The feelings and thoughts of mental health nurses concerning the management of distressed and disturbed in-patients: A comparative qualitative European study

High levels of distress and disturbance amongst those experiencing acute mental illness can be a major problem for mental health nurses. The feelings experienced by these nurses when caring for and supporting disturbed and/or distressed patients along with their concurrent thoughts are not well described in the literature. To date, this complex issue has not been explored within a comparative European context. The objective of this qualitative study was to explore the feelings and thoughts of mental health nurses when supporting and caring for distressed and/or disturbed patients in 6 European countries. Methods: Focus groups were used to collect data from 130 mental health nurses working in acute inpatient psychiatric settings. Results: Data were analysed using content analysis. Findings highlighted 6 broad themes: 1) Mixed emotions: expressive and responsive, 2) Procedure for caring for and supporting disturbed and/or distressed patients, 3) Use of guidelines for caring and supporting disturbed and/or distressed patients, 4) Team and organisational support, 5) Ethical concerns: Cognitive dissonance and 6) Education and training. Commonalities and differences were?found across all themes. Approaches to care, nurses’ role and education, clinical guidelines and/or standards vary from country to country, therefore the care, treatment and management of distressed and/or disturbed patients are various. As a result, mental health nurses have different experiences, various emotional quandaries concurrent with cognitive dissonance and different coping strategies when caring for and supporting distressed and disturbed patients. Conclusions: More emphasis needs to be given to the emotional quandaries and concurrent cognitive dissonance experienced by mental health nurses caring for distressed and/or disturbed inpatients in acute psychiatric settings. Increased access to education and training with particular attention to interpersonal communication and relationship building

Challenges and improvements in testosterone and estradiol testing

Assays that measure steroid hormones in patient care, public health, and research need to be both accurate and precise, as these criteria help to ensure comparability across all clinical and research applications. This review addresses major issues relevant to assay variability and describes recent activities by the US Centers for Disease Control and Prevention （CDC） to improve assay performance. Currently, high degrees of accuracy and precision are not always met for testosterone and estradiol measurements; although technologies for steroid hormone measurement have advanced significantly, measurement variability within and across laboratories has not improved accordingly. Differences in calibration and specificity are discussed as sources of variability in measurement accuracy. Ultimately, a combination of factors appears to cause inaccuracy of steroid hormone measurements, with nonuniform assay calibration and lack of specificity being two major contributors to assay variability. Within-assay variability for current assays is generally high, especially at low analyte concentrations. The CDC Hormone Standardization （HoSt） Program is improving clinical assays, as evidenced by a 50% decline in mean absolute bias between mass spectrometry assays and the CDC reference method from 2007 to 2011. This program provides the measurement traceability to CDC reference methods and helps to minimize factors affecting measurement variability.

Cell-free therapy:a neuroregenerative approach to sensory neuropathy?

Stem cells are considered as key therapeutic tools in the field of regenerative medicine. In the last decade, based on a growing body of knowledge about their mechanisms of action, a new way of exploring the therapeutic potential of stem cells without the need for cell transplantation has been evidenced. This approach, called cell-free therapy, uses stem cells as a source of therapeutic molecules rather than the therapeutic agent per se, and several studies have shown relevant therapeutic effects under a variety of experimental conditions, including nervous system disorders.

Short-term complementary and alternative medicine on quality of life in women with fibromyalgia

BACKGROUND： Fibromyalgia （FMS） is a syndrome characterized by chronic widespread musculoskeletal pain, whose etiology is not completely understood. Different therapeutic approaches have been used with inconsistent results. This observation does not invalidate the continued search for alternative treatments aimed at improving quality of life （QoL） in FMS. OBJECTIVE： This study compared three classica acupuncture （AC）, electroacupuncture （EAC） and promotion of QoL in FMS patients. traditional Chinese medicine （TCM） therapies： moxibustion （MX） in the management of pain and DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： A preliminary, group-assigned, comparative study enrolled 30 women, mean age （46.90_＋9.24） years （range 20-60 years）, who met the 1990 American College of Rheumatology criteria for FMS diagnosis and a pain-pressure threshold （PPT）〈 4 kg/cm2. The study was conducted in a teaching tertiary-care medical institution from May 2010 through April 2012. AC, EAC and MX were delivered for 30 min, once a week, for 8 weeks, bilaterally at Neiguan （PC6）, Hegu （LI4）, Yanglingquan （GB34）, Sanyinjiao （SP6） and Taichong （LR3） acupoints. MAIN OUTCOME MEASURES： Each week, immediately before treatment and after treatment, subjects were tested for PPTs, Wong-Baker Faces Pain Scale （WBFPS; for pain intensity） and Medical Outcomes Study 36-item Short Form Health Survey （SF-36： for QoL）. RESULTS： There was no significant improvement in pain or reduction of tender points in any of the groups studied, at the end of the 8th session. Significant improvement of QoL was perceived in vitality （after AC treatment） and in mental health （after EAC and MX treatments）. CONCLUSION： TCM therapies （AC, EAC and MX） promoted an improvement in the QoL in two areas （vitality and mental health） in FMS women. Further large-scale clinical trials are required to confirm this effect.

