other hand, 72.22% (13/18) of the TNBC cases were IDC grade 3. Of the 18 IDC grade 3, 22.22% (4/18) were CK5/6 positive. 41.30% and 10.86% of the cases showed reduced expression of e-cadherin and beta-catenin respectively. Beta-catenin nuclear and cytoplasmic staining was noted in 20% and 97.82% respectively. p53 was overexpressed in 10.86% of the cases whereas PTeN loss or reduced expression was noted in 86.95% of the cases. Conclusion: The aim of our study was to introduce TMA technique in our hospital which is considered a reference institution for cancer in Morocco. Although no statistical study was performed to look for any significance of the results obtained, we found good correlation with some of the data in the literature. To determine the molecular characteristics, if any, of the Moroccan patient, larger multidisciplinary and prospective studies would be interesting in the aim to personalize therapeutic decisions.

IgG Check calf test has been used in this study to identify if a failure of passive transfer occurs in neonatal calves by measuring the level of IgG in blood. An adequate level of IgG measured in all calves at 48 hours, 7 days and 14 days after birth showed level of IgG 1000 mg/dl. This level indicated that they have good passive transfer of immunity. Also, in this study colostrum quality fed to calves was detected using Brix Refractometer. The colostrum is of high quality as it contains 50 mg/ml of IgG. This Brix refractometer can be used on the farm level to estimate colostrum IgG content and monitoring colostrum feeding practices. Colostrum with high quality IgG could provide calves with enough IgG to attain successful passive transfer of immunity. Brix and Obione refractometers provide simple, rapid method for estimating IgG concentration on calf serum, thus considered to be the most common method for determining passive transfer failure.

The aim of this paper is to present a distributed algorithm for big data classification, and its application for Magnetic Resonance Images (MRI) segmentation. We choose the well-known classification method which is the c-means method. The proposed method is introduced in order to perform a cognitive program which is assigned to be implemented on a parallel and distributed machine based on mobile agents. The main idea of the proposed algorithm is to execute the c-means classification procedure by the Mobile Classification Agents (Team Workers) on different nodes on their data at the same time and provide the results to their Mobile Host Agent (Team Leader) which computes the global results and orchestrates the classification until the convergence condition is achieved and the output segmented images will be provided from the Mobile Classification Agents. The data in our case are the big data MRI image of size (m × n) which is splitted into (m × n) elementary images one per mobile classification agent to perform the classification procedure. The experimental results show that the use of the distributed architecture improves significantly the big data segmentation efficiency.

Fosfomycin Therapy for Non-Complicated Lower Urinary Tract Infections during Pregnancy: Tanta University experience

Objectives: To evaluate the efficacy, compliance, safety and economic cost for Fosfomycin trometamol and Nitofurantoin in uncomplicated lower urinary tract infections during pregnancy. Background: Nitofurantoin and Fosfomycin trometamol are recommended as the first-line agents for treatment of urinary tract infections (UTIs) in the latest guidelines endorsed by the Infectious Diseases Society of America (IDSA) and the european Society for Clinical Microbiology and Infectious Diseases (eSCMID). Fosfomycin is bactericidal and inhibits bacterial cell wall biogenesis and reduces bacterial adherence to uroepithelial cells. Fosfomycin has broad antibacterial activity against both Gram-positive and Gram-negative pathogens, as escherichia coli, escherichia faecalis, and various Gram-negatives like Citrobacter and Proteus. Both Nitofurantoin and Fosfomycin are category B in pregnancy. Patients and Methods: This study was conducted at Tanta University Hospitals in the period from June, 1, 2015 to January, 1, 2017. Patients were recruited from outpatient clinics of Obstetrics and Gynecology and Urology Departments presenting with asymptomatic bacteruria or cystitis. Patients were allocated randomly into 2 groups: group I (n = 50 cases) received Fosfomycin therapy and group II (n = 50 cases) received Nitofurantoin therapy (n = 50 cases). After treatment, evaluation of patient symptoms, organism count, patient compliance and cost of treatment were done. Results: The enrolled patients were suffering from lower urinary tract infections;asymptomatic bacteruria (17 cases) or cystitis (83 cases). Ten patients were excluded. The demographic data of included patients were not significant for both groups. Complete relief (100%) of symptoms 5 days after start of treatment was noticed in Fosfomycin group while improvement of symptoms after 5 day-treatment was noticed in 86.49% in Nitofurantoin group (p-value = 0.030). The side effects were recorded in 7 cases (18.42%) in Fosfomycin group

Multilevel inverters are used in many industrial applications because of good power quality, minimum losses and less harmonics contents. Multilevel inverters require no series connected synchronized switching devices, transformer and complex filters. In this paper 10, 18, 24 diode clamped multi-level inverters (DCMLI) are implemented using trust region dog leg optimization method to find the optimized values of switching angles (θ). It decreases the total harmonic distortion (THD) of the output voltages and to reduce the complexity of external filter required. The multi-level inverters are implemented in MATLAB Simulation and results are compared in terms of harmonics, system complexity and efficiency.

