Preoperative Serum albumin Levels and Postoperative Acute Kidney Injury in Off-Pump Coronary Artery Bypass Surgery: A Single-Center Study in Bangladesh

Background: Serum albumin, a vital plasma protein, helps maintain intravascular colloidal osmotic pressure, cardiac output, and renal function. Low preoperative serum albumin is linked to poor outcomes, including acute kidney injury (AKI), after off-pump coronary artery bypass (OPCAB) surgery. This study aimed to assess the relationship between preoperative serum albumin levels and early postoperative renal injury. Methods: This prospective comparative cross-sectional study was conducted from August 2019 to February 2021 at the National Heart Foundation Hospital & Research Institute, Bangladesh. It included 160 adult patients with normal preoperative renal function undergoing OPCAB. Patients were divided into two groups: Group A (serum albumin ≥ 4.0 gm/dl) and Group B (serum albumin Results: Preoperative serum albumin was significantly different between groups (Group A: 4.21 ± 0.05 gm/dl, Group B: 3.69 ± 0.04 gm/dl, p = 0.028). Group B had a higher incidence of hypertension (71.25% vs. 51.25%, p st and 3rd postoperative days were higher in Group B (p th day. Postoperative AKI occurred in 18.75% of Group A and 36.25% of Group B. Multivariate regression indicated that low preoperative serum albumin is an independent risk factor for postoperative AKI (p = 0.012, OR = 1.815, CI: 0.675 - 1.162). Conclusion: Preoperative serum albumin level is a valuable predictor of postoperative renal function. Ensuring high normal serum albumin levels before surgery can help minimize the risk of postoperative AKI.

Off-Pump Coronary Artery Bypass Grafting in Patients with Left Ventricular Dysfunction: Short-Term Results from a Single Center in Bangladesh

Background: Off-pump coronary artery bypass grafting (OPCAB) is considered a safer alternative to on-pump surgery, especially in patients with left ventricular dysfunction (LVD). Objectives: This study assessed short-term outcomes and functional improvements in LVD patients post-OPCAB. Methods: The study included 200 coronary artery disease patients who underwent isolated off-pump coronary artery bypass grafting (OPCAB) at the National Heart Foundation Hospital and Research Institute between January 2019 and June 2020. Patients were categorized into Group 1, with a left ventricular ejection fraction (LVEF) of 30% - 39%, and Group 2, with an LVEF of 40% or higher. Echocardiographic assessments of left ventricular dimensions and ejection fraction were performed preoperatively, at discharge, and one month postoperatively. Results: In Group 1, preoperative left ventricular internal dimensions during diastole (LVIDd) and systole (LVIDs) were 53.48 ± 4.40 mm and 44.23 ± 3.93 mm, respectively, with a left ventricular ejection fraction (LVEF) of 35.28% ± 2.26%. At discharge, these values improved to 51.58 ± 4.04 mm (LVIDd), 41.23 ± 5.30 mm (LVIDs), and 39.25% ± 3.75% (LVEF). One month postoperatively, further improvements were observed: 46.29 ± 3.76 mm (LVIDd), 37.45 ± 3.68 mm (LVIDs), and 43.22% ± 4.67% (LVEF). Group 2 showed similar positive outcomes, with preoperative values of 47.09 ± 5.06 mm (LVIDd), 35.11 ± 5.25 mm (LVIDs), and 50.13% ± 7.25% (LVEF), improving to 42.37 ± 4.18 mm (LVIDd), 31.05 ± 4.19 mm (LVIDs), and 55.33% ± 7.05% (LVEF) at one month postoperatively. Both groups demonstrated significant improvements in left ventricular function and NYHA class, with most patients moving from class III/IV to I/II. Complications were minimal, and no mortality was observed. Conclusion: OPCAB is safe and effective for patients with LVEF 30% - 39% and LVEF ≥ 40%, providing significant short-term functional improvements without increased risk.

Tricuspid annular plane systolic excursion has been proposed as a simple and reproducible parameter for quantitative assessment of the right ventricular ejection fraction. The prognostic importance of preoperative TAPSE in patients with mitral valve replacement for rheumatic mitral stenosis patients is still under focused. Therefore, the objective of the study was to predict the outcome after MVR in rheumatic mitral stenosis patients in relation to preoperative TAPSE. This comparative cross-sectional study was conducted at the Department of Cardiac Surgery, National Heart Foundation Hospital and Research Institute. A total of 72 patients of rheumatic mitral stenosis patients who underwent mitral valve replacement were included in the study. They were divided into two groups: Group A and B. Group A included 36 patients with TAPSE 0.05) except for the preoperative TAPSE. Mean TAPSE of Group A was 13.17 (±1.40) and Group B was 18.61 (±1.57), the difference was statistically significant (p 0.05). Among the postoperative complications, including postoperative atrial fibrillation was higher in Group A (30.56%) than Group B (11.11%), mean ventilation time was higher in Group A (27.78%) than Group B (5.56%), length of intensive care was higher in Group A (33.33%) than Group B (11.12%), and hospital stay was higher in Group A (25.0%) than Group B (5.56%), (p al stenosis patients in our settings.

