Is there a window of opportunity to optimize trastuzumab cardiac monitoring?

BACKGROUND It remains unclear whether the current arbitrary screening recommendations of trastuzumab-related cardiotoxicity provides an adequate balance between preventing heart damage and curtailing a curative *** To determine the incidence rate and consequences of trastuzumab-induced cardiotoxicity as adjuvant treatment in a real-world *** We present a retrospective analysis of cardiac function measured by echocardiogram at baseline and every 3 mo during trastuzumab *** was defined as a drop in left ventricular ejection fraction(LVEF)≥10%from baseline and/or any drop<50%.RESULTS Between January 2011 and December 2014,407 patients were ***(93.6%)were treated with an anthracycline followed by a taxane-based regimen and trastuzumab for 12 *** patients(9.8%)had *** of them were symptomatic,and 28(72.5%)completely recovered *** happened early as shown by LVEF measured on echocardiogram 2 to 4 as compared to 5 to 7(odds ratio=2.47,95%confidence interval:1.09,5.63,P=0.024).There were 54 deaths(13.3%)during the 70-mo follow-up period;1(0.2%)was attributed to late cardiotoxicity(4 years after treatment).The absence of symptomatic cardiotoxicity during trastuzumab treatment and moreover the early occurrence on the treatment period may translate into a strategy to evaluate less *** We observed a 10%rate of asymptomatic cardiotoxicity,which mirrors the results from the large adjuvant *** being transient,an LVEF drop led to frequent treatment delays and *** remains unclear whether LVEF decline is predictive of late cardiotoxicity,and treatment efficacy is compromised.

Acute heart failure(AHF)is a major cause of unplanned hospitalisations in the elderly and is associated with high *** prevalence has grown in the last years due to population aging and longer life expectancy of chronic heart failure *** international societies have provided guidelines for the management of AHF in the general population,scientific evidence for geriatric patients is often lacking,as these are underrepresented in clinical *** have a different risk profile with more comorbidities,disability,and frailty,leading to increased morbidity,longer recovery time,higher readmission rates,and higher ***,therapeutic options are often limited,due to unfeasibility of invasive strategies,mechanical circulatory support and cardiac ***,the in-hospital management of AHF should be tailored to each patient's clinical situation,cardiopulmonary condition and geriatric *** care should be considered in some cases,in order to avoid unnecessary diagnostics and/or *** discharge,a strict follow-up through outpatient clinic or telemedicine is can improve quality of life and reduce rehospitalisation *** aim of this review is to offer an insight on current literature and provide a clinically oriented,patient-tailored approach regarding assessment,treatment and follow-up of elderly patients admitted for AHF.

目前,汉语在全球范围内的影响力与日俱增,全球范围内汉语教学机构的数量也越来越多。其中,孔子学院是目前汉语教学最主要的语言学习交流机构,拉丁美洲也是如此。智利目前有两所孔子学院,分别是圣托马斯大学孔子学院和智利天主教大学孔子学院。根据拉丁美洲孔子学院区域中心(CRICAL)制定的“承认拉丁美洲的文化特性”的目标,智利孔子学院提供的项目有效性评价对教学质量评估起到了非常重要的作用。基于有效性评价可以明确孔子学院教学质量的有效性和亟待解决的方面,帮助当地孔子学院调整教学方向。本研究借助修订版SEEQ(教学效果评价)问卷,旨在研究学生对智利天主教大学孔子学院汉语和中国文化学习的项目反馈。本文共分为五章:第一章,首先介绍了研究背景和目的;其次,简述SEEQ问卷的起源、发展和实施情况。接着,概述了本文的研究内容和方法,并介绍了智利天主教大学孔子学院的起源、发展和现状,最后说明了本文的研究意义。第二章介绍了本文的研究设计,其中详细说明了调研工具的特点和对象概况。第三章是对智利天主教大学孔子学院学生SEEQ问卷结果的描述和统计分析。此外,本文还探讨了被调查者的个体因素,如汉语学习水平差异对孔子学院课程评价结果的影响。第四章为本文主要结论,并详细提出了一系列建议,为智利天主教大学孔子学院制定制定因地制宜的未来发展规划提出建议。第五章简述了本研究的创新点和局限性。

Future drug discovery and toxicology testing could benefit significantly from more predictive and multi-parametric readouts from in vitro *** the recent advances in the field of microfluidics,and more recently organ-on-a-chip technology,there is still a high demand for real-time monitoring systems that can be readily embedded with *** addition,multi-parametric monitoring is essential to improve the predictive quality of the data used to inform clinical studies that *** we present a microfluidic platform integrated with in-line electronic sensors based on the organic electrochemical *** goals are twofold,first to generate a platform to host cells in a more physiologically relevant environment(using physiologically relevant fluid shear stress(FSS))and second to show efficient integration of multiple different methods for assessing cell morphology,differentiation,and *** include optical imaging,impedance monitoring,metabolite sensing,and a wound-healing *** illustrate the versatility of this multi-parametric monitoring in giving us increased confidence to validate the improved differentiation of cells toward a physiological profile under FSS,thus yielding more accurate data when used to assess the effect of drugs or ***,this platform will enable high-content screening for in vitro drug discovery and toxicology testing and bridges the existing gap in the integration of in-line sensors in microfluidic devices.

