Hepatobiliary manifestations in inflammatory bowel disease: The gut,the drugs and the liver

In this paper,we develop and study numerical methods for the two-mode shallow water equations recently proposed in[***,***,and ***,*** Dynamics,22(2008),pp.407–432].Designing a reliable numerical method for this system is a challenging task due to its conditional hyperbolicity and the presence of nonconservative *** present several numerical approaches—two operator splitting methods(based on either Roe-type upwind or central-upwind scheme),a central-upwind scheme and a path-conservative centralupwind scheme—and test their performance in a number of numerical *** obtained results demonstrate that a careful numerical treatment of nonconservative terms is crucial for designing a robust and highly accurate numerical method。

Background:This study is aimed at describing the prevalence of and risk factors associated with early post-operative complications after Crohn’s disease-related intestinal ***:This was a retrospective analysis of data from the PRACTICROHN *** Crohn’s disease patients who underwent ileocolonic resection with ileocolonic anastomosis between January 2007 and December 2010 were *** complications evaluated included death,ileus,anastomotic leak,abscess,wound infection,catheter-related infection,digestive bleeding and other extra-abdominal infections that occurred in the 30 days after ***:A total of 364 patients(median age at surgery 38 years and 50%men)were *** for surgery was:stricturing disease(46.4%),penetrating disease(31.3%),penetrating and stricturing disease(14.0%)or resistance to medical treatment(5.8%).Early complications were recorded in 100(27.5%)patients,with wound infection,intra-abdominal abscess and anastomotic leakage being the most frequent *** hospitalization duration was 16 days for patients with complications vs.9 days without complications(P<0.001).Complications were more common among patients with penetrating disease(36/114,31.6%)and those refractory to treatment(9/21,42.9%)compared with stricturing disease(45/169,26.6%)or stricturingtpenetrating disease(6/51,11.8%)(P=0.040).The rate of complications was higher among patients with diagnosis made at the time of surgery(15/31,48.4%)compared with the rest(85/331,25.7%)(P=0.013).Medication received at the time of surgery did not affect the rate of ***:Almost a quarter of patients developed early complications after intestinal *** disease and urgent surgery were associated with an increased risk of complications.

Ultrasound-Guided Infraclavicular Axillary Vein Cannulation

Background and Objective: Percutaneous central venous cannulation is a common invasive procedure. In comparison with an external landmark technique, the advantages of ultrasound-guided venous access include direct visualization of the anatomy and in vivo visualization of venous cannulation. Methods: We evaluated an ultrasound-guided technique for infraclavicular axillary vein cannulation, focusing on its ease of use, success rate and complications rate. One hundred and twenty patients who submitted to central venous catheter placement were punctured using our technique. The patients were positioned so that their ipsilateral upper limb was abducted at 90° to the longitudinal axis, which makes it possible to visualize the infraclavicular vessels due to the elevation of the clavicle, thereby improving accessibility. Results: Cannulation was successful in all patients. The median time from the start of the first puncture (of the skin) until the aspiration of blood was 15 s (range 7 - 135 s). Both infraclavicular axillary veins were cannulated, and the vein was punctured successfully at the first attempt in 95% of the patients, without complications during the procedure. Conclusion: We propose an ultrasound-guided infraclavicular approach of the axillary vein, with a high success rate and no complications in the present cohort.

BACKGROUND The gut microbiota plays a key role in the maintenance of intestinal homeostasis and the development and activation of the host immune system. It has been shown that commensal bacterial species can regulate the expression of host genes. 16 S rRNA gene sequencing has shown that the microbiota in inflammatory bowel disease(IBD) is abnormal and characterized by reduced diversity. Micro RNAs(miRNAs) have been explored as biomarkers and therapeutic targets, since they are able to regulate specific genes associated with Crohn's disease(CD). In this work, we aim to investigate the composition of gut microbiota of active treatment-na?ve adult CD patients, with miRNA profile from gut *** To investigate the composition of gut microbiota of active treatment-na?ve adult CD patients, with miRNA profile from gut *** Patients attending the outpatient clinics at Valme University Hospital without relevant co-morbidities were matched according to age and gender. Faecal samples of newonset CD patients, free of treatment, and healthy controls were collected. Faecal samples were homogenized, and DNA was amplified by PCR using primers directed to the 16 S bacterial rRNA gene. Pyrosequencing was performed using GS-Junior platform. For sequence analysis, MGRAST server with the database Ribosomal Project was used. MiRNA profile and their relative abundance were analyzed by quantitative *** Microbial community was characterized using 16 S rRNA gene sequencing in 29 samples(n = 13 CD patients, and n = 16 healthy controls). Coronary leukocyte activation in relation to progression of coronary artery disease

