Innate and adaptive T cells in influenza disease

Influenza is a major global health problem, causing infections of the respiratory tract, often leading to acute pneumonia, life-threatening complications and even deaths. Over the last seven decades, vaccination strategies have been utilized to protect people from complications of influenza, especially groups at high risk of severe disease. While current vaccination regimens elicit strain-specific antibody responses, they fail to generate cross-protection against seasonal, pandemic and avian viruses. Moreover, vaccines designed to generate influenza- specific T-cell responses are yet to be optimized. During natural infection, viral replication is initially controlled by innate immunity before adaptive immune responses （T cells and antibody-producing B cells） achieve viral clearance and host recovery. Adaptive T and B cells maintain immunological memory and provide protection against subsequent infections with related influenza viruses. Recent studies also shed light on the role of innate T- cells （MAIT cells, y~ T cells, and NKT cells） in controlling influenza and linking innate and adaptive immune mechanisms, thus making them attractive targets for vaccination strategies. We summarize the current knowledge on influenza-specific innate MAIT and γδ T cells as well as adaptive CD8＋ and CD4＋ T cells, and discuss how these responses can be harnessed by novel vaccine strategies to elicit cross-protective immunity against different influenza strains and subtypes.

Groundwater vulnerability assessment using a GIS-based DRASTIC method in the Erbil Dumpsite area (Kani Qirzhala), Central Erbil Basin, North Iraq

Groundwater vulnerability assessment is a crucial step in the efficient management of groundwater resources,especially in areas with intensive anthropogenic activities and groundwater *** the present study,the DRASTIC method was applied using Geographic Information System(GIS)to delineate groundwater vulnerability zones in the Erbil Dumpsite area,Central Erbil Basin,North *** showed that the area was classified into four vulnerability classes:Very low(16.97%),low(27.67%),moderate(36.55%)and high(18.81%).The southern,south-eastern and northern parts of the study area exhibited the highest vulnerability potential,while the central-northern,northern and north-western regions displayed the lowest vulnerability ***,results of the single-parameter sensitivity analysis indicated that amongst the seven DRASTIC parameters,the unsaturated zone and the aquifer media were the most influencing *** conclustion,the correlation of 25 nitrate concentration values with the final vulnerability map,assessed using the Pearson correlation coefficient,yielded a satisfactory result of R=0.72.

Scintigraphic detection of carcinoid tumors with a cost effectiveness analysis

AIM: To evaluate the diagnostic sensitivity and accuracy and the cost-effectiveness of this technique in the detection of gastroenteropancreatic cardnoid tumors and their metastases in comparison with conventional imaging ***: Somatostatin receptor scintigraphy (SRS) was performed in 24 patients with confirmed carcinoids and 7 under investigation. The results were compared with those of conventional imaging methods (chest X-ray, upper abdominal ultrasound, chest CT, upper and lower abdominal CT). Also a cost-effectiveness analysis was performed comparing the cost in Euro of several combinations of SRS with conventional imaging ***: SRS visualized primary or metastatic sites in 71.0% of cases and 61.3% of conventional imagings. The diagnostic sensitivity of the method was higher in patients with suspected lesions (85.7% vs57.1%). SRS was less sensitive in the detection of metastatic sites (78.9% vs 84.2%). The undetectable lesions by SRS metastatic sites were all in the liver. Between several imaging combinations, the combinations of chest X-ray/upper abdominal CT/SRS and chest CT/upper abdominal CT/SRS showed the highest sensitivity (88.75%) in terms of the number of detected lesions. The combinations of chest X-ray/upper abdominal US/SRS and chest CT/upper abdominal ultrasound/SRS yielded also a quite similar sensitivity (82%). Compared to the cost of the four sensitive combinations the combination of chest X-ray/upper abdominal ultrasound/SRS presented the lower cost,1183.99 Euro vs 1251.75 Euro for chest CT/upper abdominal ultrasound/SRS, 1294.93 Euro for chest X/ray/upper abdominal CT/SRS and 1362.75 Euro for chest CT/upper abdominal CT/***: SRS imaging is a very sensitive method for the detection of gastroenteropancreatic carcinoids but is less sensitive than ultrasound and CT in the detection of liver metastases. Role of adipose-derived stromal cells in pedicle skin flap survival in experimental animal models

The use of skin flaps in reconstructive surgery is the first-line surgical treatment for the reconstruction of skin defects and is essentially considered the starting point of plastic surgery. Despite their excellent usability, their application includes general surgical risks or possible complications, the primary and most common is necrosis of the flap. To improve flap survival, researchers have used different methods, including the use of adiposederived stem cells, with significant positive results. In our research we will report the use of adipose-derived stem cells in pedicle skin flap survival based on current literature on various experimental models in animals.

