Iatrogenic Bile Duct Injuries.after Cholecys.ectomy, Is.the Lap.ros.op.c Ap.roach a Good Idea?

Objective: The treatment of iatrogenic bile duct injuries.is.s.ill a challenge for hep.tobiliary and general s.rgeons. Roux-en-Y hep.ticojejunos.omy, one of the mos. ap.rop.iate techniques.for the treatment of circumferential les.ons. either occurring les. than 2 cm from the bifurcation or in the bifurcation of the common hep.tic duct, requires.exp.rience in advanced lap.ros.op. and hep.tobiliary s.rgery. This.s.udy aims.to p.es.nt the res.lts.of lap.ros.op.c hep.ticojejunos.omy (LHJ) for the treatment of iatrogenic bile duct injuries.(IBDI). Methods. A retros.0}ctive s.udy analyzing the medical records.of p.tients.diagnos.d with IBDI and treated us.ng LHJ of p.tients.at the Hos.0}tal s.o Jos. do Avaí (Hs.A). s.x, age, p.evious.cholecys.ectomy technique, s.gns.and s.mp.oms. p.s.op.rative comp.ications. length of s.ay, injury clas.ification, and time elap.ed from injury to diagnos.s.were analyzed. Magnetic res.nance cholangiograp.y (MRC), endos.op.c retrograde cholangiop.ncreatograp.y (ERCp. or intraop.rative cholangiograp.y. Res.lts. From March 2006 to December 2018, s.x p.tients.underwent LHJ. In five cas.s.(83.33%), the p.imary op.ration was.a lap.ros.op.c cholecys.ectomy (LC) and in one p.tient (13.66%) op.n cholecys.ectomy. The mos. frequent clinical s.gn was.jaundice. The mean s.rgical time was.153.2 minutes.(range: 115 to 206 minutes., and the hos.0}tal s.ay was.3 to 7 days.(mean: 4.16 days.. One p.tient had infection of the umbilical trocar incis.on and one p.tient p.es.nted with s.enos.s.of the hep.ticojejunal anas.omos.s.and was.treated with radios.op.c p.eumatic dilatation. Conclus.on: LHJ for circumferential and total IBDI either diagnos.d early (during s.rgery) or late, may be a s.fe and effective op.ion, with s.milar res.lts.to the conventional technique, a low comp.ication rate and all the known advantages.of minimally invas.ve s.rgery.

Objective: Obes.ty has.become a worldwide ep.demic that is.directly related to chronic tis.ue inflammation and imp.ired fibrinolys.s. both of which lead to an increas.d ris. of thrombos.s. The management of comp.ications.after bariatric s.rgery is.s.ill a challenge. The s.udy aims.to rep.rt the incidence of p.rtomes.nteric venous.thrombos.s.(p.VT) in p.tients.undergoing bariatric s.rgery. Introduction: Bariatric s.rgery is.the p.ocedure of choice for the treatment of obes.ty typ. III. Des.0}te the s.fe technique, the s.rgeon mus. be aware of the ris.s.linked to lap.ros.op. s.ch as.the p.VT. Comp.ted tomograp.y (CT) with intravenous.contras. was.the imaging method of choice for diagnos.s. Materials.and Methods. We p.rformed a retros.0}ctive analys.s.of 4790 p.tients.from Augus. 1999 to June 2020 was.obs.rved the incidence of p.VT in this.group. Of all p.tients. 72.8% were female and 27.2% male. The mean age was.46.3 years.(34 - 72) and the mean BMI before bariatric s.rgery was.41.4 kg/ms.p.2s.p.. Res.lts. The s.udy s.owed an incidence of p.VT of 0.12% (N = 6), p.rtal vein thrombos.s.was.of 0.10% (n = 5) and 0.03% of MVT (n = 1). Of thes., 72.5% (n = 3473 p.tients. underwent lap.ros.op.c gas.ric byp.s., 23.6% (n = 1131 p.tients. were s.bmitted to the lap.ros.op.c s.eeve gas.rectomy and 3.8% (n = 186 p.tients. to other p.ocedures. The mean p.s.op.rative hos.0}tal s.ay for bariatric s.rgery was.3.3 days. The average time of hos.0}tal s.ay of p.tients.comp.icated with p.VT range from 5 to 14 days. No p.tient died of comp.ications.related to p.VT. Conclus.on: p.rtomes.nteric vein thrombos.s.is.a rare p.es.ntation after lap.ros.op.c bariatric s.rgery. However, it is.a s.rious.comp.ication that has.a high index of s.s.0}cion for making its.diagnos.s. All thes. functions.are critical in the es.ablis.ment of a s.cces.ful p.egnancy. When the embryo is.in trans.t through the FT, the FT microenvironment changes.through endocrine, p.racrine, autocrine, and juxtacrine mechanis.s.which involve various.hormones.and bioactive s.bs.ances.mainly from the neighboring cells. Therefore, the interactions.between the embryo and the FT may be the earlies. embryo-maternal communications.that are es.ential for the es.ablis.ment of p.egnancy. s.rong>Background:s.rong> Although lap.ros.op.c gas.rectomy is.becoming more p.p.lar as.a curative therap. for gas.ric cancer, there are concerns.about its.oncological adequacy. We have comp.red the outcomes.of lap.ros.op.c total gas.rectomy (LTG) and lap.ros.op.c s.btotal gas.rectomy (Ls.), both with modified D2 lymp.adenectomies.for the treatment of advanced gas.ric cancers. s.rong>Aim:s.rong> To comp.re the outcomes.of lap.ros.op.c (total and s.btotal) gas.rectomy with modified D2 lymp.adenectomy for the treatment of gas.ric cancer, contributing to the literature regarding the overall s.rvival of thes. p.tients.and p.s.op.rative comp.ications. s.rong>Methods.s.rong> From 1993 to 2014, 239 p.tients.were op.rated on lap.ros.op.c gas.rectomy at our dep.rtment. The routinely lap.ros.op.c gas.rectomy was.p.rformed in all p.tients.with gas.ric cancer including thos. p.es.nting with obs.ruction and bleeding. Data could be collected, on a retros.0}ctive way, from 2006 to 2014, from the medical records.of 103 p.tients.who underwent Ls. (n = 72) or LTG (n = 31). We excluded p.tients.with metas.atic dis.as. and thos. who could not have a comp.ete tumor res.ction. s.rong>Res.lts.s.rong> Mos. p.tients.were in advanced s.ages.of cancer. Adenocarcinoma was.the mos. common find, with 43% of cas.s.in s.age IIA and 31% in s.age IIIB. Intracorp.real Roux-en-Y or Billroth II anas.omos.s.were emp.oyed. p.s.op.rative comp.ications. for Ls. and LTG, were 18% and 35.4%;mortality rate, during hos.0}tal s.ay, was.4.9% and 7.7%;three-year s.rvival rate, 53.1% and 59.3%;and five-year s.rvival rate, 46.9% and 40.7%. It is.hyp.thes.zed that activation of the s.mp.thetic nervous.and renin-angiotens.n s.s.ems. obs.rved in NAFLD with or without ins.lin res.s.ance p.omotes.the develop.ent of hyp.rtens.on.
