High Resolution Computed Tomography Scan Findings of Chest among Hospitalized COVID-19 Patients at a Dedicated Hospital in Dhaka City during the First Wave of Pandemic

Background: The use of chest imaging in COVID-19 can be especially useful for patients with moderate to severe symptoms or comorbidities. Objective: This study aimed to demonstrate the high resolution computed tomography (CT) findings observed among the coronavirus disease 2019 (COVID-19) patients presented with pneumonia and to reveal the most frequent infiltration and distribution patterns of the disease. Methodology: This was a retrospective study. This study was performed in the Department of Radiology & Imaging at Kurmitola General Hospital, Dhaka, Bangladesh. This was the first dedicated COVID-19 hospital with a bed capacity of 500 and well-equipped ICU facilities. The recorded HRCT scan data were collected in the period from April 2020 up to May 2020 during the first wave of COVID-19 in Bangladesh. As this was a retrospective study, verbal or written consent was not obtained from all potential participants or guardians. The available demographic data as well as the medical history of all data were collected and thoroughly reviewed from the record book. These patients were RT-PCR confirmed cases of COVID-19 patients presented with pneumonia and were admitted to Kurmitola General Hospital, Dhaka, Bangladesh. All these patients underwent HRCT scans of the chest. Result: A total number of 155 COVID-19 patients with HRCT scan were evaluated. The mean age with SD of the study population was 58.03 ± 14.08 years with the range of 22 to 97 years. The male and female ratio was 2.04:1. Fibrosis of the lungs and thickening of pleura were found in 38 (24.5%) cases and 33 (21.3%) cases respectively. The involvement of both lungs was found in 32 (20.6%) cases. The presence of pneumonitis and bronchiectasis were detected in 77 (49.7%) cases and 5 (3.2%) cases respectively. Left-sided mild pleural effusion was also noted in 6 (3.9%) cases. Hepatitis B Vaccination in Medical and Dental Students: A Cross-Sectional Study

Background: Medical and dental students are at risk of Hepatitis B Virus (HBV) infection. The study aimed to assess the vaccination status against Hepatitis B Virus of students in clinical and non-clinical years of a private medical and dental college, and their knowledge, attitude, and awareness about the subject. Methods: A cross-sectional study was conducted using a pretested, self-administered questionnaire among 203 medical and dental students of CMH Lahore Medical College and Institute of Dentistry (CMH LMC & IOD) in Lahore, Pakistan. Participants were evaluated for their knowledge and vaccination status against Hepatitis B Virus. Students were considered to be fully vaccinated (recipients of 3 doses), partially vaccinated (recipients of 1 or 2 doses), and unvaccinated. Comparisons were made between students of clinical and non-clinical years. Data was entered and analysed using Statistical Package for the Social Sciences (SPSS) version 23. Results: Only 66% (n = 134) of the 203 participants had ever received a Hepatitis B Virus vaccine out of which a meagre 17.2% (n = 35) were fully vaccinated. No significant difference was found in vaccine uptake between students of clinical and non-clinical years (p-value = 0.181) despite significant differences seen in the knowledge of vaccination schedule (p-value = 0.001), the prevalence of needle-stick injuries (p-value = 0.001), and knowledge of protocols to be followed after a needle-stick injury (p-value = 0.001). Conclusion: Our study found that a large proportion of the student population is vulnerable to HBV infection. There is a need to create awareness regarding the subject to increase vaccine uptake. HBV vaccination should be offered to all currently enrolled students and be made mandatory at the time of admission in the future.

Background: Women have experienced different menopause-related symptoms. Objective: The purpose of the present study was to see the menopause-related symptoms and help seeking behavior among the women attended at a tertiary care hospital. Methodology: This cross-sectional study was conducted in the outpatient Department of Kushtia Medical College Hospital, Kushtia, Bangladesh from October 2015 to September 2016 for a period of one (01) year. Menopausal women who were attended in the outpatient Department of Hospital were included as study population. The eligible women had at least one year of amenorrhoea. Surgical menopause or due to severe illness, extreme weight loss endocrine disorders or radio therapy were excluded. Each subject completed a questionnaire that elicited their sociodemographic information, personal and family history and current medical problems. Result: A total number of 437 women were recruited for this study. The most prevalent menopausal symptom among the participants was generalized body ache 91.30%. Followed by low back pain 88.55%, muscle & joint pain 87.18%, tiredness 86.04%, hot flush 64.30%, urinary symptoms 43.93%, vaginal dryness 48.28%, insomnia 73.91%, irritability 23.56%. Most of the participants consulted at first with village doctors (63.15%) followed by general practitioners 20.82% due to their health problems. Some sought help from traditional healers like kobiraj 3.20%. Conclusion: In conclusion, body ache, low back pain, muscle and joint pain and tiredness are the most commonly reported menopause-related symptoms that are mostly consulted with village doctors.

