Intestinal microcirculation dysfunction in sepsis: pathophysiology, clinical monitoring, and therapeutic interventions

BACKGROUND:Intestinal microcirculation dysfunction is an important factor that causes poor prognosis in sepsis patients and is an important pathophysiological basis for the occurrence and development of sepsis.DATA RESOURCES:PubMed,Web of Science,and China National Knowledge Infrastructure(CNKI)were searched from inception to August 1,2021.The search was limited to the English language only.Two reviewers independently identified studies related to intestinal microcirculation dysfunction in sepsis.Exclusion criteria were duplicate articles according to multiple search criteria.RESULTS:Fifty articles were included,and most of them were animal studies.These studies reported pathogenesis,including endothelial dysfunction,leukocyte recruitment and adhesion,microthrombus formation,microcirculation hypoperfusion,and redistribution of intestinal wall blood flow.The monitoring methods of intestinal microcirculation were also diverse,including handheld microscopes,intravital microscopy(IVM),laser Doppler blood flow instruments,laser speckle contrast imaging,tissue refl ectance spectrophotometry,biochemical markers of intestinal ischemia,and histopathological examination.In view of the related pathogenesis of intestinal microcirculation disorder in sepsis,existing studies also have diff erent opinions on its treatment.CONCLUSIONS:Limited by monitoring,there are few clinical studies on intestinal microcirculation dysfunction in sepsis.Related research mainly focuses on basic research,but some progress has also been made.Therefore,this review may provide a reference for future research on intestinal microcirculation dysfunction in sepsis.

Trichostatin A improves the inflammatory response and liver injury in septic mice through the FoxO3a/autophagy signaling pathway

BACKGROUND:Sepsis-induced liver injury is a fatal complication of sepsis.Trichostatin A(TSA)regulates inflammation and autophagy in some human diseases,and forkhead box O3a(FoxO3a)has been shown to regulate autophagy.The present study aims to investigate whether TSA exerts its effects on septic liver injury through the FoxO3a/autophagy signaling pathway.METHODS:A sepsis mouse model was constructed by the cecal ligation and puncture(CLP)method,and AML12 cells were pretreated with lipopolysaccharide(LPS)(1μg/mL)to establish a sepsis cell model.Forty mice were divided into four groups,namely control group,TSA group,CLP group,and CLP+TSA group,with 10 mice in each group.Cells were divided into control group,TSA group,LPS group,and LPS+TSA group.Hematoxylin-eosin(H&E)staining and biochemical methods were used to evaluate liver tissue injury.Enzyme-linked immunosorbent assay(ELISA)was applied to detect the expression of proinflammatory cytokines,and Western blotting and immunofluorescence were used to measure autophagy-related protein expression.RESULTS:Compared with the CLP group(mice),the proinflammatory cytokines(interleukin-β[IL-β]2,665.27±324.90 pg/mL to 2,080.26±373.66 pg/mL;interleukin-6[IL-6]399.01±60.98 pg/mL to 221.90±46.89 pg/mL)and the hepatocyte injury markers(aspartate transaminase[AST]from 198.18±27.07 U/L to 128.42±20.55 U/L;alanine aminotransferase[ALT]from 634.98±74.10 U/L to 478.60±32.56 U/L)were notably decreased after TSA intervention.Moreover,LC3 II and FoxO3a showed an obvious increase and P62 showed an obvious decrease in the CLP+TSA group.Cell experiment results showed the similar trend.After Fox O3a gene was knocked down in AML12 cells,the promotion of autophagy and the improvement of liver enzyme index and inflammation by TSA were weakened.CONCLUSION:TSA may improve the inflammatory response and liver injury in septic mice through Fox O3a/autophagy.

Metadherin promotes stem cell phenotypes and correlated with immune infiltration in hepatocellular carcinoma

BACKGROUND Metadherin(MTDH)is a key oncogene in most cancer types,including hepato-cellular carcinoma(HCC).Notably,MTDH does not affect the stemness pheno-type or immune infiltration of HCC.AIM To explore the role of MTDH on stemness and immune infiltration in HCC.METHODS MTDH expression in HCC tissues was detected using TCGA and GEO databases.Immunohistochemistry was used to analyze the tissue samples.MTDH was stably knocked down or overexpressed by lentiviral transfection in the two HCC cell lines.The invasion and migration abilities of HCC cells were evaluated using Matrigel invasion and wound healing assays.Next,we obtained liver cancer stem cells from the spheroids by culturing them in a serum-free medium.Gene expression was determined by western blotting and quantitative reverse transcri-ption PCR.Flow cytometry,immunofluorescence,and tumor sphere formation assays were used to characterize stem-like cells.The effects of MTDH inhibition on tumor growth were evaluated in vivo.The correlation of MTDH with immune cells,immunomodulators,and chemokines was analyzed using ssGSEA and TISIDB databases.RESULTS HCC tissues expressed higher levels of MTDH than normal liver tissues.High MTDH expression was associated with a poor prognosis.HCC cells overex-pressing MTDH exhibited stronger invasion and migration abilities,exhibited a stem cell-like phenotype,and formed spheres;however,MTDH inhibition attenuated these effects.MTDH inhibition suppressed HCC progression and CD133 expression in vivo.MTDH was positively correlated with immature dendritic,T helper 2 cells,central memory CD8^(+)T,memory B,activated dendritic,natural killer(NK)T,NK,activated CD4^(+)T,and central memory CD4^(+)T cells.MTDH was negatively correlated with activated CD8^(+)T cells,eosinophils,activated B cells,monocytes,macrophages,and mast cells.A positive correlation was observed between the MTDH level and CXCL2 expression,whereas a negative correlation was observed between the MTDH level and CX3CL1 and CXCL12 expression.CONCLUSION High levels of M

