Conventional therapy for moderate to severe inflammatory bowel disease: A systematic literature review

BACKGROUND Despite the advent of biological drugs, conventional therapy continues to be used in moderate to severe inflammatory bowel disease(MS-IBD). This study hypothesized that as a standard of treatment and the primary alternative to biologics, conventional therapy should present robust effectiveness results in IBD *** To investigate the effectiveness of conventional therapy for *** A systematic review with no time limit was conducted in July 2017 through the Cochrane Collaboration, MEDLINE, and LILACS databases. The inclusion criteria encompassed meta-analyses, systematic reviews, randomized clinical trials, observational and case-control studies concerning conventional therapy in adult patients with MS-IBD, including Crohn's disease(CD) and ulcerative colitis(UC). Corticosteroids(prednisone, hydrocortisone, budesonide, prednisolone,dexamethasone), 5-aminosalicylic acid(5-ASA) derivatives(mesalazine and sulfasalazine) and immunosuppressants [azathioprine(AZA), methotrexate(MTX), mycophenolate, cyclosporine, tacrolimus, 6-mercaptopurine(6-MP)] were considered conventional therapy. The exclusion criteria were sample size below50; narrative reviews; specific subpopulations(e.g., pregnant women,comorbidities); studies on postoperative IBD; and languages other than English,Spanish, French or Portuguese. The primary outcome measures were clinical remission(induction or maintenance), clinical response and mucosal healing. As secondary outcomes, fecal calprotectin, hospitalization, death, and surgeries were analyzed. Marginal discrepancy and microleakage in crown-copings fabricated by three CAD/CAM systems: An <i>in vitro</i>study

The purpose of this study was to evaluate and compare the microleakage and marginal discrepancy of ceramic crown-copings fabricated by three different CAD/CAM systems. Material and methods: Thirty extracted premolars were prepared for complete all-ceramic crowns with: total convergence 10-degree, 2 mm occlusal reduction, 4.5 mm height, 1.3 mm circular reduction and chamfer marginal preparation. They were divided into three groups and each group (n = 10) received a ceramic crown-coping fabricated by one of three CAD/CAM systems: Procera, KaVo Everest and Cerec inLab. The copings were cemented with Panavia F 2.0. After thermal cycling, the teeth were immersed in 2% basic fuchsine dye for 24 hours. Microleakage was scored using a five-point scale and evaluated under a stereomicroscope at ×18. Marginal discrepancies were assessed with scanning electronic microscope at ×150, with measurements at 50 different points. Data were subjected to a chisquare test, Kolmogorov-Smirnov test and using Rho Spearman’s correlation test (P 0.05). No significant differences were found in marginal discrepancies for the Procera (62.58 ± 8.98 μm) and KaVo Everest systems (65.54 ± 18.56 μm), (P = 0.941). Significant differences were found for Cerec inLab (132.18 ± 27.75 μm) compared with Procera and KaVo Everest (P = 0.0001). No correlation between the marginal discrepancies and microleakage for the three CAD/CAM crown-copings systems. Conclusions: Cerec inLab CAD/CAM crown-copings showed significantly higher values for microleakage and marginal discrepancy in comparison with Procera and KaVo Everest CAD/CAM crown-copings.

Purpose: This study aims to compare adolescents' cardiometabolic risk score through an integrative classification of physical activity(PA),which involves the combination of moderate-to-vigorous physical activity(MVPA) and sedentary behavior(SB).Methods: A cross-sectional study derived from the Healthy Lifestyle in Europe by Nutrition in Adolescence Cross-Sectional Study database(2006-2008) was conducted in adolescents(n = 548; boys, 47.3%; 14.7 ± 1.2 years) from 10 European cities. MVPA and SB were objectively measured using accelerometry. Adolescents were divided into 4 categories according to MVPA(meeting or not meeting the international recommendations) and the median of SB time(above or below sex-and age-specific median) as follows: High-SB & Inactive, Low-SB & Inactive,High-SB & Active, and Low-SB & Active. A clustered cardiometabolic risk score was computed using the homeostatic model assessment, systolic blood pressure, triglycerides, total cholesterol/high-density lipoprotein cholesterol, sum 4 skinfolds, and cardiorespiratory fitness(CRF).Analyses of covariance were performed to discern differences on cardiometabolic risk scores among PA categories and each health ***: The cardiometabolic risk score was lower in adolescents meeting the MVPA recommendation and with less time spent in SB in comparison to the high-SB & Inactive group(p < 0.05). However, no difference in cardiometabolic risk score was established between High-SB or Low-SB groups in inactive adolescents. It is important to note that CRF was the only variable that showed a significant modification(higher)when children were compared from the category of physically inactive with "active" but not from high-to ***: Being physically active is the most significant and protective outcome in adolescents to reduce cardiometabolic risk. Lower SB does not exhibit a significant and extra beneficial difference.