Background Opisthorchiid flukes, particularlyOpisthorchis viverrini,Opisthorchis felineus,Clonorchis sinensis, andMetorchis spp. are the most common fish-borne zoonotic human liver flukes (hLFs). Liver fluke infections are more prevalent in resource-deprived and underprivileged areas. We herein estimated the prevalence of the metacercariae (MC) of major hLFs in common large freshwater fishes (lFWF) marketed for human consumption from some selected areas of Bangladesh along with detection of their molluscan vectors and *** The current status of fish-borne zoonotic hLF infections in lFWF was investigated along with their molluscan vectors and mammalian reservoir hosts in Mymensingh and Kishoreganj in Bangladesh from July 2018–June 2022 using conventional and multiple molecular techniques, such as PCR, PCR-restriction fragment length polymorphism (RFLP), sequencing, and bioinformatic analyses. The infection rate of fishes was analyzed using theZ-test and the loads of MC were compared using the chi-squared (χ2) *** The MC ofC. sinensis,Opisthorchis spp., andMetorchis spp. were detected in 11 species of common and popular lFWF. In lFWF, the estimated prevalence was 18.7% and the mean load was 137.4 ± 149.8 MC per 100 g of fish. The prevalence was the highest (P 0.05) distributed between the head and body of fishes. The infection rate was slightly higher in cultured (19.6%) fishes. The MC ofC. sinensis,O. felineus,O. viverrini, andMetorchis orientalis in fishes were confirmed using PCR, PCR-RFLP and bioinformatics. The cercariae of opisthorchiid (Pleurolophocercus cercariae) flukes were only recovered fromBithynia spp. (3.9%, 42 out of 1089). Primary Sites of Gastric Carcinoma with Metastatic Spread Detected by Transabdominal Hydrosonography: Experience of 76 Cases in Bangladesh

Background: Metastasis is frequently occurred among the gastric carcinoma patients. Objectives: The purpose of the present study was to detect the different primary sites of gastric carcinoma by ultrasonography with the metastatic spread. Methodology: This cross-sectional study was carried in the department of Radiology and Imaging of Dhaka Medical College, Dhaka from July 2009 to June 2011 for a period of two (2) years. Clinically suspected gastric carcinoma patients attended at indoor and outdoor Surgery department of Dhaka Medical College Hospital, Dhaka were selected as study population. All the patients were examined by ultrasonography of upper abdomen and or Ba-meal X-ray. Detailed clinical history and relevant points in clinical examination and investigations were noted. The sonographic findings were noted. The extent of tumour infiltration and metastasis was assessed in all patients with suspected gastric malignancy. All these patients were properly follow up and collect the endoscopic biopsy and/or surgical resection biopsy reports. In all patients who underwent operation the extent of tumour infiltration and metastasis was looked for and was noted. Results: A total number of 76 patients were included in this study. Among 76 patients, 3 unwilling to take part in the study, 4 patients refused to undergo endoscopy or surgery, 5 patients were unfit for the same and in 4 cases, and histopathological reports were not available. Ultimately 60 patients were evaluated finally. The commonest site of involvement of gastric carcinoma as determined by ultrasound was the antrum 13 out of 23 cases (56.52%). Among them metastatic para-aortic lymph nodes were seen in seven patients and hepatic metastases was detected in five patients’ ascites was seen in eleven patients. Conclusion: In conclusion, the most common site of gastric carcinoma is the antral part of stomach and majority are metastasized in peritoneum.

Clinico-Demographic Characteristics of Intestinal Tuberculosis in Pre-Operative Unsuspected Laparotomy as Incidental Findings in Bangladesh

Background: Clinical presentation of intestinal tuberculosis is varied. It is also essential to know the clinical and demographic characteristics of incidentally detected intestinal tuberculosis patient. Objective: The purpose of the present study was to find out the clinic-demographic characteristics of intestinal tuberculosis in clinically and preoperatively unsuspected laparotomies. Methodology: This cross-sectional study was carried out in the Department of Surgery at Dhaka Medical College Hospital, Dhaka from March 2002 to March 2004 for a period of two (2) years. All the patients underwent laparotomy presented without any suspicion of pulmonary or intestinal tuberculosis were included as study population. Patients who were without the history of cough, haemoptysis, antitubercular drug therapy were included. Per operative suspicion was raised due to enlarged lymph nodes in mesentery, omental masses, ileocaecal masses, suspicious growth in ileum, omental thickening, perforation suspected by tuberculosis. Biopsy was taken from suspicious lesions. Histopathology was confirmed the tubercular lesion. Results: A total number of 300 patients under laparotomy were recruited for this study of which 38 positive cases were found in the study with a percentage of 12.7%. Maximum affected age group was 21 to 30 years which was 16 (42%) cases. Male was predominant to female which was 28 (73.7%) cases and 10 (26.3%) cases respectively. Low socioeconomic people are affected more (76.3%). Most common presentation in positive cases was acute and sub-acute intestinal obstruction 20 (52.6%) followed by intestinal perforation (26.31%). Conclusion: In conclusion young age male patient is the most commonly affected intestinal tuberculosis which is frequently presented with sub-acute intestinal obstruction among the preoperatively unsuspected laparotomy patients.