Anti-OX40 Antibody Combined with HBc VLPs Delays Tumor Growth in a Mouse Colon Cancer Model

Objective Combination immunotherapy strategies targeting OX40,a co-stimulatory molecule that can enhance antitumor immunity by modulating the proliferation,differentiation,and effector function of tumor-infiltrating T cells,have attracted much attention for their excellent therapeutic effects.In this study,we aimed to evaluate the antitumor efficacy of combined anti-OX40 and hepatitis B core viruslike particles(HBc VLPs)therapy using a mouse colon cancer model.Methods Humanized B-h OX40 mice were injected subcutaneously with MC38 colon tumor cells and treated with HBc VLPs+anti-h OX40 antibody.Tumor growth was monitored.Flow cytometric analysis was performed to evaluate the populations of T cell subsets in the tumors.Results The combination of anti-OX40 with HBc VLPs resulted in a significant delay in tumor growth,suggesting that a potent antitumor immunity was induced by the combination therapy.Further studies revealed that HBc VLPs+anti-OX40 treatment induced a significant increase in effector T cells(Teffs)and a significant decrease in regulatory T cells(Tregs)in the tumor microenvironment(TME),which accounted for the synergistic antitumor effect of anti-OX40 in combination with HBc VLPs.Conclusion Combination therapy of anti-h OX40 and HBc VLPs provides synergistic antitumor activity in colon cancer-bearing mice,which may represent a potential design strategy for cancer immunotherapy.

Design method and experimental study of a cathode tool with an extremely high leveling ratio for electrochemical machining of blisk

To obtain final parts with the desired dimensional accuracy and repeatability via electrochemical machining(ECM), the machining process must enter an ECM balanced state. However,for the ECM processing of blisk, a key component of aerospace engines, the surface of the blade blank often has an uneven allowance distribution due to the narrow passage of the cascade. It is difficult to remedy this issue in subsequent processing steps, which is necessary to ensure the dimensional accuracy and repeatability of the final blade profile. To solve this problem, electrolytic machining must be preceded by electrolytic shaping, which requires cathode tools with large leveling ratios to quickly homogenize the blank surface of the blade. In this study, to obtain a cathode tool with an extremely high leveling ratio, a design method based on the variation in the electrode gap in the non-equilibrium electrolytic state is proposed, and a dissolution model based on the nonequilibrium state is established. In this design method, the allowance on the blank to be machined is first divided into many discrete allowances with the normal direction. The initial machining clearance, feed rate, and total machining time are then calculated using classical ECM equilibrium state theory based on the maximum allowance. Meanwhile, the point coordinates of the cathode tool at maximum allowance can be determined. The non-equilibrium model can then be used to calculate the relative coordinate positions corresponding to the remaining discrete allowances. Finally, the entire cathode tool profile is designed. Simulations, fundamental experiments, and blisk unit workpiece experiments were carried out to validate the design approach. In the simulated processing of the plane workpiece, the leveling ratio of the cathode tool designed by the proposed method(0.77)was 83% higher than that of the cathode tool designed using the traditional method. Evaluation of the effects of health education interventions for hypertensive patients based on the health belief model

BACKGROUND Hypertension is a major risk factor for cardiovascular disease and stroke,and its prevalence is increasing worldwide.Health education interventions based on the health belief model(HBM)can improve the knowledge,attitudes,and behaviors of patients with hypertension and help them control their blood pressure.AIM To evaluate the effects of health education interventions based on the HBM in patients with hypertension in China.METHODS Between 2021 and 2023,140 patients with hypertension were randomly assigned to either the intervention or control group.The intervention group received health education based on the HBM,including lectures,brochures,videos,and counseling sessions,whereas the control group received routine care.Outcomes were measured at baseline,three months,and six months after the intervention and included blood pressure,medication adherence,self-efficacy,and perceived benefits,barriers,susceptibility,and severity.RESULTS The intervention group had significantly lower systolic blood pressure[mean difference(MD):-8.2 mmHg,P<0.001]and diastolic blood pressure(MD:-5.1 mmHg,P=0.002)compared to the control group at six months.The intervention group also had higher medication adherence(MD:1.8,P<0.001),self-efficacy(MD:12.4,P<0.001),perceived benefits(MD:3.2,P<0.001),lower perceived barriers(MD:-2.6,P=0.001),higher perceived susceptibility(MD:2.8,P=0.002),and higher perceived severity(MD:3.1,P<0.001)than the control group at six months.CONCLUSION Health education interventions based on the HBM effectively improve blood pressure control and health beliefs in patients with hypertension and should be implemented in clinical practice and community settings.