Background Opisthorchiid flukes, particularlyOpisthorchis viverrini,Opisthorchis felineus,Clonorchis sinensis, andMetorchis spp. are the most common fish-borne zoonotic human liver flukes (hLFs). Liver fluke infections are more prevalent in resource-deprived and underprivileged areas. We herein estimated the prevalence of the metacercariae (MC) of major hLFs in common large freshwater fishes (lFWF) marketed for human consumption from some selected areas of Bangladesh along with detection of their molluscan vectors and *** The current status of fish-borne zoonotic hLF infections in lFWF was investigated along with their molluscan vectors and mammalian reservoir hosts in Mymensingh and Kishoreganj in Bangladesh from July 2018–June 2022 using conventional and multiple molecular techniques, such as PCR, PCR-restriction fragment length polymorphism (RFLP), sequencing, and bioinformatic analyses. The infection rate of fishes was analyzed using theZ-test and the loads of MC were compared using the chi-squared (χ2) *** The MC ofC. sinensis,Opisthorchis spp., andMetorchis spp. were detected in 11 species of common and popular lFWF. In lFWF, the estimated prevalence was 18.7% and the mean load was 137.4 ± 149.8 MC per 100 g of fish. The prevalence was the highest (P almost equally (P > 0.05) distributed between the head and body of fishes. The infection rate was slightly higher in cultured (19.6%) fishes. The MC ofC. sinensis,O. felineus,O. viverrini, andMetorchis orientalis in fishes were confirmed using PCR, PCR-RFLP and bioinformatics. The cercariae of opisthorchiid (Pleurolophocercus cercariae) flukes were only recovered fromBithynia spp. (3.9%, 42 out of 1089). Adequate Closed Form Wave Solutions to the Generalized KdV Equation in Mathematical Physics

In this paper, we consider the generalized Korteweg-de-Vries (KdV) equations which are remarkable models of the water waves mechanics, the shallow water waves, the quantum mechanics, the ion acoustic waves in plasma, the electro-hydro-dynamical model for local electric field, signal processing waves through optical fibers, etc. We determine the useful and further general exact traveling wave solutions of the above mentioned NLDEs by applying the exp(−τ(ξ))-expansion method by aid of traveling wave transformations. Furthermore, we explain the physical significance of the obtained solutions of its definite values of the involved parameters with graphic representations in order to know the physical phenomena. Finally, we show that the exp(−τ(ξ))-expansion method is convenient, powerful, straightforward and provide more general solutions and can be helping to examine vast amount of travelling wave solutions to the other different kinds of NLDEs.

Improving organizational performance can be achieved through business process redesign (BPR). When it comes to modern work practices, organizations must often redesign their processes to keep up. This is where enterprise software comes in, offering a common data infrastructure that enables different parts of the organization to access all necessary financial data. The architecture of network accounting business process is divided into three layers. The core layer lies in the enterprise LAN that it can integrate six business processes information into one unity of financial information and non-financial information, including purchasing, inventory, production, sales, finance and customer service, and it can generate flexibly a variety of accounting statements to meet fully the information users’ demands by providing more complete and more valuable related information. The second layer lies in Intranet among enterprises based on the core layer that it can support the link of business processes and the inter-sharing of information among node enterprises of SCM and other external enterprises. The third layer lies in Internet based on the second layer that it can fit society, enterprise and individual together, the specific lays the solid foundation of business collaboration, remote management, online management and centralized management. The operating mechanisms of the network accounting business processes are as follows. When the business event occurs, the probe can get the event information in real-time and the result is stored in the database. Meanwhile, the drive controller can control the business events in real-time by gaining control criteria and standards from database. In case the standard is exceeded, the event is not allowed to occur. Enterprise’ business staff or external personnel can output automatically the required results through report tools if they had the reading authority. Electric vehicles use electric motors, which turn electrical energy into mechanical energy. As electric motors are conventionally used in all the industry, it is an established development site. It’s a mature technology with ideal power and torque curves for vehicular operation. Conventional vehicles use oil and gas as fuel or energy storage. although they also have an excellent economic impact, the continuous use of oil and gas threatened the world’s reservation of total oil and gas. also, they emit carbon dioxide and some toxic ingredients through the vehicle’s tailpipe, which causes the greenhouse effect and seriously impacts the environment. So, as an alternative, electric car refers to a green technology of decarbonization with zero emission of greenhouse gases through the tailpipe. So, they can remove the problem of greenhouse gas emissions and solve the world’s remaining non-renewable energy storage problem. Pure electric vehicles (PEV) can be applied in all spheres, but their special implementation can only be seen in downhole operations. They are used for low noise and less pollution in the downhole process. In this study, the basic structure of the pure electric command vehicle is studied, the main components of the command vehicle power system, namely the selection of the drive motor and the power battery, are analyzed, and the main parameters of the drive motor and the power battery are designed and calculated. The checking calculation results show that the power and transmission system developed in this paper meets the design requirements, and the design scheme is feasible and reasonable.

