本章是2023年国际糖尿病足工作组(IWGDF)关于糖尿病相关的足溃疡预防的指南,是在2019年版指南基础上的更新。本指南面向临床医生和其他医务人员。我们遵循推荐、评估、发展和评价分级(GRADE)方法,采用患者-干预-比较-结局(PICO)的方法来设计临床问题和关键结局,并进行了包括必要的Meta分析在内的医学科学文献的系统综述,同时撰写了推荐及其理由。这些推荐基于系统综述中证据质量、在没有足够证据的情况下的专家意见、同时考虑干预措施的有益和不良效果,以及患者的偏好、成本、公平性、可行性和适用性的权衡。我们推荐对足溃疡风险较低的糖尿病患者每年进行一次保护性感觉丧失和周围动脉病变的筛查,并对高危患者进行更高频率的筛查以及其他危险因素的筛查。为了预防足溃疡,要对有危险因素的患者进行健康教育,教育其对足部进行适当地自我护理,在没有适当的足部保护的情况下不要行走,并对溃疡前兆进行有效治疗。指导足溃疡中高危风险的糖尿病患者穿着合适、宽松的治疗性鞋类,并考虑指导他们监测足部皮肤温度。开具处方使用治疗性鞋类,这些鞋类在行走时具有缓解足底压力的效果,能够防止足底溃疡复发。推荐足溃疡中低风险人群最好制定一个有监督的足踝运动计划,以减少溃疡风险;就溃疡的风险而言,每天总共增加1000步的负重活动可能是安全的。对于有溃疡前兆的非僵直锤状趾患者,可考虑屈肌腱切断术。我们建议不要用神经松解减压术来预防足溃疡。要对足溃疡中高危的糖尿病患者给予综合足部护理,以预防溃疡复发。贯彻这些推荐有助于医疗专业人员对有足溃疡风险的糖尿病患者提供更好的护理,增加无溃疡的天数,减少患者糖尿病相关的足病的医疗负担。

糖尿病相关的足病给全球糖尿病患者以及卫生保健系统带来沉重的负担。国际糖尿病足工作组(IWGDF)自1999年起一直致力于制定基于循证的糖尿病相关的足病的预防与管理指南。2023年,在系统综述以及全球多学科领域专家推荐意见的基础上,IWGDF的所有指南进行了更新。此外,新增加的急性夏科神经性骨关节病的临床指南已经完成。本实践指南在IWGDF指南7个部分的基础上阐述了糖尿病相关的足病的预防、分类和管理的基本原则,同时阐述了如何基于这些原则从组织层面成功地预防与治疗糖尿病相关的足病,并提供了用于辅助足部筛查的附录。本实践指南面向全球参与糖尿病患者医疗照护的医务人员。全球大量研究证实,实施这些防治原则与降低糖尿病相关的下肢截肢发生率有关。足部疾病和截肢的负担正在迅速增加,中低收入国家更是如此。这些指南可协助这些国家制定糖尿病相关的足病的预防和管理标准。总之,我们希望更新的这些实践指南可继续作为有效的参考文件,帮助医务人员减轻全球糖尿病相关的足病的负担。

国际糖尿病足工作组(IWGDF)自1999年以来一直以循证为基础发布糖尿病足病的预防和管理指南。本文中针对日常临床工作中糖尿病足溃疡的分类提出了新指南,并对既有的分类进行了综述。新指南只对现患糖尿病足溃疡进行分类,不包括用于定义未来溃疡风险的分类系统。本指南的制定是基于对现有文献的综述以及专家对最大程度影响临床结局的8个关键因素的鉴定。分类级别是根据关键因素的数量、内部和外部有效性验证以及使用意向而制定的。判定分类评分的关键因素有3种:患者相关的因素(终末期肾衰竭)、肢体相关的因素(周围动脉病变和保护性感觉丧失)和溃疡相关的因素(面积、深度、部位、单发或多发以及感染)。特殊分类系统要考虑以下5种临床情况:①卫生专业人员之间交流;②预测单个溃疡预后;③对个案临床决策的辅助;④伤口评估,有感染或无感染及周围动脉病变(评估灌注及血管重建的潜在益处);⑤用于地方、区域或国家人群结局的稽查。指南推荐:①医务人员之间交流使用SINBAD系统。②无适用于预测个体溃疡预后的分类。③感染的评估采用美国传染病学会/国际糖尿病足工作组(IDSA/IWGDF)分类系统。④评估灌注及血管重建的潜在获益使用创面、缺血、足感染(WIfI)系统。⑤人群结局的稽查使用SINBAD系统。
