Although aberrant DNA methylation changes are significantly associated with the pathogenesis of many diseases,little is known about the molecular mechanisms underlying interactions between DNA methylation and gene expression in metabolic syndrome(MetS).The aim of this study was to identify biomarkers and molecular mechanisms regulated by DNA methylation in *** profiled genome-wide DNA methylation using the Infinium MethylationEPIC BeadChIP and characterized the transcriptome by RNA sequencing in our study population comprised of subjects with MetS(n=11)and controls(n=9).

BACKGROUND The optimal treatment for heart failure(HF)is a combination of appropriate *** the disease using only medical therapy is difficult in patients with HF,severe hypercapnia,and *** patients should first receive ventilator support followed by pulmonary rehabilitation(PR).CASE SUMMARY We report two cases in which arterial blood gas(ABG)improved and PR was possible with appropriate ventilator *** patients with extreme obesity complaining of worsening dyspnea–a 47-year-old woman and a 36-year-old man both diagnosed with HF–were hospitalized because of severe hypercapnia and *** proper medical treatment,hypercapnia and desaturation resolved in neither case,and both patients were transferred to the rehabilitation department for *** the time of the first consultation,the patients were bedridden because of *** demand was successfully reduced once noninvasive ventilation was *** the patients’dyspnea gradually improved to the point where they could be weaned off the ventilator during the daytime,they started engaging in functional training and aerobic *** 4 mo of followup,both patients were able to perform activities of daily living and maintain their lower body weight and normalized ABG *** Symptoms of patients with obesity and HF may improve once ABG levels are normalized through ventilator support and implementation of PR.

BACKGROUND： Acute cholangitis in old people is a cause of mortality and prolonged hospital stay. We evaluated the effects of methods and timing of biliary drainage on the outcomes of acute cholangitis in elderly and very elderly patients. METHODS： We analyzed 331 patients who were older than 75 years and were diagnosed with acute calculous cholangitis. They were admitted to our hospital from 2009 to 2014. Patients＇ demographics, severity grading, methods and timing of biliary drainage, mortality, and hospital stay were retrospectively obtained from medical records. Clinical parameters and outcomes were compared between elderly （75-80 years, n= 156） and very elderly （≥81 years, n=175） patients. We analyzed the effects of methods [none, endoscopic retrograde cholangio- pancreatography （ERCP）, percutaneous transhepatic biliary drainage, or failure] and timing （urgent or early） of biliary drainage on mortality and hospital stay in these patients. RESULTS： Acute cholangitis in older patients manifested as atypical symptoms characterized as infrequent Charcot＇s triad （4.2%） and comorbidity in one-third of the patients. Patients were graded as mild, moderate, and severe cholangitis in 104 （31.4%）, 175 （52.9%）, and 52 （15.7%）, respectively. Urgent bili-ary drainage （≤24 hours） was performed for 80.5% （247/307） of patients. Very elderly patients tended to have more severe grades and were treated with sequential procedures of transient hiliary drainage and stone removal at different sessions. Hospital stay was related to methods and timing of biliary drainage. Mortality was very low （1.5%） and not related to patient age but rather to the success or failure of biliary drainage and severity grading of the acute cholangitis. CONCLUSIONS： The methods and timing used for biliary drainage and severity of cholangitis are the major determinants of mortality and hospital stay in elderly and very elderly patients with acute cholangitis. Predictive proteomic biomarkers for inflammatory bowel disease-associated cancer:Where are we now in the era of the next generation proteomics?

Recent advances in genomic medicine have opened up the possibility of tailored medicine that may eventually replace traditional “one-size-fits all” approaches to the treatment of inflammatory bowel disease (IBD). In addition to exploring the interactions between hosts and microbes, referred to as the microbiome, a variety of strategies that can be tailored to an individual in the coming era of personalized medicine in the treatment of IBD are being investigated. These include prompt genomic screening of patients at risk of developing IBD, the utility of molecular discrimination of IBD subtypes among patients diagnosed with IBD, and the discovery of proteome biomarkers to diagnose or predict cancer risks. Host genetic factors influence the etiology of IBD, as do microbial ecosystems in the human bowel, which are not uniform, but instead represent many different microhabitats that can be influenced by diet and might affect processes essential to bowel metabolism. Further advances in basic research regarding intestinal inflammation may reveal new insights into the role of inflammatory mediators, referred to as the inflammasome, and the macromolecular complex of metabolites formed by intestinal bacteria. Collectively, knowledge of the inflammasome and metagenomics will lead to the development of biomarkers for IBD that target specific pathogenic mechanisms involved in the spontaneous progress of IBD. In this review article, our recent results regarding the discovery of potential proteomic biomarkers using a label-free quantification technique are introduced and on-going projects contributing to either the discrimination of IBD subtypes or to the prediction of cancer risks are accompanied by updated information from IBD biomarker research.