Leukocyte activation has been linked to atherogenesis, but there is little in vivo evidence for its role in the progression of atherosclerosis. We evaluated the predictive value for progression of coronary artery disease （CAD） of leukocyte activation markers in the coronary circulation. Monocyte and neutrophil CDllb, neutrophil CD66b expression and intracellular neutrophil myeloperoxidase （MPO） in the coronary arteries were determined by flow cytometry in patients undergoing coronary angiography. The primary outcome included fatal and nonfatal myocardial infarction or arterial vascular intervention due to unstable angina pectoris. In total 99 subjects who were included, 70 had CAD at inclusion （26 patients had single-vessel disease, 18 patients had two- vessel disease and 26 patients had three-vessel disease）. The median follow-up duration was 2242 days （interquartile range： 2142-2358）. During follow-up, 13 patients （13%） developed progression of CAD. Monoeyte CDllb, neutrophil CDllb and CD66b expression and intraceilular MPO measured in blood obtained from the coronary arteries were not associated with the progression of CAD. These data indicate that coronary monocyte CDllb, neutrophil CDllb and CD66b expression and intracellular MPO do not predict the risk of progression of CAD.

Conducting clinical research in community mental health settings: Opportunities and challenges

Tremendous progress has been made in the past decade surrounding the underlying mechanisms and treatment of neuropsychiatric disease. Technological advancements and a broadened research paradigm have contributed to the understanding of the neurochemistry, brain function and brain circuitry involved in neuropsychiatric disorders. The predominant area of unmet medical need in the United States is major psychiatric disorders, and major depressive disorder is the leading cause of disability for ages 15-44. Total spending on research and development by the pharmaceutical industry has grown exponentially during the past decade, but fewer new molecular entities(NME) for the treatment of major psychiatric disorders have received regulatory approvals compared to other therapeutic areas. Though significant expansion has occurred during the "decade of the brain", the translation of clinical trials outcomes into the community mental health setting is deficient. Randomized controlled trials(RCTs) have been the standard approach to clinical evaluation of the safety and efficacy of NMEs for the past 60 years; however, there are significant barriers and skepticism in the implementation of evidence-based outcomes into clinical practice. Recruitment of patients, shortages of experiencedclinical researchers, regulatory requirements and later translation of outcomes into clinical practice are ever growing problems faced by investigators. The community mental health setting presents particular barriers in the replication of therapeutic outcomes from RCTs. The diagnostic complexity of major psychiatric diseases and the highly selective patient populations involved in clinical trials lend to the gap in translation from the "bench to the bedside". The community mental health setting lends to a diverse patient population with numerous co-morbidities and environmental factors that are unaccounted in the average RCT. Flow-Injection Flame Atomic Absorption Determination of Hexavalent Chromium with On-Line Preconcentration on an Anion Imprinted Polymer

A flow injection preconcentration system for the flame atomic absorption spectrometric determination of hexavalent chromium has been developed. The method employs on-line preconcentration of Cr(VI) on a minicolumn packed with Cr(VI)-imprinted poly(4-vinyl pyridineco-2-hydroxyethyl methacrylate) placed into a flow injection system. Hexava-lent chromium was eluted with a small volume of diluted hydrochloric acid into the nebulizer-burner system of a flame atomic absorption spectrometer. An enrichment factor of 550 and a 3σ detection limit of 0.04 μg·L-1 along a sampling frequency of 4 h-1 at a sample flow rate of 3.5 mL·min-1. The relative standard deviation is 2.9% for 1 μg·L-1 Cr(VI) (n = 11). The flow injection system proposed has the advantage of being simpler because the use of expensive and sophisticated instruments is avoided. Ease of use, continuous process and selectivity make this method suitable for Cr(VI) determination in different environmental samples such as sea and river waters, soils and sediments.

Nucleation of Fe-Rich Intermetallic Phases on α-Al_(2)O_(3) Oxide Films in Al-Si Alloys

Nucleation of Fe-rich intermetallic phases on α-Al2O3 oxide films was analyzed in five experimental Al-Fe-Mn-Si alloys. In an attempt to verify the role of α-Al2O3 films in nucleating Fe-rich intermetallics, experiments have been conducted under conditions where formed in situ oxide films were present. Some Fe-rich intermetallics are observed to be related with the oxide films in the microstructure, the present results are in accordance with some research that suggest that the Fe-rich intermetallic phases nucleate upon the oxide films. The intermetallic phases and α-Al2O3 films were unambiguously identified with the help of complementary techniques such as thermal analysis, SEM/EDS and X-ray diffraction.