Numerical Study and Optimization of CZTS-Based Thin-Film Solar Cell Structure with Different Novel Buffer-Layer Materials Using SCAPS-1D Software

This study explored the performances of CZTS-based thin-film solar cell with three novel buffer layer materials ZnS, CdS, and CdZnS, as well as with variation in thickness of buffer and absorber-layer, doping concentrations of absorber-layer material and operating temperature. Our aims focused to identify the most optimal thin-film solar cell structure that offers high efficiency and lower toxicity which are desirable for sustainable and eco-friendly energy sources globally. SCAPS-1D, widely used software for modeling and simulating solar cells, has been used and solar cell fundamental performance parameters such as open-circuited voltage (), short-circuited current density (), fill-factor() and efficiency() have been optimized in this study. Based on our simulation results, it was found that CZTS solar cell with Cd0.4Zn0.6S as buffer-layer offers the most optimal combination of high efficiency and lower toxicity in comparison to other structure investigated in our study. although the efficiency of Cd0.4Zn0.6S, ZnS and CdS are comparable, Cd0.4Zn0.6S is preferable to use as buffer-layer for its non-toxic property. In addition, evaluation of performance as a function of buffer-layer thickness for Cd0.4Zn0.6S, ZnS and CdS showed that optimum buffer-layer thickness for Cd0.4Zn0.6S was in the range from 50 to 150nm while ZnS offered only 50 – 75 nm. Furthermore, the temperature dependence performance parameters evaluation revealed that it is better to operate solar cell at temperature 290K for stable operation with optimum performances. This study would provide valuable insights into design and optimization of nanotechnology-based solar energy technology for minimizing global energy crisis and developing eco-friendly energy sources sustainable and simultaneously.

Nipah virus (NiV) is a highly infectious zoonotic pathogen that poses a significant threat to human and animal health. First identified in Malaysia in 1998, NiV has since caused several outbreaks in Southeast Asia, with sporadic cases reported in Bangladesh and India. The virus is primarily transmitted to humans through direct contact with infected animals, primarily fruit bats, or through the consumption of contaminated fruits and their juices. NiV infection presents a wide spectrum of clinical features, ranging from mild respiratory illness to severe encephalitis, with a high case fatality rate. The incubation period typically ranges from 4 to 14 days, during which patients develop fever, headache, myalgia, and respiratory symptoms such as cough and sore throat. As the disease progresses, neurological signs become prominent, including altered consciousness, seizures, and focal deficits. Severe cases may exhibit acute respiratory distress syndrome and multi organ failure. Laboratory findings often include lymphocytopenia, thrombocytopenia, and elevated liver enzymes. Diagnosis of NiV infection requires specialized laboratory testing, including reverse transcription-polymerase chain reaction (RT-PCR) and serological assays. Currently, no specific antiviral treatment exists for NiV infection, and management primarily focuses on supportive care. Prevention and control strategies encompass public health interventions, surveillance, and raising awareness among healthcare providers and the general population. The emergence and re-emergence of NiV highlight the urgent need for continued research, improved diagnostic capabilities, and the development of effective vaccines and therapeutics to mitigate the impact of this deadly virus.

Grass pea ( Lathyrus sativus L.) is an imperative food crop cultured in dryland agricultural ecology. It is a vital source of dietary protein to millions of populaces living in low-income countries in South-East Asia and Africa. This study highlights the improvement of genomic properties and their application in marker-trait relationships for 17 yield-related characters in 400 grass pea genotypes from China and Bangladesh. These characters were assessed via 56 polymorphic markers using general linear model (GLM) (P+G+Q) and mixed linear model (MLM) (P+G+Q+K) in the tassel software based on the linkage disequilibrium and population structure analysis. Population structure analysis showed two major groups and one admixed group in the populace. Statistically signi fi cant loci pairs of linkage disequilibrium (LD) mean value (D ' ) was 0.479. A total of 99 and 61 marker-trait associations in GLM and MLM models allied to the 17 traits were accepted at a 5% level of signi fi cance. Among these markers, 21 markers were associated with more than one trait; 12 marker-trait associations passed the Bonferroni correction threshold. Both models found six markers C41936, C39067, C34100, C47146, C47638, and C43047 signi fi cantly associated with days to maturity, fl ower color, plant height, and seed per pod were detected in the Hebei and Liaoyang location ( p alue) 11.2% to 43.6%. Conferring to the consequences, the association analysis methodology may operative system for quantitative, qualitative, and biochemical traits related to gene position mapping and support breeders in improving novel approaches for advancing the grass pea quality.