Comparison between Preoperative and Postoperative Sublingual Misoprostol for Prevention of Postpartum Hemorrhage during Cesarean Section: A Randomized Clinical Trial

Background: Blood loss is one of the important complications during cesarean section (CS). Previous reports have shown that misoprostol is effective in reducing blood loss during and after CS. However, the optimum time for its administration to decrease the amount of PPH is still under discussion. Objective: To compare the effect of preoperative and postoperative administration of sublingual misoprostol (400 μg) in reducing the amount of blood loss during and 24 hours after CS. Setting: Obstetrics and Gynecology Department, Faculty of Medicine, Assiut University, Assiut, egypt, between January 2017 and July 2018. Study Design: A prospective, randomized clinical trial. Methods: Four-hundred thirty women fulfilling the inclusions criteria: elective lower segment CS at term (≥37 weeks) with normal fetal heart tracing who accepted to participate in the study. Patients were divided into two groups;Patients assigned to group 1 received 400 μg sublingual misoprostol immediately after urinary catheterization and before skin incision, while patients assigned to group 2 received sublingual misoprostol immediately after skin closure. The primary outcome was the estimation of intraoperative and postoperative blood loss for 24 hours. Results: There was a significant reduction in the intraoperative blood loss in group 1 compared with group 2 (403.51 ± 72.99 vs. 460.99 ± 74.66 ml, respectively). Also, there was a significant reduction in postoperative blood loss in group 1 compared with group 2 with a statistical significance (169.45 ± 12.03 vs. 195.77 ± 13.34 ml, respectively). Postoperative hemoglobin and Hematocrit values were significantly higher in group 1 compared with group 2. Conclusions: Preoperative administration of sublingual misoprostol (400 μg) during CS is better than postoperative administration as it is associated with a reduction in the amount of intraoperative and postoperative blood loss and drop in hemoglobin level.

GIS-Mapping of Soil Available Plant Nutrients (Potentiality, Gradient, Anisotropy)

The research was elaborated in Kafr el-Dawar area (egypt northern region) to study the availability of the soil plant nutrients. The research introduced three parameters to comprehensively and carefully describe the availability of the soil plant nutrients: potentiality, gradient and anisotropy. Potentiality defines the categories of soil ability to supply plant nutrients;meanwhile gradient expresses the increasing rate of the availability of the soil plant nutrients. The gradient anisotropy refers to the directions or orientation of the increasing rate of the availability of the soil plant nutrients. The introduced parameters enabled to spatially study the availability of the soil plant nutrients. Analytical data, of soil available phosphorus (P), indicated that P ranged from 0.2 ppm to 11.4 ppm to locate all studied soil samples into the low class of the soil nutritional P ability. This was not the case of available potassium (K), where the soil samples were distributed into three available K soil categories: medium, high, and very high. GIS map of soil P nutritional potentiality for plant (potato), displayed the soil studied area in one category, as low P soil nutritional potentiality to coincide with the analytical data classification. Contrary, the K map classified the soil studied area into three categories of soil K nutritional potentiality: medium, high and excessive. This obviously referred that the individual determination of soil K nutritional potentiality is misleading for interpretation of soil tests because it does care of the spatial distribution of soil available K. Nearly, all soil samples had high available micronutrients that they were located in the high category in both classification of analytical data and GIS maps. GIS gradient maps of the soil available plant nutrients referred that the soil plant nutrients, exception of K, had two gradients: non increasing-slight increasing and build up. The Role of Humic Acid and Proline on Growth, Chemical Constituents and Yield Quantity and Quality of Three Flax Cultivars Grown under Saline Soil Conditions

In order to mitigate the salinity effects on flax grown on moderate saline sandy soil (3275 - 3430 ppm) and irrigated with moderate saline water (2300 - 2460 ppm) field experiments were carried out at the experimental Station of the Faculty of Agriculture, Wadi el-Natrun district el-Behera Governorate—egypt, during two successive winter seasons of 2012/2013 and 2013/2014. Three flax varieties (Opal, Giza-8 and Mayic) were grown and treated with some chemical additives humic acid (50 kg/fed) and/or foliar applied proline (Control, 50 and 100 mg/L). The results showed the positive responses of Giza-8 variety to the combined application of humic acid and proline and mitigated the salinity effects of soil and irrigation water and reflected on most of the studied characters. Such results indicate the potentiality of mitigation the hazardous effect of salinity with these chemical additives. The data indicated that the highest seed yield, straw yield and oil yield were obtained at humic acid (50 kg/fed) with foliar treatment of proline at rate of (100 mg/L). The interaction of proline at (100 mg/L) with humic acid at rate of (50 kg/fed) improved plant fresh and dry weight in all flax cultivars under salinity conditions. Fresh weight increased by 66.6%, 48.7% and 65.5% over controls for Opal, Giza-8 and Mayic varieties, respectively. The interaction of proline at (100 mg/L) with humic acid at rate of (50 kg/fed) with Giza-8 variety gave the highest values of seed yield, straw yield and oil yield.
