Echinococcal cysts are usually found in liver and lungs, but any other organ can potentially be involved. Extrahepatic disease due to hydatid cyst may develop in the abdominal and pelvic cavity, aside from in other less common locations, which may make both diagnosis and treatment more complex. We present a rare case of extrahepatic echinococcosis in a 70-year old patient with a 4-d history of dull abdominal pain, anemia within the transfusion range and fever. She underwent surgery for left renal hydatid cysts 30 years ago. After non operative treatment, imaging studies showed a calcified hydatid cyst in a retrogastric location communicating with a proximal jejunal loop. En-block resection of the mass together with the adrenal gland was performed including closure of the enteric fistula. Anatomic pathology confirmed the diagnosis of a calcified hydatid cyst of left adrenal origin. Surgery is the treatment of choice and most authors recommend removal of cyst and adrenal gland.

In recent years,the small pelagic fishery on the Pacific northwest coast of Mexico has significantly increased fishing pressure on thread herring Opisthonema *** fishery is regulated using a precautionary approach(acceptable biological catch(ABC)and minimum catch size).However,due to fishing dynamics,fish aggregation habits and increased fishing mortality,periodic biomass assessments are necessary to estimate ABC and assess the resource *** Catch-MSY approach was used to analyze historical series of thread herring catches off the western Baja California Sur(BCS,1981–2018)and the Gulf of California(GC,1972–2018)to estimate exploitable biomass and target reference points in order to obtain catch *** to the results,in GC,the maximum biomass reached in 1972(at the beginning of fishery)and minimum biomass reached in 2015;the estimated exploitable biomass for 2019 was 42.2×10^(4) t;and the maximum sustainable yield(MSY)was 15.4×10^(4) *** the western BCS coast,the maximum biomass was reached in 1981(at the beginning of fishery)and minimum biomass was reached in 2017;the estimated exploitable biomass for 2019 was 3.2×10^(4) t;and the MSY was 1.2×10^(4) *** stocks showed a decrease in biomass over the past years and were currently near to point of full *** results suggest that the use of the Catch-MSY method is suitable to obtain annual biomass estimates,in order to establish an ABC,to know the current state of the resource,and to avoid overcoming the potential recovery of the stocks.

Disulfiram and calcium carbimide are two alcohol deterrants widely used in alcoholism treatment, however, there exist great concerns over their safety. Reports on hepatotoxicity, mainly related to disulfiram therapy, have been published. The hepatotoxic potential of calcium carbimide is less well characterized. Here, we describe four cases of liver damage related to this therapeutic group that were submitted to a Registry of hepatotoxicity and point out the limitations that we face when prescribing these compounds. A reassessment of the role of these compounds in the management of alcohol dependence is clearly needed.

BACKGROUND The gut microbiota plays a key role in the maintenance of intestinal homeostasis and the development and activation of the host immune system. It has been shown that commensal bacterial species can regulate the expression of host genes. 16 S rRNA gene sequencing has shown that the microbiota in inflammatory bowel disease(IBD) is abnormal and characterized by reduced diversity. Micro RNAs(miRNAs) have been explored as biomarkers and therapeutic targets, since they are able to regulate specific genes associated with Crohn's disease(CD). In this work, we aim to investigate the composition of gut microbiota of active treatment-na?ve adult CD patients, with miRNA profile from gut *** To investigate the composition of gut microbiota of active treatment-na?ve adult CD patients, with miRNA profile from gut *** Patients attending the outpatient clinics at Valme University Hospital without relevant co-morbidities were matched according to age and gender. Faecal samples of newonset CD patients, free of treatment, and healthy controls were collected. Faecal samples were homogenized, and DNA was amplified by PCR using primers directed to the 16 S bacterial rRNA gene. Pyrosequencing was performed using GS-Junior platform. For sequence analysis, MGRAST server with the database Ribosomal Project was used. MiRNA profile and their relative abundance were analyzed by quantitative *** Microbial community was characterized using 16 S rRNA gene sequencing in 29 samples(n = 13 CD patients, and n = 16 healthy controls). AIM: To address the therapeutic efficacy of various treatment regimens in genotype 3 selecting randomized clinical trials and prospective National Cohort ***:(1) PEG-INF-based therapy including sofosbuvir(SOF) + RBV for 12 wk vs SOF + RBV 24 wk;(2) SOF + RBV therapy 12 wk/16 wk vs 24 wk; and(3) the role of RBV in SOF + daclatasvir(DCV) and SOF + ledipasvir(LDV) combinations. This metaanalysis provides robust information with the intention of addressing treatment strategy for hepatitis C virus genotype ***: A combination treatment including SOF + RBV + PEG-IFN for 12 wk notes better SVR than with only SOF + RBV for 12 wk, although its association with more frequent adverse effects may be a limiting factor. Longer duration therapy with SOF + RBV(24 wk) has achieved higher SVR rates than shorter durations(12 or 16 wk). SOF + LDV are not an ideal treatment for genotype 3. CONCLUSION: Lastly, SOF + DCV combination is probably the best oral therapy option and the addition of RBV does not appear to be needed to increase SVR rates substantially.

Objective: To determine the outcomes in solid organ transplant recipients following inpatient rehabilitation, as a result of a unique partnership between the rehabilitation hospital and the multi-organ transplant program in an acute hospital. Design: Retrospective observational study. Setting: Community rehabilitation hospital affiliated with a university. Participants: A cohort of 173 organ transplant patients admitted consecutively over a four-year period (2004-2008) was compared to a cohort of all rehabilitation patients (n = 9762) admitted to the same inpatient rehab facility during the same period. Interventions: Inpatient rehab program to all participants. Main Outcome Measures: Length of hospital stay, Functional Independence Measure (FIMTM) change (admission-discharge), and rate of discharges to home. Results: Outcomes were measured using components of the FIMTM instrument, admission and discharge data. Chi-square and independent two-sample t-tests were used for statistical analysis. Compared to a general rehabilitation inpatient population, transplant rehabilitation inpatients had: more immediate (TM change (8.9 vs. 20.9, p TM efficiency (1.1 vs. 1.4, p < 0.001);and a higher rate of discharges to home in patients not readmitted to acute care (98.5% vs. 94.5% p < 0.001). Conclusion: Outcomes of rehabilitation in solid organ transplant patients are comparable but not identical to those in other patient groups. Inpatient rehabilitation for transplant patients is therefore fully justifiable and necessary. The ten times higher rate of transplant patient readmission to acute hospital must be communicated, facilitated, accepted and managed within a partnership strategy.

Computational Method for Extracting and Modeling Periodicities in Time Series

Periodicity is common in natural processes, however, extraction tools are typically difficult and cumbersome to use. Here we report a computational method developed in MATLAB through a function called Periods with the aim to find the main harmonic components of time series data. This function is designed to obtain the period, amplitude and lag phase of the main harmonic components in a time series (Periods and lag phase components can be related to climate, social or economic events). It is based on methods of periodic regression with cyclic descent and includes statistical significance testing. The proposed method is very easy to use. Furthermore, it does not require full understanding of time series theory, nor require many inputs from the user. However, it is sufficiently flexible to undertake more complex tasks such as forecasting. Additionally, based on previous knowledge, specific periods can be included or excluded easily. The output results are organized into two groups. One contains the parameters of the adjusted model and their F statistics. The other consists of the harmonic parameters that best fit the original series according to their importance and the summarized statistics of the comparisons between successive models in the cyclic descent process. Periods is tested with both, simulated and actual sunspot and Multivariate ENSO Index data to show its performance and accuracy.
