Liver pathology in COVID-19 related death and leading role of autopsy in the pandemic

BACKGROUND Information on liver involvement in patients with coronavirus disease 2019 is currently *** To highlight the pathological changes found during the autopsy of severe acute respiratory syndrome coronavirus 2 positive *** A systematic literature search on PubMed was carried out until June 21,*** A literature review reveals that pre-existing liver disease and elevation of liver enzyme in these patients are not common;liver enzyme elevations tend to be seen in those in critical *** the poor expression of viral receptors in the liver,it seems that the virus is able to infect this organ and therefore cause liver ***,to date,the search for the virus inside the liver is not frequent(16%of the cases)and only a small number show the presence of the *** most of the autopsy cases,macroscopic assessment is lacking,while microscopic evaluation of livers has revealed the frequent presence of congestion(42.7%)and steatosis(41.6%).Less frequent is the finding of hepatic inflammation or necrosis(19%)and portal inflammation(18%).The presence of microthrombi,frequently found in the lungs,is infrequent in the liver,with only 12%of cases presenting thrombotic formations within the vascular *** To date,the greatest problem in interpreting these modifications remains the association of the damage with the direct action of the virus,rather than with the inflammation or alterations induced by hypoxia and hypovolemia in patients undergoing oxygen therapy and decompensated patients.

Colorectal cancer is the third most common cancer in the *** is man-datory to treat patients with colorectal *** colorectal cancer be treated in laparoscopy?Scientific literature has validated the oncological quality of laparo-scopic approach for the treatment of patients with colorectal *** non-inferiority trials with good remote control have answered positively to this long-debated *** as 1994,first publications demonstrated technical feasibility and compliance with oncological imperatives and,as far as short-term outcomes are concerned,there is no difference in terms of mortality and post-operative morbidity between open and minimally invasive surgical approaches,but only longer operating times at the beginning of the ***,from 2007 onwards,long-term results were published that demonstrated the ab-sence of a significant difference regarding overall survival,disease-free survival,quality of life,local and distant recurrence rates between open and minimally in-vasive *** this editorial,we aim to summarize the clinical and technical aspects which,even today,make the use of open surgery relevant and necessary in the treatment of patients with colorectal cancer.

How to identify early complications in patients undergoing distal gastrectomy?

In this editorial we comment on the article by Zhang et al published in a recent issue of the World Journal of Gastrointestinal *** with appropriate lymph node dissection is still standard curative treatment in locally advanced gastric *** studies point out that gastric cancer surgery is a complex procedure that leads to a high risk of morbidity and *** factors can contribute to the onset of complications with consequent effects on prognosis and increased *** complications can be divided in complications related to anastomosis,to motility and to surgical site *** study presented by Zhang B et al represent an interesting analysis on the possibility to prevent postoperative *** study was performed on 131 patients with distal gastric cancer who underwent gastrectomy with D2 lymph node *** these patients,16%developed early postoperative *** univariate analysis showed that prealbumin level,hypertension,diabetes,history of abdominal surgery,R0 resection,and blood transfusion were factors influencing early postoperative complications after distal ***,the inclusion of the above significant variables in the logistic regression analysis revealed that hypertension,diabetes,a history of abdominal surgery,and blood transfusion were independent predictors of postoperative *** conclusion,preoperative and intraoperative factors can be used to establish an early postoperative nomogram *** results of the study presented by Zhang et al suggest that the prediction model can be used to guide the detection of postoperative complications and has clinical reference value.

The realization of a stable lithium-metal free(LiMF)sulfur battery based on amorphous carbon anode and lithium sulfide(Li_(2)S)cathode is here *** particular,a biomass waste originating full-cell combining a carbonized brewer's spent grain(CBSG)biochar anode with a Li_(2)S-graphene composite cathode(Li_(2)S70Gr30)is *** design is particularly attractive for applying a cost-effective,high performance,environment friendly,and safe anode material,as an alternative to standard graphite and metallic lithium in emerging battery *** anodic and cathodic materials are characterized in terms of structure,morphology and composition through X-ray diffraction,scanning and transmission electron microscopy,X-ray photoelectron and Raman ***,an electrochemical characterization comprising galvanostatic cycling,rate capability and cyclic voltammetry tests were carried out both in half-cell and full-cell *** systematic investigation reveals that unlike graphite,the biochar electrode displays good compatibility with the electrolyte typically employed in sulfur *** CBSG/Li_(2)S70Gr30 full-cell demonstrates an initial charge and discharge capacities of 726 and 537 mAh g^(-1),respectively,at 0.05C with a coulombic efficiency of 74%.Moreover,it discloses a reversible capacity of 330 mAh g^(-1)(0.1 C)after over 300 *** on these achievements,the CBSG/Li_(2)S70Gr30 battery system can be considered as a promising energy storage solution for electric vehicles(EVs),especially when taking into account its easy scalability to an industrial level.

Influence of sarcopenia and frailty in the management of elderly patients with acute appendicitis

In developed countries,the average life expectancy has been increasing and is now well over 80 *** life expectancy is associated with an increased number of emergency surgical procedures performed in later age *** appendicitis is one of the most common surgical diseases,with a lifetime risk of 8%.A growing incidence of acute appendicitis has been registered in the elderly population and in the oldest groups(>80 years).Among patients>50-year-old who present to the emergency department for acute abdominal pain,15%have acute *** these patients,emergency surgery for acute appendicitis is challenging,and some important aspects must be *** the elderly,surgical treatment outcomes are influenced by *** must be considered a precursor of frailty,a risk factor for physical function *** has a negative impact on both elective and emergency surgery regarding mortality and *** from morbidity and mortality,the most crucial outcomes for older patients requiring emergency surgery are reduction in function decline and preoperative physical function ***,prediction of function decline is *** emergency surgery,preoperative interventions are difficult to implement because of the narrow time window before *** this editorial,we highlight the unique aspects of acute appendicitis in elderly patients and the influence of sarcopenia and frailty on the results of surgical treatment.