A Deep Look into Extractive Text Summarization

This investigation has presented an approach to Extractive Automatic Text Summarization (EATS). A framework focused on the summary of a single document has been developed, using the Tf-ldf method (Frequency Term, Inverse Document Frequency) as a reference, dividing the document into a subset of documents and generating value of each of the words contained in each document, those documents that show Tf-Idf equal or higher than the threshold are those that represent greater importance, therefore;can be weighted and generate a text summary according to the user’s request. This document represents a derived model of text mining application in today’s world. We demonstrate the way of performing the summarization. Random values were used to check its performance. The experimented results show a satisfactory and understandable summary and summaries were found to be able to run efficiently and quickly, showing which are the most important text sentences according to the threshold selected by the user.

The cement raw mixtures often contain minor components as magnesium oxide and various kind of sulphates . Generally, if the content in such oxides is under 4-6%. they do not have a significant influence upon the Portland cement properties. In the case of fcrroportland clinkers, the presence of the above mentioned oxides. MgO and SO3, has a significant influence both upon the clinkering temperature and the mechanical properties of the corresponding cements. In the present study, brownmillcrit with or without MgO or/and SO3 (as CaSO4) was synthesized by solid state reaction from pure reagents: CaCO3, Al2O3 and Fe2O3 at 1250℃for 30 minutes. Four different ways of cooling were used: air quenching, water quenching, cooling slowly in the furnace atmosphere and cooling up to 1000oC in furnace atmosphere followed bv air quenching. Rapid cooling of the binder (air and water quenching) determines shorter setting times in comparison with the standard sample cooled in the same conditions and almost doubling of the comprcssive strengths, after 28 days of hardening.

Fatty Acids and Triglycerides Composition in Uruguayan Cow, Sheep and Goat Cheeses

Adverse health consequences are associated with consumption of so-called trans fatty acids （FAs） generated during chemical hydrogenation processes. Naturally-occurring trans fats, including trans vaccenic acid （TVA） derived from a conjugated isomer of linoleic acid （18：2 cis-9, trans-11）, （CLA） in contrast, are recognized as having potentially beneficial effects on human health. These natural trans FAs are found in fat from dairy products and meat of ruminants, and have distinct effects compared with trans FAs industrially produced. The lipid composition of cheeses depends on the milk used. In this work was determined CLA and TVA content in milk fat of sheep and cow to be 1.4% and 3.2%, respectively, and from goat, 0.7% and 1.8%. Also, differences in CLA and TVA content were found in fat extracted from samples of the same cheese type made from cow＇s milk from the same company but with different production dates during a year. The levels of CLA and TVA found in Uruguayan cheeses were generally higher than levels reported in previous studies with comparable cheeses produced in other countries. Milk fat is well-known to confer specific properties to foods. This in turn will affect the rheology and sensory attributes as food. For this reason, it is of interest to determine the triglyceride （TAG） composition in milk-derived foods. Results show that there are characteristic differences in the TAG composition of the different cheeses. We found minimal differences between cheeses from goat and sheep but found distinct characteristics for TAGs from cow milk-derived cheese.