Diagnostic Test Validity of Oxacillin Screen Agar for the Detection of Methicillin Resistant <i>Staphylococcus aureus</i>

Background: Detection of Methicillin Resistant Staphylococcus aureus may vary in phenotypic and genotypic methods. Objective: The aim of this present study was to validate the oxacillin screen agar test for the detection of methicillin-resistant Staphylococcus aureus isolated from clinical specimens. Methodology: This cross-sectional study was planned to carry out in the Department of Microbiology and Immunology at Bangabandhu Sheikh Mujib Medical University, Dhaka from January, 2010 to December, 2010 for a period of one (01) year. Different clinical specimens were collected from the patients presented with infections at anybody sites. Isolation and identification of Staphylococcus aureus (S. aureus) were performed by staining, culture and biochemical tests. Oxacillin screen agar test was performed to detect the methicillin-resistant Staphylococcus aureus. PCR was performed for detection of the mecA gene for MRSA. Result: Out of the 22 suspected MRSA isolates 19 were mecA positive by PCR. The sensitivity and specificity of oxacillin screen agar were 100.0% (95% CI 82.3 to 100.0%) and 33.3% (95% CI 24.0 to 90.6%) respectively. The positive predictive value and negative predictive value of oxacillin screen agar test were 90.5% (95% CI 81.0 to 95.5%) and 100.0% (95% CI 20.6 to 100.0%) respectively. The accuracy of the oxacillin screen agar test was 90.9% (95% CI 71.7 to 96.3%). Conclusion: In conclusion oxacillin screen agar test has high sensitivity with low specificity for the detection of MRSA.

Nipah virus (NiV) is a highly infectious zoonotic pathogen that poses a significant threat to human and animal health. First identified in Malaysia in 1998, NiV has since caused several outbreaks in Southeast Asia, with sporadic cases reported in Bangladesh and India. The virus is primarily transmitted to humans through direct contact with infected animals, primarily fruit bats, or through the consumption of contaminated fruits and their juices. NiV infection presents a wide spectrum of clinical features, ranging from mild respiratory illness to severe encephalitis, with a high case fatality rate. The incubation period typically ranges from 4 to 14 days, during which patients develop fever, headache, myalgia, and respiratory symptoms such as cough and sore throat. As the disease progresses, neurological signs become prominent, including altered consciousness, seizures, and focal deficits. Severe cases may exhibit acute respiratory distress syndrome and multi organ failure. Laboratory findings often include lymphocytopenia, thrombocytopenia, and elevated liver enzymes. Diagnosis of NiV infection requires specialized laboratory testing, including reverse transcription-polymerase chain reaction (RT-PCR) and serological assays. Currently, no specific antiviral treatment exists for NiV infection, and management primarily focuses on supportive care. Prevention and control strategies encompass public health interventions, surveillance, and raising awareness among healthcare providers and the general population. The emergence and re-emergence of NiV highlight the urgent need for continued research, improved diagnostic capabilities, and the development of effective vaccines and therapeutics to mitigate the impact of this deadly virus.

Test Accuracy of CT-Scan for the Detection of Malignant Liver Mass

Background: Detection of malignant liver mass is very important for the treatment modalities. Objective: The purpose of the present study was to establish the usefulness of CT scan in the diagnosis of malignant hepatic mass. Methodology: This cross sectional study was carried out in the Department of Radiology and Imaging at Mymensingh Medical College Hospital (MMCH), Mymensingh, Banghabandhu Sheikh Mujib Medical University (BSMMU), Dhaka and Dhaka Medical College Hospital (DMCH), Dhaka during the period of 1st January 2006 to 31st December 2007. Patients admitted in the Department of Medicine and Department of Hepatobiliary of MMCH, BSMMU, and DMCH with the clinical diagnosis of fever, abdominal pain, anorexia, nausea/vomiting, loss of appetite, jaundice, weight loss and ascites were selected as study population. CT scan and histopathology were performed to all the patients. Result: A total number of 50 patients were recruited for this study. Mean age of all patients was 51.28 ± 14 years with a range of 17 year to 78 years. Among all patients 28 had multiple lesion, of them 71.4% was malignant and 28.6% was benign. On the other side 22 patients had solitary lesion, of them 36.4% was malignant and 63.6% was benign
