Objective:To report the outcomes of surgery for a contemporary series of patients with locally advanced non-metastatic renal cell carcinoma(RCC)treated at a referral academic centre,focusing on technical nuances and on the value of a multidisciplinary ***:We queried our prospective institutional database to identify patients undergoing surgical treatment for locally advanced(cT3-T4 N0-1 M0)renal masses suspected of RCC at our centre between January 2017 and December ***:Overall,32 patients were included in the analytic *** these,12(37.5%)tumours were staged as cT3a,8(25.0%)as cT3b,5(15.6%)as cT3c,and 7(21.9%)as cT4;6(18.8%)patients had preoperative evidence of lymph node ***(28.1%)patients underwent nephron-sparing surgery while 23(71.9%)received radical nephrectomy.A template-based lymphadenectomy was performed in 12 cases,with evidence of disease in 3(25.0%)at definitive histopathological *** cases of RCC with level IV inferior vena cava thrombosis were successfully treated using liver transplant techniques without the need for extracorporeal *** intraoperative complications were recorded in 3(9.4%)patients,no postoperative major complications(Clavien-Dindo3)were *** histopathological analysis,2(6.2%)patients who underwent partial nephrectomy harboured oncocytoma,while the most common malignant histotype was clear cell RCC(62.5%),with a median Leibovich score of 6(interquartile range 5e7).Conclusion:Locally advanced RCC is a complex and heterogenous disease posing several challenges to surgical *** experience confirms that provided careful patient selection,surgery in experienced hands can achieve favourable perioperative,oncological,and functional outcomes.

The very different effects of Cholera Toxin (CT) on cell growth and proliferation may depend on the type of ganglio-side receptors in cell membranes and different signal transduction mechanisms triggered,but other functions relatedto the drug resistance mechanisms can not be *** effect of CT treatment on the "in vitro" clonogenicity,thePopulation Doubling Time (PDT),apoptosis,PKA activation and Bax and Bcl-2 expression was evaluated in WEHI-3Bcell line and its CT-resistant subclone (WEHI-3B/CTRES).In WEHI-3B parental cells the dramatic accumulation ofcAMP induced by CT correlated well with PKA activation,increased PDT value,inhibition of clonogenicity and apop-tosis.H-89 treatment inhibited PKA activation by CT but did not protect the cells from apoptosis and growth *** WEHI-3B/CTRES no significant CT-dependent accumulation of cAMP occurred with any increase of PKA activityand *** CT resistant cells (WEHI-3B/CTRES),Bcl-2 expression was down regulated by both CT or drug treatment(eg.,ciprofloxacin,CPX) although these cells were protected from CT-dependent apoptosis but not from *** from other cell models described,down regulation of Bcl-2 is proved to be independent on cAMPaccumulation and PKA *** observations support the implication of cAMP dependent kinase (PKA) in theinhibition of WEHI-3B cells growth and suggest that,in WEHI-3B/CTRES,Bcl-2 expression could be modulated byCT in the absence of cAMP *** in consideration of many contradictory data reported in literature,ourcell models(of one sensitive parental cell strain and two clones with different uncrossed specific resistance to CT andCPX)provides a new and interesting tool for better investigating the relationship between the CT signal transductionmechanisms and Bcl-2 expression and function.

Evaluation of Endectocides and Conventional Acaricides in the Control of the <i>Boophilus microplus</i>on Field-Kept Dairy Cattle in the State of Pernambuco-Brazil

The aim of the present study was to evaluate different acaricide treatments for the control of Boophilus microplus on field-kept dairy cattle in the state of Pernambuco, Brazil. The first phase of the experiment consisted of collecting the ingurgitated female Boophilus microplus directly from the hosts for attainment of larvae. After the attainment of larvae, the artificial infestation procedure began on the cattle, with larvae ages ranging from 7 to 14 days. On Day 0, animals were separated into 12 groups to receive the corresponding treatment: Abamectin;Ivermectin;Ivermectin LA;Amitraz;Amitraz + Ivermectin;Amitraz + Ivermectin LA;Amitraz + Abamectin;Association (Cypermethrin + Chlorpyrifos + Citronella);Association + Ivermectin;Association + Ivermectin LA;Association + Abamectin;and Control. Subsequent evaluations were made on post-treatment days +7, +14, +21, +28, +35, +42, +49, +56 and +63. Analyzing the post-treatment effectiveness per day, indices revealed considerable variation ranging from 0% to 96.63%. Such indices demonstrate the significant reduction in the number of ticks on the animals in some groups, especially in the Abamectin group. The analysis of the results demonstrates that the use of different avermectines can assist in the development of Boophilus microplus control programs, thereby reducing the number of acaricide applications and production costs related to ticks.

