Men with a complete absence of normal sperm morphology exhibit high rates of success without assisted reproduction

In couples with infertility, abnormal strict morphology of 0% normal forms （NF） is a criterion to proceed rapidly to in vitro fertilization （IVF）. Since no data currently exist, we investigated the outcomes for men with 0% NF to determine reproductive success without the use of assisted reproductive technologies （ART）. A cohort of 24 men with 0% NF were identified （2010-2013） with 27 randomly selected men with 〉4% NF as controls. Patient charts were reviewed with men contacted and administered an Institutional Review Board （IRB）-approved telephone questionnaire to ascertain outcomes. After a median follow-up time of 2.5 years, 29.2% of men with 0% NF did not require ART for their first pregnancy （controls = 55.6%, P〈 0.05）. When all pregnancies were analyzed together, men with 0% NF achieved twenty pregnancies of which 75% did not require IVF （controls - thirty pregnancies; 76.7% did not require IVF）. The average age of men and female partners was similar between men with 0% NF and 〉4% NF. All men had normal follicle-stimulating hormone （FSH）, testosterone, prolactin, sex hormone-binding globulin （SHBG）, and estradiol. Although, global semen parameters were worse in men with 0% NF, when a first pregnancy was a natural conception （NC）, 100% of men with 0% NF （n = 7/7） and 37.5% of controls in = 3/8） went on to have a subsequent pregnancy via NC. Men with 0% NF conceived without IVF in 29.2% of cases compared to 55.6% of controls. Strict morphology should not be used to predict fertilization, pregnancy, or live birth potential. In men with 0% NF, alternative modalities should be considered before immediate IVF.

Biliary rhabdomyosarcoma (BRMS) is an uncommon childhood malignancy which has been managed surgically. We present a case of a 3-year-old boy with BRMS, in whom endoscopic retrograde cholangiopancreatography (ERCP) was successfully used both diagnostically and therapeutically, thus obviating the need for surgery and its attendant risks of morbidity and mortality. We conclude that ERCP is an effective alternative to surgery for BRMS in some patients.

Hearing loss is the most common neurosensory *** results froma variety of heritable and acquired causes and is linked to multiple deleterious effects on a child’s development that can be ameliorated by prompt identification and individualized *** hearing loss in newborns is challenging,especially in mild or progressive cases,and its management requires a multidisciplinary team of healthcare providers comprising audiologists,pediatricians,otolaryngologists,and genetic *** physiologic newborn hearing screening has resulted in earlier diagnosis of hearing loss than ever before,a growing body of knowledge supports the concurrent implementation of genetic and cytomegalovirus testing to offset the limitations inherent to a singular screening *** this review,we discuss the contemporary role of screening for hearing loss in newborns as well as future directions in its diagnosis and treatment.

Ultrasound-Guided Dorsal Scapular Nerve Blockade in the Diagnosis and Management of Neck Pain

Neck pain is common and has multiple sources, but correct diagnosis and matched treatment provide the best outcomes. The first description of ultrasound-guided dorsal scapular nerve blockade using a single-shot local anesthetic technique for the diagnosis and treatment of neck pain is reported. A 38-year-old female patient presented with neck pain, and the history and clinical examination strongly suggested myofascial pain affecting the middle scalene muscle. The pain had been unresponsive to pharmacological therapy or physiotherapy. After identifying the dorsal scapular nerve (DSN) in the body of the middle scalene muscle, an ultrasound-guided nerve block was performed using a single inj.ction of local anesthetic to alleviate the patient’s pain. It has been demonstrated that the dorsal scapular nerve can be identified in the neck and effectively blocked using ultrasound guidance. This technique has the potential to assist in the diagnosis and treatment of neck pain originating from the middle scalene muscle.

WHAT ARE THE DIFFERENCES OF POLYMER SURFACE RELAXATION FROM THE BULK?

The polymer surface relaxation in thin films has been a long debating issue. We report a new method on studying surface relaxation behaviors of polymer thin films on a solid substrate. This method involved utilizing a rubbed polyimide surface with a pretilting angle in a liquid crystalline cell. Due to the surface alignment, the liquid crystals were aligned along the rubbing direction. During heating the liquid crystalline cell, we continuously monitored the change of orientation of the liquid crystals. It is understood that at a temperature where the orientation of liquid crystal is lost, the surface relaxation on the glass substrate takes place to lose the polyimide surface orientation. It was found that the relaxation temperature at which the liquid crystals lose their orientation depends on the film thickness of the polyimide. A quantitative linear relationship between the relaxation temperature and reciprocal of the film thickness can be observed. Furthermore, different topologies of the rubbed and relaxed thin films were amplified using the polyethylene decoration method and observed using atomic force microscopy.

