Capsule enteroscopy is useful for the therapeutic management of Crohn's disease

AIM: To analyze therapeutic changes in Crohn's disease(CD) patients following video capsule endoscopy(VCE) and to assess the usefulness of Lewis score and the Patency ***: Patency Capsule was performed in every patient that had indication for VCE, and those with negative patency did not undergo VCE. Patients with established CD that underwent VCE between January 2011 and February 2014 were selected for this study; those with suspected CD were excluded, independent of VCE results, since our purpose was to address differences in therapeutic regimen in CD patients before and after VCE. Patients with inconclusive VCE were also excluded. Patients had to be free of non-steroidal anti-inflammatories for at least 1 mo. Those patients who met these criteria were allocated into one of three groups: Staging group(asymptomatic CD patients that underwent VCE for staging of CD), Flare group(patients with active CD), or Post-op group(CD patients evaluated for post-operative recurrence). Lewis score was calculated for every VCE procedure. Statisticalanalysis was performed to address the impact of VCE findings on the therapeutic management of CD patients and to evaluate the utility of the Lewis ***: From a total of 542 VCEs, 135 were performed in patients with CD. Patency capsule excluded nearly 25% of the patients who were supposed to undergo VCE. No videocapsule retention during VCE was reported. From these 135 patients, 29 were excluded because CD diagnosis was not established at the time of VCE. Therefore, a total of 106 patients were included in the final analysis. From these, the majority were in the Staging group(n = 73, 69%), and the remaining were in the Flare(n = 23, 22%) or Post-op(n = 10, 9%) group. Median time between diagnosis and VCE was 5.5 years. Overall, VCE determined changes in the treatment of 40% of patients: only 21% remained free of immunosuppressors after VCE compared to 44% before VCE(P < 0.001). This article describes cases of anti-tumor necrosis factor(TNF)-α-induced autoimmune hepatitis and evaluates the outcome of these patients in relation to their immunosuppressive strategy. A retrospective analysis of medical records was performed in our center, in order to detect cases of autoimmune hepatitis(AIH) associated with anti-TNF biologic agents. We describe and analyze eight cases of AIH following anti-TNF therapy, 7 with infliximab and 1 with adalimumab. A distinction should be made between induction of autoimmunity and clinically evident autoimmune disease. Liver biopsy is useful in detecting the role of the TNF-α antagonist in the development of AIH. The lack of relapse after discontinuing immunosuppressive therapy favors, as in this case series, an immune-mediated drug reaction as most patients with AIH have a relapse after treatment is suspended. Although AIH related to anti-TNF therapy is rare, a baseline immunological panel along with liver function tests should be performed in all patients with autoimmune disease before starting biologics.

The Patient Satisfaction Scale： Validation of a Portuguese Version in Women Receiving Obstetric Care

Satisfaction of the obstetric patient is an indicator taken in account in health reports. Previous studies point towards several dimensions. In order to better evaluate health care services, it is curtail to have Portuguese language tools of easy appliance. The aim of the study is to validate the PSS in Portuguese. This is a descriptive study using a convenience sample of ninety one mothers. Participants were primary healthcare clients with children aged of I or less. The analysis regarded sensitivity, reliability, as well as concurrent, discriminant, converging and factorial validity. After Promax rotation, three factors were extracted explaining 82.13% of the variance. Reliability analysis was observed by split-half method showing a Spearman-Brown correlation coefficient of 0.91. Cronbach＇s alpha of the Portuguese PSS total scale and subscales were satisfactory with coefficients between 0.829 and 0.941. Concurrent validity was supported by a continuous variable of support from nurses. Discriminant validity showed in women with a normal vaginal delivery, a higher satisfaction both globally and on the three dimensions. The properties observed at the PSS have shown to be satisfactory. PSS reveals itself as a valid measure of the obstetric client＇s satisfaction.

Comparison of the apical seal on filled root canals with Topseal<sup>®</sup>vs MTA Fillapex<sup>®</sup>sealers: A quantitative scintigraphic analysis

This study investigated the microleakage of two different root canal obturation systems, using the nuclear medicine approach, with sodium pertechnetate 99mTc. Twenty six single-rooted extracted teeth were selected. The crowns were sectioned to obtain15 mmlong root segments and each tooth was prepared using rotary ProFile? instruments. The roots were divided into 2 experimental groups and two control groups. Twenty root canals were filled, using Thermafil? and Topseal? or MTA Fillapex? as a sealer. On the 7th and the 28th day the apices were submersed in a solution of 99mTc-Pertechnetate during 3 hours. The radioactivity was counted using a gamma camera. Although apical leakage on the 7th day in the Topseal group was reduced compared with RealSeal1, with a statistical significant difference (p = 0.057), on the 28th day, the MTA Fillapex increased the sealing properties (p = 0.017).

