Venous Catheterization Experimental Model in Rabbits:Histological Alterations in the Catheter Region

Central venous access is one of the most common surgery procedures worldwide, especially in pediatric surgery. Local and regional complications as the result of venous catheter permanence time are frequently described as: thrombosis, infection, edema and local cellulite, movement and loss of the catheter. Other severe complications such as endocardiac and hemorrhagic lesions are also described and considered the cause of catheter early removal. In the literature few studies have addressed vascular and perivascular lesions and complications as the result of central venous access to peripheral veins, given the difficulty of setting up venous catheterization experimental models to study blood vessels and perivascular tissue alterations after catheterization. In the present venous catheterization experimental model, rabbits were divided into two groups based on the time that the venous catheters were maintained in their veins. Group a composed of 7 New Zealand male rabbits was submitted to a 15-day treatment;and the 6 New Zealand male rabbits of group B were treated during 90 days. Both groups presented similar inflammatory conditions since there was no significant difference between groups. Therefore, the results may well suggest that the endothelial inflammatory reaction could have developed at an early initial short period and by maintaining the catheter, the inflammatory reactions would have decreased or disappeared. Aimed at studying these vascular and perivascular alterations in venous catheterization, the present study proposes an experimental rabbit model that allows the analysis of differences in local vascular and perivascular histological variations and compares histological differences between both venous catheterization groups each of them with different periods of treatment.

Review of the Ahmed versus Baerveldt study-5-year treatment outcomes

The results from the tube versus trabeculectomy(TVT)study provided the evidence to support the use of aqueous shunt surgery and its increasing popularity in the *** Ahmed versus Baerveldt(AVB)study,a randomized controlled study,compared the efficacy and safety between two of the most commonly used glaucoma drainage implants.A significant proportion of the participants had failed trabeculectomy,neovascular or uveitic *** 5-year results showed that the cumulative probability for failure is significantly lower for Baerveldt compared to Ahmed group(52.3%vs.40.0%,P=0.039).The most common reason for failure was high intraocular pressure(IOP)but 4%of the Baerveldt group has refractory *** surgeries were also effective in reducing dependency on intra-ocular pressure lowering medications but the Baerveldt group is superior in this aspect(median of 1 medication compared to 2 medications for Ahmed group).Both surgeries experienced 60-70%rate of complications but most were *** most common long-term complication was corneal decompensation(10%).Overall,the results of the AVB study mirrored the results from the Ahmed Baerveldt Comparison(ABC)*** conclusion,the 5-year report from the AVB study suggested that the Baerveldt tube is more appropriate for eyes which require much lower IOP such as eyes with advanced glaucoma or in young *** Ahmed tube would be more suitable for patients who are at risk of hypotony(such as uveitic and neovascular glaucoma)and those who require immediate IOP lowering after surgery.

Reduced lysosomal acid lipase activity: A new marker of liver disease severity across the clinical continuum of non-alcoholic fatty liver disease?

Lysosomal acid lipase(LAL)plays a key role in intracellular lipid *** LAL activity promotes increased multi-organ lysosomal cholesterol ester storage,as observed in two recessive autosomal genetic diseases,Wolman disease and Cholesterol ester storage *** liver steatosis and accelerated liver fibrosis are common features in patients with genetic LAL *** contrast,few reliable data are available on the modulation of LAL activity in vivo and on the epigenetic and metabolic factors capable of regulating its activity in subjects without homozygous mutations of the Lipase A *** the last few years,a less severe and non-genetic reduction of LAL activity was reported in children and adults with non-alcoholic fatty liver disease(NAFLD),suggesting a possible role of LAL reduction in the pathogenesis and progression of the *** with NAFLD show a significant,progressive reduction of LAL activity from simple steatosis to non-alcoholic steatohepatitis and cryptogenic *** cirrhosis of different etiologies,those with cryptogenic cirrhosis show the most significant reductions of LAL *** findings suggest that the modulation of LAL activity may become a possible new therapeutic target for patients with more advanced forms of ***,the measurement of LAL activity may represent a possible new marker of disease severity in this clinical setting.