A Weakly-Supervised Crowd Density Estimation Method Based on Two-Stage Linear Feature Calibration

In a crowd density estimation dataset,the annotation of crowd locations is an extremely laborious task,and they are not taken into the evaluation metrics.In this paper,we aim to reduce the annotation cost of crowd datasets,and propose a crowd density estimation method based on weakly-supervised learning,in the absence of crowd position supervision information,which directly reduces the number of crowds by using the number of pedestrians in the image as the supervised information.For this purpose,we design a new training method,which exploits the correlation between global and local image features by incremental learning to train the network.Specifically,we design a parent-child network(PC-Net)focusing on the global and local image respectively,and propose a linear feature calibration structure to train the PC-Net simultaneously,and the child network learns feature transfer factors and feature bias weights,and uses the transfer factors and bias weights to linearly feature calibrate the features extracted from the Parent network,to improve the convergence of the network by using local features hidden in the crowd images.In addition,we use the pyramid vision transformer as the backbone of the PC-Net to extract crowd features at different levels,and design a global-local feature loss function(L2).We combine it with a crowd counting loss(LC)to enhance the sensitivity of the network to crowd features during the training process,which effectively improves the accuracy of crowd density estimation.The experimental results show that the PC-Net significantly reduces the gap between fullysupervised and weakly-supervised crowd density estimation,and outperforms the comparison methods on five datasets of Shanghai Tech Part A,ShanghaiTech Part B,UCF_CC_50,UCF_QNRF and JHU-CROWD++.

Preparation and characterization of Rb-doped TiO_(2) powders for photocatalytic applications

TiO_(2)nanoparticles doped with different concentrations of Rb were prepared by sol–gel method.The crystal structures of samples were characterized by X-ray diffraction(XRD)and transmission electron microscopy(TEM).It was confirmed by the Rietveld refinement method that the as-prepared samples crystallize in anatase phase and Rb doping induces expansion and distortion of the crystal lattice.Compared with the pure TiO_(2),the Rb-doped nanoparticles have smaller crystal size and better dispersibility.Degradation rate of methylene blue was used to evaluate the photocatalytic activity of Rb–TiO_(2).It indicated that doping Rb into TiO_(2)can effectively improve the photocatalytic activity of TiO_(2)and the mechanisms were discussed in detail.When using the sample doped with 0.20%Rb,methylene blue degraded with the fastest rate(97%within 60 min).

BACKGROUND Radiotherapy stands as a promising therapeutic modality for colorectal cancer(CRC);yet,the formidable challenge posed by radio-resistance significantly undermines its efficacy in achieving CRC remission.AIM To elucidate the role played by microRNA-298(miR-298)in CRC radio-resistance.METHODS To establish a radio-resistant CRC cell line,HT-29 cells underwent exposure to 5 gray ionizing radiation that was followed by a 7-d recovery period.The quantification of miR-298 levels within CRC cells was conducted through quantitative RT-PCR,and protein expression determination was realized through Western blotting.Cell viability was assessed by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay and proliferation by clonogenic assay.Radio-induced apoptosis was discerned through flow cytometry analysis.RESULTS We observed a marked upregulation of miR-298 in radio-resistant CRC cells.MiR-298 emerged as a key determinant of cell survival following radiation exposure,as its overexpression led to a notable reduction in radiation-induced apoptosis.Intriguingly,miR-298 expression exhibited a strong correlation with CRC cell viability.Further investigation unveiled human dual-specificity tyrosine(Y)-regulated kinase 1A(DYRK1A)as miR-298’s direct target.CONCLUSION Taken together,our findings underline the role played by miR-298 in bolstering radio-resistance in CRC cells by means of DYRK1A downregulation,thereby positioning miR-298 as a promising candidate for mitigating radioresistance in CRC.

Defect Detection in c-Si Photovoltaic Modules via Transient Thermography and Deconvolution Optimization

Defects may occur in photovoltaic(PV)modules during production and long-term use,thereby threatening the safe operation of PV power stations.Transient thermography is a promising defect detection technology;however,its detection is limited by transverse thermal diffusion.This phenomenon is particularly noteworthy in the panel glasses of PV modules.A dynamic thermography testing method via transient thermography and Wiener filtering deconvolution optimization is proposed.Based on the time-varying characteristics of the point spread function,the selection rules of the first-order difference image for deconvolution are given.Samples with a broken grid and artificial cracks were tested to validate the performance of the optimization method.Compared with the feature images generated by traditional methods,the proposed method significantly improved the visual quality.Quantitative defect size detection can be realized by combining the deconvolution optimization method with adaptive threshold segmentation.For the same batch of PV products,the detection error could be controlled to within 10%.
