Background:Physical activity(PA)is important for cancer *** of remotely delivered interventions are needed to assist in reaching under-served non-metropolitan cancer *** objective of this study was to ascertain whether wearable technology,coupled with health coaching was effective in increasing PA in breast and colorectal cancer survivors living in regional and remote areas in ***:Cancer survivors from 5 states were randomized to intervention and control *** participants were given a Fitbit Charge 2TMand received up to 6 telephone health coaching *** participants received PA print *** assessments at baseline and 12 weeks measured moderate-to-vigorous PA(MVPA),light PA,and sedentary ***:Eighty-seven participants were recruited(age=63±11 years;74(85%)female).There was a significant net improvement in MVPA of 49.8 min/week,favoring the intervention group(95%confidence interval(95%CI):13.6-86.1,p=0.007).There was also a net increase in MVPA bouts of 39.5 min/week(95%CI:11.9-67.1,p=0.005),favoring the intervention *** groups improved light PA and sedentary behavior,but there were no between-group ***:This’s the first study to demonstrate that,when compared to standard practice(i.e.,PA education),a wearable technology intervention coupled with distance-based health coaching,improves MVPA in non-metropolitan cancer *** results display promise for the use of scalable interventions using smart wearable technology in conjunction with phone-based health coaching to foster increased PA in geographically disadvantaged cancer survivors.

Supramolecular Manipulation of Intracellular Acidic Vesicles to Improve Cellular Self-Repair and Self-Defense

Lysosomes are the“cell stomach”that digests various species including macromolecules and pathogens for self-repair and self-defense upon fusion with endosomes,autophagosomes,and ***,artificially manipulating intracellular vesicles could potentially promote lysosome-mediated intracellular digestion for disease prevention and ***,a supramolecular strategy to efficiently tether and fuse acidic vesicles(i.e.,lysosomes,late endosomes,autolysosomes,and phagosomes)has been developed for the first time to enhance the digestion of cellular wastes and foreign matter,including pathogens.A linear polyethylene glycol(PEG)derivative dually tagged with morpholine(MoR)and adamantane(ADA)on the opposite side,namely,MoR-PEG-ADA,was designed to target intracellular acidic vesicles via MoR and to decorate their surfaces with ***,the addition of cucurbit[7]uril(CB[7])grafted hyaluronic acid(HA)induced supramolecular tethering and fusion of acidic vesicles via strong host-guest interactions between CB[7]of CB[7]-HA and ADA residing on the surface of acidic *** a proof-ofconcept,the overall cellular metabolism,including endogenous autophagy and the exogenous endocytosis and phagocytosis,was effectively upregulated,demonstrating that supramolecular regulation of the dynamics of intracellular acidic vesicles may promote the lysosome’s digestion to significantly improve cellular self-repair and self-defense.

Assimilation of GoES-R Geostationary Lightning Mapper Flash Extent Density Data in GSI 3DVar, EnKF, and Hybrid En3DVar for the Analysis and Short-Term Forecast of a Supercell Storm Case

Capabilities to assimilate Geostationary operational Environmental Satellite “R-series ”(GoES-R) Geostationary Lightning Mapper(GLM) flash extent density(FED) data within the operational Gridpoint Statistical Interpolation ensemble Kalman filter(GSI-EnKF) framework were previously developed and tested with a mesoscale convective system(MCS) case. In this study, such capabilities are further developed to assimilate GoES GLM FED data within the GSI ensemble-variational(EnVar) hybrid data assimilation(DA) framework. The results of assimilating the GLM FED data using 3DVar, and pure En3DVar(PEn3DVar, using 100% ensemble covariance and no static covariance) are compared with those of EnKF/DfEnKF for a supercell storm case. The focus of this study is to validate the correctness and evaluate the performance of the new implementation rather than comparing the performance of FED DA among different DA schemes. only the results of 3DVar and pEn3DVar are examined and compared with EnKF/DfEnKF. Assimilation of a single FED observation shows that the magnitude and horizontal extent of the analysis increments from PEn3DVar are generally larger than from EnKF, which is mainly caused by using different localization strategies in EnFK/DfEnKF and PEn3DVar as well as the integration limits of the graupel mass in the observation operator. overall, the forecast performance of PEn3DVar is comparable to EnKF/DfEnKF, suggesting correct implementation.

Generative pretrained transformer 4:an innovative approach to facilitate value-based healthcare

objective Appropriate medical imaging is important for value-based *** aim to evaluate the performance of generative pretrained transformer 4(GPT-4),an innovative natural language processing model,providing appropriate medical imaging automatically in different clinical *** Institutional Review Boards(IRB)approval was not required due to the use of nonidentifiable ***,we used 112 questions from the American College of Radiology(ACR)Radiology-TEACHES Program as prompts,which is an open-sourced question and answer program to guide appropriate medical *** included 69 free-text case vignettes and 43 simplified *** the performance evaluation of GPT-4 and GPT-3.5,we considered the recommendations of ACR guidelines as the gold standard,and then three radiologists analyzed the consistency of the responses from the GPT models with those of the *** set a five-score criterion for the evaluation of the consistency.A paired t-test was applied to assess the statistical significance of the *** For the performance of the GPT models in free-text case vignettes,the accuracy of GPT-4 was 92.9%,whereas the accuracy of GPT-3.5 was just 78.3%.GPT-4 can provide more appropriate suggestions to reduce the overutilization of medical imaging than GPT-3.5(t=3.429,P=0.001).For the performance of the GPT models in simplified scenarios,the accuracy of GPT-4 and GPT-3.5 was 66.5%and 60.0%,*** differences were not statistically significant(t=1.858,P=0.070).GPT-4 was characterized by longer reaction times(27.1 s in average)and extensive responses(137.1 words on average)than *** As an advanced tool for improving value-based healthcare in clinics,GPT-4 may guide appropriate medical imaging accurately and efficiently。

Incidental renal cell carcinoma post bilateral nephrectomy in autosomal dominant polycystic kidney disease

BACKGRoUND Renal cell carcinoma(RCC)is more common in patients with autosomal dominant polycystic kidney disease(ADPKD)than in the general *** RCC in ADPKD is challenging due to the presence of multiple renal cysts,often leading to delays and difficulties in distinguishing RCC from cyst infection or hemorrhage.A total of 38 kidneys were excised from 19 patients,with a mean age of 56.8 years and an average hemodialysis duration of 84.2 *** patients underwent open nephrectomies,and 11 underwent hand-assisted laparoscopic *** was detected in 15.8%of kidneys,affecting 21.1%of *** patients had multifocal RCC in both *** RCC cases were pT1 stage,with the largest lesion averaging 16.5 mm in *** average operative duration was 120 minutes,with intraoperative blood loss averaging 184.2 *** patients required blood *** complications occurred in five patients,with a mean hospital stay of 17.1 *** mean follow-up period was 28.1 *** The prevalence of RCC is higher in patients with ADPKD with ESRD than in those with ESRD ***,clinicians should be cautious and implement surveillance programs to monitor the development of RCC in patients with ADPKD,particularly those on dialysis.