Hypertension in kidney transplant recipients

Kidney transplantation is considered the treatment of choice for end-stage kidney disease ***,the residual cardiovascular risk remains significantly higher in kidney transplant recipients(KTRs)than in the general *** is highly prevalent in KTRs and represents a major modifiable risk factor associated with adverse cardiovascular outcomes and reduced patient and graft *** definition of hypertension and recognition of special phenotypes and abnormal diurnal blood pressure(BP)patterns is crucial for adequate BP *** by office BP is commonly encountered in these patients,and a high proportion of masked and uncontrolled hypertension,as well as of white-coat hypertension,has been revealed in these patients with the use of ambulatory BP *** pathophysiology of hypertension in KTRs is multifactorial,involving traditional risk factors,factors related to chronic kidney disease and factors related to the transplantation *** the absence of evidence from large-scale randomized controlled trials in this population,BP targets for hypertension management in KTR have been extrapolated from chronic kidney disease *** most recent Kidney Disease Improving Global Outcomes 2021 guidelines recommend lowering BP to less than 130/80 mmHg using standardized BP office *** calcium channel blockers and angiotensin-converting enzyme inhibitors/angiotensin-II receptor blockers have been established as the preferred first-line agents,on the basis of emphasis placed on their favorable outcomes on graft *** aim of this review is to provide previous and recent evidence on prevalence,accurate diagnosis,pathophysiology and treatment of hypertension in KTRs.

Desiccation tolerance （DT）, the ability to survive loss of more than 90% of cellular water （≤0.1 g H2O g^-1 dry mass） for prolonged periods, is common in seeds （90% of angiosperm species） but rare in vegetative tissues （0.04% of land plants; reviewed in Gaff and Oliver, 2013）. Vegetative DT is more widespread in non-tracheophytes, being present in 25 species of algae, 158 bryophytes species, and all lichens, but is rare in tracheophytes, being displayed in 64 pteridophyte and 135 angiosperm species （Gaff and Oliver, 2013）.

A systematic review and meta-analysis of clinical trials implementing aromatase inhibitors to treat male infertility

Aromatase activity has commonly been associated with male infertility characterized by testicular dysfunction with low serum testosterone and/or testosterone to estradiol *** this subset of patients,and particularly in those with hypogonadism,elevated levels of circulating estradiol may establish a negative feedback on the hypothalamic–pituitary–testicular axis by suppressing follicle-stimulating hormone(FSH)and luteinizing hormone(LH)production and impaired *** manipulation via different agents such as selective estrogen modulators or aromatase inhibitors to increase endogenous testosterone production and improve spermatogenesis in the setting of infertility is an off-label option for *** carried out a systematic review and meta-analysis of the literature of the past 30 years in order to evaluate the benefits of the use of aromatase inhibitors in the medical management of infertile/hypoandrogenic ***,eight original articles were included and critically *** steroidal(Testolactone)or nonsteroidal(Anastrozole and Letrozole)aromatase inhibitors were found to statistically improve all the evaluated hormonal and seminal outcomes with a safe tolerability *** the evidence is promising,future prospective randomized placebo-controlled multicenter trials are necessary to better define the efficacy of these medications.

Programmed death-ligand-1(PD-L1)plays a crucial role in the suppression of the antitumor immune response upon interaction with programmed cell death protein-1(PD-1)on cytotoxic T ***-L1 is constitutively expressed on antigen-presenting cells or tumor ***,PD-L1 has been detected by immunohistochemistry in a variety of tumors,including malignant pleural mesothelioma(MPM),in which it has emerged as a predictive biomarker for PD-1/PD-L1 immune checkpoint blockers(ICBs).1,2 PD-L1 expression on tumor cells can be upregulated by several factors including the interferon gamma(IFN-γ)cytokine produced by tumor-infiltrating lymphocytes(TILs).3 In MPM cells,it has been reported that IFN-γupregulates PD-L1 mRNA expression due to the activation of the interferon regulatory factor 1(IRF1)transcription factor,4 but no data have been shown regarding cell surface PD-L1 that is functionally relevant for its contact with PD-1-positive cells.

