AIM: Standard immunosuppression after organ transplantation stimulates tumor growth. Sirolimus has a strong antiproliferative and a tumor inhibiting effect. The purpose is to assess the effect on tumor growth of the immunosuppressive compounds sirolimus and tacrolimus alone and in combination on cells of human hepatocellular ***: We used the human cell lines SK-Hep 1 and Hep 3B derived from hepatocellular carcinoma. Proliferation analyses after treatment with sirolimus, tacrolimus, or the combination of both were performed. FACS analyses were done to reveal cell cycle changes and apoptotic cell death. The expression of apoptosis-related proteins was estimated by Western ***: Sirolimus alone or combined with tacrolimus inhibited the growth of both cell lines after 5 d by up to 35% in SK-Hep 1 cells, and by up to 68% in Hep 3B cells at 25 ng/mL. Tacrolimus alone stimulated the growth by 12% after 5 ng/mL and by 25% after 25 ng/mL in Hep 3B cells. We found an increase of apoptotic Hep 3B cells from 6 to 16%, and a G1-arrest in SK-Hep 1 cells with an increase of cells from 61 to 82%, when sirolimus and tacrolimus were combined. Bcl-2 was down-regulated in Hep 3B, but not in SK-Hep 1 cells after combined ***: Sirolimus appears to inhibit the growth of hepatocellular carcinoma cells alone and in combination with tacrolimus. Sirolimus seems to inhibit the growth stimulation of tacrolimus.

diffuse Idiopathic Neuroendocrine Cell Hyperplasia (dIPNECH) is a rare pulmonary disease. It was first described by Aguayo et al. in 1992, and recognised by the World Health Organisation in 2004 as a precursor lesion to the development of pulmonary carcinoid tumour. dIPNECH has been described in several isolated case reports and series. This article describes a case of dIPNECH and summaries the recent literature in an attempt to raise awareness of this disease and management options.

The concept of autonomic regulation （aR） reflects the relevance of the function of different autonomic systems for health, aR can be captured by questionnaires. We differentiate between a trait or constitutional aR questionnaire version including 12 （short-version） or 18 items, respectively, with three subscales （orthostatic-circulatory, rest/activity and digestive regulation）, and an 18-item state aR questionnaire on the preceding week with four subscales （rest/activity, orthostatic-circulatory, thermo- and digestive regulation）. The validated questionnaires show satisfying to good reliability and robust validity with clear construct validity. In this article, we summarized the actually available literature on aR and the use of aR questionnaires in clinical and observational studies. We described the relationship of high aR with health and in case of low aR or loss of regulation with disease and functional disorder in the three （four） different subscales and functional systems, such as rest/activity, orthostatic-circulatory or digestive regulation （thermoregulation） with the consecutive therapeutJc need. Finally, we gave perspectives of its further application in clinical research.

BACKGROUNd Advanced chronic kidney disease(CKd) is a common complication for people with type 1 and 2 diabetes and can often lead to glucose instability. To date, CGM and intermittent scanning CGM are only approved for people with diabetes but not for those with advanced *** To compare the performance of dexcom G5 and FreeStyle Libre sensors in adults with type 1 or 2 diabetes and advanced *** This was a non-randomized clinical trial that took place in two outpatient clinics in western Sweden. All patients with type 1 or 2 diabetes and an estimated glomerular filtration rate(eGFR) of d to participate. Forty patients(full analysis set = 33) carried the dexcom G5 sensor for 7 d and FreeStyle Libre sensor for 14 d simultaneously. For referencing capillary blood glucose(SMBG) was measured with a high accuracy glucose meter(HemoCue®) during the study period. At the end of the study, all patients were asked to answer a questionnaire on their experience using the *** The mean age was 64.1(range 41-77) years, hemoglobin A1 c was 7.0% [standard deviation(Sd) 3.2], and diabetes duration was 28.5(Sd 14.7) years. A total of 27.5% of the study population was on hemodialysis and 22.5% on peritoneal dialysis. The mean absolute relative difference for dexcom G5 vs SMBG was significantly lower than that for FreeStyle Libre vs SMBG [15.2%(Sd 12.2) vs 20.9%(Sd 8.6)], with a mean difference of 5.72 [95% confidence interval(CI): 2.11-9.32;P = 0.0036]. Endoscopic biliary stent insertion is a well-established procedure. It is especially successful in treating postoperative biliary leaks, and may prevent surgical intervention. A routine change of endoprostheses after 3 mo is a common practice but this can be prolonged to 6 too. We reported a colonic perforation due to biliary stent dislocation and migration to the rectosigmoid colon, and reviewed the literature.

To study the behaviour of reinforced concrete (RC) structures with sections of concrete removed and the reinforcement exposed, 3d nonlinear numerical analysis was performed upon both intact and debonded RC beams by using finite element techniques. The deformational characteristics and the ultimate loads were obtained through numerical models, as well as crack and stress distributions. The failure modes can also be deduced from computational results. Compared with intact beams, the normal assumptions of plane section behaviour is not hold true and the patterns of stress and strain are different in debonded RC beams. The numerical results show good consistency with experimental data. This kind of numerical simulation is a supplement to existing codes.

Background: Currently, surgical resection represents the only curative treatment for pancreatic cancer(PC), however, the majority of tumors are no longer resectable by the time of diagnosis. The aim of this study was to describe time trends and distribution of pancreaticoduodenectomies(Pds) performed for treating PC in Brazil in recent years. Methods: data were retrospectively obtained from Brazilian Health Public System(namely dATASUS) regarding hospitalizations for PC and Pd in Brazil from January 2008 to december 2015. PC and Pd rates and their mortalities were estimated from dATASUS hospitalizations and analyzed for age, gender and demographic characteristics. Results: A total of 2364 Pds were retrieved. Albeit PC incidence more than doubled, the number of Pds increased only 37%. Most Pds were performed in men(52.2%) and patients between 50 and 69 years old(59.5%). Patients not surgically treated and those 70 years or older had the highest in-hospital mortality rates. The most developed regions(Southeast and South) as well as large metropolitan integrated municipalities registered 76.2% and 54.8% of the procedures, respectively. LMIM Pd mortality fluctuated, ranging from 13.6% in 2008 to 11.8% in 2015. Conclusions: This study suggests a trend towards regionalization and volume-outcome relationships for Pd due to PC, as large metropolitan integrated municipalities registered most of the Pds and more stable mortality rates. The substantial differences between Pd and PC increasing rates reveals a limiting step on the health system resoluteness. Reduction in the number of hospital beds and late access to hospitalization, despite improvement in diagnostic methods, could at least in part explain these findings.
