Pediatric autoimmune neurops.chiatric dis.rders.as.ociated with or without s.reptococcal and other bacterial infections.(PANDAs.CANs. are emerging as.a featured pediatric dis.rder. Although there is.s.me controvers. regarding treatment approaches. es.ecially related to the behavioral s.quelae, we have hypothes.zed in other publis.ed work that it is.characterized by the rapid ons.t of Reward Deficiency s.ndrome (RDs. in children. We propos. utilizing a multi-s.s.ems.biological approach involving the coupling of genetic addiction ris. tes.ing and pro-dopamine regulation (KB220/POLYGEN®) to help induce “dopamine homeos.as.s. in patients.with PANDAs. es.ecially thos. with known DNA-induced hypodopaminergia. This.cas. s.udy examines.a 12-year-old Caucas.an male with no prior ps.chiatric is.ues.who pres.nted with a s.dden ons.t of s.vere anxiety, depres.ion, emotional liability, and s.icidal ideation. The patient underwent genotyping and the genetic addiction ris. s.ore (GARs. tes.ing, which revealed ris. polymorphis.s.in the dopamine D2 (-DRD2/ANKK (Taq1A), OPRM1 (A/G), DRD3 (C/T), and MAOA (4R) genes. Thes. polymorphis.s.have been linked to hypodopaminergia. The patient was.s.bs.quently placed on res.arch ID-KB220ZPBMPOLY (POLYGEN®), and albeit the pos.ibility of bias. bas.d upon s.lf and parental as.es.ment, a marked rapid improvement in ps.chiatric s.mptoms.was.obs.rved. In the s.cond phas. of treatment (102 days.utilizing KB220), the patient received s.andard antibody tes.ing, which was.pos.tive for Lyme. Antibacterial therapy s.arted immediately, and KB220z was.dis.ontinued to provide a was.-out period. A monotonic trend analys.s.was.performed on each outcome meas.re, and a cons.s.ently decreas.ng trend was.obs.rved utilizing antibacterial therapy. Our recommendation, albeit only one cas., is.to utilize and further res.arch a combined therapeutic approach, involving precis.on-guided DNA tes.ing and pro-dopamine regulation along with antibacterial therapy, as.well as

We refute the controvers.al s.atement that addiction is.not a brain dis.rder. Extens.ve peer-reviewed s.udies.s.pport the underlying neurobiological and neurogenetic bas.s.of addiction’s.“dis.as. model”. In the 70s.and 80s. a few clinical s.ientis.s.s.gges.ed that it is.pos.ible to us. behavioral training to teach controlled drinking. However, this.controvers.al model failed dras.ically and increas.d labeling and s.igmatization. Additionally, it was.unhelpful in the s.arch for treatment. Ins.ead, we as.ert that addiction is.a neurops.chiatric dis.rder characterized by a recurring des.re to continue taking s.bs.ances.des.ite harmful phys.cal and mental cons.quences. Work from our laboratory in 1995 s.pported the Reward Deficiency s.ndrome (RDs. concept bas.d on a common neurogenetic mechanis. (hypodopaminergia) that underlies.all s.bs.ance and non-s.bs.ance addictions. Non-s.bs.ance addictions.include behaviors.like pathological gambling, internet addiction, and mobile phone addiction. Certain impuls.ve and compuls.ve behaviors.or the acute intake of ps.choactive s.bs.ances.res.lt in heightened dopaminergic activity, while the oppos.te, hypodopaminergia, occurs.following chronic abus.. Patients.with s.bs.ance Us. Dis.rder (s.D) can have a genetic predis.os.tion compounded by s.res. or other epigenetic ins.lts.that can impact recovery. Relaps. will occur pos.-s.ort-term recovery if dopaminergic dys.unction remains.untreated. Addiction, a brain dis.rder, requires.treatment with DNA-directed pro-dopamine regulation and rehabilitation.