Comprehensive studies identify motor neuron spectrum disorders including amyotrophic lateral sclerosis(ALS)as globally rising fatal disorders with the highest prevalence in aging populations,influenced by ethnicity and ancestry(GBD 2016 Motor Neuron Disease Colla borators,2018).While~10% of diagnoses involve a family history(fALS),most cases are considered sporadic(sALS).However,population-based studies suggest that even cases without a common index mutation impart heritability(ryan et al.,2019),indicating a crucial role of rare and as yet unknown genetic denominators.

Background:Retreatments are sometimes necessary to correct residual or induced refractive errors following refractive *** different combinations of primary treatment methods and retreatment techniques have been studied,however,few studies have investigated wavefront-optimized(WFO)technology for retreatment following primary refractive *** study aimed to report the outcomes of WFO photorefractive keratectomy(PRK)retreatments of refractive error following previous laser refractive surgery with PRK,laser in situ keratomileusis(LASIK),or laser-assisted subepithelial keratectomy(LASEK).Methods:We reviewed records of patients who underwent WFO PRK retreatments using the Allegretto Wave Eye-Q 400 Hz Excimer Laser System(Alcon Surgical)between j.nuary 2008 and April 2011 at Walter Reed Army Medical Center and Madigan Army Medical *** were recorded in terms of uncorrected distance visual acuity(UDVA),manifest refraction spherical equivalent(MRSE),corrected distance visual acuity(CDVA),and complications at 1 month(M),3 M,and 6 M ***:Seventy-eight patients(120 eyes)underwent WFO PRK retreatment during the study *** surgery was surface ablation in 87 eyes(78 PRK,9 LASEK)and LASIK in 33 *** mean spherical equivalent before retreatment was−0.79±0.94 D(−3.00 to 1.88 D).UDVA was≥20/20 in 69 eyes(60.0%)at 1 M,54 eyes(71.1%)at 3 M,and 27 eyes(73.0%)at 6 M *** was within±0.50 D of emmetropia in 78 eyes(67.8%)at 1 M,59 eyes(77.6%)at 3 M,and 25 eyes(67.6%)at 6 M *** was maintained within±1 line of pre-op in 113 of 115 eyes(98.3%)at 1 M,74 of 76 eyes(97.4%)at 3 M,and 37 eyes(100%)at 6 M ***:Although follow-up was limited beyond 3 M,WFO PRK retreatments in patients with residual refractive error may be a safe and effective *** studies are necessary to determine the long-term safety and stability of outcomes.

Modern societal pressures and expectations over the past several decades have resulted in the tendency for couples to delay conception. While women experience a notable decrease in oocyte production in their late thirties, the effect of age on spermatogenesis is less well described. While there are no known limits to the age at which men can father children, the effects of advanced paternal age are incompletely understood. This review summarizes the current state of knowledge regarding advanced paternal age and its implications on semen quality, reproductive success and offspring health. This review will serve as a guide to physicians in counseling men about the decision to delay paternity and the risks involved with conception later in life.

Alosa pseudoharengus is an anadromous fish that migrates from marine to freshwaters to spawn. The early larval and juvenile forms are known to be planktivorous, where heavy feeding upon their preferred food source of large crustacean zooplankton often results in changes to composition and size structure within this trophic guild which in turn can result in shifts within the trophic spectrum and a classic trophic cascade. In this study of Lower Mill Pond, Brewster MA, we evaluated the feeding strategy of juvenile Alosa to determine whether juvenile alewife switches to feeding largely on cyanobacteria and whether cyanotoxins microcystin (MC) and β-methlyamino-L-alanine (BMAA) bioaccumulate in their muscle tissue. Within 15 - 30 days of their estimated spawning date, overexploitation of crustacean zooplankton resulted in a shift from planktivory to benthic detritivory for the majority of their life history, although this did not reduce their condition based on weight-length relationships (Log Wwt. = -5.503 + (3.101 × Log Length).
