In this editorial,we comment on the article by Liakina V:“Antibiotic resistance in patients with liver cirrhosis:Prevalence and current approach to tackle”(World J Clin Cases 2023,11:7530-7542).In this excellent review,Liakina presents current data on bacterial complications in patients with *** infections are the most common complication in patients with liver *** focus specifically on spontaneous bacterial peritonitis(SBP)which is the most repres-entative infectious *** V suggested starting empirically,in all patients with suspected SBP,third-generation cephalosporins when the number of polymorphonuclear leukocytes(PMNs)in ascites is greater than 250/mm^(3).This statement creates some doubts in our clinical practice so we discuss on the unsolved pitfalls of diagnosis and treatment that are often encountered in patients with ascitic fluid infections,especially on bacterascites that is defined as ascitic bacterial growth with PMNs below 250/mm^(3).The severity of liver disease and overall prognosis are highly comparable for patients with bacterascites and SBP in some recent well-conducted ***,we present a brief analysis of the prevalence of antibiotic-resistant isolates with an introduction of currently approved antibiotic drug options to treat ascitic fluid infections avoiding antibiotic *** light of the most recent epidemiological data,third-generation cephalosporins should not be considered as an empirical antibiotic treatment of choice for ascitic fluid infections.

In a previous paper "to retrofit or not to retrofit?"(Nuti and Vanzi, 2003) a straightforward procedure able to forecast the economic return of seismic structural upgrading was presented. More recently, the authors realized that the final mathematical results can be much simplified so as to allow back-of-an-envelope computation. The title of this paper tries to highlight precisely this aspect, namely that for many a regular seismic structural upgrading cases, nearly no computation is needed(apart from one subtraction and one multiplication) to assess their economic convenience. These findings are presented and discussed in this paper, together with a state of the art on the cost-studies available in literature and technical codes. The mathematical formulation leading to the proposed approximation is suitably explained, underlining its applicability field and comparing it with the rigorous solution. Also a table and a formula are furnished that alternatively allows to calculate the maximum estimation errors, in order to obtain an upper and lower bound for the maximum amount of money which should be allocated for seismic structural upgrading.

Assessment of cardiac function in infants with transposition of the great arteries after surgery: comparison of two methods

Background Assessment of cardiac function is crucial in pediatric patients undergoing cardiovascular surgery, monitoring cardiac output and changing hemodynamic conditions during surgery accordingly is important to improve post-surgical outcome. We aimed to measure cardiac index (CI) and maximal rate of the increase of left ventricular pressure dp/dt(max) with the pressure recording analytic method (PRAM, MostCare?) and compared it with transthoracic echocardiographic cardiac index estimation in infants with transposition of the great arteries (TGA) undergoing surgical correction. Methods We enrolled 74 infants with TGA consecutively into this study. CI and dp/dt(max) were measured with PRAM and echocardiography at 0, 4, 8, 12, 24 and 48 h postoperatively. Blood brain natriuretic peptide (BNP) and blood lactate (Lac) were measured at baseline and after operation. Results The median age at surgery was 13 days (range 1–25 days) with an average weight of 3.24 kg (range 2.31–4.17 kg). CI esti-mated by PRAM was 1.11 ± 0.12 L/min/m2 (range 0.69–1.36) and by Doppler echocardiography was 1.13 ± 0.13 L/min/m2 (range 0.76–1.40). dp/dt(max) estimated by PRAM was 1.31 ± 0.03 mmHg/s (range 1.23–1.43) and by Doppler echocardiography was 1.31 ± 0.04 L/min/m2 (range 1.25–1.47). CI (r = 0.817,P< 0.001) and dp/dt(max) (r = 0.794,P< 0.001) measured by two meth-ods were highly correlated with a linear relation. Blood BNP and lactate increased to the highest level at 8–12 h post-operatively. Conclusions In the early post-operative period, PRAM provides reliable estimates of cardiac index and dp/dt(max) value compared with echocardiographic measurements. PRAM through mostcare? is a reliable continuous monitoring method for peri-operative management in children with congenital heart disease.

Tumour cells have exquisite flexibility in reprogramming their metabolism in order to support tumour initiation, progression,metastasis and resistance to therapies. These reprogrammed activities include a complete rewiring of the bioenergetic, biosyntheticand redox status to sustain the increased energetic demand of the cells. Over the last decades, the cancer metabolism field hasseen an explosion of new biochemical technologies giving more tools than ever before to navigate this complexity. Within a cell ora tissue, the metabolites constitute the direct signature of the molecular phenotype and thus their profiling has concrete clinicalapplications in oncology. Metabolomics and fluxomics, are key technological approaches that mainly revolutionized the fieldenabling researchers to have both a qualitative and mechanistic model of the biochemical activities in cancer. Furthermore, theupgrade from bulk to single-cell analysis technologies provided unprecedented opportunity to investigate cancer biology at cellularresolution allowing an in depth quantitative analysis of complex and heterogenous diseases. More recently, the advent offunctional genomic screening allowed the identification of molecular pathways, cellular processes, biomarkers and noveltherapeutic targets that in concert with other technologies allow patient stratification and identification of new treatmentregimens. This review is intended to be a guide for researchers to cancer metabolism, highlighting current and emergingtechnologies, emphasizing advantages, disadvantages and applications with the potential of leading the development ofinnovative anti-cancer therapies.

Delay/Disruption-Tolerant Networking(DTN) originated from research on Interplanetary Internet and still today space applications are the most important application field and research stimulus. This paper investigates DTN communications between the Earth and the far side of the Moon, by means of a lunar orbiter acting as relay. After an introductory part, the paper presents a comprehensive analysis of the DTN performance that can be achieved on the identified communication scenario. The focus is on the evaluation of the stateof-the-art ability of Interplanetary Overlay Network(ION), the NASA DTN implementation of Bundle Protocol(BP) and Contact Graph Routing(CGR), to meet the many challenges of the space communication scenario investigated(and more generally of a future interplaynetary Internet): intermittent links, network partitioning, scarce bandwidth, long delays, dynamic routing, handling of high priority and emergency traffic, interoperability issues. A study of security threats and Bundle Security Protocol(BSP) countermeasures complete the *** many results provided, confirm the essential role of DTN in future space communications.