Wound irrigation for preventing surgical site infections

Distribution and quantification of Pb in an evergreen broadleaved forest in three hydrological years

The distribution,quantification and fluxes of Pb were examined in an evergreen broadleaved forest in western Greece for three hydrological *** specifically,concentrations and annual fluxes of Pb were determined in bulk and throughfall deposition as well as *** Pb concentrations were also measured in forest floor and mineral soil up to 80 cm and the isotopic ratios of 206Pb/207Pb were determined in soil layers and the parent rock *** variability in the fluxes of the metal among the three hydrological years were found,evidence of the variability of Pb deposition in *** fractions with a large surface area,like holm oak flowers,had high Pb *** a steady state model and considering the Pb amounts in throughfall and litterfall as inputs on the forest floor,the mean residence time of Pb in the forest floor was 94 years with a coefficient of variation equal to 41%.More observations are needed to lower the variability of the mean residence *** isotopic ratio in the rock material was defined as the lithogenic *** statistical tests showed that the petrol derived Pb migrated to the depth of 20 cm and its percentages in the soil pedon was in the range of 62%in the L horizon to 11%in the 10–20cm *** higher depths(>40 cm)preindustrial anthropogenic Pb affected the isotopic *** the forest under consideration is remote from industrial activities,the results can serve as a baseline for future studies on Pb distribution and quantification.

Ponseti method compared with soft-tissue release for the management of clubfoot:A meta-analysis study

AIM: To compare the functional outcomes of patients who underwent open surgery vs Ponseti method for the management of idiopathic clubfoot and to determine whether correlations exist between functional outcome and radiographic ***: A meta-analysis of the literature was conducted for studies concerning primary treatment of patients with idiopathic clubfoot. We searched PubM ed Medline, EMBASE, and the Cochrane Library databases from January 1950 to October 2011. Meta-analyses were performed on outcomes from 12 studies. Pooled means, SDs, and sample sizes were either identified in the results or calculated based on the results of each ***: Overall, 835 treated idiopathic clubfeet in 516 patients were reviewed. The average follow-up was 15.7 years. Patients managed with Ponseti method did have a higher rate of excellent or good outcome than patients treated with open surgery(0.76 and 0.62, respectively), but not quite to the point of statistical significance(Q = 3.73, P = 0.053). Age at surgery wasnot correlated with the functional outcome for the surgically treated patients(r =-0.32, P = 0.68). A larger anteroposterior talocalcaneal angle was correlated with a higher rate of excellent or good outcomes(r = 0.80, P = 0.006). There were no other significant correlations between the functional and radiographic ***: The Ponseti method should be considered the initial treatment of idiopathic clubfeet, and open surgery should be reserved for clubfeet that cannot be completely corrected.

Dysbetalipoproteinemia: Two cases report and a diagnostic algorithm

Dysbetalipoproteinemia is a rare familial dyslipidemia characterized by approximately equally elevated serum cholesterol and triglyceride levels due to accumulated remnant lipoproteins in apolipoprotein E2/E2 *** is associated with an increased risk for premature cardiovascular disease. Thus, making a diagnosis of dysbetalipoproteinemia aids in assessing cardiovascular risk correctly and allows for genetic counseling. However, the diagnostic work-up can be challenging. Diagnosis of dysbetalipoproteinemia should be considered in patients mixed dyslipidemia when the apolipoprotein B concentration is relatively low in relation to the total cholesterol concentration or when there is significant disparity between the calculated low density lipoprotein(LDL) and directly measured LDL cholesterol concentrations. Other indices are also informative in the diagnostic process. We present herein two phenotypically different cases(a 44-year-old man with severe hypertriglyceridemia and a 49-year-old woman with mixed dyslipidemia) of genotypically proven familial dysbetalipoproteinemia and a diagnostic algorithm of the disease.

Background Next-generation sequencing has revolutionized our perspective on the gut microbiome composition,revealing the true extent of the adverse effects of *** impact of antibiotic treatment on gut microbiota must be considered and researched to provide grounds for establishing new treatment strategies that are less devastating on commensal *** study investigates the impact on gut microbiome when a commonly used antibiotic,azithromycin is administered,as well as uncovers the benefits induced when it is used in combination with lactulose,a prebiotic known to enhance the proliferation of commensal *** 16S rRNA gene sequencing analysis of stool samples obtained from 87 children treated with azithromycin in combination with or without lactulose have been ***'s gut microbial profile was established at the pre-and post-treatment *** Azithromycin caused an increase in the relative abundance of opportunistic pathogens such as Streptococcus that was evident 60 days after *** few days after treatment,children who also received lactulose started to show a higher relative abundance of saccharolytic bacteria such as Lactobacillus,Enterococcus,Anaerostipes,Blautia and Roseburia,providing a protective role against opportunistic *** addition,azithromycin-prebiotic combination was able to provide a phylogenetic profile more similar to the pre-treatment *** It is suggested that during azithromycin treatment,lactulose is able to reinstate the microbiome equilibrium much faster as it promotes saccharolytic microbes and provides a homeostatic effect that minimizes the opportunistic pathogen colonization.
