Backg.ound:Data on the prevalence of human papillomavirus(HPV)infection in patients with penile lichen sclerosus(LS)-are scant and ***:To investig.te the prevalence of HPV infections in patients with penile ***:HPV infection was assessed by polymerase chain reaction(PCR)-in paraffin-embedded penile biopsies obtained from the g.ans or inner foreskin of 46 adult patients with penile LS,and in brush cytolog. smears of penile healthy mucosa from an equal number of randomly selected control males matched for *** evaluation was performed using.conditional log.stic reg.ession ***:PCR disclosed the presence of HPV infection in17.4%of LS patients(HPV 16,six cases;HPV 18,one case;HPV 45,one case).Among.t the controls,HPV infection occurred in 8.7%of patients(HPV 16,two cases;HPV 53,one case;HPV 70,one case).Statistical reg.ession analysis confirmed that the rate of HPV infection was hig.er among.t patients with g.nital LS than among.t healthy controls [odds ratio(OR),2.55;95%confidence interval(CI),0.73-8.89].Conclusions:Infection with oncog.nic “hig.-risk”HPV types in patients with g.nital LS may enhance the risk of penile cancer arising.on LS.

Several studies have compared prostate-specific antig.n （PSA） kinetics in men with and without cancer, but there has been no adequate analysis of the long.tudinal variation in PSA. The aim of this study was to assess the fluctuations in PSA in a cohort of elderly men in an attempt to define a physiolog.cal pattern of PSA kinetics. We searched a specific cohort of patients ag.d 〉 75 years and with PSA value 〈 2.0 ng.mL^-1. A history of all PSA values over the past 10 years was compiled for each patient to create a database of patients fitting.the following.criteria：（1） minimum of five PSA measurements, （2） over at least 5 years. Exclusion criteria were：（1） PSA 〈 0.2 ng.mL^-1 at each measurement and （2） having.had more than one PSA test per year. In all, 1 327 patients （mean ag.： 78.52 years） fit the inclusion criteria. The mean variation from the first to the last PSA test was 0.05 ± 0.43, with a mean follow-up of 6.79 ± 1.71 years. Over the same period, the mean fluctuation from the lowest to the hig.est PSA value was 0.04 ± 0.55 （P = 0.925）. The mean annual PSA velocity （PSAV） was calculated by dividing.the mean variation from the first to the last PSA test by the number of years of observation for each patient and was set at 0.0104 ± 0.1050. Concluding. in a larg.-scale cohort of elderly individuals considered healthy and evaluated for a considerable follow-up, the averag. annual PSAV as well as the averag. fluctuation from the lowest to the hig.est PSA value are insig.ificant.

Transperineal prostate biopsy is a procedure that can be used to obtain histolog.cal samples from the prostate. To improve both the quality of the biopsy core samples and prostate cancer detection, we are currently performing.a prospective, randomized trial comparing.prostate biopsy samples obtained using.an 18 g.needle to those obtained using.a 16 g.needle. The aim of this preliminary study was to evaluate pain and complication rates in both g.oups in order to assess whether performing.a prostate biopsy with a larg.r calibre needle is a feasible procedure. One hundred and eig.ty-seven patients underg.ing.transperineal prostate biopsy were prospectively evaluated and divided into two g.oups. The first g.oup （94 patients, g.oup A） received a transperineal prostate biopsy using.a 16 g.needle and the second g.oup （93 patients, g.oup B） underwent transperineal prostate biopsy with an 18 g.needle. Anaesthesia was obtained with a sing.e perineal injection at the prostatic apex in all subjects. A visual analog.e scale （VAS） and facial expression scale （FES） were used to assess pain during.multiple steps of the procedure in each g.oup. A detailed questionnaire was used to obtain information about drug.use because it could potentially influence the pain and complications that patients experienced. Two weeks after the procedure, early and late complications were evaluated. Statistical analysis was carried out using.non-parametric tests. Prostate Specific Antig.n （PSA） and drug.use were similar at baseline between the two g.oups. Pain during.prostate biopsy, which was measured with both the VAS and FES instruments, did not differ sig.ificantly between the 18- and 16 g.needle g.oups, and no sig.ificant differences were found in early or late complication rates between the g.oups. We present the case of a 55-year-old man admitted for dyspnea, coug. and edema of the leg. and feet. The patient, a smoker with psoriasis at hands and feet noted the comparison, three years ag., of marked thickening.and yellow discoloration of the nails. The chest X-ray showed a rig.t-sided pleural effusion with parenchymal atelectasis. The cytolog.c examination of pleural fluid revealed mesothelial cells with inflammatory chang.s. The bacterial culture of the pleural fluid was neg.tive and pleural biopsies revealed fibrosis. To reduce dyspnoea and formation of pleural fluid, was performed a pleurodesis by talcag.. The patient was discharg.d with itraconazole, hig. dose vitamin E and zinc supplements as home therapy. The correct diag.osis was not easily achieved because the patient had symptoms and sig.s referable to other diseases as heart failure or venous insufficiency or psoriasis.

The aim of this study was to evaluate the effectiveness of low molecular weig.t hyaluronic acid oral tablets for the treatment of atrophic vag.nitis. 12 women with atrophic vag.nitis were recruited for this double blind, placebo-controlled study. Patients were randomized to receive either low molecular weig.t hyaluronic acid (HA) oral tablets or placebo for three months. Vag.nal biopsies were taken at baseline and after three months of treatment, and vag.nal epithelium was analysed using.lig.t microscopy. The evaluation of symptoms was self-assessed by the patients. Biopsies from HA g.oup showed an epithelium thicker than in placebo g.oup. The lamina propria from the HA g.oup also showed a denser appearance compared to placebo g.oup. Morphometric analysis showed sig.ificant differences between HA and baseline in the number of epithelial layers and in the thickness. The evaluation of symptoms also showed an effective improvement in the patients treated with HA, compared to baseline and to placebo g.oup. In conclusion, oral administration of HA tablets improved the vag.nal epithelium, decreasing.atrophy. This can be an ideal option for patients with atrophic vag.nitis who do not want to or can not take estrog.n and show low compliance toward vag.nal administration.

在加密函数存在的假设下,我们证明了NP中的全部语言都拥有零知识证明,就是说能证明一个CNF公式是可满足的而不泄露该公式的任何其它特性,特别是,不产生满足的賦值,也不产生更弱的一些特性,比如是否存在一个满足的賦值,其中x1=真,或者是否存在一个满足的赋值,其中x_1=x_3等。上述结果使我们能够证明密码协议领域(两方及多方)里的两个基本定理。这两个定理提供自动且有效的变换,当给定一个在极弱敌情下无误的协议时,输出一个在极强敌情下也无误的协议。这样,这两个定理指明了发展两方及多方密码协议的一些强有力的方法。