Previous studies on the genus Clusia have shown anti-inflammatory and antiproliferative effects of the leaf extracts,but its antinociceptive activity has never been *** the present study,the antinociceptive activity of the hexane extract of the leaves of Clusia nemorosa ***,called HECn,was *** activity was evaluated using acetic acid-induced writhing,formalin,and hot-plate *** experiments were carried out on male Swiss *** extract(1 400 mg kg 1),given by intraperitoneal route(i.p.) 1 h prior to testing,produced a dose-dependent inhibition on the number of abdominal writhings,with an ID50 of 62 mg kg *** addition,HECn was able to prevent the visceral pain induced by acetic acid in mice for at least 2 *** the formalin test,HECn had no effect in the first phase,but produced an analgesic effect on the second phase with the inhibition of licking *** HECn did not show a significant analgesic effect in the hot plate *** with yohimbine attenuated the antinociceptive effect induced by HECn in the writhing ***,naloxone,atropine,or haloperidol did not affect antinociception induced by HECn in the writhing ***,these results indicate that the extract from the leaves of Clusia nemorosa produces antinociception in models of chemical pain through mechanisms that suggest participation of the adrenergic systems pathway.

Analysis of the histologic features in the differential diagnosis of intrahepatic neonatal cholestasis

AIM:To compare the histologic features of the liver in intrahepatic neonatal cholestasis(IHNC)with infectious,genetic-endocrine-metabolic,and idiopathic etiologies. METHODS:Liver biopsies from 86 infants with IHNC were *** inclusion criteria consisted of jaundice beginning at 3 mo of age and a hepatic biopsy during the 1st year of *** following histologic features were evaluated:cholestasis,eosinophilia,giant cells,erythropoiesis,siderosis,portal fibrosis,and the presence of a septum. RESULTS:Based on the diagnosis,patients were classified into three groups:group 1(infectious;n=18),group 2(genetic-endocrine-metabolic;n=18),and group 3(idiopathic;n=50).There were no significant differences with respect to the following variables:cholestasis,eosinophilia,giant cells,siderosis,portalfibrosis,and presence of a septum.A significant difference was observed with respect to erythropoiesis,which was more severe in group 1(Fisher's exact test,P=0.016). CONCLUSION:A significant difference was observed in IHNC of infectious etiology,in which erythropoiesis was more severe than that in genetic-endocrine-metabolic and idiopathic etiologies,whereas there were no significant differences among cholestasis,eosinophilia,giant cells,siderosis,portal fibrosis,and the presence of a septum.

Liver transplantation for recurrent hepatic adenoma

Hepatic adenoma (HA) is a rare indication for liver transplantation (LTx). So far 20 cases of LTx for HA are reported in PubMed. In rare cases HA presents as multiple hepatic adenomas or recurrent adenoma after initial liver resection and in such cases LTx is the only potential cure and prevents the risk of bleeding or cancer transformation into hepatocellular carcinoma. We report the case of a 56 years old lady who underwent a left hepatectomy for giant adenoma in 2005 and resection of segment Ⅴ-Ⅵ for recurrence of liver adenoma in 2007. She developed a second recurrence of HA with 3 new lesions in the right liver in 2008. The patient underwent LTx. After 3 years the patient is alive with no evidence of disease. LTx is indicated in patients with HA in which resection is not technically feasible.

Ten years of hip fractures in Italy: For the first time a decreasing trend in elderly women

AIM:To evaluate the hospitalization rate of femoralneck fractures in the elderly Italian population over ten ***:We analyzed national hospitalizations records collected at central level by the Ministry of Health from 2000 to ***-and sex-specific rates of fractures occurred at femoral neck in people≥65 years *** performed a sub-analysis over a three-year period(2007-2009),presenting data per five-year age groups,in order to evaluate the incidence of the hip fracture in the oldest ***:We estimated a total of 839008 hospitalizations due to femoral neck fractures between 2000 and2009 in people≥65,with an overall increase of 29.8%over 10 *** incidence per 10000 inhabitants remarkably increased in people≥75,passing from158.5 to 166.8(+5.2%)and from 72.6 to 77.5(+6.8%)over the ten-year period in women and men,*** oldest age group(people>85 years old)accounted for more than 42%of total hospital admissions in 2009(n=39000),despite representing only 2.5%of the Italian ***,women aged>85accounted for 30.8%of total fractures,although they represented just 1.8%of the general *** results of this analysis indicate that the incidence of hip fractures progressively increased from 2000 to 2009,but a reduction can be observed for the first time in women≤75(-7.9%between 2004 and 2009).CONCLUSION:Incidence of hip fractures in Italy are continuously increasing,although women aged 65-74years old started showing a decreasing trend.
