Spinal Epidural Cavernous Hemangioma of the Thoracic Spine: A Case Report

Cavernous hemangiomas can arise in any region of the body, including the central nervous system. Spinal cavernous hemangiomas account for 5% - 12% of all cases of vertebral vascular malformation. Most of these are of vertebral origin, and cases that are non-vertebral in origin are rare. We encountered a patient with a relatively rare spinal epidural cavernous hemangioma of the thoracic spine that was non-vertebral in origin. The patient was a 63-year-old man. He had become aware of bilateral leg pain and numbness about 2 months earlier, and gait disturbance appeared gradually thereafter. On MRI, a lesion showing iso-intensity on T1-weighted imaging and high intensity on T2-weighted imaging was detected at the 7th thoracic vertebra. On gadolinium contrast-imaging, the lesion was found to be a homogenously-enhanced dumbbell-shaped extradural spinal neoplasm protruding from the left 7th/8th thoracic intervertebral foramen. A neurogenic tumor was suspected based on myelography and MRI findings, and complete tumorectomy was performed, which improved the lower limb symptoms and gait disturbance. The histopathological diagnosis was cavernous hemangioma. Epidural hemangiomas arise from the vertebra in many cases, and pure spinal epidural cavernous hemangiomas are rare. It is difficult to make a preoperative diagnosis because there are no specific imaging findings that can differentiate these tumors. It may be important to consider this disease before surgery in the differential diagnosis of epidural tumors.

Separation of Inorganic Anions Using Methacrylate-Based Monolithic Column Modified with Trimethylamine in Ion Chromatography Capillary System

Methacrylate-based monolithic column was prepared in fused-silica capillary (80 ′ 0.32 mm i.d.) by in situ polymerizetion reaction using glycidyl methacrylate as monomer;ethylene dimethacrylate as crosslinker;1-propanol, 1,4-butanediol, and water as porogenic solvents. The monolith matrix was modified with trimethylamine to create strong anion exchanger via ring opening reaction of epoxy groups. The morphology of the monolithic column was studied by using Scanning Electron Microscope (SEM). This column had good mechanical stability and permeability. The effects of various mobile phases for separation of inorganic anions were investigated. Iodate, bromate, nitrite, bromide, and nitrate were separated within 11 min using100 mMpotassium chloride as mobile phase and detected at 210 nm. This method showed good precision of retention time, acceptable linearity and good sensitivity. Under the optimum condition, the RSD of the retention time was in the range of 1.09%-1.75% (n = 6). The calibration curve showed linear relationships between the peak area and the concentration. The limits of detection (LOD) and the limits of quantitation (LOQ) were between 0.08-0.18 mM and 0.26-0.61 mM, respectively. This method was applied to the determination of inorganic anions in tap water and ground water samples.

A Case of Thoracic Disc Herniation Extruded to the Dorsal Epidural Space

We encountered a patient with thoracic disc herniation that extruded to the dorsal epidural space. The patient was a 78-year-old man in whom listlessness of the lower limbs developed without an inducer and walking became difficult. The patient was diagnosed as having a thoracic spinal cord tumor on MRI by a physician, and was referred to our hospital. Since ring enhancement was observed on contrast MRI performed at our hospital, the patient was diagnosed with disc herniation and surgery was performed. A hernia continuous with the intervertebral space that extruded to the dorsal epidural space was excised. At the final follow up, 1 year and 4 months after surgery, the patient could walk with a cane. Among thoracic disc herniation cases, hernia extruded to the dorsal epidural space is rare and difficult to diagnose by imaging before surgery, but evaluation using contrast MRI is useful in the diagnosis. When a space-occupying lesion is observed in the spinal canal in the lower thoracic spinal region, it is necessary to evaluate it by contrast MRI in consideration of thoracic disc herniation.

Extraction of Palladium (Ⅱ) with p-Methylphenylalkylsulfide from Aqueous Chloride Media

p-Methylphenylalkylsulfide of various alkyl chain length as extractant were synthesized and the extraction of palladium was examined in terms of equilibrium and kinetics. Distribution ratio of Pd was independent of alkyl chain length. For aqueous chloride media, there was a significant difference in distribution ratios for the solution of NaC1 and HC1. The results of loading test and the slope analysis suggest that the extractant and Pd (Ⅱ) form 2:1 complex. Furthermore, the extraction rate based on the volume of aqueous phase was obtained in a stirred vessel, and the rate equation was presented. Unfortunately, it was difficult to construct surfactant liquid membrane system by use of the present extractant.

Autopsy study of anatomical features of the posterior gastric artery for surgical contribution

AIM: To investigate features of the posterior gastric ar-tery (PGA) with respect to incidence, location and size by using autopsy ***: Autopsies were performed on 72 cadavers of adults with no history of abdominal operations. The localization of the PGA, the distance between the root of the splenic artery and the origin of the PGA, and the ex-ternal diameter of the PGA were ***: The PGA was recognized in all patients. In 70 (97.2%) cadavers, the PGA branched from the splenic artery, and one female in this group had two PGAs. In 1 (1.4%) patient, the PGA originated from the root of the celiac trunk and in another (1.4%) patient, the PGA branched from the superior polar artery. Overall, the PGA extended for a length of 5.8-12.2 (mean, 8.4) cm from the root of the splenic artery, and the external diameter of the PGA was 1.2-3.2 (mean, 2) mm. CONCLUSION: The anatomical features of the PGA can be readily observed and characterized by autopsy. This study has provided valuable information on the features of the PGA useful in the planning of surgical treatment.

为确保高速浇铸而无漏钢,必须搞清楚初期凝固坯壳与结晶器壁之问结晶器保护渣的润滑状况,同时这一点也是控制弯月面处凝固壳的形成,保证板坯表面质量的基本前提。使用模拟方法计算了结晶器振动过程中,结晶器壁与坯壳间隙间流进的结晶器保护渣量,并分析了与板坯表面振痕深度相关的坯壳变形。模型计算结果与连铸板坯的表面质量吻合很好。
