Health Workers’ Documentation Process as a Prerequisite to the Integration of Patient Care at a Regional Referral Hospital in Uganda

Background: Integrated patient care is necessary for better care outcomes. Documentation enhances the integration of care;however, in the Ugandan setting, documentation of care is poor (e.g., omissions and incomplete records) and integration of patient care is not visible. This study presents a review of patient health records that was undertaken to understand documentation of care at a regional referral hospital in eastern Uganda. This information will help in developing a documentation model to facilitate the integration of patient care in Uganda. Methodology: This retrospective review involved 513 patient health records from the medical-surgical, pediatric, and obstetric/gynecological departments of Jinja Regional Referral Hospital. Data were collected using checklists. Stratified sampling was used to capture variations in ward unit records and identify a fair representation of each department. Data were analyzed with descriptive and inferential statistics. All analyses were performed with SPSS version 22. Results: On average, the study hospital attended to 1000 patients per day and discharged 100 patients per ward unit per month. Our record review showed that documentation by both nurses and doctors was incomplete, and care was fragmented. However, doctors documented care more often than nurses, although the integration of patient care was not evident in doctors’ documentation. Conclusion: To establish integrated patient care, documentation must meet standards set by relevant professional bodies. The findings of this study will inform the development of a feasible documentation model to facilitate the integration of patient care in Uganda.

Hematomas and Limb Skeletal Malformations in Chicken embryos Following exposure to 5-Fluoro-2'-Deoxyuridine

Vascular injury or interruption may play a role in vertebrate limb teratogenesis. Since 5fluoro- 2'- deoxyuridine (FdU) can cause vascular injury in the murine limb and skull prior to the appearance of skeletal malformations in these structures, we studied the effects of this chemical on skeletal development in the chick embryo and noted any vascular injury. The yolk sacs of day three ehick embryos (Hamburger and Hamilton states 17-19) were injected with solutions of vary concentrations of FdU in saline. The embryos developed until the 10th day of incubation when they were fixed for study. Uninjected, saline injected, and sham injected control embryo were similarly fixed. Upon gross inspection, frequent diffuse and saccular hernatomas, as well as fluid-filled blisters, were noted in the limbs of embryos treated with FdU. After the embryos were fixed and cleared, and the skeletons stained, significant skeletal malformations were observed in these limbs. Bony elements of both the upper and lower limbs were affected in at least some of the embryos. The combination of FdU-induced hematomas and blisters with associated skeletal malformations in the same regions of some embryos suggests a relationship between these phenomena.

Hepatocellular carcinoma: Review of disease and tumor biomarkers

Hepatocellular carcinoma(HCC) is a common malignancy and now the second commonest global cause of cancer death. HCC tumorigenesis is relatively silent and patients experience late symptomatic presentation. As the option for curative treatments is limited to early stage cancers, diagnosis in non-symptomatic individuals is crucial. International guidelines advise regular surveillance of high-risk populations but the current tools lack sufficient sensitivity for early stage tumors on the background of a cirrhotic nodular liver. A number of novel biomarkers have now been suggested in the literature, which may reinforce the current surveillance methods. In addition, recent metabonomic and proteomic discoveries have established specific metabolite expressions in HCC, according to Warburg's phenomenon of altered energy metabolism. With clinical validation, a simple and non-invasive test from the serum or urine may be performed to diagnose HCC, particularly benefiting low resource regions where the burden of HCC is highest.

Selection of Reference Genes in equine White Blood Cells for Real Time PCR Normalization Following extracorporeal Shock Wave Therapy

Selection of proper reference genes (RGs) is an essential step needed for accurate normalization of results from genomic studies. expression of RGs is regulated by many factors such as species, age, gender, type of tissue, the presence of disease, and the administration of therapeutic treatment. The aim of the present study was to identify optimal RGs in a set of blood samples collected at different time points (0, 24, 48, 72 h) from horses following administration of extracorporeal shock wave therapy (eSWT). The mRNA expression of twelve RGs: HPRT1, ACTB, HSP90A, SDHA, GUSB, B2M, UBC, NONO, TBP, H6PD, RPL32, GAPDH was determined using real time quantitative polymerase chain reaction (qPCR). An SAS program developed on the algorithm of geNorm, SASqPCR, was used to determine stability of the expression and the number of optimal RGs. The results showed that the range of quantification cycle (Cq) values of the evaluated genes varied between 17 and 26 cycles, and that one optimal RG, ACTB, was sufficient for normalization of gene expression. Results of stability of expression demonstrated that ACTB was the optimal choice for all the samples studied. Notably, in samples collected at 72 h post eSWT, TBP showed a significant change in the expression level, and was not suitable for use as a RG. These results substantiate the importance of validating and selecting an appropriate RG.

Supercritical fluids technology for clean biofuel production

Biofuels are liquid or gaseous fuels that are predominantly produced from biomass for transport sector applications. As biofuels are renewable, sustainable, carbon neutral and environmentally benign, they have been proposed as promising alternative fuels for gasoline and diesel engines. This paper reviews state-of-the-art application of the supercritical fluid (SCF) technique in biofuels production that includes biodiesel from vegetable oils via the transesterification process, bio-hydrogen from the gasification and bio-oil from the lique- faction of biomass, with biodiesel production as the main focus. The global biofuel situation and biofuel economics are also reviewed. The SCF has been shown to be a promising technique for future large-scale biofuel production, especially for biodiesel production from waster oil and fat. Compared with conventional biofuel production methods, the SCF technology possesses a number of advantages that includes fast kinetics, high fuel production rate, ease of continuous operation and elimination of the necessity of catalysts. The harsh operation environment, i.e. the high temperature and high pressure, and its request on the materials and associated cost are the main concerns for its wide application.

Privacy Protection for Big Data Linking using the Identity Correlation Approach

Privacy protection for big data linking is discussed here in relation to the Central Statistics Office （CSO）, Ireland＇s, big data linking project titled the ＇Structure of earnings Survey - Administrative Data Project＇（SeSADP）. The result of the project was the creation of datasets and statistical outputs for the years 2011 to 2014 to meet eurostat＇s annual earnings statistics requirements and the Structure of earnings Survey （SeS） Regulation. Record linking across the Census and various public sector datasets enabled the necessary information to be acquired to meet the eurostat earnings requirements. However, the risk of statistical disclosure （i.e. identifying an individual on the dataset） is high unless privacy and confidentiality safe-guards are built into the data matching process. This paper looks at the three methods of linking records on big datasets employed on the SeSADP, and how to anonymise the data to protect the identity of the individuals, where potentially disclosive variables exist.

Murine study of portal hypertension associated endothelin-1 hypo-response
