Targeting host factors:A novel rationale for the management of hepatitis C virus

Hepatitis C is recognized as a major threat to global public health. The current treatment of patients with chronic hepatitis C is the addition of ribavirin to interferon-based therapy which has limited efficacy, poor tolerability, and significant expense. New treatment options that are more potent and less toxic are much needed. Moreover, more effective treatment is an urgent priority for those who relapse or do not respond to current regimens. A major obstacle in combating hepatitis C virus （HCV） infection is that the fidelity of the viral replication machinery is notoriously low, thus enabling the virus to quickly develop mutations that resist compounds targeting viral enzymes. Therefore, an approach targeting the host cofactors, which are indispensable for the propagation of viruses, may be an ideal target for the development of antiviral agents because they have a lower rate of mutation than that of the viral genome, as long as they have no side effects to patients. Drugs targeting, for example, receptors of viral entry, host metabolism or nuclear receptors, which are factors required to complete the HCV life cycle, may be more effective in combating the viral infection. Targeting host cofactors of the HCV life cycle is an attractive concept because it imposes a higher genetic barrier for resistance than direct antiviral compounds. However the principle drawback of this strategy is the greater potential for cellular toxicity.

A Hybrid Regularization-Based Multi-Frame Super-Resolution Using Bayesian Framework

The prime purpose for the image reconstruction of a multi-frame super-resolution is to reconstruct a higher-resolution image through incorporating the knowledge obtained from a series of relevant low-resolution images,which is useful in ***,super-resolution image reconstruction methods are usually damaged by undesirable restorative artifacts,which include blurring distortion,noises,and stair-casing ***,it is always challenging to achieve balancing between image smoothness and preservation of the edges inside the *** this research work,we seek to increase the effectiveness of multi-frame super-resolution image reconstruction by increasing the visual information and improving the automated machine perception,which improves human analysis and interpretation ***,we propose a new approach to the image reconstruction of multi-frame super-resolution,so that it is created through the use of the regularization *** the proposed approach,the bilateral edge preserving and bilateral total variation regularizations are employed to approximate a high-resolution image generated from a sequence of corresponding images with low-resolution to protect significant features of an image,including sharp image edges and texture details while preventing *** experimental results of the synthesized image demonstrate that the new proposed approach has improved efficacy both visually and numerically more than other approaches.

Impact of IL28B gene polymorphisms rs8099917 and rs12980275 on response to pegylated interferon-α/ribavirin therapy in chronic hepatitis C genotype 4 patients

Host genetic factors may predict the outcome and treatment response in hepatitis C virus（HCV）*** of these factors is the single nucleotide polymorphisms of the interleukin 28B（IL28B）*** sought to evaluate the outcome of pegylated interferon and ribavirin therapy in association with IL-28B rs8099917 and rsl2980275 in patients infected with HCV genotype 4.A total of 180 patients with chronic hepatitis C were selected from Egyptians who have received combined therapy with pegylated interferon and ribavirin for 6 months and their response was evaluated after follow-up at 0,6,12,24 and 48 weeks from the beginning of the *** samples were collected from responders and *** DNA was extracted from whole blood and genotyping was carried out by polymerase chain reaction and restriction fragment length polymorphism（PCR-RFLP）.Our results showed that TT genotype of rs8099917 was associated with higher sustained viral response（SVR）rates and G allele represented a risk factor for failure of response（OR=3.7,CI=1.8：7.64）while rs12980275 was not significantly associated with SVR in genotype 4 Egyptian *** determination of 1L-28B SNPs may be useful in enhancing correct prediction of SVR achievement in treating this group of genotype 4 patients.

Measurements of Natural Radioactivity in Some Granite Samples Using Alpha Spectrometric Analysis

Alpha spectrometry using pulse height analysis has been used for the determination of uranium concentrations in different environmental samples. The concentration of 238U was measured by both destructive and non-destructive techniques with a detection limit of less than 1.8 mBq/kg. However, because of the extremely low 234U concentrations in environmental samples, it was necessary to use a destructive technique to separate U from the sample matrices as well as remove interfering elements from the sample solution to determine 238U/234U ratio. In this study, the uranium was separated from the environmental samples using anion exchangers in (Dowex 1 × 8 Cl? form) and purified via co-precipitation with Lanthanum fluorides (LaF3) and the alpha source prepared by electrodeposition. The results obtained were validated using some certified reference samples.

Novel non-invasive biological predictive index for liver fibrosis in hepatitis C virus genotype 4 patients

AIMTo investigate the diagnostic ability of a non-invasive biological marker to predict liver fibrosis in hepatitis C genotype 4 patients with high accuracy. METHODSA cohort of 332 patients infected with hepatitis C genotype 4 was included in this cross-sectional study. Fasting plasma glucose, insulin, C-peptide, and angiotensin-converting enzyme serum levels were measured. Insulin resistance was mathematically calculated using the homeostasis model of insulin resistance (HOMA-IR). RESULTSFibrosis stages were distributed based on Metavir score as follows: F0 = 43, F1 = 136, F2 = 64, F3 = 45 and F4 = 44. Statistical analysis relied upon reclassification of fibrosis stages into mild fibrosis (F0-F) = 179, moderate fibrosis (F2) = 64, and advanced fibrosis (F3-F4) = 89. Univariate analysis indicated that age, log aspartate amino transaminase, log HOMA-IR and log platelet count were independent predictors of liver fibrosis stage (P CONCLUSIONOur predictive model is easily available and reproducible, and predicted liver fibrosis with acceptable accuracy.

