Intestinal neuronal dysplasia type B:A still little known diagnosis for organic causes of intestinal chronic constipation

Intestinal neuronal dysplasia type B(IND-B) is a controversial entity among the gastrointestinal neuromuscular disorders. It may occur alone or associated with other neuropathies, such as Hirschsprung's disease(HD). Chronic constipation is the most common clinical manifestation of patients. IND-B primarily affects young children and mimics HD, but has its own histopathologic features characterized mainly by hyperplasia of the submucosal nerve plexus. Thus, IND-B should be included in the differential diagnoses of organic causes of constipation. In recent years, an increasing number of cases of IND-B in adults have also been described, some presenting severe constipation since childhood and others with the onset of symptoms at adulthood. Despite the intense scientific research in the last decades, there are still knowledge gaps regarding definition, pathogenesis, diagnostic criteria and therapeutic possibilities for IND-B. However, in medical practice, we continue to encounter patients with severe constipation or intestinal obstruction who undergo to diagnostic investigation for HD and their rectal biopsies present hyperganglionosis in the submucosal nerve plexus and other features, consistent with the diagnosis of IND-B. This review critically discusses aspects related to the disease definitions, pathophysiology and genetics, epidemiology distribution, clinical presentation, diagnostic criteria and therapeutic possibilities of this still little-known organic cause of intestinal chronic constipation.

In the last decade, evidence-based medical practice has been supported on a large scale by computerized decision support tools, aiming to reduce diagnostic and therapeutic uncertainty, complementing the actions of the health professional. With technological developments, it is now possible to consider these systems as part of clinical intervention, both for the diagnosis and treatment of diseases. The literature has described the implementation of e-health tools, that is, technological innovations in the health area such as software, applications, serious games, among others, as a strategy to improve the process and adherence to treatment. However, there is still no standardized instrument in Brazil that can be used to guide the development, from the research phase, and the implementation of these tools as a health intervention, also impacting patient outcomes. With the objective of investigating a new therapeutic and preventive form, based on intervention with a computerized system, this work proposes the creation of guidelines for the registration and implementation of e-health tools as a clinical intervention. The proposal aims to be able to assist in the reporting standardization from the development stage to the application of the e-health tool helping in the treatment of diseases, registering all the experience lived in the research and applying it in different contexts of health.

Renin-Angiotensin system (RAs) is involved with hypertension and other cardiovascular diseases. However, the association of RAs components to cancer still causes suspicion. To try to clarify this, here we aimed to show this association for three important components: Angiotensin Converting Enzyme 1 (ACE1), Angiotensin Type 1 Receptor (AGTR1) and Angiotensin Type 2 Receptor (AGTR2). The first articles show that association of RAs components with cancer dates back to the 70’s. ECA1 and AGTR1 have close association with cancer and ACE1 inhibitors or AGTR1 blockers are candidates to treatment of some tumors. Moreover, the action of AGTR2 is still controversial, but most studies show that the increased expression of AGTR2 can attack the cancer cells. In breast cancer, these components have also been widely studied and many works have shown that the correlation exists. Therefore specific target using these RAs components could be a beneficial, novel therapy to various tumors.

suicide Attempts in a General Hospital: A Case-Control

Objective: This study aims is to conduct a comparative analysis of the clinical and sociodemographic profiles of individuals referred to Consultation-Liaison Psychiatry (CLP) at a tertiary general hospital, specifically focusing on those seeking assistance due to suicide attempts (Ts) versus CLP requests for other reasons (non-Ts) between the years 2010 and 2020. Additionally, we aim to assess factors associated with suicide attempts. Method: This retrospective controlled study involves a thorough examination of medical records encompassing 3913 patients who received CLP services at the Hospital de Base (HB) and the Children and Maternity Hospital (HCM) of são José do Rio Preto Medical school Foundation (Funfarme) from June 2010 to June 2020. The patient cohort is categorized into two groups: those with requests for sA (n = 677) and those with requests for other reasons. Results: In the sA group, as opposed to the non sA group, statistically significant differences were observed for gender (predominantly female), employment status (employed), marital status (single), and age (younger). Notably, a higher prevalence of psychiatric antecedents (88.7%, n = 555 vs. 73.6% n = 2018) was found in the sA group, indicating a significant association. The frequency of sA requests remained stable over the years. Among the sA group, mood disorders were the most prevalent (39%, n = 372), followed by disorders related to substance use (21.2%, n = 202), and personality disorders (16%, n = 152). A history of previous suicide attempts emerged as a crucial factor for the recurrence of sA (p-value ≤ 0.001). Exogenous intoxication emerged as the most frequently employed method in attempts (79.6%, n = 539), particularly prevalent among patients diagnosed with mood disorders (41%, n = 314). Essential role of small bowel capsule endoscopy in reclassification of colonic inflammatory bowel disease type unclassified

AIM To evaluate the role of small bowel capsule endoscopy(sBCE) on the reclassification of colonic inflammatory bowel disease type unclassified(IBDU).METHODs We performed a multicenter, retrospective study including patients with IBDU undergoing sBCE, between 2002 and 2014. sBCE studies were reviewed and the inflammatory activity was evaluated by determining the Lewis score(Ls). Inflammatory activity was considered significant and consistent with Crohn's disease(CD) when the Ls ≥ 135. The definitive diagnosis during follow-up(minimum 12 mo following sBCE) was based on the combination of clinical, analytical, imaging, endoscopic and histological *** Thirty-six patients were included, 21 females(58%) with mean age at diagnosis of 33 ± 13(15-64) years. The mean follow-up time after the sBCE was 52 ± 41(12-156) mo. The sBCE revealed findings consistent with significant inflammatory activity in the small bowel(Ls ≥ 135) in 9 patients(25%); in all of them the diagnosis of CD was confirmed during follow-up. In 27 patients(75%), the sBCE revealed no significant inflammatory activity(Ls se patients, the diagnosis of Ulcerative Colitis(UC) was established in 16 cases(59.3%), CD in 1 case(3.7%) and 10 patients(37%) maintained a diagnosis of IBDU during follow-up. A Ls ≥ 135 at sBCE had a sensitivity = 90%, specificity = 100%, positive predictive value = 100% and negative predictive value = 94% for the diagnosis of *** sBCE proved to be fundamental in the reclassification of patients with IBDU. Absence of significant inflammatory activity in the small intestine allowed exclusion of CD in 94% of cases.