5-ASA colonic mucosal concentrations resulting from different pharmaceutical formulations in ulcerative colitis

AIM:To compare the mucosal concentrations of 5-aminosalicylic acid(5-ASA)resulting from different pharmaceutical formulations and analyse the influence of inflammation on the mucosal ***:The study included 130 inflammatory bowel disease(IBD)patients receiving 5-ASA as pH-dependent-release formulations(73 patients),time-dependent-release formulations(11 patients),or pro-drugs(18patients).In addition,28 patients were receiving topical treatment(2-4 g/d)with pH-dependent-release *** biopsies were obtained from the sigmoid region during the *** 5-ASA concentrations(ng/mg)were measured in tissue homogenatesusing high-pressure liquid chromatography with electrochemical *** t test and Mann-Whitney test,when appropriate,were used for statistical ***:Patients receiving pH-dependent-release formulations showed significantly higher mucosal concentrations of 5-ASA(51.75±5.72 ng/mg)compared with patients receiving pro-drugs(33.35±5.78 ng/mg,P=0.01)or time-dependent-release formulations(38.24±5.53 ng/mg,P=0.04).Patients with endoscopic remission had significantly higher mucosal concentrations of5-ASA than patients with active disease(60.14±7.95ng/mg vs 35.66±5.68 ng/mg,P=0.02).Similar results were obtained when we compared patients with the histological appearance of remission and patients with active histological inflammation(67.53±9.22 ng/mg vs 35.53±5.63 ng/mg,P<0.001).Significantly higher mucosal concentrations of 5-ASA were detected in patients treated with both oral and topical treatments in combination compared with patients who received oral treatment with pH-dependent-release formulations alone(72.33±11.23 ng/mg vs 51.75±5.72 ng/mg,P=0.03).CONCLUSION:IBD patients showed significant variability in mucosal 5-ASA concentrations depending on the type of formulation,and the highest mean concentration was achieved using pH-dependent-release formulations.

Human sperm pattern of movement during chemotactic ＇e-orientation towards a progesterone source

Human spermatozoa may chemotactically find out the egg by following an increasing gradient of attractant molecules. Although human spermatozoa have been observed to show several of the physiological characteristics of chemotaxis, the chemotactic pattern of movement has not been easy to describe. However, it is apparent that chemotactic cells may be identified while returning to the attractant source. This study characterizes the pattern of movement of human spermatozoa during chemotactic re-orientation towards a progesterone source, which is a physiological attractant candidate. By means of videomicroscopy and image analysis, a chemotactic pattern of movement was identified as the spermatozoon returned towards the source of a chemotactic concentration of progesterone （10 pmol I^-1）. First, as a continuation of its original path, the spermatozoon swims away from the progesterone source with linear movement and then turns back with a transitional movement that can be characterized by an increased velocity and decreased linearity. This sperm behaviour may help the spermatozoon to re-orient itself towards a progesterone source and may be used to identify the few cells that are undergoing chemotaxis at a given time.

Bone tissue engineering represents one of the most challenging emergent fields for scientists and *** failures of autografts and allografts in many pathological conditions have prompted researchers to find new biomaterials able to promote bone repair or regeneration with specific characteristics of biocompatibility,biodegradability and *** advancements for tissue regeneration in bone defects have occurred by following the diamond concept and combining the use of growth factors and mesenchymal stem cells(MSCs).In particular,a more abundant and easily accessible source of MSCs was recently discovered in adipose *** adipose stem cells(ASCs)can be obtained in large quantities with little donor site morbidity or patient discomfort,in contrast to the invasive and painful isolation of bone marrow *** osteogenic potential of ASCs on scaffolds has been examined in cell cultures and animal models,with only a few cases reporting the use of ASCs for successful reconstruction or accelerated healing of defects of the skull and jaw in *** these reports extend our limited knowledge concerning the use of ASCs for osseous tissue repair and regeneration,the lack of standardization in applied techniques makes the comparison between studies *** clinical trials are needed to assess ASC therapy and address potential ethical and safety concerns,which must be resolved to permit application in regenerative medicine.
