Effective containment of the COVID-19 pandemic requires rapid and accurate detection of the *** chain reaction(PCR)remains the gold standard for COVID-19 *** this article,we report the performance of a cost-effective modular microfluidic reverse transcription(RT)-PCR and RT-loop mediated isothermal amplification(RTLAMP)platform,Epidax®,for the point-of-care testing and confirmation of *** platform is versatile and can be reconfigured either for screening using endpoint RT-PCR or RT-LAMP tests or for confirmatory tests using realtime ***®is highly sensitive and detects as little as 1 RNA copy perµL for real-time and endpoint RT-PCR,while using only half of the *** achieved comparable results with those of a commercial platform when detecting SARS-CoV-2 viruses from 81 clinical RNA ***®can also detect SARS-CoV-2 from 44 nasopharyngeal samples without RNA extraction by using a direct RT-PCR assay,which shortens the sample-to-answer time to an hour with minimal user ***,we validated the technology using an RT-LAMP assay on 54 clinical RNA ***,our platform provides a sensitive,cost-effective,and accurate diagnostic solution for lowresource settings.

Background: The ExcelsiusGPS?(Globus Medical, Inc., Audubon, PA) is a next-generation spine surgery robotic system recently approved for use in the United States. The objective of the current study is to assess pedicle screw accuracy and clinical outcomes among two of the first operative cases utilizing the ExcelsiusGPS?robotic system and describe a novel metric to quantify screw deviation. Methods: Two patients who underwent lumbar fusion at a single institution with the ExcelsiusGPS?surgical robot were included. Pre-operative trajectory planning was performed from an intra-operative CT scan using the O-arm (Medtronic, Inc., Minneapolis, MN). After robotic-assisted screw implantation, a post-operative CT scan was obtained to confirm ideal screw placement and accuracy with the planned trajectory. A novel pedicle screw accuracy algorithm was devised to measure screw tip/tail deviation distance and angular offset on axial and sagittal planes. Screw accuracy was concurrently determined by a blinded neuroradiologist using the traditional Gertzbein-Robbins method. Clinical variables such as symptomatology, operative data, and post-operative follow-up were also collected. Results: Eight pedicle screws were placed in two L4-L5 fusion cases. Mean screw tip deviation was 2.1 mm (range 0.8-5.2 mm), mean tail deviation was 3.2 mm (range 0.9-5.4 mm), and mean angular offset was 2.4 degrees (range 0.7-3.8 degrees). All eight screws were accurately placed based on the Gertzbein-Robbins scale (88% Grade A and 12% Grade B). There were no cases of screw revision or new post-operative deficit. Both patients experienced improvement in Frankel grade and Karnofsky Performance Status (KPS) score by 6 weeks post-op. Conclusion: The ExcelsiusGPS? robot allows for precise execution of an intended pre-planned trajectory and accurate screw placement in the first patients to undergo robotic-assisted fusion with this technology.

Ovarian cancer is the second most common gyneco-logical cancer and the leading cause of death in the United States. In this article we review the diagnosis and current management of epithelial ovarian cancer which accounts for over 95 percent of the ovarian malignancies. We will present various theories about the potential origin of ovarian malignancies. We will discuss the genetic anomalies and syndromes that may cause ovarian cancers with emphasis on Breast cancer type 1/2 mutations. The pathology and pathogenesis of ovarian carcinoma will also be presented. Lastly, we provide a comprehensive overview of treatment strategies and staging of ovarian cancer, conclusions and future directions.

A Systematic Review Incorporating Meta-Analysis on the Effectiveness of Intravenous Immunoglobulin Versus Corticosteroids in the Treatment of Pediatric Myocarditis

Background:Concerns have been raised about the efficacy of intravenous immunoglobulin and corticosteroids in pediatric myocarditis;however,the relationship between the risk and efficacy of these two therapies in children with myocarditis ***:A systematic review on seventeen studies was conducted in July 2020,which included 1,960 subjects at the baseline,with 788 receiving intravenous immunoglobulin and 142 receiving *** mean difference(md.or odds ratio(OR)with 95%confidence intervals(Cis)was calculated to assess the prognostic role of both treatments using dichotomous and continuous methods with random or fixed-effect ***:The use of intravenous immunoglobulin was significantly associated with a lower mortality rate or heart transplantation in children with myocarditis(OR,0.55;95%CI,0.40-0.77,^md.5.17;95% CI,-0.26-10.60,p=0.06).Conclusion:The use of intravenous immunoglobulin might have an independent risk relationship with a lower mortality rate or heart transplantation and is recommended in children with myocarditis to prevent complications.

补肾疏肝中药治疗早发性卵巢功能不全系统评价

目的系统评价补肾疏肝中药治疗早发性卵巢功能不全(premature ovarian insufficiency,POI)的有效性和安全性。方法检索PubMed、Cochrane Library、Embase、中国期刊全文数据库(CNKI)、维普中文科技期刊数据库(VIP)、中国生物医学文献数... 详细信息

目的系统评价补肾疏肝中药治疗早发性卵巢功能不全(premature ovarian insufficiency,POI)的有效性和安全性。方法检索PubMed、Cochrane Library、Embase、中国期刊全文数据库(CNKI)、维普中文科技期刊数据库(VIP)、中国生物医学文献数据库(CBM)、万方数字化期刊全文数据库(Wan Fang)7个数据库,检索时间从建库至2021年3月22日,纳入补肾疏肝中药治疗POI的随机对照临床试验。筛选出17篇文献,对照组采用西药治疗,观察组采用补肾疏肝中药或补肾疏肝中药联合常规西药治疗。结果与西药组相比,补肾疏肝中药联合西药组可显著提高临床治疗总有效率[RR=1.22,95%CI(1.14,1.31),Pmd.-5.32,95%CI(-7.54,-3.10),Z=4.69,Pmd.-2.34,95%CI(-3.64,-1.03),Z=3.52,P=0.0004]、升高血清E_(2)水平[Smd.0.97,95%CI(0.71,1.22),Z=7.47,P<0.00001],且未明显增加不良反应发生率[RR=0.99,95%CI(0.49,1.97),P=0.97]。结论补肾疏肝中药联合西药治疗POI患者可显著提高临床治疗总有效率、降低血清FSH及LH水平、升高血清E_(2)水平,且未明显增加不良反应发生率。然而,受纳入研究数量和质量限制,补肾疏肝中药联合西药治疗POI的疗效尚需更多大样本、多中心、高质量随机对照试验进行验证。