Copper,an essential metal for the energy transition,is primarily obtained from chalcopyrite through hydrometallurgical and pyrometallurgical *** risks and harmful impacts of these processes pose significant concerns for environmental and human safety,highlighting the need for more efficient and eco-friendly hydrometallurgical *** review article emphasizes current pro-cesses such as oxidative leaching,bioleaching,and pressure leaching that have demonstrated efficiency in overcoming the complicated chalcopyrite *** leaching operates under benign conditions within the leaching media;nevertheless,the introduction of oxidizing agents provides benefits and ***,a non-aggressive method,has shown a gradual increase in copper extrac-tion efficiency and has been explored using both primary and secondary *** leaching,known for its effectiveness and se-lectivity in copper extraction,is becoming commercially more viable with increased research *** research also provides im-portant data for advancing future research in the field.

Quantitative scintigraphic analysis of the apical seal in Thermafil/Topseal and Real Seal 1/Realseal filled root canals

AIM: To investigate the microleakage of two different root canal obturation systems, using the nuclear medicine ***: Twenty-six single-rooted extracted teeth were selected. The crowns were sectioned to obtain 15-mm long root segments and each tooth was prepared using rotary Pro File instruments. The roots were divided into 2 experimental groups using Real Seal 1 and Real Seal sealer or Thermafil and Top Seal sealer as well as two control groups. On the 7th and the 28 th day the apices were submersed in a solution of 99 m TcPertechnetate during 3 h. The radioactivity was counted using a γ camera. RESULTS: The present study showed that none of the root canal-filled teeth was leakage free. The statistical analyses were made using Kruskal-Wallis and statistical significance was assessed using α = 0.05. Although apical leakage measured in counts per minute(cpm) in the Thermafil/Top Seal group was lower than in the Real Seal/Real Seal group(363 916 ± 180 707.7 cpm vs533 427 ± 414 020.6 cpm) on 7th day and(1 678 200 ± 567 217.4 cpm vs 2 240 518 ± 383 356.7 cpm) on 28 th day, there was no statistical difference(P > 0.05). In the Thermafil/Top Seal group and Real Seal 1/Real Seal group it was found that over time, the number of counts increased between 7 d and 28 d(363 916 ± 180 707.7 cpm vs 1 678 200 ± 567 217.4 cpm) and(533 427 ± 414 020.6 cpm vs 2 240 518 ± 383 356.7 cpm), respectively, with statistically significant differences(Thermafil/Top Seal group, P = 0.015 and Real Seal 1/Real Seal group, P = 0.036).CONCLUSION:Both carrier-based Realseal 1 and Thermafil techniques showed a similar sealing effect,but none of the materials was leakage free.

This study investigated the microleakage of two different root canal obturation systems, using the nuclear medicine approach, with sodium pertechnetate 99mTc. Twenty six single-rooted extracted teeth were selected. The crowns were sectioned to obtain15 mmlong root segments and each tooth was prepared using rotary ProFile? instruments. The roots were divided into 2 experimental groups and two control groups. Twenty root canals were filled, using Thermafil? and Topseal? or MTA Fillapex? as a sealer. On the 7th and the 28th day the apices were submersed in a solution of 99mTc-Pertechnetate during 3 hours. The radioactivity was counted using a gamma camera. Although apical leakage on the 7th day in the Topseal group was reduced compared with RealSeal1, with a statistical significant difference (p = 0.057), on the 28th day, the MTA Fillapex increased the sealing properties (p = 0.017).

^(99m)Tc in the evaluation of microleakage of composite resin restorations with SonicFill^(TM).An in vitro experimental model

Introduction: The composite SonicFillTM (Kerr/Kavo) is indicated for posterior restorations, with a single increment up to 5 mm due to reduced polymerization shrinkage, thus reducing working time. Aim: Evaluation of marginal microleakage with SonicFillTM. Method and Materials: There were sectioned sixty noncarious human molars in the occluso-cervical direction. Class V cavities were prepared on each tooth with gingival margin walls in a standardized way. The specimens were divided into 4 groups: group 1—restored with SonicFillTM (Kerr/Kavo), group 2—restored with FiltekTM SupremeXTE (3M ESPE), group 3—the cavities were not restored;group 4—restored with SonicFillTM (Kerr/Kavo). In groups 1, 2 and 4 the enamel was conditioned with 37% orthophosphoric acid and applied the self-etch adhesive system Clear- fillTM SE BOND (Kuraray). The specimens were stored in distilled water at 37?C for 7 days. After, the specimens, were immersed in a solution of 99mTc-Pertechnetate and the radioactivity was assessed with a gamma camera. The nonparametric Kruskal-Wallis and Mann-Whitney test with Bonferroni correction at a significance level of 5% were used for the statistical analyses. Results: There are significant differences between the positive and negative control groups and between these and experimental groups (p TM and FiltekTM SupremeXTE. Conclusion: The new composite SonicFillTM and FiltekTM SupremeXTE showed no difference concerning dye penetration. The Sonic- FillTM restorative system showed no influence in concerning microleakage.