Poly(ether imide)(PEI)has shown satisfactory corrosion protection capability with good adhesion strength as a coating for magnesium(Mg),a potential candidate of biodegradable orthopedic implant ***,its innate hydrophobic property causes insufficient osteoblast affinity and a lack of ***,we modify the physical and chemical properties of a PEI-coated Mg implant.A plasma immersion ion implantation technique is combined with direct current(DC)magnetron sputtering to introduce biologically compatible tantalum(Ta)onto the surface of the PEI *** PEI-coating layer is not damaged during this process owing to the extremely short processing time(30 s),retaining its high corrosion protection property and adhesion *** Ta-implanted layer(roughly 10-nm-thick)on the topmost PEI surface generates long-term surface hydrophilicity and favorable surface conditions for pre-osteoblasts to adhere,proliferate,and ***,in a rabbit femur study,the Ta/PEI-coated Mg implant demonstrates significantly enhanced bone tissue affinity and osseointegration *** results indicate that Ta/PEI-coated Mg is promising for achieving early mechanical fixation and long-term success in biodegradable orthopedic implant applications.

Objective: To investigate the clinical significance of MET gene amplification in patients with gastric cancer in the palliative ***: MET amplification was assessed using fluorescence in situ hybridization(FISH) in 50 patients and quantitative polymerase chain reaction(qPCR) in 326 patients;259 patients treated with first-line fluoropyrimidine and platinum were included for survival ***: The results of FISH and qPCR indicated that the c-MET/CEP7 ratio was correlated with gene copy number. The optimal cutoff value for the copy number using qPCR to detect MET gene amplification with FISH was 5(κ=0.778, P5 detected by qPCR. MET-amplified gastric cancer was associated with an Eastern Cooperative Oncology Group(ECOG) performance status(PS) score of ≥2(33.3% vs. 10.5% P=0.007), peritoneal metastasis(76.2% vs. 46.2%, P=0.008), and elevated bilirubin levels(28.6% vs. 7.3%, P=0.006). The median overall survival(OS) and progression-free survival(PFS) were 11.9 and 5.6 months, respectively. MET-amplified gastric cancer was not associated with survival outcomes [hazard ratio(HR)=0.68, 95% confidence interval(95% CI): 0.35-1.32,P=0.254 for PFS;HR=0.68, 95% CI: 0.35-1.32, P=0.251 for OS].Conclusions: qPCR can be used to detect MET gene amplification. MET amplification was not a predictor of poor prognosis in patients with metastatic or unresectable gastric cancer.

In this study, we induced cerebral infarction in rats by occluding the right middle cerebral artery, and tested the effects of electroacupuncture at the Baihui acupoint （DU 20）. Motor and sensory function was tested using Garcia’s scale and motor weakness grading, and the expression of vascular endothelial growth factor in the brain was quantified using immunoblotting and immunohistochemistry. We found that scalp electroacupuncture at DU 20 significantly improved motor performance and sensory function in rats with stroke, and this was accompanied by an increased expression of vascular endothelial growth factor in the ischemic brain tissue and peri-ischemic area. In addition, Pearson correlation analysis showed that the improvements in functional recovery were correlated with the increased expression of vascular endothelial growth factor.

BACKGROUND Following the global outbreak of coronavirus disease 2019(COVID-19),unlike other vaccines,COVID-19 vaccines were developed and commercialized in a relatively short period of *** large-scale administration of this vaccine in a short time-period led to various unexpected side effects,including severe cytopenia and thrombosis with thrombocytopenia *** many reports on adverse reactions,vaccination was necessary to prevent the spread of COVID-19;thus,it is essential to understand and discuss various cases of adverse reactions after *** SUMMARY A 77-year-old woman was administered the second dose of Pfizer mRNA COVID-19 *** vaccination she experienced fever,myalgia,and *** were subsequently administered for several days,but there was no improvement in the *** patient showed severe thrombocytopenia and *** and abdominopelvic computed tomography showed no infection related findings,but splenomegaly and cirrhotic liver features were observed.A large number of immature cells were observed in the peripheral blood smear;thus,bone marrow examination was performed for acute ***,there were no *** patient recovered after administration of hepatotoxins and transfusion treatment for cytopenia and was diagnosed with an adverse reaction to COVID-19 *** Adverse reactions of vaccination could be mistaken for hematologic malignancies including *** report a patient with leukocytosis following COVID-19 vaccination.
