Pancreatic paracoccidioidomycosis simulating malignant neoplasia:Case report

Paracoccidioidomycosis is a systemic granulomatous disease caused by fungus,and must be considered in the differential diagnosis of intra-abdominal tumors in endemic *** report a rare case of paracoccidioidomycosis in the pancreas.A 45-year-old man was referred to our institution with a 2-mo history of epigastric abdominal pain that was not diet-related,with night sweating,inappetence,weight loss,jaundice,pruritus,choluria,and acholic feces,without signs of sepsis or palpable *** ultrasonography(US)showed a solid mass of approximately 7 cm×5.5 cm on the pancreas *** computerized tomography showed dilation of the biliary tract,an enlarged pancreas(up to 4.5 in the head region),with dilation of the major pancreatic *** patient underwent exploratory laparotomy,and the surgical description consisted of a tumor,measuring 7 to 8 cm with a poorly-defined margin,adhering to posterior planes and mesenteric vessels,showing an enlarged bile *** drainage of the biliary tract,Rouxen-Y gastroenteroanastomosis,lymph node excision,and biopsies were performed,but malignant neoplasia was not *** analysis showed chronic pancreatitis and a granulomatous chronic inflammatory process in the choledochal lymph ***-alcohol resistant bacillus and fungus screening were ***-needle aspiration of the pancreas was performed under US *** smear was compatible with infection by Paracoccidioides *** report a rare case of paracoccidioidomycosis simulating a malignant neoplasia in the pancreas head.

Based on the concentration of Malathion used in the field, we evaluated the genotoxic potential of low concentrations of this insecticide on meristematic and F1 cells of Allium cepa and on rat hepatoma tissue culture（HTC cells）. In the A. cepa, chromosomal aberrations（CAs）, micronuclei（MN）, and mitotic index（MI） were evaluated by exposing the cells at 1.5,0.75, 0.37, and 0.18 mg/m L of Malathion for 24 and 48 hr of exposure and 48 hr of recovery time. The results showed that all concentrations were genotoxic to A. cepa cells. However,the analysis of the MI has showed non-relevant effects. Chromosomal bridges were the CA more frequently induced, indicating the clastogenic action of Malathion. After the recovery period, the higher concentrations continued to induce genotoxic effects, unlike the observed for the lowest concentrations tested. In HTC cells, the genotoxicity of Malathion was evaluated by the MN test and the comet assay by exposing the cells at 0.09, 0.009, and0.0009 mg/5 m L culture medium, for 24 hr of exposure. In the comet assay, all the concentrations induced genotoxicity in the HTC cells. In the MN test, no significant induction of MN was observed. The genotoxicity induced by the low concentrations of Malathion presented in this work highlights the importance of studying the effects of low concentrations of this pesticide and demonstrates the efficiency of these two test systems for the detection of genetic damage promoted by Malathion.

Related to complexity, there is a wide diversity of concepts, ranging from ‘‘systemic" to ‘‘complex", implying a need for a unified terminology. Per different authors, the main drivers of complexity can be found in human behaviour and uncertainty. This complexity, structural or dynamic can be organizational, technological, or nested in their relationship. ISO international standard 31000:2009 definition of risk management ‘‘coordinated activities to direct and control an organization with regard to risk", when applied to economic sectors, industry, services, project, or activity, it requires the use of models or theories as guidelines. Therefore, as its basic elements comprehend human behaviour and/or uncertainty, risk management to be effective and adapted as much as possible to reality, must be operational within complex systems, as already demonstrated in different R&D environments. Risk management faces demanding challenges when approaching specific and endogenous needs, such as the mining sector. This paper presents a multivariable function analysis methodology approach based on complex system modelling and through real data corresponding to a risk management tool in the mining sector.

Background: This study aimed to determine the litterfall production, accumulation, decomposition rate and nutrient stocks, and to estimate the soil carbon(C) and nitrogen(N) stocks in three palm swamp ecosystems with different land use intensities in the Southeast of ***: Three palm swamp ecosystems with different land use intensities were evaluated: Agua Doce(AD),conserved area;Capivara(CV), area with small agricultural practices;and Buriti Grosso(BG), area with an intensive land use. Produced and accumulated litterfall from the Cerrado areas surrounding the palm swamps were collected from October 2014 to September 2015, and the Ca, Mg, K, and P concentrations were determined in the dry and rainy seasons. Soil samples were collected in the surrounding Cerrado and within the palm swamps until 100 cm soil depth to determine bulk density and soil C and N contents and ***: Annual litterfall production in the Cerrado surrounding palm swamps was similar in AD(3.58 Mg·ha^-1·year^-1) and CV(3.79 Mg·ha^-1·year^-1), and was lower in BG(2.84 Mg·ha^-1·year^-1), and was more intense during the dry season. Furthermore, litterfall accumulation was higher in CV(7.12 Mg·ha^-1·year^-1) and BG(6.75 Mg·ha^-1·year^-1), culminating in lower decomposition rates. AD showed the highest decomposition rate(0.60) due to its vegetation structure and conservation. The macronutrient contents from the production and accumulated litterfall had a low influence of the land use, decreasing in the following order: Ca > K > Mg > P. Soils in CV and BG palm swamp areas showed higher values of C and N contents, due to the use of land for agricultural crops and cattle raising in an extensive(rangeland) system. AIM: To evaluate and compare the quality of life of patients submitted to XEN~? implant or trabeculectomy and the relationship with potentially involved ***: A cross-sectional study of patients with advanced open-angle glaucoma who underwent implantation of XEN~?(group 1) and trabeculectomy(group 2) between October 2015 and February 2017. The studied variables were: age, gender, follow-up time, need of topical anti-hypertensive therapy, visual acuity and intraocular pressure(IOP). The quantification of the quality of life was attained through the Glaucoma Symptom Scale(GSS) questionnaire. RESULTS: Totally 34 eyes(34 patients) were included, 17 in each group. The mean GSS scores for group 1 were 42.6±6.8(median, 47; p25, 36.5; p75, 48.5) and for group 2 it was 41.6±7.0(median, 43; p25, 36.5; p75, 47.0; P=0.34). There was a strong negative correlation between the need for topical anti-hypertensive drugs and the GSS result in both groups(r=-0.88, P<0.01, r=-0.59, P=0.01, respectively) and a moderate negative correlation with IOP in group 1(r=-0.50, P=0.03).CONCLUSION: The analysis demonstrates the noninferiority of medium-term quality of life of one group in relation to the other(XEN~? implant and trabeculectomy). The number of topical anti-hypertensive drugs and IOP negatively influenced the quality of life.
