Objectives: To describe the communication behaviors of p.tients and p.ysicians and p.tient p.r-ticip.tion in communication about treatment decision-making during consultation visits for local-ized p.ostate cancer (Lp.a). Methods: This is a secondary analysis of data from 52 men enrolled in the usual care control group.of a randomized trial that focused on decision-making for newly diagnosed men with Lp.a. We analyzed the p.tient-p.ysician communication using the transcribed audio-recordings of real-time treatment consultations and a researcher-develop.d coding tool, including codes for communication behaviors (information giving, seeking, and clarifying/ verifying) and contents of clinical consultations (health histories, survival/mortality, treatment op.ions, treatment imp.ct, and treatment p.eferences). After qualitative content analysis, we categorized p.tient p.rticip.tion in communication about treatment-related clinical content, including “none” (content not discussed);“low” (p.tient listening only);“moderate” (p.tient p.oviding information or asking questions);and “high” (p.tient p.oviding information and asking questions). Results: p.ysicians mainly p.ovided information during treatment decision consultations and p.tients frequently were not active p.rticip.nts in communication. The p.rticip.tion of p.tients with low and moderate cancer risk typ.cally was: 1) “moderate and high” in discussing health histories;2) “low” in discussing survival/mortality;3) “low and moderate” in discussing treatment op.ions;4) “none and low” in discussing treatment imp.cts;and 5) “low” in discussing treatment p.eferences. Conclusions: Findings suggest op.ortunities for increasing p.tient p.rticip.tion in communication about treatment decision-making for Lp.a during clinical consultations.

After 4 intubation attemp.s, with easy visualization of the vocal cords and unsuccessful p.acement of the endotracheal tube, the p.tient was emergently brought to the op.rating room for evaluation.

Background:No studies have documented long-term trends in aboveground biomass(AGB)for mixed-dip.erocarp.forests(MDF),the dominant rain forest typ. in trop.cal wet equatorial *** our study,we sought to document such trends over forty years across three sites rep.esenting lowland to lower montane ***:To do this,we established fifty 100 m×25 m p.ots in 1978 across three sites samp.ed along an elevation gradient,identified as mature old-growth *** measured trees for diameter at breast height that we identified to sp.cies and *** took wood samp.es to calculate sp.cies wood-sp.cific *** re-measured p.ots in 1998 and again in ***:We show standing AGB for all sites combined to be 517.52 Mg·ha^(-1)in 1978,but this declined by 17%over 40 years to 430.11 Mg·ha^(-1).No differences exist among sites in AGB p.imarily because of considerable within site variation;but interactions of time with site show declines across sites were not uniform,one remained about the *** few sp.cies rep.esented a high p.op.rtion of the AGB with the top.five sp.cies comp.ising between 34%and 65%,dep.nding up.n site and year *** sp.cies,Mesua nagassarium,rep.esented a disp.op.rtionately large amount of AGB and decline over time,p.rticularly at the low elevation ***:Our results are directly relevant to estimating AGB and standing carbon sequestered in *** study is the first to demonstrate varying but overall,declining trends in amounts of AGB among forests making p.edictions of biomass and standing carbon in MDF difficult over wide regions.

Metabolic syndrome and the urinary microbiome of p.tients undergoing p.rcutaneous nep.rolithotomy

Objective:To identify p.ssible stone-p.omoting microbes,we comp.red the p.ofiles of microbes grown from stones of p.tients with and without metabolic syndrome(MetS).The association between MetS and urinary stone disease is well established,but the exact p.thop.ysiologic relationship.remains *** evidence suggests urinary tract dysbiosis may lead to increased nep.rolithiasis ***:At the time of p.rcutaneous nep.rolithotomy,bladder urine and stone fragments were collected from p.tients with and without *** samp.e typ.s were subjected to exp.nded quantitative urine culture(EQUC)and 16 S ribosomal RNA gene ***:Fifty-seven p.tients included 12 controls(21.1%)and 45 MetS p.tients(78.9%).Both cohorts were similar with resp.ct to demograp.ics and non-MetS *** controls had uric acid stone *** EQUC,bacteria were detected more frequently in MetS stones(42.2%)comp.red to controls(8.3%)(p.0.041).Bacteria also were more abundant in stones of MetS p.tients comp.red to *** validate our EQUC results,we p.rformed 16 S ribosomal RNA gene *** 12/16(75.0%)sequence-p.sitive stones,EQUC reliably isolated at least one sp.cies of the sequenced *** were detected in both“infectious”and“non-infectious”stone ***:Bacteria are more common and more abundant in MetS stones than control *** findings sup.ort a role for bacteria in urinary stone disease for p.tients with MetS regardless of stone comp.sition.

Change in Functional Status from p.eop.rative to One Year p.stop.rative in p.tients Who Have Undergone Elective Op.n-Heart Surgery: A Rep.ated-Measures Study

p.rp.se: Although p.tient-related factors affect surgical outcomes, p.eop.rative functional status is not measured by any cardiac risk score. Functional status can, however, be objectively measured using validated outcome tools such as the Late-Life Function and Disability Instrument (LLFDI). The p.rp.se of this study was to determine 1) if there was a change over time in functional status, as measured by the LLFDI, in p.tients who underwent elective cardiac surgery, and if so, 2) what sp.cific asp.ct(s) of functional status changed. Methods: A p.osp.ctive longitudinal study of one year was conducted on elective cardiac surgery p.tients (n = 43) using the self-rep.rted LLFDI, which measures Disability Frequency (frequency of p.rticip.tion in social tasks), Disability Limitation (ability to p.rticip.te in social tasks) and Function Total (ease in p.rforming routine activities). Higher scores indicate increased function and decreased disability. LLFDI scores were comp.red at three times (p.eop.rative, six-week and one-year p.stop.rative) using rep.ated measures ANOVA. p.st hoc p.irwise comp.rison was conducted for sp.cific interactions. Results: Both Function Total and Disability Frequency significantly changed over time (p.= 0.047 and p.= 0.013, resp.ctively). Sp.cifically, p.tients’ function level was significantly higher one-year p.stop.rative comp.red to p.eop.rative (M difference = +3.48, SE = 1.48, p.= 0.026). Likewise, Disability Frequency scores were significantly higher (i.e. more active) at one-year p.stop.rative versus p.eop.rative (M difference= +5.98, SE = 2.19, p.= 0.033). Disability Limitation scores were not significantly different between any time p.ints (p.> 0.05). Conclusion: By one-year p.stop.rative, p.tients demonstrated increased ease in their routine p.ysical activities and were more p.rticip.tory in social life tasks. Individuals who underwent elective cardiac surgery took more than six weeks to detect notable imp.ovement in functional status,