Objective: Sustained withdrawal is a sign of infant distress and may be determined by extreme conditions like organic pathology or relationship problems. The Alarm Distress Baby Scale is the best instrument to identify withdrawal in infants between 2 and 24 months of age. The aim of this study was to assess the association between infant social withdrawal behavior and maternal pathology in the perinatal period. Method: The target sample of the study was 134 mother-child couples examined at the Policlinico Agostino Gemelli General Pediatric Outpatient Clinic and Psychiatric Outpatient Clinic, in Rome. We analyzed the relationship between withdrawal behavior and psychological and organic prenatal/postnatal pathology, using the Chi Square exact test. Results: When the mother suffers from both organic and psychological pathologies in the perinatal period, the risk of withdrawal is increased: the probability of infant withdrawal behavior is increased by maternal organic postnatal pathology by 14 times, by maternal psychological postnatal pathology by 9 times, and by maternal psychological postnatal depression, measured by Edinburgh Postnatal Depression Scale, by almost 4 times. Conclusion: Increased or sustained withdrawal reactions can be observed in unsatisfactory mother-child interaction. Maternal risk factors should be strictly considered in the pediatric clinical evaluation.

Formation of new blood vessels is paramount for tumour growth and metastatic dissemination and vascular endothelial growth factor (VEGF) is one of the key regulators of this process. The purpose of this study was to evaluate the immunohistochemical expression of VEGF in 23 splenic hemangiosarcomas and 7 splenic hemangiomas in dogs. Blood tests performed previous to splenectomy were analysed for correlation with tumour VEGF expression. Results showed significantly higher VEGF expression in hemangiosarcomas than hemangiomas and lower hematocrit values and red cell count in dogs affected with malignant neoplasia (P < 0.05). These findings suggest the presence of high VEGF levels may be related to the malignant vascular proliferation seen in hemangiosarcomas.

AIM: To evaluate carotid intima-media thickening (IMT) and plaques, gallstone disease (GD) and fatty liver (FL) as a function of age. METHODS: In 449 subjects, FL and carotid disease were assessed ultrasonographically. In a subgroup of 65/449 patients with non-alcoholic fatty liver disease (NAFLD), carotid disease, GD and associated factors were determined. RESULTS: FL of unspecified etiology was more common in younger and GD in older individuals. FL subjects had an increased prevalence of IMT and a decreased prevalence of plaques and manifested carotid disease earlier. Plaques were more common in subjects with GD. Age was an independent predictor of carotid disease outcome and FL was a protective factor for plaques. In NAFLD, there was an inverse correlation between body weight and age and the latter independently predicted carotid findings. CONCLUSION: Cardiovascular risk in patients with FL and NAFLD needs to be assessed as a function of age and body weight.

Mesenchymal stromal cells(MSC)have shown their benefits in graft-versus-host disease(GVHD), with five unsettled matters: 1) MSCs expansion in medium with Fetal Bovine Serum(FBS)and its risk of xenoreaction;2) The number of cells indicated for therapy that is relatively high, with the need to optimize the expansion, number and time wise;3) The utilization of third party donors;4) Culture passage number(P);and 5) Source of the cells. This study was designed to determine the superiority of the Platelet Lysates(PL)over FBS on the expansion of MSC, the optimal cell’ plating density and days between each pass, and to investigate if donor total nucleated cells(TNC)obtained from the washouts of discharged bags and filters of hematopoietic stem cell transplantation(HSCT)can be expanded to be used at clinical grade. TNC were removed, plated and after the first passage were cultivated in different concentrations with FBS or PL, and the number of days to reach 80% of confluence was observed. Next, cultures with the same plating density were fed either with PL or with FBS and after seven days counted to analyze how much they had grown in that period. The proliferation of mesenchymal stromal cells in the presence of PL and SFB was averaged 11.88 and 2.5 times, respectively, in a period of 7 days. The highest concentration of plating cells using PL took less time to reach confluence as compared with the three lower ones. This study suggests that the PL is the best choice as a supplement to expand MSC and to allow the proliferation of enough number of MSC at P2 for clinical use.

Cystic dystrophy of the duodenal wall is a rare form of the disease which was described in 1970 by French authors who reported the presence of focal pancreatic disease localized in an area comprising the C-loop of the duodenum and the head of the ***-man authors have defined this area as a"groove".We report our recent experience on cystic dystrophy of the paraduodenal space and systematically review the data in the literature regarding the alterations of this space.A MEDLINE search of papers published between 1966 and 2010 was carried out and 59 paperswere considered for the present study;there were 19 cohort studies and 40 case *** majority of patients having groove pancreatitis were middle *** age was significantly higher in patients having groove *** diagnosis of cystic dystrophy of the duodenal wall can now be assessed by multi-detector computer tomography,magnetic resonance imaging and endoscopic *** latter two techniques may also add more information on the involvement of the remaining pancreatic gland not involved by the duodenal malformation and they may help in differentiating"groove pancreatitis"from "groove adenocarcinoma".In conclusion,chronic pan-creatitis involving the entire pancreatic gland was present in half of the patients with cystic dystrophy of the duodenal wall and,in the majority of them,the pan-creatitis had calcifications.