Clinical Outcome of Low Dose Contrast during Percutaneous Coronary Intervention in Patients with Moderate to Severe Kidney Impairment

Background: Chronic kidney disease patients are at a greater risk for nephropathy requiring dialysis after percutaneous coronary intervention. Such patients are usually deferred due to fear of “Renalism”.?Objectives This study assesses the outcome of Low dose contrast protocol during PCI in CKD patients whose e-GFR 60 ml/min/1.72 m and investigates a safety margin for contrast use in these high-risk categories.?Methods: Patients were into three groups according to CV/e-GFR ratio: Group (A) low-dose: CV/e-GFR ratio 2.0 Group (B) medium-dose: CV/e-GFR ratio > 2.0 and × bodyweight\***). Group (C) high-dose: CV/e-GFR ratio > MACD. Results: A total of 73 patients were enrolled. Average age was 54 ± 8 years,81.4% were male and 18.6% were females and 52% were diabetic. Mean baseline e-GFR was 40 ± 8.0 ml/min/1.73m2. Contrast Volume used in group A was (58.26 ± 15.05) (n = 24), in group B (109.42 ± 17.11) (n = 26) and in group C (304.5 ± 60.30) (n = 23), respectively. The incidences of CI-AKI in the 3 groups were 0%, 11.5% and 35%, respectively (p = 0.02). All-cause death 0%, 17% and introduction of maintenance hemo dialysis was 0%, 11.5% and 26%, respectively (p Conclusion: Low dose contrast protocol is safe, effective and easily applicable technique without CI-AKI or death.

Joint Optimal Power Source Sizing and Data Collection Trip Planning for Advanced Metering Infrastructure Enabled by Unmanned Aerial Vehicles

The use of unmanned aerial vehicles(UAVs) in the collection of data from wireless devices, sensor nodes, and the Internet of Things(IoT) devices has recently received significant attention. In this paper, we investigate the data collection process from a set of smart meters in advanced metering infrastructure(AMI) enabled by UAVs. The objective is to minimize the total annual cost of the electric utility by jointly optimizing the number of UAVs, their power source sizing, the charging locations as well as the data collection trip planning. This is achieved while considering the energy budgets of batteries of UAVs and the required amount of collected data. The problem is formulated as a mixed-integer nonlinear programming(MINLP), which is decoupled into two sub-problems where a candidate UAV and a number of buildings are first grouped into trips via genetic algorithms(GAs), and then the optimum trip path is found using a traveling salesman problem(TSP) branch and bound algorithm. Simulation results show that the battery capacity or the number of UAVs increases as the coverage area or the density increases.

Profile of Extra-Pulmonary Tuberculosis in Internal Medicine

Introduction: Tuberculosis is an infectious disease that mainly affects the lung. Extrapulmonary localizations are a reason for hospitalization in our health facilities. The objective of this study was to describe the epidemiological, clinical, paraclinical and evolutionary aspects of extra pulmonary tuberculosis (EPT) at the Abass Ndao Hospital Center. Patients and Methods: This was a descriptive cross-sectional study conducted over a period of 11 years (January 1, 2010 to December 30, 2021). All patients with extrapulmonary tuberculosis hospitalized in the department of internal medicine during the recruitment period were included. Results: Fifty-two (52) patients were collected. The year 2019 recorded the most cases 23.08% (n = 20). The mean age of the patients was 40.56 ± 18.24 years. The age group 20 - 34 years 42.31% (n = 22) was the most represented. Females were in the majority 61.54% (n = 32) with a sex ratio (M/F) was 0.63. Housewives were in the majority 40.38% (n = 21). 60.87% of the cases (n = 14) came from a health facility. 38.46% of the cases had been infected. 21.74% (n = 9) were smokers. The reasons for consultation were dominated by fever (67.44%), AEG (62.79%) and cough (41.86%). Eighteen patients (40.91%) had fever. The mean time to consultation was 77.37 ± 90.3 days with extremes of 3 and 365 days. The median was 45 days. More than half of the patients 61.90% (n = 26) had anemia. Positive retroviral serology was noted in 21.43% of cases. All patients had a CRP greater than 6. More than half of the patients 51.92% (n = 27) had multifocal tuberculosis. The peritoneum 44.23% (n = 23) was the main organ affected. The average hospital stay was 9.8 ± 4.9 days with extremes of 1 and 19 days. All patients had received the protocol in force at the national level. Death was noted in 4 patients (9.52%).