Traumatic brain injury is among the most common causes of death and disability in youth and young *** addition to the acute risk of morbidity with moderate to severe injuries,traumatic brain injury is associated with a number of chronic neurological and neuropsychiatric sequelae including neurodegenerative diseases such as Alzheimer’s disease and Parkinson’s ***,despite the high incidence of traumatic brain injuries and the established clinical correlation with neurodegeneration,the causative factors linking these processes have not yet been fully *** from removal from activity,few,if any prophylactic treatments against post-traumatic brain injury neurodegeneration ***,it is imperative to understand the pathophysiological mechanisms of traumatic brain injury and neurodegeneration in order to identify potential factors that initiate neurodegenerative *** stress,neuroinflammation,and glutamatergic excitotoxicity have previously been implicated in both secondary brain injury and *** particular,reactive oxygen species appear to be key in mediating molecular insult in neuroinflammation and *** such,it is likely that post injury oxidative stress is a key mechanism which links traumatic brain injury to increased risk of ***,reactive oxygen species and their subsequent byproducts may serve as novel fluid markers for identification and monitoring of cellular ***,these reactive species may further serve as a suitable therapeutic target to reduce the risk of post-injury neurodegeneration and provide long term quality of life improvements for those suffering from traumatic brain injury.

Microencapsulation of Lipase and Savinase Enzymes by Spray Drying Using Arabic Gum as Wall Material

Enzymes have been used in detergents over the years. They can improve the detergent’s efficiency due to their activities against hard stains. Nevertheless, enzymes cannot maintain their properties indefinitely, since they are exposed to stress factors, like temperature, pH, mechanical processes and others. Consequently, enzymes lose their structure and they are not functional. For this reason, microencapsulating these proteins is a feasible solution to improve their use in industrial processes and commercial products. Spray drying technology has been selected because a lot of scientific literature proved its useful application in a variety of industries. In particular, savinase and lipase are the two encapsulated enzymes in this work. Savinase attacks proteins and lipase removes fats, so they are suitable enzymes for detergent industry. Arabic gum has been used as wall material. Morphology, size and activity of the obtained microcapsules have been analyzed in order to find the best conditions to produce them. In conclusion, useful microcapsules of lipase and savinase can be obtained with the mentioned technology.

CYP1A1,CYP2E1 and EPHX1 polymorphisms in sporadic colorectal neoplasms

AIM To investigate the contribution of polymorphisms in the CYP1A1, CYP2E1 and EPHX1 genes on sporadic colorectal cancer(SCRC) risk. METHODS Six hundred forty-one individuals(227 patients with SCRC and 400 controls) were enrolled in the study. The variables analyzed were age, gender, tobacco and alcohol consumption, and clinical and histopathological tumor parameters. The CYP1A1 *2A, CYP1A1 *2C CYP2E1 *5B and CYP2E1 *6 polymorphisms were analyzed by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP). The EPHX1 Tyr113 His, EPHX1 His139 Arg and CYP1A1 *2C polymorphisms were detected by real-time PCR. Chisquared test and binary logistic regression were used in the statistical analysis. Haplotype analysis was conducted using the Haploview program, version *** Age over 6 2 years was a risk factor for SCRC development(OR = 7.54, 95%CI: 4.94-11.50, P < 0.01). Male individuals were less susceptible to SCRC(OR = 0.55, 95%CI: 0.35-0.85, P < 0.01). The CYP2E1*5B polymorphism was associated with SCRC in the codominant(heterozygous genotype: OR = 2.66, 95%CI: 1.64-4.32, P < 0.01), dominant(OR = 2.82, 95%CI: 1.74-4.55, P < 0.01), overdominant(OR = 2.58, 95%CI: 1.59-4.19, P < 0.01), and log-additive models(OR = 2.84, 95%CI: 1.78-4.52, P < 0.01). The CYP2E1*6 polymorphism was associated with an increased SCRC risk in codominant(heterozygous genotype: OR = 2.81, 95%CI: 1.84-4.28, P < 0.01; homozygous polymorphic : OR = 7. 3 2, 9 5 % C I : 1.85-28.96, P < 0.01), dominant(OR = 2.97, 95%CI: 1.97-4.50, P < 0.01), recessive(OR = 5.26, 95%CI: 1.35-20.50, P = 0.016), overdominant(OR = 2.64, 95%CI: 1.74-4.01, P < 0.01), and log-additive models(OR = 2.78, 95%CI: 1.91-4.06, P < 0.01). The haplotype formed by the minor alleles of the CYP2E1*5B(C) and CYP2E1*6(A) polymorphisms was associated with SCRC(P = 0.002).