^(99m)Tc in the evaluation of microleakage of composite resin restorations with SonicFill^(TM).An in vitro experimental model

Introduction: The composite SonicFillTM (Kerr/Kavo) is indicated for posterior restorations, with a single increment up to 5 mm due to reduced polymerization shrinkage, thus reducing working time. Aim: Evaluation of marginal microleakage with SonicFillTM. Method and Materials: There were sectioned sixty noncarious human molars in the occluso-cervical direction. Class V cavities were prepared on each tooth with gingival margin walls in a standardized way. The specimens were divided into 4 groups: group 1—restored with SonicFillTM (Kerr/Kavo), group 2—restored with FiltekTM SupremeXTE (3M ESPE), group 3—the cavities were not restored;group 4—restored with SonicFillTM (Kerr/Kavo). In groups 1, 2 and 4 the enamel was conditioned with 37% orthophosphoric acid and applied the self-etch adhesive system Clear- fillTM SE BOND (Kuraray). The specimens were stored in distilled water at 37?C for 7 days. After, the specimens, were immersed in a solution of 99mTc-Pertechnetate and the radioactivity was assessed with a gamma camera. The nonparametric Kruskal-Wallis and Mann-Whitney test with Bonferroni correction at a significance level of 5% were used for the statistical analyses. Results: There are significant differences between the positive and negative control groups and between these and experimental groups (p TM and FiltekTM SupremeXTE. Conclusion: The new composite SonicFillTM and FiltekTM SupremeXTE showed no difference concerning dye penetration. The Sonic- FillTM restorative system showed no influence in concerning microleakage.

Analysis of the Deformation of Composite Materials of Scrapes of Tire and Latex with Temperature Variation

The great major applications of thermal isolation are in the strip of drops and averages temperatures （up to 180 ℃）, which is used from aggressive materials to the nature, such as glass wool, rock wool, polystyrene, EPS among others. In spite of the effectiveness in the retention of the flow of heat, such materials possess considerable cost and they are discarded for long years to be decomposed. In that context, to adapt to the world politics about the preservation of the environment, a study began with intention of developing a material composite with thermal properties, originating from insulating industrial residues. In this research, the behavior of this composite was analyzed, when it submitted traction efforts. For this, a term-mechanic was used （DMA）, where the composite was differentiated under variation temperatures. It was also certain that the gradient temperature of the composite was the source of heat, the conductivity and thermal diffusion. Consequently, the function of the answers of the system was possible to be observed with the deformation of the composite with the tension and temperature variation, as well as the temperature gradient for each proportion of the composite.

Quantitative scintigraphic analysis of the apical seal in Thermafil/Topseal and Real Seal 1/Realseal filled root canals

AIM: To investigate the microleakage of two different root canal obturation systems, using the nuclear medicine ***: Twenty-six single-rooted extracted teeth were selected. The crowns were sectioned to obtain 15-mm long root segments and each tooth was prepared using rotary Pro File instruments. The roots were divided into 2 experimental groups using Real Seal 1 and Real Seal sealer or Thermafil and Top Seal sealer as well as two control groups. On the 7th and the 28 th day the apices were submersed in a solution of 99 m TcPertechnetate during 3 h. The radioactivity was counted using a γ camera. RESULTS: The present study showed that none of the root canal-filled teeth was leakage free. The statistical analyses were made using Kruskal-Wallis and statistical significance was assessed using α = 0.05. Although apical leakage measured in counts per minute(cpm) in the Thermafil/Top Seal group was lower than in the Real Seal/Real Seal group(363 916 ± 180 707.7 cpm vs533 427 ± 414 020.6 cpm) on 7th day and(1 678 200 ± 567 217.4 cpm vs 2 240 518 ± 383 356.7 cpm) on 28 th day, there was no statistical difference(P > 0.05). In the Thermafil/Top Seal group and Real Seal 1/Real Seal group it was found that over time, the number of counts increased between 7 d and 28 d(363 916 ± 180 707.7 cpm vs 1 678 200 ± 567 217.4 cpm) and(533 427 ± 414 020.6 cpm vs 2 240 518 ± 383 356.7 cpm), respectively, with statistically significant differences(Thermafil/Top Seal group, P = 0.015 and Real Seal 1/Real Seal group, P = 0.036).CONCLUSION:Both carrier-based Realseal 1 and Thermafil techniques showed a similar sealing effect,but none of the materials was leakage free.