N-tosyl-nitropyrroles as Dienophiles in Polar Cycloaddition Reactions Developed in Protic Ionic Liquids

N-tosyl-2-nitropirrole and N-tosyl-3-nitropirrole reacts with poorly and activated dienes using protic ionic liquids as reaction media. They exhibit a dienophilic character producing the corresponding indoles through a Diels-Alder process. In all cases the presence of protic ionic liquids as reaction media improve the yields respect to the use of molecular solvent, while the temperature and the reaction time decrease. A similar behavior was observed in disubstituted N-tosyl-pyrroles.

A significant number of stallions produce low quality ejaculates with high sensibility to chilling. single Layer Centrifugation (sLC) with Andro-coll-ETM has been presented as an efficient method of selecting good quality spermatozoa. In the current study, changes in sperm quality (motility, viability, acrosome integrity and DNA damage) occurring during storage at 5?C for a maximum of 72 h, were investigated. For that, one ejaculate from 12 stallions was split in two aliquots: control and sLC-selected. Both aliquots were chilled and stored at 5?C and spermatozoa were evaluated for motility, viability and acrosome integrity at 24, 48 and 72 h post collection. DNA damage was evaluated at 48 h post collection using the comet assay. In the sLC-selected aliquots, there was a significant improvement in terms of progressive motility (0 h: P = 0.005;24 h: P 0.05). sLC with Androcoll-ETM improved semen quality prolonging sperm longevity of chilled semen (P = 0.012). This positive effect was more evident in ejaculates most sensitive to chilling that had a sharp decrease in motility in the first 24 h of refrigeration and for all ejaculates at 72 h post-chilling. Therefore, this method reveals to be a useful technique for selecting spermatozoa and maintain sperm quality during storage.

Artemisia annua: A New Version of a Traditional Tea under Randomized, Controlled Clinical Trial for the Treatment of Malaria

Introduction: The traditional antimalarial tea Artemisia annua, indicated for centuries in China to treat fevers, is again arousing interest for the treatment of malaria due to improvements attained in the plant composition by a few Institutions throughout the world, including the state University of Campinas (UNICAMP), Brazil, increasing its principal component by more than 100 times as from standard varieties, giving 1% in artemisinin and an expressive biomass yield such as 2 tons of dried leaves/hectare. Clinical trials carried out with this material in African countries have proven its therapeutic potential for a new generation of Artemisia tea in the treatment of falciparum malaria. In addition to artemisinin, recent studies have identified and quantified other compounds present in the crude extract and characterized their contributions to the anti-malarial efficacy, including their action against chloroquine-resistant strains. The majority of the clinical trials carried out with Artemisia tea in African countries have shown that the control of the parasitaemia is efficient in the initial treatment period, but few trials have followed the patients up to the 28sup>thsup> day. This first clinical trial carried out in Brazil with the A. annua infusion, after toxicological trials that defined the safety of this form of medication. Methods: The therapeutic efficacy of the tea was measured in patients with falciparum malaria over 28 days, comparing it with the current first-line treatment namely artemether-lumefantrine (Coartemsup>?sup>). The trial was carried out in controlled groups according to official protocol approved by the National Ethics in Research Committee (CONEP: 77/ 2011) and a rigorous control of the 17 patients with non-serious cases of falciparum malaria, recruited in the following three municipalities of the state of Pará, Brazil: Tucuruí, Goianésia do Pará and Anajás. Dissociation of the N400 component between linguistic and non-linguistic processing: A source analysis study

The N400 component is commonly associated with the detection of linguistic incongruity. A few studies have shown that the N400 can also be elicited by non-linguistic stimuli. Different spatiotemporal patterns were observed between the typical Linguistic N400 and the Non-linguistic N400, suggesting distinct brain generators. The aim of this study was to investigate the presence of an N400 in response to linguistic and non-linguistic stimuli, and to specify anatomical sources of both N400s using a novel analysis method: the Bayesian Model Averaging (BMA) distributed source model. Picture-word and environmental soundpicture associations, either congruent or incongruent, were presented to ten young healthy adults while highdensity ERP recordings were made. standard electrophysiological analyses confirmed that the N400 was not specific to linguistic incongruity but was also elicited by environmental sound-picture incongruities. Different topographic distributions were obtained for the Linguistic N400 and Non-linguistic N400. BMA analysis showed that the Linguistic N400 generators were mostly located in the left superior temporal gyrus, whereas the sources of the Non-linguistic N400 were identified mostly in the right middle and superior temporal gyri. Detection of linguistic incongruities recruited cerebral areas commonly associated with language processing, whereas non-linguistic incongruities recruited right cerebral regions usually associated with auditory processing of non-linguistic stimuli. The Linguistic and Non-linguistic N400s appear to be elicited by similar cognitive mechanisms assumed by different cerebral areas depending on the type of material to be processed. The present findings support the existence of parallel pathways for the processing of linguistic and non-linguistic incongruities.
